Traditional Chinese medicines(TCMs),usually comprising hundreds of different compounds with distinctive physiochemical properties,have been practiced for thousands of years worldwide for the treatment of various human diseases.Significant pharmacological properties have been clarified for many TCM in recent decades,while great efforts are needed to identify the active biomedical materials from TCM due to challenges such as technical barriers to screening,isolation,characterization,and optimization[1].Recent improvements in analytical tools,genome mining,and bioengineering technologies such as CRISPR-Cas systems are overcoming such challenges and offering new opportunities,promoting the integrative studies of TCM to systematically dig out the potential pharmaceutical compounds of TCMs[2-4].

To evaluate outcomes between extraperitoneal robotic single-port radical prostatectomy(epR-spRP)and extraperitoneal robotic multiport radical prostatectomy(epR-mpRP)performed with the da Vinci Si Surgical System,comparison was performed between 30 single-port(SP group)and 26 multiport(MP group)cases.Comparisons included operative time,estimated blood loss(EBL),hospital stay,peritoneal violation,pain scores,scar satisfaction,continence,and erectile function.The median operation time and EBL were not different between the two groups.In the SP group,the median operation time of the first 10 patients was obviously longer than that of the latter 20 patients(P<0.001).The median postoperative hospital stay in the SP group was shorter than that in the MP group(P<0.001).The rate of peritoneal damage in the SP group was less than that in the MP group(P=0.017).The pain score and overall need for pain medications in the SP group were lower than those in the MP group(P<0.001 and P=0.015,respectively).Patients in the SP group were more satisfied with their scars than those in the MP group 3 months postoperatively(P=0.007).At 3 months,the cancer control,recovery of erectile function,and urinary continence rates were similar between the two groups.It is safe and feasible to perform epR-spRP using the da Vinci Si surgical system.Therefore,epR-spRP can be a treatment option for localized prostate cancer.Although epR-spRP still has a learning curve,it has advantages for postoperative pain and self-assessed cosmesis.In the absence of the single-port robotic surgery platform,we can still provide minimally invasive surgery for patients.

Several studies suggest that mesenchymal stem cells(MSCs) possess antitumor properties;however,the exact mechanisms remain unclear. Recently,microvesicles(MVs) are considered as a novel avenue intercellular communication,which may be a mediator in MSCs-related antitumor effect. In the present study,we evaluated whether MVs derived from human umbilical cord Wharton’s jelly mesenchymal stem cells(h WJMSCs) may inhibit bladder tumor T24 cells growth using cell culture and the BALB/c nu/nu mice xenograft model. CCK-8 assay and Ki-67 immunostaining were performed to estimate cell proliferation in vitro and in vivo. Flow cytometry and TUNEL assay were used to assess cell cycle and apoptosis. To study the conceivable mechanism by which h WJMSC-MVs attenuate bladder tumor T24 cells,we estimated the expression of Akt/p-Akt,p-p53,p21 and cleaved Caspase 3 by Western blot technique after exposing T24 cells to h WJMSC-MVs for 24,48 and 72 h. Our data indicated that h WJMSC-MVs can inhibit T24 cells proliferative viability via cell cycle arrest and induce apoptosis in T24 cells in vitro and in vivo. This study showed that h WJMSC-MVs down-regulated phosphorylation of Akt protein kinase and up-regulated cleaved Caspase 3 during the process of anti-proliferation and pro-apoptosis in T24 cells. These results demonstrate that h WJMSC-MVs play a vital role in h WJMSC-induced antitumor effect and may be a novel tool for cancer therapy as a new mechanism of cell-to-cell communication.

Acupuncture is one of the most effective complementary therapies for allergic rhinitis(AR)and has been recommended by several clinical practice guidelines(CPGs)for AR.However,these CPGs mentioned acupuncture without making recommendations for clinical implementation and therapeutic protocols,therefore limiting the applicability of acupuncture therapies for AR.Hence,for the benefit of acupuncture practitioners around the world,the World Federation of Acupuncture-moxibustion Societies have initiated a project to develop the CPG for the use of acupuncture and moxibustion to treat AR.This CPG was developed according to the Grading of Recommendations Assessment,Development,and Evaluation(GRADE)methodology,referring to the principles of the World Health Organization Handbook for Guideline Development.During the development of the CPG,the guideline development group(GDG)played an important role.The clinical questions,recommendations and therapeutic protocols were all formulated by the GDG using the modified Delphi method.The CPG contains recommendations for 15 clinical questions about the use of acupuncture and moxibustion interventions.These include one strong recommendation for the intervention based on high-quality evidence,three conditional recommendations for either the intervention or standard care,and 11 conditional recommendations for the intervention based on very low quality of evidence.The CPG also provides one filiform needle acupuncture protocol and five moxibustion protocols extracted based on the protocols presented in randomized controlled trials reviewed by the GDG.

