Nurse anesthetists’perceptions and experiences of managing emergence delirium:A qualitative study

BACKGROUND This study employs a descriptive phenomenological approach to investigate the challenges anesthesia nurses face in managing emergence delirium(ED),a common and complex postoperative complication in the post-anesthesia care *** role of nurses in managing ED is critical,yet research on their understanding and management strategies for ED is *** To investigate anesthetic nurses’cognition and management experiences of ED in hopes of developing a standardized management *** This study employed a descriptive phenomenological approach from qualitative research *** sampling was utilized to select 12 anesthetic nurses from a tertiary hospital in Shanghai as research ***-structured interviews were conducted,and the data were organized and analyzed using Colaizzi’s seven-step analysis method,from which the final themes were *** After analyzing the interview content,four main themes and eight subthemes were distilled:Inefficient cognition hinders the identification of ED(conceptual ambiguity,empirical identification),managing diversity and challenges(patientcentered safe care,low level of medical-nursing collaboration),work responsibilities and pressure coexist(heavy work responsibilities,occupational risks and stress),demand for high-quality management(expecting the construction of predictive assessment tools and prevention strategies,and pursuing standardized management processes to enhance management effectiveness).CONCLUSION Nursing managers should prioritize the needs and suggestions of nurses in order to enhance their nursing capabilities and provide guidance for standardized management processes.

Bowel preparation experiences and needs before follow-up colonoscopy in older adult postoperative colorectal cancer patients: A qualitative study

BACKGROUND The bowel preparation process prior to colonoscopy determines the quality of the bowel preparation,which in turn affects the quality of the ***-scopy is an essential procedure for postoperative follow-up monitoring of colorec-tal cancer(CRC)*** studies have shown that advanced age and a history of colorectal resection are both risk factors for inadequate bowel ***,little attention has been paid to the bowel preparation experiences and needs of predominantly older adult postoperative CRC *** To explore the experiences and needs of older adult postoperative CRC patients during bowel preparation for follow-up *** Fifteen older adult postoperative CRC patients who underwent follow-up colonoscopy at a tertiary hospital in Shanghai were selected using purposive sampling from August 2023 to November *** phenomenological method in qualitative research was employed to construct an interview outline and conduct semi-structured interviews with the ***'s seven-step analysis was utilized to organize,code,categorize,summarize,and verify the interview *** The results of this study were summarized into four themes and eight sub-themes:(1)Inadequate knowledge about bowel preparation;(2)Decreased physiological comfort during bowel preparation(gastrointestinal discomfort and sleep deprivation caused by bowel cleansing agents,and hunger caused by dietary restrictions);(3)Psychological changes during different stages of bowel preparation(pre-preparation:Fear and resistance due to previous experiences;during preparation:Irritation and helplessness caused by taking bowel cleansing agents,and post-preparation:Anxiety and worry while waiting for the colonoscopy);and(4)Needs related to bowel preparation(detailed instructions from healthcare professionals;more ideal bowel cleansing agents;and shortened waiting times for colonoscopy).CONCLUSION Older adult postoperative CRC patients'kn

Noise-Induced Entanglement Transition in One-Dimensional Random Quantum Circuits

A random quantum circuit is a minimally structured model to study entanglement dynamics of many-body quantum *** consider a one-dimensional quantum circuit with noisy Haar-random unitary gates using density matrix operator and tensor contraction *** is shown that the entanglement evolution of the random quantum circuits is properly characterized by the logarithmic entanglement *** performing exact numerical calculations,we find that,as the physical error rate is decreased below a critical value p;≈0.056,the logarithmic entanglement negativity changes from the area law to the volume law,giving rise to an entanglement *** critical exponent of the correlation length can be determined from the finite-size scaling analysis,revealing the universal dynamic property of the noisy intermediate-scale quantum devices.

