Socio-demographic factors impact time to discharge following total knee arthroplasty

AIM To determine social, logistical and demographic factors that influence time to discharge in a short stay pathway(SSP) by following total knee arthroplasty(TKA). METHODS The study included primary TKA's performed in a highvolume arthroplasty center from January 2016 through December 2016. Potential variables associated with increased hospital length of stay(LOS) were obtained from patient medical records. These included age, gender, race, zip code, body mass index(BMI), number of pre-operative medications used, number of narcotic medications used, number of patient reported allergies(PRA), simultaneous bilateral surgery, tobacco use, marital status, living arrangements, distance traveled for surgery, employment history, surgical day of the week, procedure end time and whether the surgery was performed during a major holiday week. Multivariate step-wise regression determined the impact of social, logistical and demographic factors on LOS. RESULTS Eight hundred and six consecutive primary SSP TKA's were included in this study. Patients were discharged at a median of 49 h(post-operative day two). The following factors increased LOS: Simultaneous bilateral TKA [46.1 h longer(P nder [4.3 h longer(P = 0.012)], age [3.5 h longer per ten-year increase in age(P nt-reported allergies [1.1 h longer per allergy reported(P = 0.005)], later procedure endtimes [0.8 h longer per hour increase in end-time(P = 0.004)] and Black or African American patients [6.1 h longer(P = 0.047)]. Decreased LOS was found in married patients [4.8 h shorter(P = 0.011)] and TKA's performed during holiday weeks [9.4 h shorter(P = 0.011)]. non-significant factors included: BMI, median income, patient's living arrangement, smoking status, number of medications taken, use of pre-operative pain medications, distance traveled to hospital, and the day of surgery. COnCLUSIOn The cost of TKA is dependent upon LOS, which is affected by multiple factors. The clinical care team should acknowledge socio-demographic factors

AIM To investigate the impact of inflammatory bowel disease (IBD) on α2-Heremans-Schmid Glycoprotein(AHSG/fetuin A) and potential associations with disease and patient characteristics.METHODS AHSG serum levels were determined in treatment-na?ve newly-diagnosed patients, 96 with ulcerative colitis(UC), 84 with Crohn's disease (CD), 62 with diarrheapredominant or mixed irritable bowel syndrome (IBS, D- and M- types) and 180 healthy controls (HC), by an enzyme linked immunosorbent assay (ELISA).All patients were followed for a minimum period of 3 years at the Gastroenterology Department of the University Hospital of Larissa, Greece. C-reactive protein(CRP), anti-glycan antibodies, anti-Saccharomyces cerevisiae mannan antibodies Ig G, anti-mannobioside carbohydrate antibodies Ig G, anti-laminariobioside carbohydrate antibodies Ig G and anti-chitobioside carbohydrate antibodies Ig A were also determined via immunonephelometry and ELISA, respectively.RESULTS The mean ± SE of serum AHSG, following adjustment for confounders, was 0.32 ± 0.02 g/L in IBD, 0.32 ± 0.03 g/L in CD and 0.34 ± 0.03 g/L in UC patients, significantly lower than in IBS patients(0.7 ± 0.018 g/L) and HC (0.71 ± 0.02 g/L) (P n all cases). AHSG levels were comparable between the CD and UC groups. Based on AHSG levels IBD patients could be distinguished from HC with about 90% sensitivity and specificity. Further adjusted analysis verified the inverse association between AHSG and penetrating, as well as stricturing CD (partial correlation coefficient: -0.45 and -0.33, respectively) (P ng for confounding factors, inverse correlations between AHSG and CRP and the need for anti-TnFα therapy or surgery, were found (partial correlation coefficients:-0.31,-0.33,-0.41, respectively, P n all cases). Background:Recognizing sport-related concussion(SRC)is challenging and relies heavily on subjective symptom reports.An objective,biological marker could improve recognition and understanding of SRC.There is emerging evidence that salivary micro-ribonucleic acids(miRnAs)may serve as biomarkers of concussion;however,it remains unclear whether concussion-related miRnAs are impacted by exercise.We sought to determine whether40 miRnAs previously implicated in concussion pathophysiology were affected by participation in a variety of contact and non-contact sports.Our goal was to refine a miRnA-based tool capable of identifying athletes with SRC without the confounding effects of exercise.Methods:This case-control study harmonized data from concussed and non-concussed athletes recruited across 10 sites.Levels of salivary miRnAs within 455 samples from 314 individuals were measured with RnA sequencing.Within-subjects testing was used to identify and exclude miRnAs that changed with either(a)a single episode of exercise(166 samples from 83 individuals)or(b)season-long participation in contact sports(212 samples from 106 individuals).The miRnAs that were not impacted by exercise were interrogated for SRC diagnostic utility using logistic regression(172 samples from 75 concussed and 97 non-concussed individuals).Results:Two miRnAs(miR-532-5p and miR-182-5p)decreased(adjusted pngle episode of exercise,and 1 miRnA(miR-4510)increased only after contact sports participation.Twenty-three miRnAs changed at the end of a contact sports season.Two of these miRnAs(miR-26b-3p and miR-29c-3p)were associated(R>0.50;adjusted pnumber of head impacts sustained in a single football practice.Among the 15 miRnAs not confounded by exercise or season-long contact sports participation,11 demonstrated a significant difference(adjusted pn concussed and non-concussed participants,and 6 displayed moderate ability(area under curve>0.70)to identify concussion.A single ratio(miR-27a-5p/miR-30a-3p)displayed the

