Clinical analysis of patients with deep sternal wound infection-induced sepsis:a retrospective cohort study

Background:This study aimed to summarize the clinical characteristics of patients with deep sternal wound infection-induced sepsis after median sternotomy and improve the treatment outcomes of infection-related sepsis.Methods:A retrospective cohort study was conducted on 21 patients with deep sternal wound infection-induced sepsis after median sternotomy who were admitted to the Department of Critical Care.The clinical manifestations,laboratory test results,infection control,and organ and nutritional support of the patients were summarized,and the follow-up data were obtained.Results:The primary symptoms of deep sternal wound infection-induced sepsis included dyspnea,high fever,chills,and altered state of consciousness.Laboratory test results revealed increased inflammatory markers and decreased oxygenation index.Renal and liver function injury were observed in 8 and 4 patients,respectively;18 and 12 patients demonstrated elevated D-dimer and N-terminal Pro B type natriuretic peptide levels,respectively.Of the 8 patients whose wound secretions tested positive for bacteria,Acinetobacter baumannii and Staphylococcus aureus infections were present in 6 and 2 patients,respectively.One of the 6 patients whose blood cultures tested positive for bacteria demonstrated Candida albicans infection.Fifteen patients received ventilator-assisted ventilation and 2 patients received renal replacement therapy.Of all the 21 patients,17 were cured,2 died,and 2 were discharged.Conclusion:Postmedian sternotomy sepsis attributed to a deep sternal wound infection usually results from a preexisting condition.The most prominent clinical manifestation is dyspnea,which is sometimes accompanied by the impairment of organ function.Infection prevention,proper nutrition support,and maintenance of healthy organ function are the cornerstones for successful treatment outcomes.

Multiplicity of Solutions for a Class of Noncooperative Elliptic Systems

In this paper, we consider the following noncooperative elliptic systems where Ω is a bounded domain in RN with smooth boundary ∂Ω, λ,δ,γ are real parameters, and . We assume that F is subquadratic at zero with respect to the variables u,v. By using a variant Clark’s theorem, we obtain infinitely many nontrivial solutions (uk,vk) with as k → ∞. Compared with the existing literature, we do not need to assume the behavior of the nonlinearity ∇F at infinity.

利用马弗炉高温真空煅烧碳酸钙烧制氧化钙熟料,后经XRD分析煅烧熟料矿物组成、SEM(扫描电镜)观察和分析固体物质的表面形态和结构,并利用煅烧熟料配制静态破碎剂,给出了静态破碎剂的最优配比,研究了水灰比、环境温度以及拌和水温对静态破碎剂水化反应的影响。结果表明:随着煅烧温度的升高,碳酸钙分解成氧化钙的程度越高,氧化钙晶体收缩愈紧密,晶体微孔隙逐渐消失且变得越致密,表观密度增大,反应活性降低。静态破碎剂在测试钢管中的峰值温度随水灰比的增大先增大后减小;随环境温度以及拌和水温的升高而升高;但其温度随时间变化趋势大体一致。

目的:现有众多临床研究提示残余胆固醇(remnant cholesterol, RC)与心血管疾病风险紧密相关。本研究旨在此基础上,进一步探索RC与药物洗脱支架(drug-eluting stents, DES)再狭窄的相关性。方法:本研究回顾性纳入2016年1月至2020年12月,于北京市大兴区人民医院置入DES并于术后18~30个月复查冠状动脉造影的急性冠状动脉综合征(acute coronary syndrome,ACS)患者。通过造影结果,将纳入人群分为支架内再狭窄(in-stent restenosis,ISR)组与非ISR组。通过多元回归分析模型评估RC与DES-ISR的相关性。创建受试者工作特征曲线(receiver operating characteristic curve,ROC)评估RC预测DES-ISR的效应。结果:本研究共纳入603例ACS患者,其中69例在随访时发生了DES-ISR。与非DES-ISR组相比,ISR组RC浓度水平明显偏高[0.45(0.37,0.63)vs. 0.37(0.27,0.48)mmol/L,P<0.001]。单因素回归分析,RC与DES-ISR显著相关(OR=3.620,95%CI:1.817~7.211,P <0.001);校正其他混在因素后多因素回归分析模型,RC仍与DES-ISR独立相关(OR=3.255,95%CI:1.562~6.659,P=0.002)。ROC曲线分析显示,RC可有效预测ACS患者术后中远期发生DES-ISR的概率(曲线下面积=0.671,95%CI:0.605~0.736,P<0.0001)。结论:RC与ACS患者中远期DES-ISR风险独立相关。

