Potential impact of specific therapy on pregnant women with pulmonary arterial hypertension without cardiac shunt:a descriptive study in northern China

Background:Pregnancy in women with pulmonary arterial hypertension(PAH)is a fatal condition,despite the effectiveness of PAH-specific therapies.The coverage status and effect of specific therapies in pregnant patients with PAH without cardiac shunts in China remain unclear.To investigate this issue,we conducted a multicenter retrospective study in northern China.Methods:The study included 85 patients who were admitted to 4 clinical centers in Shandong Province between October 2010 and August 2020.Maternal endpoint events included(1)maternal death and/or(2)major adverse cardiac events,both occurring during pregnancy or within 6 weeks postpartum.Results:Although the overall mortality rate was encouraging(11.8%),the number of patients receiving PAH-specific therapies was extremely low(28.2%).Moreover,only 15.3%of patients received adequate duration of PAH-specific therapy(≥4 weeks)before delivery,and this subgroup showed the lowest major adverse cardiac events rate(7.7%)compared with that in the untreated(19.7%)and short-time treated groups(lity risks.Short-term PAH-specific therapy does not guarantee favorable maternal outcomes.Prepregnancy screening,early identification,and timely intervention are expected to improve maternal outcomes in pregnant women with PAH.

Impacts of pancreatic exocrine insufficiency on gut microbiota

Pancreatic exocrine insufficiency(PEI)can be induced by various kinds of diseases,including chronic pancreatitis,acute pancreatitis,and post-pancreatectomy.The main pathogenetic mechanism of PEI involves the decline of trypsin synthesis,disorder of pancreatic fluid flow,and imbalance of secretion feedback.Animal studies have shown that PEI could induce gut bacterial overgrowth and dysbiosis,with the abundance of Lactobacillus and Bifidobacterium increasing the most,which could be partially reversed by pancreatic enzyme replacement therapy.Clinical studies have also confirmed the association between PEI and the dysbiosis of gut microbiota.Pancreatic exocrine secretions and changes in duodenal p H as well as bile salt malabsorption brought about by PEI may affect and shape the abundance and composition of gut microbiota.In turn,the gut microbiota may impact the pancreatic exocrine acinus through potential bidirectional crosstalk.Going forward,more and higher-quality studies are needed that focus on the mechanism underlying the impact of PEI on the gut microbiota.

A unified stochastic damage model for concrete based on multi-scale energy dissipation analysis

This work proposes a unified damage model for concrete within the framework of stochastic damage mechanics. Based on the micro-meso stochastic fracture model(MMSF), the nonlinear energy dissipation process of the microspring from nanoscale to microscale is investigated. In nanoscale, the rate process theory is adopted to describe the crack growth rate;therefore, the corresponding energy dissipation caused by a representative crack propagation can be obtained. The scale gap from nanoscale to microscale is bridged by a crack hierarchy model. Thus, the total energy dissipated by all cracks from the nanoscale to the microscale is gained. It is found that the fracture strain of the microspring can be derived from the above multi-scale energy dissipation analysis. When energy dissipation is regarded as some microdamage to the microspring, the constitutive law of the microspring is no longer linearly elastic, as previously assumed. By changing the expression of the damage evolution law from fracture strain to energy dissipation threshold, the new damage evolution model is derived. The proposed model can not only replicate the original static model but also extend to cases of rate dependence. By deriving the fracture strain under different strain rates, the rate sensitivity of concrete materials can be reflected. The model parameters can be conveniently obtained by identifying them with experimental data. Finally, several numerical examples are presented to verify the proposed model.