Impact of oxygen-containing carbonyl and ether groups on dielectric properties of poly(oxa)norbornene cyclic olefins

Flexible dielectric polymers that can withstand high electric field and simultaneously have high dielectric constant are desired for high-density energy storage.Here,we systematically investigated the impact of oxygen-containing ether and carbonyl groups in the backbone structure on dielectric properties of a series of cyclic olefin.In comparison to the influence of the-CF3 pendant groups that had more impact on the dielectric constant rather than the band gap,the change of the backbone structure affected both the dielectric constant and band gaps.The one polymer with ether and carbonyl groups in the backbone has the largest band gap and highest discharge efficiency,while it has the lowest dielectric constant.The polymer without any ether groups in the backbone has the smallest band gap and lowest discharge efficiency,but it has the highest dielectric constant.Polymers that have no dipolar relaxation exhibit an inversely correlated dielectric constant and band gap.Enhancing the dipolar relaxation through rational molecular structure design can be a novel way to break through the exclusive constraint of dielectric constant and band gap for high-density energy storage.

RnA-based therapies in animal models of Leber congenital amaurosis causing blindness

Leber congenital amaurosis(LCA)is a severe,genetically heterogeneous recessive eye disease in which∼35%of genemutations are in-frame nonsensemutations coding for loss-of-function premature termination codons(PTCs)inmRnA.nonsense suppression therapy allows read-through of PTCs leading to production of full-length protein.A limitation of nonsense suppression is that nonsense-mediated decay(nMD)degrades PTC-containing RnA transcripts.The purpose of this study was to determine whether inhibition of nMD could improve nonsense suppression efficacy in vivo.Using a high-throughput approach in the recessive cep290 zebrafish model of LCA(cep290;Q1223X),we first tested the nMD inhibitor Amlexanox in combination with the nonsense suppression drug Ataluren.We observed reduced retinal cell death and improved visual function.With these positive data,we next investigated whether this strategy was also applicable across species in two mammalianmodels:Rd12(rpe65;R44X)and Rd3(rd3;R107X)mouse models of LCA.In the Rd12 model,cell death was reduced,RPE65 protein was produced,and in vivo visual function testing was improved.We establish for the first time that the mechanism of action of Amlexanox in Rd12 retina was through reduced UPF1 phosphorylation.In the Rd3 model,however,no beneficial effect was observed with Ataluren alone or in combination with Amlexanox.This variation in response establishes that some forms of nonsensemutation LCA can be targeted by RnA therapies,but that this needs to be verified for each genotype.The implementation of precision medicine by identifying better responders to specific drugs is essential for development of validated retinal therapies.

Background To characterize pediatric patients supported with continuous positive airway pressure and bilevel positive airway pressure(CPAP/BiPAP)or high-flow nasal cannula(HFnC)during interfacility transport(IFT).Methods A retrospective study with a provincial pediatric transport team from a tertiary hospital pediatric intensive care unit.Pediatric patients aged 28 days ton January 2017 and December 2018,were identified through a transport registry and were included in the study.Results A total of 118(26.7%)patients received CPAP/BIPAP or HFnC support for IFT.The most common respiratory diagnosis was bronchiolitis(46%).These patients were placed on respiratory support,31.4 minutes after the transport team’s arrival.none required intubation during their IFT,despite mean transport times of 163 minutes.Conclusions This study may provide important information for programs with large catchment areas,in which large distances and transport times should not be barriers to nIV implementation.

Origin of major ions in monthly rainfall events at the Bamenda Highlands, north West Cameroon

Rainwater characteristics can reveal emissions from various anthropogenic and natural sources into the atmosphere. The physico-chemical characteristics of 44 monthly rainfall events （collected between January and December 2012） from 4 weather stations （Bamenda, ndop plain, ndawara and Kumbo） in the Bamenda Highlands （BH） were investigated. The purpose was to determine the sources of chemical species, their seasonal inputs and suitability of the rainwater for drinking. The mean pH of 5 indicated the slightly acidic nature of the rainwater. Average total dissolved solids （TDS） were low （6.7 mg/L）, characteristic of unpolluted atmospheric moisture/air. Major ion concentrations （mg/L） were low and in the order K＋〉 Ca2＋〉 Mg2~ 〉 na＋ for cations and nO3 〉〉 HCO3 〉 SO] 〉 CI- 〉 PO3- 〉 F- for anions. The average rainwater in the area was mixed Ca-Mg-SO4-CI water type. The CI-/na＋ ratio （1.04） was comparable to that of seawater （1.16）, an indication that n a＋ and CI originated mainly from marine （Atlantic Ocean） aerosols. High enrichments of Ca2＋, Mg2＋ and SO2- to na＋ ratios relative to seawater ratios （constituting 44% of the total ions） demonstrated their terrigenous origin, mainly from Saharan and Sahelian arid dusts. The K＋/na＋ ratio （2.24）, which was similar to tropical vegetation ash （2.38）, and nO3 was essentially from biomass burning. Light （〈 100 mm） pre-monsoon and post-monsoon convective rains were enriched in major ions than the heavy （〉 100 mm） monsoon rains, indicating a high contribution of major ions during the low convective showers. Despite the acidic nature, the TDS and major ion concentrations classified the rainwater as potable based on the WHO guidelines.

