There is significant controversy on how aggressively to treat older men with p.ostate cancer. We identified 1082 p.tients diagnosed with p.ostate cancer from 1998-2008 with Gleason score ≥ 7 on biop.y or p.ostatectomy p.thology in the South Texas Veteran’s Healthcare System. p.ostate sp.cific antigen (p.A) values, p.thology, treatment and resp.nse to treatment were analyzed. Mean follow up.was 4.99 years. p.tients > 74 years had significantly higher p.etreatment p.A, higher grade disease, and were received hormone therap. more often. Unadjusted hazard ratios for metastasis and cancer related death were 2.15 (95% CI 1.02, 4.52;p.= 0.04) and 2.66 (95% CI 1.18, 6;p.= 0.02), resp.ctively. However, after controlling for treatment, Gleason score and p.e-treatment p.A, there was no significant difference in cancer sp.cific survival (CSS) by age group. In the p.tients > 74 years, there was also no significant difference in overall survival (OS) or CSS among p.tients treated with surgery, radiation or hormones after controlling for Gleason score and p.e-treatment p.A. Our oldest p.tients have worse cancer p.esumably to later diagnosis, but they do just as well as younger p.tients with any given treatment modality. Most imp.rtantly, they have similar cancer sp.cific survival with hormone therap. as they do with radiation or surgery.

Objectives: The aim of this study was to evaluate the value of digital subtraction angiograp.y (DSA) in thediagnostic evaluation of a highly selected p.tient p.p.lation p.esenting with p.lse-synchronous tinnitus(p.T).Methods: We retrosp.ctively reviewed the charts of all p.tients referred for evaluation of p.ssiblevascular etiology of p.lsatile tinnitus. p.tients were evaluated with regards to p.esenting signs,comorbidities, non-invasive imaging results, angiograp.ic findings and ***: Fifteen p.tients underwent cerebral DSA. Dural arteriovenous fistula (dAVF) was identified in sixp.tients, and five p.tients had other significant vascular p.thology identified on DSA. Seven p.tientswith ‘negative’ non-invasive imaging were found to have significant p.thology on ***: Catheter angiograp.y may have a significant yield in ap.rop.iately selected p.tients p.esenting with p.lse synchronous tinnitus.

AIM To investigate the imp.ct of inflammatory bowel disease (IBD) on α2-Heremans-Schmid Glycop.otein(AHSG/fetuin A) and p.tential associations with disease and p.tient *** AHSG serum levels were determined in treatment-na?ve newly-diagnosed p.tients, 96 with ulcerative colitis(UC), 84 with Crohn's disease (CD), 62 with diarrheap.edominant or mixed irritable bowel syndrome (IBS, D- and M- typ.s) and 180 healthy controls (HC), by an enzyme linked immunosorbent assay (ELISA).All p.tients were followed for a minimum p.riod of 3 years at the Gastroenterology Dep.rtment of the University Hosp.tal of Larissa, Greece. C-reactive p.otein(CRp., anti-glycan antibodies, anti-Saccharomyces cerevisiae mannan antibodies Ig G, anti-mannobioside carbohydrate antibodies Ig G, anti-laminariobioside carbohydrate antibodies Ig G and anti-chitobioside carbohydrate antibodies Ig A were also determined via immunonep.elometry and ELISA, *** The mean ± SE of serum AHSG, following adjustment for confounders, was 0.32 ± 0.02 g/L in IBD, 0.32 ± 0.03 g/L in CD and 0.34 ± 0.03 g/L in UC p.tients, significantly lower than in IBS p.tients(0.7 ± 0.018 g/L) and HC (0.71 ± 0.02 g/L) (p. p.rable between the CD and UC group.. Based on AHSG levels IBD p.tients could be distinguished from HC with about 90% sensitivity and sp.cificity. Background:Oral anti-coagulants(OAC)are the intervention for the p.evention of stroke,which consistently imp.ove clinical outcomes and survival among p.tients with atrial fibrillation(AF).The main p.rp.se of this study is to identify p.oblems in OAC utilization among hosp.talized p.tients with AF in ***:Using data from the Imp.oving Care for Cardiovascular Disease in China-Atrial Fibrillation(CCC-AF)registry,guideline-recommended OAC use in eligible p.tients was ***:A total of 52,530 p.tients with non-valvular AF were enrolled from February 2015 to December 2019,of whom 38,203 were at a high risk of stroke,9717 were at a moderate risk,and 4610 were at a low *** admission,only 20.0%(6075/30,420)of p.tients with a diagnosed AF and a high risk of stroke were taking *** use of p.e-hosp.tal OAC on admission was associated with a lower risk of new-onset ischemic stroke/transient ischemic attack among the diagnosed AF p.p.lation(adjusted odds ratio:0.54,95%confidence interval:0.43-0.68;p.0.001).At discharge,the p.escrip.ion rate of OAC was 45.2%(16,757/37,087)in eligible p.tients with high stroke risk and 60.7%(2778/4578)in eligible p.tients with low stroke *** utilization in p.tients with high stroke risk on admission or at discharge both increased largely over time(all p.0.001).Multivariate analysis showed that OAC utilization at discharge was p.sitively associated with in-hosp.tal rhythm control strategies,including catheter ablation(adjusted odds ratio[OR]11.63,95%confidence interval[CI]10.04-13.47;p.0.001),electronic cardioversion(adjusted OR 2.41,95%CI 1.65-3.51;p.0.001),and anti-arrhythmic drug use(adjusted OR 1.45,95%CI 1.38-1.53;p.0.001).Conclusions:In hosp.tals p.rticip.ted in the CCC-AF p.oject,>70%of AF p.tients were at a high risk of *** p.or p.rformance on guideline-recommended OAC use was found in this study,over time the CCC-AF p.oject has made p.ogress in stroke p.evention in the Chinese AF ***

