Clinical features, treatment strategies and outcomes of craniocervical junction arteriovenous fistulas: a cohort study of 193 patients

Background Craniocervical junction(CCJ)arteriovenous fistulas(AVFs)are rare.The current treatment strategies for AVFs with different angioarchitecture need to be clarified.The present study aimed to analyse the correlation between angioarchitecture and clinical characteristics,share our experience in treating this disease and identify risk factors associated with subarachnoid haemorrhage(SAH)and poor outcomes.Methods A total of 198 consecutive patients with CCJ AVFs from our neurosurgical centre were retrospectively reviewed.The patients were grouped according to their clinical manifestations,and their baseline clinical characteristics,angioarchitecture,treatment strategies and outcomes were summarised.Results The patients’median age was 56 years(IQR 47–62 years).The majority of patients were men with 166(83.8%)patients.The most common clinical manifestation was SAH(52.0%),followed by venous hypertensive myelopathy(VHM)(45.5%).The most common CCJ AVFs type was dural AVF,with 132(63.5%)fistulas.The most frequent fistula location was C-1(68.7%)and dural branch of vertebral artery(70.2%)was the most involved arterial feeders for fistulas.The most common direction of venous drainage was descending intradural drainage(40.9%),followed by ascending intradural drainage(36.5%).Microsurgery was the most common treatment strategy applied for 151(76.3%)patients,15(7.6%)patients were treated with interventional embolisation only,and 27(13.6%)received both interventional embolisation and microsurgical treatment.The learning curve for microsurgery only was analysed by cumulative summation method,and the turning point was the 70th case,and blood loss in post-group was lower than that in pre-group(p=0.034).At the last follow-up,there were 155(78.3%)patients with favourable outcomes(modified Rankin Scale(mRS)<3).Age≥56(OR 2.038,95%CI 1.039 to 3.998,p=0.038),VHM as the clinical manifestation(OR 4.102,95%CI 2.108 to 7.982,p<0.001)and pretreatment mRS≥3(OR 3.127,95%CI 1.617 to 6.047,p<0.001)were significantly associated with poor out

A Cyclic Constitutive Model Based on Crystal Plasticity for Body-Centered Cubic Cyclic Softening Metals

Under the framework of the small deformation crystal plasticity theory,a crystal plastic cyclic constitutive model for body-centered cubic(BCC)cyclic softening polycrystalline metals is established.The constitutive model introduces the isotropic softening rule that includes two different mechanisms:namely softening under monotonic deformation and softening under cyclic deformation on each slip system.Meanwhile,a modified Armstrong-Frederick nonlinear kinematic hardening rule is adopted.The appropriate explicit scale transition rule is selected to extend the single crystal constitutive model to the polycrystalline constitutive model.Then the model is used to predict the uniaxial and multiaxial ratcheting deformation of BCC axle steel EA4T to verify the rationality of the proposed model.The simulation results indicate that the newly established crystal plasticity model can not only describe the cyclic softening characteristics of BCC axle steel EA4T well,but also reasonably describe the evolution laws of uniaxial ratcheting and nonproportional multiaxial ratcheting deformation.Moreover,the established crystal plastic cyclic constitutive model can reasonably predict the ratcheting behavior of BCC single crystal as well.

Selective synthesis and physical properties of a bismacrocycle:Cycloparaphenylene-pillar[5]arene

Macrocyclic materials have attracted much attention due to their particular chemical and physical properties.Herein we report the precise synthesis and characterization of a new bismacrocycle structure base on cycloparaphenylene(CPP)and pillar[5]arene,named cycloparaphenylene-pillar[5]arenes(CPPn[5]).The bismacrocycle was fully characterized by NMR and HR-MS.The photophysical properties of CPPn[5]were investigated by UV–vis,and the maximum absorption peak was located at 331 nm,which was consistent with density functional theory(DFT)calculations.The fluorescence spectrum was further studied and the emission peak was maximized at 458 nm.The computational results indicate the strain energy of CPPn[5]is 27.80 kcal/mol and the HOMO-LUMO gap is 3.39 eV Notably,CPPn[5]showed interesting supramolecular properties.

