Immediate in-hospital outcomes after percutaneous revascularization of acute myocardial infarction complicated by cardiogenic shock

BACKGROUND Cardiogenic shock(CS)is a life-threatening complication of acute myocardial infarction with high morbidity and mortality rates.Primary percutaneous coro-nary intervention(PCI)has been shown to improve outcomes in patients with CS.AIM To investigate the immediate mortality rates in patients with CS undergoing primary PCI and identify mortality predictors.METHODS We conducted a retrospective analysis of 305 patients with CS who underwent primary PCI at the National Institute of Cardiovascular Diseases,Karachi,Pak-istan,between January 2018 and December 2022.The primary outcome was immediate mortality,defined as mortality within index hospitalization.Uni-variate and multivariate logistic regression analyses were performed to identify predictors of immediate mortality.RESULTS In a sample of 305 patients with 72.8%male patients and a mean age of 58.1±11.8 years,the immediate mortality rate was found to be 54.8%(167).Multivariable analysis identified Killip class IV at presentation[odds ratio(OR):2.0;95%co-nfidence interval(CI):1.2-3.4;P=0.008],Multivessel disease(OR:3.5;95%CI:1.8-6.9;Pality.CONCLUSION Immediate mortality rate in patients with CS undergoing primary PCI remains high despite advances in treatment strategies.Killip class IV at presentation,multivessel disease,and high thrombus burden(grade≥4)were identified as independent predictors of immediate mortality.These findings underscore the need for aggressive management and close monitoring of patients with CS undergoing primary PCI,particularly in those with these high-risk characteristics.

In this note,we investigate conformal Killing vectors(CKVs)of locally rotationally symmetric(LRS)Bianchi type V spacetimes.Subject to some integrability conditions,CKVs up to implicit functions of(t,x)are obtained.Solving these integrability conditions in some particular cases,the CKVs are completely determined,obtaining a classification of LRS Bianchi type V spacetimes.The inheriting conformal Killing vectors of LRS Bianchi type V spacetimes are also discussed.

Risk factors and outcomes of peptic ulcer bleed in a Pakistani population:A single-center observational study

BACKGROUND Peptic ulcer disease(PUD)remains a significant healthcare burden,contributing to morbidity and mortality worldwide.Despite advancements in therapies,its prevalence persists,particularly in regions with widespread nonsteroidal antiinflammatory drugs(NSAIDs)use and Helicobacter pylori infection.AIM To comprehensively analyse the risk factors and outcomes of PUD-related upper gastrointestinal(GI)bleeding in Pakistani population.METHODS This retrospective cohort study included 142 patients with peptic ulcer bleeding who underwent upper GI endoscopy from January to December 2022.Data on demographics,symptoms,length of stay,mortality,re-bleed,and Forrest classification was collected.RESULTS The mean age of patients was 53 years,and the majority was men(68.3%).Hematemesis(82.4%)and epigastric pain(75.4%)were the most common presenting symptoms.Most patients(73.2%)were discharged within five days.The mortality rates at one week and one month were 10.6%and 14.8%,respectively.Re-bleed within 24 h and seven days occurred in 14.1%and 18.3%of patients,respectively.Most ulcers were Forrest class(FC)Ⅲ(72.5%).Antiplatelet use was associated with higher mortality at 7 and 30 d,while alternative medications were linked to higher 24-hour re-bleed rates.NSAID use was associated with more FCⅢulcers.Re-bleed at 24 h and 7 d was strongly associated with one-week or one-month mortality.CONCLUSION Antiplatelet use and rebleeding increase the risk of early mortality in PUD-related upper GI bleeding,while alternative medicines are associated with early rebleeding.