All laser direct writing process for temperature sensor based on graphene and silver

A highly sensitive temperature sensing array is prepared by all laser direct writing(LDW)method,using laser induced silver(liS)as electrodes and laser induced graphene(liG)as temperature sensing layer.A finite element analysis(FEA)photothermal model incorporating a phase transition mechanism is developed to investigate the relationship between laser parameters and liG properties,providing guidance for laser processing parameters selection with laser power of 1–5 W and laser scanning speed(greater than 50 mm/s).The deviation of simulation and experimental data for widths and thickness of liG are less than 5%and 9%,respectively.The electrical properties and temperature responsiveness of liG are also studied.By changing the laser process parameters,the thickness of the liG ablation grooves can be in the range of 30–120μm and the resistivity of liG can be regulated within the range of 0.031–67.2Ω・m.The percentage temperature coefficient of resistance(TCR)is calculated as−0.58%/°C.Furthermore,the FEA photothermal model is studied through experiments and simulations data regarding liS,and the average deviation between experiment and simulation is less than 5%.The liS sensing samples have a thickness of about 14μm,an electrical resistivity of 0.0001–100Ω・m is insensitive to temperature and pressure stimuli.Moreover,for a liS-liG based temperature sensing array,a correction factor is introduced to compensate for the liG temperature sensing being disturbed by pressure stimuli,the temperature measurement difference is decreased from 11.2 to 2.6°C,indicating good accuracy for temperature measurement.

Small cell lung cancer(SCLC)is a highly malignant tumor with a very poor prognosis;therefore,more effective treatments are urgently needed for patients afflicted with the disease.In recent years,emerging molecular classifications based on key transcription factors of SCLC have provided more information on the tumor pathophysiology,metastasis,immune microenvironment,and acquired therapeutic resistance and reflected the intertumoral heterogeneity of the various SCLC phenotypes.Additionally,advances in genomics and single-cell sequencing analysis have further revealed the high intratumoral heterogeneity and plasticity of the disease.Herein,we review and summarize these recent lines of evidence and discuss the possible pathogenesis of SCLC.

Predicting torsional capacity of reinforced concrete members by data-driven machine learning models

Due to the complicated three-dimensional behaviors and testing limitations of reinforced concrete(RC)members in torsion,torsional mechanism exploration and torsional performance prediction have always been difficult.In the present paper,several machine learning models were applied to predict the torsional capacity of RC members.Experimental results of a total of 287 torsional specimens were collected through an overall literature review.Algorithms of extreme gradient boosting machine(XGBM),random forest regression,back propagation artificial neural network and support vector machine,were trained and tested by 10-fold cross-validation method.Predictive performances of proposed machine learning models were evaluated and compared,both with each other and with the calculated results of existing design codes,i.e.,GB 50010,ACI 318-19,and Eurocode 2.The results demonstrated that better predictive performance was achieved by machine learning models,whereas GB 50010 slightly overestimated the torsional capacity,and ACI 318-19 and Eurocode 2 underestimated it,especially in the case of ACI 318-19.The XGBM model gave the most favorable predictions with R^(2)=0.999,RMSE=1.386,MAE=0.86,andλ=0.976.Moreover,strength of concrete was the most sensitive input parameters affecting the reliability of the predictive model,followed by transverse-to-longitudinal reinforcement ratio and total reinforcement ratio.

Stable graphene oxide-based lyotropic liquid crystals for interfacial lubrication

Lyotropic liquid crystals have lubricating properties due to their ordered assembly and fluidity,whose mesogens are often characterized by amphiphilic properties.Despite the attention that graphene oxide(GO)has been studied as a novel amphiphilic lyotropic mesogen this decade,and GO applied as a lubrication additive has been demonstrated in both oil and water-based systems,little research reveals the interfacial lubrication of GO liquid crystals yet.This work reports that GO aqueous dispersion can form lyotropic liquid crystals above a specific critical concentration of 5.00 mg/mL,providing a form of stable water-based lubricant,which can keep stable for several months and can reduce friction by 37.3%and wear by 25.24%.The liquid crystal phase was verified by polarizing microscope and synchrotron radiation small-angle X-ray scattering,and its rheological properties and viscoelasticity were studied by interfacial rheometer.The formation of lyotropic liquid crystals can enhance the stability of GO aqueous dispersions at high density,simultaneously ensuring friction decrease and anti-wear effect.It is attributed to the stable nematic network by the ordered GO sheets.The ordered assembly structure bears vertical shear force,therefore,reducing the wear.It is also assumed that the wide lateral size of graphene oxide promotes the nematic phase thus smoothes the graphene oxide film composed spontaneously under the coincidence of lamellar liquid crystal and 2D layered material.Through this work,the interlayer lubrication of GO was optimized,and the problem of GO dispersion sedimentation was solved by self-assembly.The range of interfacial lubrication of GO aqueous dispersion has been expanded and the synergistic effect is conducive to the environmentally friendly lubricants.