To the Editor:The morbidity of thoracic lesions is very high during the treatment of malignant hematological diseases,and tumor recurrence or invasion is common,in addition to infections.Percutaneous lung biopsy or bronchoscopy could result in severe hemoptysis,pneumothorax,or infection because of low platelets and hypo-immunity,and treatment is often ineffective due to the ambiguous diagnosis.Even when a patient is diagnosed with a fungal disease,it is difficult to eliminate the lesion with only antifungal medicine.

Esophageal disease is a common disorder of the digestive system that can severely affect the quality of life andprognosis of patients. Esophageal stenting is an effective treatment that has been widely used in clinical practice.However, esophageal stents of different types and parameters have varying adaptability and effectiveness forpatients, and they need to be individually selected according to the patient’s specific situation. The purposeof this study was to provide a reference for clinical doctors to choose suitable esophageal stents. We used 3Dprinting technology to fabricate esophageal stents with different ratios of thermoplastic polyurethane (TPU)/(Poly-ε-caprolactone) PCL polymer, and established an artificial neural network model that could predict the radial forceof esophageal stents based on the content of TPU, PCL and print parameter. We selected three optimal ratios formechanical performance tests and evaluated the biomechanical effects of different ratios of stents on esophagealimplantation, swallowing, and stent migration processes through finite element numerical simulation and in vitrosimulation tests. The results showed that different ratios of polymer stents had different mechanical properties,affecting the effectiveness of stent expansion treatment and the possibility of postoperative complications of stentimplantation.

Electronic origin of structural degradation in Li-rich transition metal oxides:The case of Li_(2)MnO_(3)and Li_(2)RuO_(3)

Li_(2)MnO_(3)and Li_(2)RuO_(3)represent two prototype Li-rich transition metal(TM)oxides as high-capacity cathodes for Li-ion batteries,which have similar crystal structures but show quite different cycling performances.Here,based on the first-princi-ples calculations,we systematically studied the electronic structures and defect properties of these two Li-rich cathodes,in order to get more understanding on the structural degradation mechanism in Li-rich TM oxides.Our calculations indicated that the structural and cycling stability of Li_(2)MnO_(3)and Li_(2)RuO_(3)depend closely on their electronic structures,especially the energy of their highest occupied electronic states(HOS),as it largely determines the defect properties of these cathodes.For Li_(2)MnO_(3)with low-energy HOS,we found that,due to the defect charge transfer mechanism,various defects can form spontaneously in its host structure as Li ions are extracted upon delithiation,which seriously deteriorates its structural and cycling stability.While for Li_(2)RuO_(3),on the other hand,we identified that the high-energy HOS prevents it from the defect formation upon delithi-ation and thus preserve its cycling reversibility.Our studies thus illustrated an electronic origin of the structural degradation in Li-rich TM oxides and implied that it is possible to improve their cycling performances by carefully adjusting their TM compo-nents.

Thermal and mechanical behavior of casting copper alloy wheel during wheel and belt continuous casting process

To investigate the thermal and mechanical behavior of casting wheel,a two-dimensional thermoelastic-plastic finite element model was used to predict the temperature,stress and distortion distribution of the casting wheel during the wheel and belt continuous casting process.The effects of grinding thickness and casting speed on the thermal and mechanical behaviors of the center of the hot face of the casting wheel were discussed in detail.In each rotation,the casting wheel passes through four different spray zones.The results show that the temperature distribution of the casting wheel in different spray zones is similar,the temperature of the hot face is the highest and the temperature reaches the peak in the spray zoneⅢ.The stress and distortion depend on the temperature distribution,and the maximum stress and distortion of the hot face are 358.2 MPa and 1.82 mm,respectively.The temperature at the center of the hot face decreases with increasing grinding thickness and increases with increasing casting speed.
