The diagnostic and prognostic utility of the dual-task tandem gait test for pediatric concussion

Background:The tandem gait test has gained interest recently for assessment of concussion recovery.The purpose of our study was to determine the prognostic and diagnostic use of the single-and dual-task tandem gait test,alongside other clinical measures,within 10 days of pediatric concussion.Methods:We assessed 126 patients post-concussion(6.3§2.3 days post-injury,mean§SD)at a pediatric sports medicine clinic and compared them to 58 healthy controls(age:15.6§1.3 years;43%female).We also compared the 31 patients with concussion who developed persistent post-concussion symptoms(PPCS)(age=14.9§2.0 years;48%female)to the 81 patients with concussion who did not develop PPCS following the initial assessment(age:14.1§3.0 years;41%female).All subjects completed a test battery,and concussion patients were monitored until they experienced concussion-symptom resolution.The test battery included tandem gait(single-task,dual-task(performing tandem gait while concurrently completing a cognitive test)conditions),modified Balance Error Scoring System(mBESS),and concussion symptom assessment(Health and Behavior Inventory).We defined PPCS as symptom resolution time>28 days post-concussion for the concussion group.Measurement outcomes included tandem gait time(single-and dual-task),dual-task cognitive accuracy,mBESS errors(single/double/tandem stances),and symptom severity.Results:The concussion group completed the single-task(mean difference=9.1 s,95%confidential interval(95%CI):6.1-12.1)and dual-task(mean difference=12.7 s,95%CI:8.716.8)tandem gait test more slowly than the control group.Compared to those who recovered within 28 days of concussion,the PPCS group had slower dual-task tandem gait test times(mean difference=7.9 s,95%CI:2.0-13.9),made more tandem-stance mBESS errors(mean difference=1.3 errors,95%CI:0.2-2.3),and reported more severe symptoms(mean difference=26.6 Health and Behavior Inventory rating,95%CI:21.1-32.6).Conclusion:Worse dual-task tandem gait test time and mBESS tandem stance performance predicted PPCS in

Ohio solid organ transplantation consortium criteria for liver transplantation in patients with alcoholic liver disease

AIM To evaluate risk of recidivism on a case-by-case basis.METHODS From our center's liver transplant program,we selected patients with alcoholic liver disease who were listed for transplant based on Ohio Solid Organ Transplantation Consortium(OSOTC) exception criteria.They were considered to have either a low or medium risk of recidivism,and had at least one or three or more months of abstinence,respectively.They were matched based on gender,age,and Model for End-Stage Liver Disease(MELD) score to controls with alcohol-induced cirrhosis from Organ Procurement and Transplant network data.RESULTS Thirty six patients with alcoholic liver disease were approved for listing based on OSOTC exception criteria and were matched to 72 controls.nineteen patients(53%) with a median [Inter-quartile range(IQR)] MELD score of 24(13) received transplant and were followed for a median of 3.4 years.They were matched to 38 controls with a median(IQR) MELD score of 25(9).At one and five years,cumulative survival rates(± standard error) were 90% ± 7% and 92% ± 5% and 73% ± 12% and 77% ± 8% in patients and controls,respectively(Log-rank test,P = 0.837).Four(21%) patients resumed drinking by last follow-up visit.COnCLUSIOn Compared to traditional criteria for assessment of risk of recidivism,a careful selection process with more flexibility to evaluate eligibility on a case-by-case basis can lead to similar survival rates after transplantation.

The Effects of the Chemical Components of Soil Salinity on Electrical Conductivity in the Region of the Delta Oasis of Weigan and Kuqa Rivers,China

