Given their numerous functional and architectural benefits,such as improved bearing capacity and increased resistance to elastic instability modes,cold-formed steel(CFS)built-up sections have become increasingly developed and used in recent years,particularly in the construction *** paper presents an analytical and numerical study of assembled CFS two single channel-shaped columns with different slenderness and configurations(backto-back,face-to-face,and box).These columns were joined by double-row rivets for the back-to-back and box configurations,whereas they were welded together for the face-to-face *** built-up columns were filled with ordinary concrete of good *** element models were applied,using ABAQUS software,to assess mechanical performance and study the influence of assembly techniques on the behavior of cold-formed columns under axial *** approaches based on Eurocode 3 and Eurocode 4 recommendations for un-filled and concrete-filled columns respectively were followed for the numerical analysis,and concrete confinement effects were also considered per American Concrete Institute(ACI)standards for face-to-face and box *** obtained results indicated a good correlation between the numerical results and the proposed analytical methodology which did not exceed 8%.The failure modes showed that the columns failed due to instabilities such as local and global buckling.

BACKGROUND In severe cases of coronary artery disease,percutaneous coronary intervention provide promising *** stent used could be a drug-eluting stent(DES)or a titanium-nitride-oxide coated stent(TiNOS).AIM To compare the 5-year effectiveness and safety of the two stent *** The following systematic review and meta-analysis was conducted in accordance with the preferred reporting items for systematic reviews and meta-analysis guidelines,and PubMed/MEDLINE,Scopus,and Cochrane Central were searched from inception till August *** outcomes were major adverse cardiac events(MACE),cardiac death,myocardial infarction(MI),cardiac death or MI,and ischemia-driven total lesion revascularization(ID-TLR).RESULTS Four randomized controlled trials(RCT),which analyzed a sum total of 3045 patients with acute coronary syndrome(ACS)after a median follow-up time of 5 years were *** statistically insignificant,an increase in the ID-TLR was observed in patients receiving TiNOSs vs *** addition,MI,cardiac death and MI,and definite stent thrombosis(DST)were significantly decreased in the TiNOS *** analysis revealed no significant results with meta-regression presenting non-ST elevated MI(NSTEMI)as a statistically significant covariate in the outcome of *** TiNOS was found to be superior to DES in terms of MI,cardiac death or MI,and DST outcomes,however,the effect of the two stent types on ID-TLR and MACE was not significant.A greater number of studies are required to establish an accurate comparison of patient outcomes in TiNOS and DES.

Optical and near-infrared photometric study of NGC 6724

BVRI CCD photometry of the poorly studied open cluster NGC 6724 has been carried out down to a limiting magnitude of V-20 mag. The stars of the cluster have been observed using the Newtonian focus （f/4.84） of the 74-inch telescope at Kottamia Astronomical Observatory in Egypt. Also, the 2MASS - JHK system is used to confirm the results we obtained. The main photometric parameters have been estimated for the present object; the diameter is found to be 6 arcmin, the distance is 15304-60pc from the Sun and the age is 900＋50Myr. The optical reddening E（B - V） = 0.65 mag, while the infrared reddening is E（J - H） = 0.20 mag. The slope of the mass function distribution and the relaxation time estimations indicate that cluster NGC 6724 is dynamically relaxed.

Unified Approach to Assess Engineering Performance of Fill Improved by Shallow to Deep Compaction Based Techniques Using Relative Density

The setting of pre assessment criteria for soil compaction is hardly determined, especially, in case of undecided structure locations. Different design guidelines recommend achieving a specific value of relative density for the compaction of fill placement works. Alternatives were discussed through the literature to predict the value of relative density based on soil field tests (e.g. cone and standard penetration tests). This paper presents the weakness of using the Over Consolidation Ratio (OCR) as guidance to assess the value of cone tip resistance using the soil relative density. The variation of OCR (from 1 to 10) has a significant effect on the qc value up to 110% when compared to the normally consolidated state. Then normally consolidated state can logically cover the compaction process with variation of 20%, 33%, and 4% for relative density values 85%, 70%, and 60%, respectively. A unified approach is recommended to predict the compaction qc-performance line using normally consolidated condition and sand relative density.

In our case, we present a case of a 27-year-old male who presented with progressively worsening altered mental status and seizures. Over the course of his admission to the hospital and intensive care unit, laboratory testing failed to find an offending agent to his presentation. Testing did result in the diagnosis of encephalitis, but an underlying cause was not found. After careful exclusion of bacterial, viral, and other types of encephalopathy, autoimmune encephalopathy was diagnosed despite the absence of commonly used markers of autoimmune encephalopathy. The presentation and symptoms of our patient led to a wide range of differentials, and a high index of suspicion was needed throughout his admission in order to obtain the appropriate tests. Although appropriate testing might be ordered, due to the sensitivities and specificities of all laboratory tests, these objective tests do produce false negative results at times. It is in these times that one must weigh the physical exam, clinical judgment, and the process of elimination to diagnose an underlying pathology. Autoimmune Encephalitis diagnosis can be broken down into possible, probable, and definitive diagnoses based on antibody testing results. In this case, we present a patient with probable autoimmune encephalitis that failed to yield positive autoimmune markers after extensive testing of other possible causes of encephalitis.

