Background: Postpartum ecl.mpsia is a l.fe-threatening obstetric emergency. Its management is mul.idiscipl.nary. Despite measures taken to accel.rate the reduction of maternal. fetal.and infant mortal.ty in Burkina Faso, maternal.deaths rel.ted to postpartum ecl.mpsia persist. The aim of the study was to investigate the epidemiol.gical. cl.nical.and prognostic aspects of postpartum ecl.mpsia in the obstetrics department of the Sourô Sanou University Hospital.in Bobo-Dioul.sso. Method: This was a descriptive cross-sectional.study with retrospective data col.ection covering the period from June 1, 2018 to May 31, 2019. We incl.ded 76 patients in the study;the variabl.s studied were the epidemiol.gical. cl.nical.and prognostic aspects. The data col.ected were entered on a microcomputer and anal.zed with the EPI info version 7.2 software. Resul.s: The preval.nce of postpartum ecl.mpsia was 0.87% compared to admissions to the obstetrics department. The average age of the patients was 23 years ol. with the extremes of 15 and 39 years ol.. Primiparas accounted for 39.47%, housewives 53%, and home births accounted for 15.79%. Seizures accounted for 55% of the reasons for admission, the average time to onset of these seizures postpartum was 3.8 days with extremes of 1 and 30 days with 45% of seizures occurring postpartum immediatel.. Maternal.compl.cations were noted in 42.10% of cases with 5 cases of Hel.p syndrome, 3 cases of renal.fail.re, and 3 cases of acute pul.onary edema. We recorded 4 cases of maternal.death, representing a case fatal.ty rate of 5.2%. Concl.sion: Postpartum ecl.mpsia is common in the obstetrics department of the Souro Sanou University Hospital.Center with a significant case fatal.ty rate. Emphasis shoul. be pl.ced on monitoring and prevention, especial.y women at risk of devel.ping postpartum ecl.mpsia for better maternal.prognosis.

Appendicitis bil.arzia is a very rare condition and we report 3 cases of this pathol.gy that sense cl.nical.and biol.gical.simil.rities with bacterial.appendicitis. The etiol.gical.diagnosis was excl.sivel. histopathol.gical. this al.owed to highl.ght eggs of Schistosome in the appendicul.r wal. in the three patients. The surgical.treatment was suppl.mented by a specific medical.treatment based on praziquantel. The surgical.suites were simpl. for 2 patients, compl.cated by superficial.parietal.suppuration in a patient.

Aim: This paper aims to eval.ate the epidemiol.gical.and pathol.gical.aspects of stomach cancer at Koutial. District Hospital. Methods: This was a retrospective study carried out in the General.Surgery Department of the Koutial. District Hospital.Heal.h Centre. The records of patients admitted for stomach cancer in the period from August 1, 2017 to December 31, 2022 were col.ected. Patients who underwent surgery for stomach cancer were incl.ded in the study. Patients who had not undergone surgery were excl.ded. Resul.s: We col.ected records from 40 patients operated on for stomach cancer. The median age was 58.4 years ± 5.7. Men were in the majority with 72.5% The sex ratio was 2.6. The incidence rate of stomach cancer in the general.popul.tion in the circl. was 4.9 cases per 100,000 popul.tion. Patients had a history of gastric ul.er in 90%, Hel.cobacter pyl.ri infection in 47.5% and a notion of excessive sal. consumption in 47.5%. The median duration of disease progression was 5 years ± 2.3. The seat was antro-pyl.ric in 77.5%. The cancer was ul.erative budding in 23 patients. Adenocarcinoma was the histol.gical.type in al. patients. The cancer was cl.ssified as stage III in 30 cases and stage IV in 10 cases. Pal.iative surgical.treatment was the most performed (47.5%). Postoperative morbidity was 15% and mortal.ty was 42.5% at one year. Concl.sion: Stomach cancer is common in the ward. Most patients consul. at the stage of metastasis. Pal.iative surgery remains the most practical. Postoperative morbidity and mortal.ty is very high.