Acute pulmonary embolism immediately after cesarean section despite dilatation of the left ventricle:a case report and literature review

Introduction Pulmonary embolism is a lethal complication during pregnancy and the puerperium.Compared with vaginal delivery,the risk of pulmonary disease is almost twice as high with cesarean section.However,diagnosing a pulmonary embolism can be challenging.Case presentation A 31-year-old nulliparous woman underwent cesarean section.Postoperatively,the patient suddenly developed dyspnea and dull pain in the left back region and cardiopulmonary function deteriorated rapidly.Venous ultrasonography confirmed thrombosis of the left common iliac vein.Transthoracic echocardiography revealed a normal right heart and a dilated left ventricle with a patent foramen ovale.An acute pulmonary embolism was confirmed by pulmonary angiography.Catheter-directed thrombus fragmentation and thrombolysis were then performed.Conclusion Dilated left ventricular echocardiography does not exclude postpartum acute pulmonary embolism.In extreme emergencies,when acute pulmonary embolism is the most likely diagnosis and life-threatening,catheter-directed angiography may be preferred over other approaches.

Ultrasensitive photochromism and impedance dual response to weak visible light by solvated Pb(Ⅱ) modified polyoxomolybdate

Polyoxometalates (POMs) are important inorganic photochromic materials to be potentially applied in photo-induced switch,energy storage,and even the detection of light.However,due to the limited sensitivity of POMs,it is difficult to realize the photochromic response to weak visible light.In this paper,by the coordination of solvated Pb(Ⅱ),a new structure-defined chain-like polyoxomolybdate complex of[(Pb(DMF)_(4))^(3)(P_(2)Mo_(18)O_(62))_(2)]n(Pb_(3)Mo_(18) ,DMF=dimethylformamide) has been demonstrated by a facile solvent-diffusion approach.By virtue of interactions between Pb(DMF)_(4) and polyoxoanions,Pb_(3)Mo_(18) shows an ultrasensitive photochromic response to weak visible lights and forms the reduced’heteropoly blue’species through ligand-to-metal charge transfer (LMCT) process.A new mechanism is firstly proposed here that the 6s orbital lone electron pair on Pb(II) can effectively stabilize the generated hole of oxygen atoms as a result of O→Mo charge transfer.Through the proposed mechanism,the LMCT barrier is drastically lowered and allows the coloration to be occurred even upon weak visible light.Also,because the conductivity of Pb_(3)Mo_(18) enhances with the increase of reduction extent,its electrochemical impedance signals are proportionally response to irradiation intensity.Especially,for the first time,the polyoxomolybdate composite can be used to detect weak visible light,in which the optical signal can be converted into electrical signal output.Moreover,Pb_(3)Mo_(18) can be drip-coated on the surface of the screen printed chip electrode,which is facile to the detection of light by portable devices compatible with computers,mobile phones and other electronic equipment.This work not only highlights a new approach to the molecular design of photochromic POMs by the coordination of metal ions with the effect of inert electron pair,but also lays a foundation to extend the application of POMs as light signal sensors.

