Survival in men older than 75 years with low- and intermediate-grade prostate cancer managed with watchful waiting with active surveillance

Objective:Recent studies have reported the underuse of active surveillance or watchful wait-ing for low-risk prostate cancer in the United *** study examined prostate cancer-specific and all-cause death in elderly patients older than 75 years with low-risk tumors managed with active treatment versus watchful waiting with active surveillance(WWAS).methods:We performed survival analysis in a cohort of 18,599 men with low-risk tumors(early and localized tumors)who were 75 years or older at the time of prostate cancer diagnosis in the linked Surveillance,Epidemiology,and End Results(SEER)-medicare database(from 1992 to 1998)and who were followed up through December *** was defined as having an-nual screening for prostate-specific antigen and/or digital rectal examination during the follow-up *** risks of prostate cancer-specific and all-cause death were compared by Cox regression *** propensity score matching technique was used to address potential selection ***:In patients with well-differentiated(Gleason score 2-4)and localized disease,those managed with WWAS without delayed treatment had higher risk of all-cause death(hazard ratio 1.20,95%confidence interval 1.13-1.28)but a substantially lower risk of prostate cancer-specific death(hazard ratio 0.62,confidence interval 0.51-0.75)than patients undergoing active *** managed with WWAS with delayed treatment had comparable mortality ***-tivity analyses based on propensity score matching yielded similar ***:In men older than 75 years with well-differentiated and localized prostate cancer,WWAS without delayed treatment had a lower risk of prostate cancer-specific death and compa-rable all-cause death as compared with active *** patients in whom treatment was delayed had comparable mortality *** results support WWAS as an initial management option for older men with well-differentiated and localized prostate cancer.

Subclinical hypothyroidism (SCHT) is common, with an occurrence of up to 10% of the adult population and defined biochemically only by elevated TSH and normal T4. SCHT affects negatively on lipid and carbohydrate metabolism increasing the risk of ischemic heart disease, affects negatively on cardiac performance and have a close correlation with renal function. The aim of this study is to compare presentation characteristics and outcomes between euthyroid pts and pts with SCHT who underwent cardiac surgery. methods: 474 pts from June 2003 through September 2004 had TSH and T4 measured. 365 pts were euthyroid (Group 1), 41 pts had SCHT (Group 2). Groups were compared by demographics and EuroSCORE (ES) risk profiles. Operative and hospital outcomes were compared as was follow-up mortality up to 96 months. Results: There were more females in Group 2, p = 0.04, more pts with CHF and number of NYHA III-IV pts (p more pts in Group 2 had elevated s-crea (p mparing the Groups by EuroSCORE (ES) showed higher risk scores in Group 2 pts (Additive ES 6.8 vs 8.5 and Logistic ES 12.3% vs 18.1%, p = 0.01 and 0.03). Hospital mortality was higher in Group 2 (12.2% vs 4.1%, p = 0.04) and the number of pts needing extended care was higher in Group 2 (p = 0.01). Follow up mortality was doubled in Group 2 pts up to 96 months compared to Group 1 (p mpared with euthyroid pts. Hospital and follow-up mortality are increased in SCHT pts.

Cohesin is a multiprotein complex that not only is essential for cell division but also has key roles in genome organization that underpin its gene regulatory *** mutations of genes encoding cohesin subunits occur in myeloid malignancies at 10%–12%(Kon et al.,2013),and the frequency of cohesin mutation in Down syndrome-associated megakaryoblastic leukaemia is even higher(~50%)(Yoshida et al.,2013).Cohesin insufficiency reinforces stem cell programmes and impairs differentiation in haematopoietic stem cells(mazumdar et al.,2015;mullenders et al.,2015;Viny et al.,2015).The STAG2 subunit of cohesin is the most frequently mutated in myeloid malignancies(Kon et al.,2013).In contrast to other cohesin subunits,complete loss of STAG2 is tolerated due to partial compensation by ***2 and STAG1 have redundant roles in cell division(Benedetti et al.,2017;van der Lelij et al.,2017).However,cohesin-STAG1 and cohesin-STAG2 have non-redundant roles in facilitating 3D genome organization to delineate tissue-specific gene expression(Kojic et al.,2018).

Objectives: The aim of this study was to evaluate the value of digital subtraction angiography (DSA) in thediagnostic evaluation of a highly selected patient population presenting with pulse-synchronous tinnitus(PST).methods: We retrospectively reviewed the charts of all patients referred for evaluation of possiblevascular etiology of pulsatile tinnitus. Patients were evaluated with regards to presenting signs,comorbidities, non-invasive imaging results, angiographic findings and ***: Fifteen patients underwent cerebral DSA. Dural arteriovenous fistula (dAVF) was identified in sixpatients, and five patients had other significant vascular pathology identified on DSA. Seven patientswith ‘negative’ non-invasive imaging were found to have significant pathology on ***: Catheter angiography may have a significant yield in appropriately selected patients presenting with pulse synchronous tinnitus.

In this review,we introduce the current state of the art of the growth technology of pure,lightly doped,and heavily doped(solid solution)nonlinear gallium selenide(GaSe)crystals that are able to generate broadband emission from the near infrared(IR)(0.8 mm)through the mid-and far-IR(terahertz(THz))ranges and further into the millimeter wave(5.64 mm)*** the first time,we show that appropriate doping is an efficient method controlling a range of the physical properties of GaSe crystals that are responsible for frequency conversion efficiency and exploitation *** appropriate doping,uniform crystals grown by a modified technology with heat field rotation possess up to 3 times lower absorption coefficient in the main transparency window and THz ***,doping provides the following benefits:raises by up to 5 times the optical damage threshold;almost eliminates two-photon absorption;allows for dispersion control in the THz range independent of the mid-IR dispersion;and enables crystal processing in arbitrary directions due to the strengthened ***,doped GaSe demonstrated better usefulness for processing compared with GaSe grown by the conventional technology and up to 15 times higher frequency conversion efficiency.

As atmospheric CO_2 levels rise, the development of physical or chemical adsorbents for CO_2 capture and separation is of great importance on the way towards a sustainable low-carbon future. Porous organic polymers are promising candidates for CO_2 capture materials owing to their structural flexibility, high surface area, and high stability. In this review, we highlight high-performance porous organic polymers for CO_2 capture and summarize the strategies to enhance CO_2 uptake and selectivity, such as increasing surface area, increasing interaction between porous organic polymers and CO_2, and pore surface functionalization.