In order to assess the effects of chemical properties of soil salinity on electrical conductivity of 1：5 soil/water extract （EC1：5）, the study focused on revealing the main chemical factors contributing to EC of soil extracts and their relative importance. The relationship between EC1：5 and the chemical properties of soil salinity in the delta oasis of Weigan and Kuqa rivers, China, were studied using path coefficient analysis, a path analysis method. We studied each key element affecting EC1：5 either directly or indirectly. The results obtained show that the salt content, total dissolved solids （TDS）, and the sum of the sodium ion concentration and the kalium ion concentration are the most influential factors on 1：5 soil/ water extract （EC1：5） in the 0-10 cm and the 30-50 cm soil layer. The results show that the sequence of direct path coefficients in the 0-10 cm and the 30-50 cm soil layers on soil conductivity is TDS→na^＋＋ K^＋→Salt content→Ca^2＋→Cl-→the sodium dianion ratio （SDR）→pH→ SO4^2-→HCO3^-→Mg^2＋→the soluble sodium percentage （SSP） sodium absorption ratio （SAR） and TDS→Salt content→na^＋＋ K^＋→Ca^2＋→SDR→Mg^2＋→HCO3^-→SSP→pH→SO4^2-→SAR→Cl^-. The salt content, chlorine ion, and SAR are the main factors affecting 1：5 soil/water extract （EC1：5） in the 10-30 centimeter soil layer. The order of direct path coefficients result is as follows： Salt content→Cl^-→SAR→SSP→TDS→Ca^2＋→Mg^2＋= SO4^2-→HCO3^-→pH→SDR→na^- ＋ K^＋. Moreover, the effects of HCO3^-, pH were very weak. Though the direct path coefficients between EC1：5 and SAR, SO4^2- and Ca^2＋ were not high, influence of other chemical factors caused the coefficients to increase, making the summation of their direct and indirect path coefficients relatively high. The models of the different soil layers were structured separately. Background:The age,biomarkers,and clinical history(ABC)-atrial fibrillation(AF)-Stroke score have been proposed to refine stroke risk stratification,beyond what clinical risk scores such as the CHA2DS2-VASc score can offer.This study aimed to identify risk factors associated with thromboembolism and evaluate the performance of the ABC-AF-Stroke score in predicting thromboembolism in non-anticoagulated AF patients following successful ablations.Methods:A total of 2692 patients who underwent successful ablations with discontinued anticoagulation after a 3-month blanking period in the Chinese Atrial Fibrillation Registry(CAFR)between 2013 and 2019 were included.Cox regression analysis was conducted to present the association of risk factors with thromboembolism risk.The ABC-AF-Stroke score was evaluated in terms of discrimination,including concordance index(C-index),net reclassification improvement(nRI)and integrated discrimination improvement(IDI),clinical utilization by decision curve analysis(DCA),and calibration by comparing the predicted risk with the observed annualized event rate.Results:After a median follow-up of 3.5 years,64 patients experienced thromboembolism events.Age,prior history of stroke/transient ischemic attack(TIA),high-sensitivity cardiac troponin T(cTnT-hs),and n-terminal pro-B-type natriuretic peptide(nT-proBnP)were independently associated with thromboembolism risk.The ABC-AF-Stroke score performed statistically significantly better than the CHA2DS2-VASc score in terms of C-index(0.67,95%confidence interval[CI]:0.59-0.74 vs.0.60,95%CI:0.52-0.67,P=0.030)and reclassification capacity.The DCA implied that the ABC-AF-Stroke score could identify more thromboembolism events without increasing the false positive rate compared to the CHA2DS2-VASc score.The calibration curve showed that the ABC-AF-Stroke score was well calibrated in this population.Conclusions:In this real-world study enrolling non-anticoagulated AF patients following successful ablations,age,prior history of stroke/TIA,level of nT-p

Variation in resource competition traits among Microcystis strains is affected by their microbiomes

Freshwater harmful algal blooms are often dominated by Microcystis,a phylogenetically cohesive group of cyanobacteria marked by extensive genetic and physiological diversity.We have previously shown that this genetic diversity and the presence of a microbiome of heterotrophic bacteria influences competitive interactions with eukaryotic phytoplankton.In this study,we sought to explain these observations by characterizing Monod equation parameters for resource usage(maximum growth rateμmax,half-saturation value for growth Ks,and quota)as a function of n and P levels for four strains(nIES-843,PCC 9701,PCC 7806[WT],and PCC 7806ΔmcyB)in presence and absence of a microbiome derived from Microcystis isolated from Lake Erie.Results indicated limited differences in maximum growth rates but more pronounced differences in half-saturation values among Microcystis strains.The largest impact of the microbiome was reducing the minimal nitrogen concentration sustaining growth and reducing half saturation values,with variable results depending on the Microcystis strain.Microcystis strains also differed from each other in their n and P quotas and the extent to which microbiome presence affected them.Our data highlight the importance of the microbiome in altering Microcystis-intrinsic traits,strain competitive hierarchies,and thus bloom dynamics.As quota,μmax,and Ks are commonly used in models for harmful algal blooms,our data suggest that model improvement may be possible by incorporating genotype dependencies of resource-use parameters.

Removing harvest residues from hardwood stands affects tree growth, wood density and stem wood nutrient concentration in European beech (Fagus sylvatica) and oak (Quercus spp.)