Inter-valley spiral order in the Mott insulating state of a heterostructure of trilayer graphene-boron nitride

Recent experiment has shown that the ABC-stacked trilayer graphene-boron nitride Moire super-lattice at half-filling is a Mort insulator. Based on symmetry analysis and effective band structure calculation, we propose a valley-contrasting chiral tight-binding model with local Coulomb interaction to describe this Moire super-lattice system. By matching the positions of van Hove points in the low-energy effective bands, the valley-contrasting staggered flux per triangle is determined around π/2. When the valence band is half-filled, the Fermi surfaces are found to be perfectly nested between the two valleys. Such an effect can induce an inter-valley spiral order with a gap in the charge excitations, indicating that the Mott insulating behavior observed in the trilayer graphene-boron nitride Moire super-lattice results predominantly from the inter-valley scattering,

Insertion of Supramolecular Segments into Covalently Crosslinked Polyurethane Networks towards the Fabrication of Recyclable Elastomers

Thermoset plastics have become one of the most important chemical products in the world. The consequent problem is that although the thermosets possess excellent performance in mechanical strength, they cannot be reprocessed because of the internal permanent network structures. Optimizing the molecular design of thermosets is one of the most feasible ways to improve their recyclability. Here we present a facile and robust strategy to engineer the reprocessability of thermoset polyurethanes without compromising their mechanical toughness and chemical resistance via adding supramolecular additives during the polymer synthesis process. By using a multiple hydrogen bonding moiety as the model supramolecular additive, we demonstrate that the mechanical properties, recyclability, and chemical resistance of the crosslinked polyurethanes can be precisely controlled by adjusting the contents of the supramolecular additive. Systematic studies on the relations between molecular design and material properties are performed, and the optimized polyurethane network with a moderate amount of the supramolecular additive achieves the right balance between the robustness and recyclability. This work provides a cost-effective and practical way to chemically engineer thermoset plastics, aiming to enable the recycling of mechanically tough and chemically stable polymer materials.

Arterioles in cerebral amyloid angiopathy and vascular dementia

Background Small cerebrovascular lesions are one of the most important factors in cerebral amyloid angiopathy （CAA） and vascular dementia （VaD）. We analyzed the difference of arteriolar pathology between CAA patients （CAAs） and vascular dementia patients without CAA （VaDs）. Methods Ten deceased CAAs and twelve deceased VaDs were available for this study. Five deceased patients without known cerebrovascular diseases served as controls. These patients were all autopsy cases. All transversely cut arterioles in the gray matter and white matter with an external diameter equal to or larger than 30 um and with a maximum of 300 um were examined. The internal and external diameters of arterioles were measured. Results The external diameter of gray matter arterioles in the CAAs was significantly greater than in controls. In gray matter arterioles, the diameter of the lumen in VaDs was markedly smaller than in the CAAs, whereas there was no significant difference between CAAs and controls. CAAs and VaDs may cause remarkable thickening of the arteriolar walls in either white matter or gray matter. The sclerotic index of arterioles in VaDs was significantly greater than in CAAs and controls. Conclusions Stenosis of arterioles occurred in both CAA and VaD, but the tendency was greater in VaD. Arterioles of CAA were also expanded in gray matter, which may be related to lobar hemorrhage. The loss and/or degeneration of vascular smooth muscle cells was predominant in CAA, while the over-proliferation of vascular smooth muscle cells was areater in VaD.

Effects of Solution Depletion and Segregation Oxidation on Morphology of Modified 310 Austenitic Stainless Steel

The oxidation morphologies of modified 310 steel exposed in 900 and 1 100 ℃ air were investigated. A double layer morphology consisting of a （Cr, Mn）-rich outer layer and a fine Cr-rich inner layer was formed at 900℃. It was related to the breakaway oxidation induced by the Cr-depletion and the Mn-segregation in inner layer. Some Cr-rich oxides with amorphous state were formed along grain boundaries. And some new finer oxide grains, voids and Cr-rich precipitates were observed in spallation areas at 1 100℃. Correspondingly, the oxidation kinetic curve dropped with the spallation of scale and increased with the formation of some new oxide grains. It was caused by segregation of Cr and the transformation of oxides from Cr2O3 to the volatile oxides at elevated temperature. XRD analysis showed that the precipitates were mainly composed of CrO3. Segregation and depletion for solutions were also discussed by oxidation diffusion mechanisms.

