Contribution of CT Scanning in the Investigation of Cranioencephalic Lesions in Bangui

Introduction: Cranioencephalic exploration has always played a major role in CT scans. In the Central Africa Republic (CAR), the lack of cross-sectional imaging before the year 2020 meant that no study had focused on cranioencephalic lesions. The aim of this study was to contribute to improving the management of cranioencephalic pathologies in CAR. Patients and Method: The study took place at the Bangui National Medical Imaging Centre (CNIMB). It was a retrospective study over a two-year period (March 1, 2021 to February 30, 2023). All patients referred for cranioencephalic CT scans were included, regardless of age or sex. Results: 1745 CT scans were performed, 575 of which were cranioencephalic CT scans. The majority of patients were male (53%). Most lived in the capital Bangui (90.9%). Patients aged 61 and over were the most representative. The distribution of patients by requesting department showed that the reception and emergency department was one of the least requesting departments. The main abnormalities observed were strokes, 82.1% of which were isch.e.ic strokes and 17.9% h.e.orrhagic strokes. Strokes were followed by degenerative lesions. Post-traumatic injuries included h.e.orrhagic contusions (38.3%), subdural h.e.atomas in 20.5% of cases, and extradural h.e.atomas (9.3%). Craniofacial lesions (fractures) were observed in 45.8% of cases. Conclusion: Cranioencephalic scans accounted for 1/3 of CT examinations performed during the study period. It revealed pathologies that could not be detected by conventional means. All in all, CT scans contributed to the diagnosis of cerebral pathologies.

Contribution of CT in the exploration of Non-Traumatic Pathologies of the Lumbar Spine in Adults in Bangui

Background: In Central African Republic, a study carried out in 2019 highligh.e. the limits of conventional radiography in the diagnosis of non-traumatic lower back pain in adults. Objective: The objective of this study is to show the value of CT scanning in the exploration of non-traumatic pathologies of the lumbar spine in adults. Patients and Method: Retrospective and descriptive cross-sectional study covering the files of patients aged at least 18 years old, sent for lumbar scanning from March 1 to December 31, 2021 at the medical imaging center. Results: In total, 593 scan examinations were performed including 159 (26.8%) examinations of the lumbar spine. The average age of the patients was 49.84 years. The majority were male (66%). 127 (79.8%) were referred mainly to the neurology and rheumatology departments. Common low back pain (n = 97, 61%) was the main reason for requesting a CT scan. The lumbar scan was performed without contrast product in 88.7% of cases. In total, 148 (93.1%) examinations were pathological and dominated by overall disc overhang (62.2%) and somatic and interapophyseal osteoarthritis (16.9%). The other lesions were spondylodiscitis (7.4%), tumor-like lesions (3.4%) and narrow lumbar canal (1.4%).

Introduction :Congenital adrenal hyperplasia (CAh) is the most common cause of primary adrenal insufficiency. It is a rare monogenic recessive disorder. In African setting in absence of neonatal screening, the diagnosis is still late, based on a clinical approach. During this clinical enquiry, information from past history or pedigree of the patient is of a huge importance and may revealed surprises. Patients and Methods: In this observational study, we retrospectively included all patients with a diagnosis of CAh. The diagnosis of CAh was retained based on a high 17 hydroxyprogesterone level in addition to clinical and morphological findings. From patients’ files, we extracted data on family history of disease, pedigree, clinical findings and genetics when available of 39 patients from two endocrinopeadiatric centers. Results: In 13 (30%) families, we found 20 reported deaths of infant less than 12 months. In these 13 families, half of the patients followed had 21 hydroxylase deficiencies and had 11 hydroxylase deficiencies. Unsurprisingly, we suspected adrenal insufficiency in these patients at verbal autopsy even in families with a patient with 11 hydroxylase deficiency. Other non DSD malformations or genetic disorders with apparently no link with CAh were reported in 3 families. The father of a patient reported to have hypospadias. Conclusion: each diagnosis of CAh made in our context is visible part of an iceberg. Behind a diagnosis of CAh made in our setting, is a long course of care, a dramatic past history revealing access to appropriate care disparity. Neonatal screening should thus be considered as an emergency.

Condition monitoring of the insulating system within power transformers has a massive importance according to the electrical *** gas analysis(DGA)is frequently used for this ***,DGA lacks the necessary level of accuracy to identify all equipment faults,particularly in their initial stages of ***,it does not have the capability for real-time monitoring and relies on manual sampling and laboratory testing,causing potential delays in fault ***,the interpretation of DGA data necessitates specialised expertise,which may pose difficulties for smaller entities that have limited access to ***,the contribution of this research is to use infrared spectroscopy measurements as a new effective technique substituting the DGA method for fault diagnosis in insulating *** inception faults that were considered in this study were the electrical fault(discharges of high.e.ergy)and the thermal fault(300°Cemperatureegarding that,two test cells were crafted especially for serving the simulation processes inside the laboratory for both types of inception ***,six samples of pure paraffinic mineral oil were taken to be degraded in the *** that,all of them besides another sample that were not subjected to any kind of faults were taken to be examined by Fourier transform infrared(FTIR)spectroscopy to obtain an overview of the oil's behaviour in each fault *** that,the FTIR analysis was initially verified utilising the DGA ***,for further affirmation,the dielectric dissipation factor(DDF)for all samples was *** the final analysis,the verification tests provide experimental evidence about the outperformance of this new optical technique in detecting the transformer's inception faults in addition to proving its potential for being a superior alternative to the well-known traditional diagnostic techniques.

The search for new biomarkers predictive of type 2 diabetes currently constitutes a research avenue in Bioclinical. Total homocysteine remains a preferred target due to its involvement in the occurrence of degenerative complications in type 2 diabetics. The aim of this work was to study hyperhomocysteinemia and other biochemical markers associated with T2D in the Congolese population. This was an analytical case-control study carried out between October 2022 and October 2023. The study population consisted of 150 subjects including 100 T2D patients and 50 control subjects. The main clinical data were collected on a pre-established form. homocysteine determination was carried out by the sandwich.e.ISA method. The other biochemical markers were measured by colorimetric enzymatic methods. hyperhomocysteinemia was present in 27.3% (41/150) of the entire study population. Type 2 diabetics had a frequency of hyperhomocysteinemia of 36% (36/100) and control 10% (5/50) (p = 0.001). The mean hyperhomocysteinemia concentration was 31.9 μmol/l with.e.tremes ranging from 18 to 103 μmol/l. Means of biological markers between diabetics and controls showed a statistically significant difference (p = 0.01). The risk factors associated with this hhcy were: sex (OR = 3.5), age (OR = 9.4), sedentary lifestyle (OR = 3.4) and glycosylated hemoglobin (OR = 12) with a p-value espectively. Our results suggest that hyperhomocysteinemia can be considered as a predictive biomarker in the bioclinic of Congolese type 2 diabetic patients.