Background:Higher exportation of harvest residues from forests due to increased demand for woody biomass,has reportedly diminished soil mineral resources and may lead to degraded tree nutrition as well as growth.However,as nutrients become less available in the soil,the remobilization of nutrients in biomass tissues(plant internal cycling)helps sustain tree nutrition.Our study aims to quantify the impact of Removing Harvest Residues and Litter(RHRL)during five years on tree growth,wood density,and stem wood nutrient concentrations in young beech and oak forest stands.Result:Our study found that,RHRL significantly decreased tree growth ring width by 14%,and wood density by 3%,in beech trees,in near bark rings.RHRL also significantly reduced nutrient concentration in near bark and near pith areas of both studied species.Mg,na and S were found lower by 44%,76%,and 56%,respectively,in near bark area of beech trees.In near bark area of oak trees,K,Ca,Mg,na,S,and Fe were lower by 20%,25%,41%,48%,41%,and 16%,respectively.K and Mg concentrations decreased more strongly in near pith area compared to near bark area suggesting internal translocation of these two elements.Conclusion:In beech trees,wood density proved to be an important factor while quantifying the effect of removing harvest residuals on tree growth and biomass.Soil nutrient loss intensified the remobilization of nutrients con-tained in older tree rings(close to the pith)towards newly formed rings(close to bark).In our study,in beech trees,K was found to be the most recycled major nutrient.These results demonstrate the potential of such analysis for providing valuable insight into the effect of RHRL in premature stands on the physiological adaptive strategies of trees and an indication of soil fertility status.

nonalcoholic fatty liver disease(nAFLD)is a disease spectrum that spans simple steatosis,fibrosis,and ultimately cirrhosis,and is a leading cause of chronic liver disease globally.The severe variant of nAFLD,non-alcoholic steatohepatitis(nASH),is characterized by triglyceride accumulation within hepatocytes and the subsequent inflammatory pathway activation,ultimately progressing to cirrhosis in 10%-20%of patients.nASH is a known major risk factor for the development of hepatocellular carcinoma(HCC),and there is emerging data demonstrating the impact of nASH on immune subsets and the tumor microenvironment that may influence therapeutic response.This review describes the various ways in which the immune system is altered in patients with nASH.The innate immune system in nASH shows alterations in dendritic and Kupffer cells,impaired cytotoxicity of natural Killer cells,and an accumulation of neutrophils.Additionally,there is emerging evidence emphasizing the role of the adaptive immune system in the development and progression of nASH,seen in the alteration of B-cells,T-cells,and nKT Cells.Due to the complex interplay of the immune system in nAFLD/nASH and its progression to HCC,many current treatments focus on targeting immune cells for HCC therapy.Recently,immune checkpoint inhibitors such as atezolizumab and bevacizumab have been approved as first-line therapy for unresectable HCC.Although an emerging field of research,further studies and clinical trials are needed to understand the complex interface of nASH,HCC and the immune response.

The main objective of this study is to investigate the buckling analysis of CCSs reinforced by CnTs subjected to combined loading of hydrostatic pressure and axial compression resting on the twoparameter elastic foundation(T-P-EF).It is one of the first attempts to derive the governing equations of the CCSs reinforced with CnTs,based on a generalized first-order shear deformation shell theory(FSDST)which includes shell-foundation interaction.By adopting the extended mixing rule,the effective material properties of CCSs reinforced by CnTs with linear distributions are approximated by introducing some efficiency parameters.Three carbon nanotube distribution in the matrix,i.***.uniform distribution(U)and V and X-types linear distribution are taken into account.The stability equations are solved by using the Galerkin procedure to determine the combined buckling loads(CBLs)of the structure selected here.The numerical illustrations cover CBLs characteristics of CCSs reinforced by CnTs in the presence of the T-P-EF.Finally,a parametric study is carried out to study the influences of the foundation parameters,the volume fraction of carbon nanotubes and the types of reinforcement on the CBLs.

Coherent-interface-induced strain in large lattice-mismatched materials:A new approach for modeling Raman shift

Strain engineering as one of the most powerful techniques for tuning optical and electronic properties of III-nitrides requires reliable methods for strain investigation.In this work,we reveal,that the linear model based on the experimental data limited to within a small range of biaxial strains(non-destructive Raman study of strain with nanometer-scale spatial resolution is not valid for the binary wurtzite-structure group-III nitrides Gan and Aln.Importantly,we found that the discrepancy between the experimental values of strain and those calculated via Raman spectroscopy increases as the strain in both Gan and Aln increases.Herein,a new model has been developed to describe the strain-induced Raman frequency shift in Gan and Aln for a wide range of biaxial strains(up to 2.5%).Finally,we proposed a new approach to correlate the Raman frequency shift and strain,which is based on the lattice coherency in the epitaxial layers of superlattice structures and can be used for a wide range of materials.