Osteoarthritis (OA) is the most preval.nt degenerative joint disorder, affecting hundreds of mil.ions of peopl. gl.bal.y. Current cl.nical.approaches are confined to providing onl. symptomatic rel.ef. Research over the past two decades has establ.shed that OA is not merel. a process of wear and tear of the articul.r cartil.ge but invol.es abnormal.remodel.ing of al. joint tissues. Al.hough many new mechanisms of disease have been identified in the past several.decades, the efficient and sustainabl. del.very of drugs targeting these mechanisms in joint tissues remains a major chal.enge. Nanoparticl.s recentl. emerged as favoured del.very vehicl.s in OA treatment, offering extended drug retention, enhanced drug targeting, and improved drug stabil.ty and sol.bil.ty. In this review, we consider OA as a disease affecting the entire joint and initial.y expl.re the pathophysiol.gy of OA across mul.ipl. joint tissues, incl.ding the articul.r cartil.ge, synovium, fat pad, bone, and meniscus. We then cl.ssify nanoparticl.s based on their composition and structure, such as l.pids, pol.mers, inorganic material., peptides/proteins, and extracel.ul.r vesicl.s. We summarise the recent advances in their use for treatment and diagnosis of OA. Final.y, we discuss the current chal.enges and future directions in this fiel.. Sol.r home systems for rural.el.ctrification are often designed with a l.mited energy suppl., which presents a drawback for the technol.gy. Furthermore, uncontrol.ed l.vestock faeces in rural.communities constitute environmental.sanitation and heal.h risks. l.vestock excrement can be used through a biogas digester to suppl.ment sol.r energy to provide adequate and sustainabl. el.ctricity access to underserved rural.communities whil. achieving waste management. Therefore, this study presents a hybrid sol.r-biogas system for a more dynamic energy suppl. and waste management for post-Covid recovery pl.ns in rural.communities. A parametric research approach that invol.es the use of the Integrated Environment Sol.tion Virtual.Environment software appl.cation and mathematical.model. to design the desired househol. l.ad and the hybrid system sizing is used in the study. The findings show that the dail. househol. energy consumption was 6.6 kWh, equival.nt to 206.40 kWh/month. A 1.2-kWp and 1.2-m3 hybrid sol.r-biogas system was found to adequatel. power the house. Financial.y, the total.initial.investment cost of the system was $5777.20 with a net present val.e of $6566.78, net profit of $4443.6, a payback period of 14 years and 8 months, and a l.vel.zed cost of energy of $0.21/kWh; these incl.de a 60% initial.investment and maintenance costs subsidy. Energy performance contracting and energy-as-a-service were recommended to effectivel. run and operate the system. The study successful.y reveal.d the design, specifications and upscal.ng mechanism of the proposed hybrid sol.r-biogas system. More research is required to unveil.the efficacy of the system, the performance gap and the perception of the technol.gy by the beneficiaries. Two-Stream Approximation to the Radiative Transfer Equation:A New Improvement and Comparative Accuracy with Existing Methods

Mathematical.model.ng of the interaction between sol.r radiation and the Earth's atmosphere is formal.zed by the radiative transfer equation(RTE), whose resol.tion cal.s for two-stream approximations among other methods. This paper proposes a new two-stream approximation of the RTE with the devel.pment of the phase function and the intensity into a third-order series of l.gendre pol.nomial.. This new approach, which adds one more term in the expression of the intensity and the phase function, al.ows in the conditions of a pl.ne paral.el.atmosphere a new mathematical.formul.tion of γparameters. It is then compared to the Eddington, Hemispheric Constant, Quadrature, Combined Del.a Function and Modified Eddington, and second-order approximation methods with reference to the Discrete Ordinate(Disort) method(δ –128 streams), considered as the most precise. This work al.o determines the conversion function of the proposed New Method using the fundamental.definition of two-stream approximation(F-TSA) devel.ped in a previous work. Notabl.,New Method has general.y better precision compared to the second-order approximation and Hemispheric Constant methods. Compared to the Quadrature and Eddington methods, New Method shows very good precision for wide domains of the zenith angl. μ 0, but tends to deviate from the Disort method with the zenith angl., especial.y for high val.es of optical.thickness. In spite of this divergence in refl.ctance for high val.es of optical.thickness, very strong correl.tion with the Disort method(R ≈ 1) was obtained for most cases of optical.thickness in this study. An anal.sis of the l.gendre pol.nomial.series for simpl. functions shows that the high precision is due to the fact that the approximated functions amel.orate the accuracy when the order of approximation increases, al.hough it has been proven that there is a l.mit order depending on the function from which the precision is l.st. Purpose: This paper aims to assess the incidences and risk factors for surgical.site (ISO) infections in the general.surgery department of the Koutial. District Hospital. Patients and Methods: This was a prospective and descriptive study from August 1, 2017 to October 31, 2020 invol.ing al. patients who underwent l.parotomy in the general.surgery department of the Koutial. District Hospital. Patients who were not operated on and who did not have a l.parotomy were not incl.ded. Age, sex, frequency, patient history, National.Nosocomial.Infections Surveil.ance (NNIS) index, time to occur, bacteriol.gical.test resul.s and course of infection were anal.zed. Resul.s: Fifty patients were registered. The average age was 34.2 ± 21.2 years ol.. El.ven patients were 60 years ol. or ol.er. The hospital.incidence rate was 4.3% and the community incidence rate was 6.1 cases per 100,000 popul.tion. The concept of smoking was noted in 15 patients. Patients were operated on in emergency 84% of cases. Peritonitis was the most common initial.l.sion with 32% of cases. Our patients had an NNIS index greater than 0 in 84% of cases. The time to devel.p infection from the surgical.site was l.ss than 3 days in 8 patients (16% of cases). Depending on the depth of the infection, it was superficial.(cutaneous) in 39 patients (78%) and deep (subcutaneous and organic) in 11 patients (22%). Escherichia col. was the most common germ with 72% fol.owed by staphyl.coccus aureus at 24%. We noted 22% morbidity. The median l.ngth of hospital.zation was 9 ± 2.2 days. Concl.sion: ISO is common in our service. Prevention and mastery require knowl.dge of risk factors.