Urinary incontinence (UI) is a common problem worldwide.It has a major impact on physical and social activities and interpersonal relationships.UI is common in women,but is under-reported and undertreated.It affects the quality of life of female patients severely.Acupuncture and moxibustion have been proposed as potentially effective interventions for female UI.Hence,for the benefit of acupuncture practitioners around the world,the World Federation of Acupuncture-moxibustion Societies initiated a project to develop a clinical practice guideline (CPG) for the use of acupuncture and moxibustion to treat female UI.This CPG was developed according to the Grading of Recommendations Assessment,Development,and Evaluation (GRADE) methodology,referring to the principles of the World Health Organization Handbook for Guideline Development.During the development of the CPG,the guideline development group (GDG) played an important role.The clinical questions,recommendations and therapeutic protocols were all formulated by GDG using the modified Delphi method.This CPG contains ten recommendations about the use of acupuncture and moxibustion interventions for ten clinical questions,which include nine conditional recommendations for the intervention and one conditional recommendation for either the intervention or the comparison.This CPG also provides one protocol for conventional filiform needle therapy,two therapy protocols for deep needling stimulation on lumbosacral acupoints,and four moxibustion therapy protocols,based on the protocols presented in randomized controlled trials reviewed by the GDG.

《体育法》司法适用:从实证到法理的考察——面向良法善治的中国式体育法治现代化

通过对150篇法院适用《体育法》裁判文书的时间序列、案件类型、援引条款以及审理程序、结果进行实证分析发现,目前我国适用《体育法》案例总体数量较少但呈逐年增长态势;民事案件占绝大多数;法律条款援引不均衡,司法与仲裁管辖争议成焦点;绝大多数案件未进入法院实体审理程序。对此我们不仅要从现象中具体描述“现状是什么”,更要从法理高度深度挖掘现状“为什么是这样”。事实上这里既有法制定层面上立法内容和立法技术不完善的立法瑕疵问题,亦存在法实施层面上“同案不同判”和司法纠纷“闭环”解决的司法公正彰显不够问题。虽然2023年新《体育法》试图回应并克服这些弊端,但仍不足以适应中国式体育法治现代化的新要求。未来应树立体育立法与司法联动的整体体育法治观,在立法层面根据新《体育法》及时制定相应制度的细化规则,完善配套制度,提升立法操作性;在司法层面及时出台针对性司法解释和指导性案例,探索增设专门的体育法院或法庭,提升司法公正性,以体育立法和司法的双向重塑推动中国式体育法治现代化的行稳致远。

中国式体育法治现代化是中国式体育现代化和体育法治建设的“思想交汇”和“实践交集”,通过对司法场域中150篇援引《体育法》的司法裁判文书分析发现,目前我国公开的体育司法案例数量总体偏少且民事案件居多;程序性条款援引居多,司法与仲裁管辖成争议焦点。对此不仅要从现象中具体描述“现状是什么”,更要从法理高度挖掘现状“为什么是这样”以及“未来怎么办”,这背后同我国体育立法精细性、系统性不足,审判人员适用法律有偏差,多元化体育纠纷解决机制不完善密切相关。虽然新《体育法》试图回应并克服这些弊端,但仍难以适应并满足中国体育法治现代化的新要求。中国式体育法治现代化应是现代体育法治规范体系、现代体育行政执法体系、现代体育司法治理体系和现代体育行业自治体系的系统建构。最高司法机关应及时出台相关涉及体育的司法解释、指导性案例,加强体育裁判文书的释法说理,探索设立专门的体育法庭或涉体审判法官工作室。

Endoplasmic reticulum stress and autophagy in cerebral ischemia/reperfusion injury:PERK as a potential target for intervention

Several studies have shown that activation of unfolded protein response and endoplasmic reticulum(ER)stress plays a crucial role in severe cerebral ischemia/reperfusion injury.Autophagy occurs within hours after cerebral ischemia,but the relationship between ER stress and autophagy remains unclear.In this study,we established experimental models using oxygen-glucose deprivation/reoxygenation in PC12 cells and primary neurons to simulate cerebral ischemia/reperfusion injury.We found that prolongation of oxygen-glucose deprivation activated the ER stress pathway protein kinase-like endoplasmic reticulum kinase(PERK)/eukaryotic translation initiation factor 2 subunit alpha(e IF2α)-activating transcription factor 4(ATF4)-C/EBP homologous protein(CHOP),increased neuronal apoptosis,and induced autophagy.Furthermore,inhibition of ER stress using inhibitors or by si RNA knockdown of the PERK gene significantly attenuated excessive autophagy and neuronal apoptosis,indicating an interaction between autophagy and ER stress and suggesting PERK as an essential target for regulating autophagy.Blocking autophagy with chloroquine exacerbated ER stress-induced apoptosis,indicating that normal levels of autophagy play a protective role in neuronal injury following cerebral ischemia/reperfusion injury.Findings from this study indicate that cerebral ischemia/reperfusion injury can trigger neuronal ER stress and promote autophagy,and suggest that PERK is a possible target for inhibiting excessive autophagy in cerebral ischemia/reperfusion injury.