Dynamics of Microbial Activity Related to N Cycling in Cd-Contaminated Soil During Growth of Soybean

The potential influences of cadmium (Cd) on the biochemical processes of the soil nitrogen (N) cycle, along with the dynamics of ammonification, nitrification, and denitrification processes in the rhizosphere and non-rhizosphere (bulk soil), respectively, were investigated in a Cd-stressed system during an entire soybean growing season. In terms of Cd pollution at the seedling stage, the ammonifying bacteria proved to be the most sensitive microorganisms, whereas the effects of Cd on denitrification were not obvious. Following the growth of soybeans, the influences of Cd on ammonification in the bulk soil were: toxic impacts at the seedling stage, stimulatory effects during the early flowering stage, and adaptation to the pollutant during the podding and ripening stages. Although nitrification and denitrification in the bulk soil decreased throughout the entire growth cycle, positive adaptation to Cd stress was observed during the ripening stage. Moreover, during the ripening stage, denitrification in the bulk soil under high Cd treatment (20 mg kg-1) was even higher than that in the control, indicating a probable change in the ecology of the denitrifying microbes in the Cd-stressed system. Changes in the activity of microbes in the rhizosphere following plant growth were similar to those in the non-rhizosphere in Cd treatments; however, the tendency of change in the rhizosphere seemed to be more moderate. This suggested that there was some mitigation of Cd stress in the rhizosphere.

Is adjuvant chemotherapy necessary for patients with ypT0–2N0 rectal cancer treated with neoadjuvant chemoradiotherapy and curative surgery?

Background and objective:The benefit from adjuvant chemotherapy for patients treated with neoadjuvant chemoradiotherapy(NCRT)and curative surgery remains controversial,particularly among those responding well to *** retrospective study aimed to clarify the benefits of adjuvant chemotherapy in terms of the oncological outcomes of patients with ypT0–2N0 rectal cancer after NCRT and curative ***:All patients with ypT0–2N0 rectal cancer after NCRT and curative resection between 2005 and 2014 were *** oncological outcomes between patients treated with adjuvant chemotherapy and those without any chemotherapy were ***:The clinicopathological characteristics of 110 patients were reviewed in this study;one patient was excluded due to lack of *** the 109 patients included,58(53.2%)underwent adjuvant chemotherapy(chemo group),whereas the remaining 51(46.8%)did not receive any chemotherapy(non-chemo group).After a median follow-up of 50 months,there were no significant differences in the 5-year overall survival(OS)or recurrence-free survival(RFS)rates between the groups(OS:92.1 vs 86.3%,P=0.375;RFS:80.9 vs 74.7%,P=0.534).Subgroup analysis also demonstrated no significant differences in 5-year OS and RFS rates between patients with ypT0N0 rectal cancer(P=0.712 and P=0.599,respectively)and those with ypT1–2N0 disease(P=0.255 and P=0.278,respectively).Conclusions:These results indicate that patients with ypT0–2N0 rectal cancer after NCRT followed by curative surgery may not derive significant benefit from adjuvant ***,further prospective randomized trials,with larger sample sizes,are warranted to confirm this conclusion.

DFT study on mechanism of the classical Biginelli reaction

The condensation of benzaldehyde, urea, and ethyl acetoacetate according to the procedure described by Biginelli was investigated at the B3LYP/6-31G（d）, B3LYP/6-31 ＋G（d,p）, and B3LYP/6-311＋G（3df,2p）//B3LYP/6-31＋G（d,p） levels to explore the reaction mechanism. According to the mechanism proposed by Kappe, structures of five intermediates were optimized and four transition states were found. The calculation results proved that the mechanism proposed by Kappe is right.