Pathological Role of Fungal Biofilms in Fungal Rhinosinusitis: A Case-Control Study

Objective: This case-control study aimed to detect the presence of fungal biofilms in cases of fungal sinusitis, trying to find its role in recurrence of fungal sinus infection and resistance to medical treatment. Patients and Methods: This study was conducted at Ain-Shams University from June-2017 to June-2018 upon 20 patients as the fungal cases are not much in Egypt. All patients were subjected to functional endoscopic sinus surgery and taking specimens. Examination of specimens by scanning electron microscope was made. Results: The prevalence of fungal biofilms was 7 (70%) in cases and 0 (0%) in controls (p-value = 0.001). There is higher risk of fungal biofilms in cases of fungal sinusitis, the odds of biofilms is higher in the study group 2.33 than in the control group 0. The lower is the invasiveness, the higher is the fungal biofilms (OR = 0.94, 95% CI 0.46 - 4.05). Also, the analysis showed that the lower is the recurrence the lower is the fungal biofilms (OR = 2.25, 95% CI 0.15 - 7.93). Conclusion: This study showed the presence of fungal biofilms in different cases of fungal sinusitis whether primary or recurrent, also whether invasive or non-invasive. Studies with large sample size are recommended to get a strong evidence.

Objective To explore if the addition of adjuvant chemotherapy with paclitaxel and carboplatin to radiotherapy confers an advantage for overall survival （OAS）, and progression free survival （PFS）; to assess the incidence of relapses over standard pelvic radiotherapy; and to evaluate the related toxicity in high-risk stage I-II endometrial carcinoma Methods Medical records were reviewed to identify high-risk stage I-1I endometrial carcinoma cases treated in the Clinical Oncology and Nuclear Medicine department between 2002 and 2008 with adjuvant radiotherapy alone （arm Ⅰ）（57 patients） or with sequential carboplatin （AUCS-6） and paclitaxel （135-175 mg/m^2） with radiotherapy （arm Ⅱ）（51 patients）. Radiotherapy was performed through the four-field box technique at doses of 45-50 Gy （1.8 Gy/day × 5 days/week）. Results The toxicity was manageable and predominantly hematologic with a grade 3 neutropenia and thrombocytopenia in 9.8% and 6% of the patients in arm Ⅰ and arm Ⅱ, respectively, without febrile neutropenia. All patients experienced hair loss. Chernoradiotherapy arm was associated with a lower incidence rate of relapse （9.8% vs. 22.7%）. After a median follow-up period of 48 months, the 5-year OAS and PFS rates for chemoradiotherapy-treated patients were significantly more favorable than those who did not receive chemotherapy （P=0.02 and 0.03, respectively）. In arm I, the OAS and PFS rates were 73.7% and 66.7% compared with those in arm II, whose rates were 90.2% and 84.3%. Conclusions Adjuvant chemoradiation with paclitaxel and carboplatin improved the survival rates and decreased the recurrence rates in patients with high-risk stage Ⅰ-Ⅱ endometrial carcinoma. Chemotherapy was associated with an acceptable rate of toxicity. However, a prospective study with a larger number of patients is needed to define a standard adjuvant treatment for high-risk stage Ⅰ-Ⅱ endometrial carcinoma.

BACKGROUND Little is known about inflammatory bowel disease(IBD)burden and its impact on bone mineral density(BMD)among adult patients in Saudi *** the best of our knowledge,our study is the only study to give an update about this health problem in adult Saudi patients with *** is a great risk factor for reduced BMD due to its associated chronic inflammation,malabsorption,weight loss and medication side ***,screening for reduced BMD among patients with IBD is of utmost importance to curb and control anticipated morbidity and mortality among those *** To assess the relationship between IBD and BMD in a sample of adult Saudi patients with *** Ninety adult patients with IBD-62 Crohn’s disease(CD)and 28 ulcerative colitis(UC)-were recruited from King Fahad Specialist Hospital gastroenterology clinics in Buraidah,*** enrolled patients were interviewed for their demographic information and for IBD-and BMD-related clinical *** patients had the necessary laboratory markers and dual-energy x-ray absorptiometry scans to evaluate their BMD *** were divided into two groups(CD and UC)to explore their clinical characteristics and possible risk factors for reduced *** The CD group was significantly more prone to osteopenia and osteoporosis compared to the UC group;44%of the CD patients had normal BMD,19%had osteopenia,and 37%had osteoporosis,while 78%of the UC patients had normal BMD,7%had osteopenia,and 25%had osteoporosis(P value<0.05).In the CD group,the lowest t-score showed a statistically significant correlation with body mass index(BMI)(r=0.45,P<0.001),lumbar z-score(r=0.77,P<0.05)and femur z-score(r=0.85,P<0.05).In the UC group,the lowest t-score showed only statistically significant correlation with the lumbar z-score(r=0.82,P<0.05)and femur z-score(r=0.80,P<0.05).The ROC-curve showed that low BMI could predict the lowest t-score in the CD group with the best cut-off value at≤23.43(m/kg2);area