Surface modification of hollow capsule by Dawson-type polyoxometalate as sulfur hosts for ultralong-life lithium-sulfur batteries

Reasonable construction of sulfur host with high conductivity,large sulfur storage gap,strong chemical adsorption,and fast oxidation–reduction kinetics of polysulfide is very significant for its practical use in lithium-sulfur batteries(LSBs).In this paper,the surface modification of MIL-88A(Fe)is carried out by Dawson-type polyoxometalate(POM),and a hollow capsule shell material with P_(2)W_(18),Fe_(3)O_(4),and C components is synthesized by the subsequent carbonization process.When applied as the sulfur host,the hollow capsule shell material can efficiently improve the conductivity of sulfur electrode and restrain the volumetric change of active sulfur while charging and discharging.On this foundation,electrochemical analysis and density functional theory(DFT)calculation show that the P_(2)W_(18)on the outer layer of the capsule shell have effective electrocatalytic activity and potent chemical bond on the lithium polysulfides(LiPSs),which is helpful to block the shuttle effect.Therefore,the as-assembled LSBs display the outstanding specific capacity and prominent cycle stability.Specifically,it delivers an excellent reversible capacity of 1063 mAh/g after 100 cycles of charge–discharge at a rate of 0.5 C,accounting for a preservation by 96%in comparison to that of the initial cycle.Moreover,even after 2000 cycles at 1 C,the reversible specific capacity of 585 mAh/g can still be maintained with an average decay rate of only 0.021%.

5种猪场常用消毒剂对PCV2和PRV的消毒效果评价

为评价含碘类、季铵盐类、过硫酸氢钾类、二氧化氯类和戊二醛类5种猪场常用消毒剂的安全性及其对病毒的消毒效果,在实验室、模拟猪场舍条件下,分别采用病毒悬液法、细胞培养病变法,根据说明书推荐的稀释比例,选择高、中、低3个工作浓度,与PCV2和PRV分别作用不同时间后接种细胞培养,观察细胞病变,通过TCID50计算病毒灭活率,荧光定量PCR检测病毒核酸含量。结果显示:含碘类和二氧化氯类消毒剂对细胞毒性较小,戊二醛类消毒剂对细胞的毒性相对较强。5类消毒剂对PCV2和PRV均有一定的杀灭效果,工作浓度越低,灭活时间越长;实验室条件下,含碘类、过硫酸氢钾类、二氧化氯类消毒剂显示出较强的消毒效果,在高、中工作浓度下10 min内即可100%灭活病毒,而猪场模拟条件下,含碘类消毒剂的实际消毒效果较差,季铵盐类消毒剂的核酸去除能力较弱。结果表明:所选用的5种常用消毒剂均可用于猪场消毒,可根据实际条件和需求选用;在保证安全的前提下,应尽量使用较高的工作浓度,且保证足够的消毒时间。本研究确定了5种常用消毒剂的病毒灭活效果与其工作浓度、消毒作用时间的关联性,为猪场消毒剂的选择与使用提供了参考。

Superior performance porous carbon nitride nanosheets for helium separation from natural gas:Insights from MD and DFT simulations

Increasing helium(He)demand in fundamental research,medical,and industrial processes necessitates efficient He purification from natural gas.However,most theoretically available membranes focus on the separation of two or three kinds of gas molecules with He and the underlying separation mechanism is not yet well understood.Using molecular dynamic(MD)and first-principle density function theory(DFT)simulations,we systematically demonstrated a novel porous carbon nitride membrane(g-C_(9)N_(7))with superior performance for He separation from natural gas.The structure of g-C_(9)N_(7) monolayer was optimized first,and the calculated cohesive energy confirmed its structural stability.Increasing temperature from 200 to 500 K,the g-C_(9)N_(7)membrane revealed high He permeability,as high as 1.48×10^(7) GPU(gas permeation unit,1 GPU=3.35×10^(-10) mol·s^(-1)·Pa^(-1)·m^(-2))at 298 K,and also exhibited high selectivity for He over other gases(Ar,N_(2),CO_(2),CH_(4),and H_(2)S).Then,the selectivity of He over Ne was found to decrease with increasing the total number of He and Ne molecules,and to increase with increasing He to Ne ratio.More interestingly,a tunable He separation performance can be achieved by introducing strain during membrane separation.Under the condition of 7.5%compressive strain,the g-C_(9)(N_(7)(membrane reached the highest He over Ne selectivity of 9.41×10^(2).It can be attributed to the low energy barrier for He,but increased energy barrier for other gases passing through the membrane,which was subject to a compressive strain.These results offer important insights into He purification using g-C_(9)N_(7)membrane and opened a promising avenue for the screening of industrial grade gas separation with strain engineering.