本研究对巴基斯坦三所大学的学生展开调查。通过对调查数据的分析,得到巴基斯坦大学学生满意度的11个基本要素,并发现了巴基斯坦大学学生满意度的基本特征:学生在教师指导、教师教学、图书馆和同伴关系的满意度水平处在第一梯队,在管理服务、课程与要求、住宿条件、交通与安全的满意度水平处在第二梯队,在咨询服务、校园支持和教室条件的满意度水平处在第三梯队。此外,学生满意度在性别、父母月收入、兄弟姐妹数量、高中成绩四个投入变量下存在显著性差异;在学校、院系、是否住校和大学成绩这四个环境变量存在显著性差异。政策的制定者需关注学校的咨询服务,提高财政和校园支持,改善教室条件;考虑不同背景特征学生的不同需求,在实践过程中有所侧重。

在巴基斯坦现行高等教育行政管理机构及组织体系下,巴基斯坦高等学校设置制度的构建缘起、设置程序及标准具有一定的特色。研究发现,巴基斯坦高等学校设置程序围绕申请、审核、考察、赋权等四个环节进行,具备系统的设置标准,涵盖师资队伍建设、设备场地规范、经费使用、管理若干方面。巴基斯坦高等学校设置制度较为完备,符合本国国情,利益主体均衡,标准刚柔相济,但也存在重硬件、轻科研,未能为高等教育国际化纵深发展预留对接平台,以及"一刀切"色彩过重等问题。

Handwritten character recognition systems are used in every field of life nowadays,including shopping malls,banks,educational institutes,etc.Urdu is the national language of Pakistan,and it is the fourth spoken language in the world.However,it is still challenging to recognize Urdu handwritten characters owing to their cursive nature.Our paper presents a Convolutional Neural Networks(CNN)model to recognize Urdu handwritten alphabet recognition(UHAR)offline and online characters.Our research contributes an Urdu handwritten dataset(aka UHDS)to empower future works in this field.For offline systems,optical readers are used for extracting the alphabets,while diagonal-based extraction methods are implemented in online systems.Moreover,our research tackled the issue concerning the lack of comprehensive and standard Urdu alphabet datasets to empower research activities in the area of Urdu text recognition.To this end,we collected 1000 handwritten samples for each alphabet and a total of 38000 samples from 12 to 25 age groups to train our CNN model using online and offline mediums.Subsequently,we carried out detailed experiments for character recognition,as detailed in the results.The proposed CNN model outperformed as compared to previously published approaches.

Estimation of soil carbon pools in the forests of Khyber Pakhtunkhwa Province, Pakistan

Forest soils have high carbon densities compared to other land-uses.Soil carbon sequestration is important to reduce CO 2 concentrations in the atmosphere.An eff ective climate change mitigation strategy involves limiting the emissions of greenhouse gases from soils.Khyber Pakhtunkhwa is the most forested province of Pakistan,hosting about one-third of the country’s 4.5×106 ha forest area.Soil organic carbon in the province’s forests was estimated through a fi eld-based study carried out during 2014–17 covering the whole province.Data was collected from 373 sample plots laid out in diff erent forest types using a stratifi ed cluster sampling technique.The total quantity of soil organic carbon was estimated at 59.4×106 t with an average of 52.4±5.3 t/ha.About 69%of the total soil carbon is present in temperate forests.Subtropical broad-leaved and subtropical pine forests constitute 11.4%and 8.8%of the soil carbon stock respectively.Similarly,subalpine and oak forests have respective shares of 5.1%and 5.7%in the soil carbon pool.The lowest carbon stock(0.1%)was found in dry-tropical thorn forests.The highest soil carbon density was found in subalpine forests(69.5±7.2 t/ha)followed by moist temperate forests(68.5±6.7 t/ha)and dry temperate forests(60.7±6.5 t/ha).Oak forests have carbon density of 43.4±7.1 t/ha.Subtropical pine,subtropical broad-leaved and dry tropical thorn forests have soil carbon densities of 36.3±3.7,32.8±6.2 and 31.5±3.5 t/ha,respectively.The forests of the Khyber Pakhtunkhwa province have substantial amounts of soil carbon which must be conserved for climate change mitigation and maintenance of sound forest health.