Identification of epididymis-specific transcripts in the mouse and rat by transcriptional profiling

As part of our efforts to identify novel contraceptive targets in the epididymis we performed transcriptional profiling on each of the 10 and 19 segments of the mouse and rat epididymidis, respectively, using Affymetrix whole genome microarrays. A total of 17 096 and 16 360 probe sets representing transcripts were identified as being expressed in the segmented mouse and rat epididymal transcriptomes, respectively. Comparison of the expressed murine transcripts against a mouse transcriptional profiling database derived from 22 other mouse tissues identified 77 transcripts that were expressed uniquely in the epididymis. The expression of these genes was further evaluated by reverse transcription polymerase chain reaction （RT-PCR） analysis of RnA from 21 mouse tissues. RT-PCR analysis confirmed epididymis-specific expression of Defensin Beta 13 and identified two additional genes with expression restricted only to the epididymis and testis. Comparison of the 16 360 expressed transcripts in the rat epididymis with data of 21 other tissues from a rat transcriptional profiling database identified 110 transcripts specific for the epididymis. Sixty-two of these transcripts were further investigated by qPCR analysis. Only Defensin 22 （E3 epididymal protein） was shown to be completely specific for the epididymis. In addition, 14 transcripts showed more than 100-fold selective expression in the epididymis. The products of these genes might play important roles in epididymal and/or sperm function and further investigation and validation as contraceptive targets are warranted. Stability study on an anti-cancer drug 4-(3,5-bis(2-chlorobenzylidene)-4-oxo-piperidine-1-yl)-4-oxo-2-butenoic acid (CLEFMA) using a stabilityindicating HPLC method

CLEFMA, 4-(3,5-bis(2-chlorobenzylidene)-4-oxo-piperidine-1-yl)-4-oxo-2-butenoic acid, is a new chemical entity with anti-cancer and anti-inflammatory activities. Here, we report its stability in solution against stress conditions of exposure to acid/base, light, oxidant, high temperature, and plasma. The identity of the degradation products was ascertained by mass and proton nuclear magnetic resonance spectroscopy. To facilitate this study, we developed and validated a reverse phase high performance liquid chromatography method for detection of CLEFMA and its degradation. The method was linear over a range of 1–100 μg/m L; the accuracy and precision were within acceptable limits; it was stability-indicating as it successfully separated cis-/trans-isomers of CLEFMA as well as its degradation product. The major degradation product was produced from amide hydrolysis at maleic acid functionality caused by an acidic buffer, oxidant(3% hydrogen peroxide),or temperature stress(40–60 °C). The log k-p H profile showed that CLEFMA was most stable at neutral p H. In accelerated stability study we found that the shelf-life(T_(90%)) of CLEFMA at 25 °C and 4 °C was 45 days and220 days, respectively. Upon exposure to UV-light(365 nm), the normally prevalent trans-CLEFMA attained cis-configuration. This isomerization also involved the maleic acid moiety. CLEFMA was stable in plasma from which it could be efficiently extracted by an acetonitrile precipitation method. These results indicate that CLEFMA is sensitive to hydrolytic cleavage at its maleic acid moiety, and it is recommended that its samples should be stored under refrigerated and light-free conditions, and under inert environment.

Thermal and microstructural characterization of a novel ductile cast iron modified by aluminum addition

In high-temperature applications,like exhaust manifolds,cast irons with a ferritic matrix are mostly used.However,the increasing demand for higher-temperature applications has led manufacturers to use additional expensive materials such as stainless steels and ni-resist austenitic ductile cast irons.Thus,in order to meet the demand while using low-cost materials,new alloys with improved high-temperature strength and oxidation resistance must be developed.In this study,thermodynamic calculations with Thermo-Calc software were applied to study a novel ductile cast iron with a composition of 3.5wt%C,4wt%Si,1wt%nb,0‒4wt%Al.The designed compositions were cast,and thermal analysis and microstructural characterization were performed to validate the calculations.The lowest critical temperature of austenite to pearlite eutectoid transformation,i.e.,A1,was calculated,and the solidification sequence was determined.Both calculations and experimental data revealed the importance of aluminum addition,as the A1 increased by increasing the aluminum content in the alloys,indicating the possibility of utilizing the alloys at higher temperature.The experimental data validated the transformation temperature during solidification and at the solid state and confirmed the equilibrium phases at room temperature as ferrite,graphite,and MC-type carbides.
