COVID-19: Feelings Experienced by Nurses, Physiotherapists, and Medical Doctors in a Brazilian Field Hospital

Facing critical situations such as the ones triggered by COVID-19 may take healthcare professionals to confront their psychological resources, which may induce a higher stress level. We aimed to understand the feelings experienced by nurses, physiotherapists, and medical doctors in a field hospital dedicated to patients with COVID-19 in the biggest city of Brazil. Methods: We performed a qualitative study in a state field hospital in S?o Paulo city, Brazil, in a sample of 25 volunteer healthcare professionals. Participants answered to a sociodemographic electronic survey and were subjected to an individual semi-structured on-site interview. Results: Participants were mostly female (68%), comprised eight nurses, eight physiotherapists, and nine medical doctors, had an average age of 36.8 years, and were professionally experienced. Speech analysis revealed two thematic categories (related to work and assistance in the pandemic) and 11 subcategories: adaptation to the scenario, stressing experience, psychological support, difficulties with workwear, safety with workwear, non-verbal communication, empathy, patient comfort, outstanding experiences, major challenges, and teamwork. Conclusion: Nurses, physiotherapists, and medical doctors’ experiences while working in the field hospital were permeated by both positive and negative feelings. Those feelings originated directly from the fear of the unknown when dealing with a new disease without well-established scientific evidence, the use of medical workwear for long periods, and communication and environmental difficulties. In contrast, positive feelings, related to teamwork and empathy, arose and overlapped with the difficulties.

Antifungal Susceptibility Profile <i>In Vitro</i>Fungal Air in a Hospital Environment

Purpose: The aim of this study was to isolate and identify potentially pathogenic airborne fungi from Hospital das Clínicas Samuel Libanio in the city of Pouso Alegre-MG and evaluate their susceptibility to natural and industrial products. Methods: The air collection was performed by passive sedimentation in the morning during the autumn and winter seasons. Petri dishes were open as the location of air conditioning. The isolates were subjected to pathogenicity test. The identification of the fungi was performed according to the macroscopic evaluation and micromorphology. Potentially pathogenic isolates were susceptibility tested by disc diffusion method. The agents used were insecticides and industrial cleaning products and essential oils of citronella plants, lemon grass, eucalyptus and Melaleuca extracted by steam distillation method. Results: We obtained a total of 356 fungal isolates. The inside door environments were 126 (35.39%) and the outside environments were 230 (64.6%) isolates. The 22 (6.18%) isolates from the inside and 25 (6.18%) outside the hospital showed pathogenic potential. This isolates were identified as Acremonium spp., A. niger, A. terreus, A. versicolor, Curvularia sp., Penicillium sp. and Scopulariopsis sp. Susceptibility testing it was observed that most of the isolates were susceptible to the principle product containing sodium hypochlorite. Citronella oil showed enormous potential inhibition against all isolates. Already lemon grass oil was effective only against isolates of Penicillium spp. Conclusions: All genres identified are significant allergens, which can cause respiratory disease in both immunocompromised individuals such as asthmatics and people with any immune deficit. The monitoring of environmental sources should be performed, especially in special areas with immunocompromised patients. despite efforts to try to reduce fungal infections hospital there are still flaws in the strategies employed.

Tuberculosis is one of the 10 leading causes of death in the world. Its symptoms include fever, malaise, weakness, weight loss, chest pain, cough, expectoration, shortness of breath and sepsis, which is its most frequent complication. Due to these symptoms, many patients with tuberculosis require admission to the Intensive Care Unit, where they usually are placed on mechanical ventilation. Tuberculosis is more prevalent in the population in situations of social and economic vulnerability. The main factors that interfere with adherence to treatment and the prognosis of these patients are the patient’s support network and their socioeconomic status. We present the following report of a long-time smoker patient, with chronic kidney disease and previous treatment for tuberculosis, who was brought to the emergency room due to lowering of the sensorium, and was immediately placed on mechanical ventilation. The tests suggested sepsis with a respiratory focus, so a search for Alcohol-Acid-Resistant Bacillus was carried out, with a positive result. The patient was transferred to the Intensive Care Unit due to tachycardia and acute respiratory failure. The patient was a smoker with a high tobacco load, chronic kidney disease, in addition to precarious economic, educational and self-care conditions. This report shows that patients with tuberculosis and its complications can be stabilized through known pharmacological treatment. However, the most effective measure to interrupt the transmission of the disease remains the early diagnosis and the provision of adequate treatment, with guidelines aimed at the cessation of harmful habits, such as smoking.

Sepsis after Administering an Intramuscular Iron Injection in the Arm of an Elder Patient with Multiple Comorbidities: A Case Report

Background: Skin reactions and musculoskeletal injuries caused by intramuscular injection are an increased risk for skin and soft tissue infections and may culminate in sepsis. Objective: To describe the history of an elderly patient who developed a lesion in her right arm after home administration of intramuscular iron injection in this arm and its outcome. Methods: This is a case report of a patient who participated in a study in an intensive care unit. Results: This study describes a 65-year-old patient with multiple comorbidities who was admitted to the hospital complaining of pain for two months and difficulty in moving her right shoulder associated with skin and soft tissue infections in her right arm after intramuscular iron administration by a relative for the treatment of multifactorial anemia and refractory melena. The patient worsened her general condition and was transferred to the intensive care unit. despite the therapy instituted, the patient developed sepsis of cutaneous origin, multiple organ dysfunction, and death. Conclusions: The administration of intramuscular medication requires the participation of qualified professionals, such as nurses, mainly within the scope of the Brazilian public health system which commonly comprises patients in a context of vulnerability.