Thermodynamic and experimental study of high-temperature roasting of blast furnace gas ash for recovery of metallic zinc and iron

A high-temperature reduction roasting method was used to achieve metallic iron and zinc recovery from blast furnace gas ash(BFA).The reduction processes for Zn-containing and Fe-containing oxides were analyzed in detail by using ther-modynamic equilibrium calculation and the principle of minimum free energy.The results showed that the main reaction in the system is the reduction of ZnFe_(2)_(4)and iron oxides.Over the full temperature range,iron oxides were more easily reduced than zinc oxides.Regardless of the amount of CO contained in the system,the reduction of ZnO to Zn was difficult to proceed below the boiling point(906℃)of Zn.When the reduction temperature is below 906℃,the reduction process of zinc ferrate was ZnFe_(2)_(4)→ZnO;when the reduction temperature is above 906℃,its reduction process becomed ZnFe_(2)_(4)→ZnO→Zn(g).The metallization and dezincification rates of the BFA gradually increased with increasing reaction temperature.As the C/O ratio increased,the metallization and dezincification rates first increased and then decreased.The effect of reduction time on BFA reduction was similar to that of reaction temperature.

Corrosion behavior of Gd_(2)Zr_(2)O_(7)thermal barrier coatings under Fe-containing environmental sediment attack

Environmental sediments mainly consisting of CaO–MgO–Al_(2)O_(3)–SiO_(2)(CMAS)corrosion are a serious threat to thermal barrier coatings(TBCs),in which Fe element is usually ignored.Gd_(2)Zr_(2)O_(7)TBCs are famous for their excellent CMAS resistance.In this study,the characteristics of Fe-containing environmental sediments(CMAS-Fe)and their corrosiveness to Gd_(2)Zr_(2)O_(7)coatings were investigated.Four types of CMAS-Fe glass with different Fe contents were fabricated.Their melting points were measured to be 1322–1344℃,and the high-temperature viscosity showed a decreasing trend with increasing Fe contents.The corrosion behavior of four types of CMAS-Fe to Gd_(2)Zr_(2)O_(7)coatings at 1350℃was investigated.At the initial corrosion stage(0.1 h),anorthite was precipitated in CMAS-Fe with a high Ca:Si ratio,while Fe-garnet was formed in the melt with the highest Fe content.Prolonging the corrosion time resulted in the formation of a reaction layer,which exhibited an interpenetrating network composed of Gd-oxyapatite,ZrO_(2),and residual CMAS-Fe.Some spinel was precipitated within the reaction layer.After 1 h or even longer time,the reaction layers tended to be stable and compact,which had comparable hardness and fracture toughness to those of Gd_(2)Zr_(2)O_(7)coatings.Under the cyclic CMAS-Fe attack,the residual CMAS-Fe in the interpenetrating network provided a pathway for the redeposited CMAS-Fe infiltration,resulting in the continuous growth of the reaction layer.As a result,the Gd_(2)Zr_(2)O_(7)coatings had a large consumption in the thickness,degrading the coating performance.Therefore,the Gd_(2)Zr_(2)O_(7)coatings exhibit unsatisfactory corrosion resistance to CMAS-Fe attack.