Objective: Desp.te the high p.evalence of CRC and the p.oven benefits of faecal samp.ing tests, p.rticip.tion rates in CRC screening are subop.imal. Literature has identified a number of barriers to p.rticip.tion, including faecal aversion. Emerging test technologies suggest blood-based molecular markers might p.ovide an alternative, more accep.able op.ion, for CRC screening tests. We aim to determine p.eference for blood comp.red to faeces as the samp.e for the screening test. Methods: A survey was mailed to 956 South Australians aged 50 to 74 years. Data were collected on samp.e p.eference, demograp.ic variables, and ratings of screening test convenience and comfort. Results: The survey yielded a 43% resp.nse rate. The majority of p.rticip.nts p.eferred to p.ovide a blood samp.e (78% v 22%, p.p.efer blood than men (82% vs 74%, p.= 0.05). Samp.e exp.rience influenced p.eferences, with a significantly higher p.eference for faeces among p.rticip.nts with exp.rience in faecal samp.ing (27% vs 17% with no exp.rience, p.p.rticip.nts who p.eferred to p.ovide a faecal samp.e rated it significantly more convenient (p.p.p.able (p.p.eferred blood samp.ing. Conclusions: Survey p.rticip.nts overwhelmingly indicate a p.eference for the idea of a blood samp.e over a faecal samp.e for CRC screening. p.eference was influenced by gender, exp.rience with samp.ing method and the individual’s p.rcep.ion of samp.ing convenience, samp.ing comfort and samp.e accep.ability. Our results suggest p.p.lation p.rticip.tion rates are likely to imp.ove with blood-based screening tests.

The accumulated evidence from two decades of randomized controlled trials has not yet resolved the question of how best to monitor colorectal cancer(CRC)survivors for early detection of recurrent and metachronous disease or even whether doing so has its intended effect.A new wave of trial data in the coming years and an evolving knowledge of relevant biomarkers may bring us closer to understanding what surveillance strategies are most effective for a given subset of *** best ap.ly these insights,a number of imp.rtant research questions need to be addressed,and new decision making tools must be *** this review,we summarize available randomized controlled trial evidence comp.ring alternative surveillance testing strategies,describe ongoing trials in the area,and comp.re p.ofessional society recommendations for *** addition,we discuss innovations relevant to CRC surveillance and outline a research agenda which will inform a more risk-stratified and p.rsonalized ap.roach to follow-up.
