Postural control quantification in minimally and moderately impaired persons with multiple sclerosis:The reliability of a posturographic test and its relationships with functional ability

Background:Postural control has been associated with the functional impairment in persons with multiple sclerosis(pwMS).However,there is a need for reliable methods to assess postural control in early stages of the disease,when subtle changes can be difficult to detect.The aims of this study were to assess the absolute and relative reliability of a standing and a sitting posturographic protocol in minimally(Expanded Disability Status Scale≤2)and moderately(2.5≤Expanded Disability Status Scale≤4)impaired pwMS,and to analyze relationships among postural control and functional mobility and gait performance.Methods:To assess postural control in an upright stance,14 minimally and 16 moderately impaired pwMS performed six 70 s trials in tandem stance,3 with their weaker leg behind(TSWL)and 3 with their stronger leg behind(TSSL).Additionally,participants completed five 70 s trials using an unstable sitting protocol(US)to assess trunk stability.The mean radial errors of TSWL,TSSL,and US trials were calculated as postural control indexes.Furthermore,participants performed the Timed Up and Go test(TUG)and the Timed 25-foot Walk test(T25FW)to measure their functional mobility and gait speed,respectively.Reliability was evaluated using the intraclass correlation coefficient(ICC3,1)and the standard error of measurement(SEM).Analyses of variances were carried out to assess between-group differences.Hedges’g index(dg)was used to estimate the effect size of differences.Pearson correlation analyses(r)were performed to examine the relationships among the postural control and the functional tests.Results:Posturographic tests showed a high reliability in both minimally(0.87≤ICC≤0.92;9.32%≤SEM≤11.76%)and moderately(0.80≤ICC≤0.92;10.33%≤SEM≤15.33%)impaired pwMS.Similarly,T25FW and TUG displayed a high consistency in minimally(0.89≤ICC≤0.94;3.43%≤SEM≤5.17%)and moderately(0.85≤ICC≤0.93;5.57%≤SEM≤6.56%)impaired individuals.Minimally impaired pwMS showed a better performance on the TUG,T25FW,and TSWL than moderately impaired individuals(p

Arroyo Verde epithermal deposit,in the North Patagonia region of Argentina,is located within the Lower Chon Aike volcanism and is developed between 192.6±2.5 Ma(mineralization host rock)and 189.5±2.6 Ma(overlying lava).Marifil volcanic complex is the host rock for these veins,veinlets and breccias.This particular small deposit has developed low to intermediate sulphidation characteristics,with gangue mineral textures associated with several ore minerals.Electron probe microanalysis determines electrum,silver minerals such as acanthite,and tetrahedrite,related Ag-Cu minerals like jalpaite,mckinstryite,stromeyerite,and base metals like chalcopyrite,sphalerite,galena and pyrite.Fluid inclusions in quartz of two out of five events indicate that these Low Jurassic veins were formed due to subtle boiling and dilution mechanisms and by low-salinity(3.4 wt.%to 6.7 wt.%NaCl Eq)magmatic-hydrothermal fluid at 225 to 310℃.The fluid inclusion measurements that indicate boiling(Event 3)were plotted,and based on this data,we interpret that the Arroyo Verde deposit has been eroded between 550 and 700 m.This particular character has not been previously recorded for other sectors of the eastern north Patagonian region.

Ten-year experience with penile prosthetic surgery for the treatment of erectile dysfunction:outcomes of a tertiary referral center and predictors of early prosthetic infection

We aimed to evaluate ten-year outcomes of penile prosthesis(PP)implantation for the treatment of erectile dysfunction and to assess predictors of early prosthetic infection(EPI).We identified 549 men who underwent 576 PP placements between 2008 and 2018.Univariate and multivariate analyses were used to identify potential predictors of EPI.An EPI predictive nomogram was developed.Thirty-five(6.1%)cases of EPI were recorded with an explant rate of 3.1%.In terms of satisfaction,82.0%of the patients defined themselves as“satisfied,”while partner’s satisfaction was 88.3%.Diabetes(P=0.012),longer operative time(P=0.032),and reinterventions(P=0.048)were associated with EPI risk,while postoperative ciprofloxacin was inversely associated with EPI(P=0.014).Rifampin/gentamicin-coated 3-piece inflatable PP(r/g-c 3IPP)showed a higher EPI risk(P=0.019).Multivariate analyses showed a two-fold higher risk of EPI in diabetic patients,redo surgeries,or when a r/g-c 3IPP was used(all P<0.03).We showed that diabetes,longer operative time,and secondary surgeries were the risk factors for EPI.Postoperative ciprofloxacin was associated with a reduced risk of EPI,while r/g-c 3IPP had higher EPI rates without an increased risk of PP explant.After further validation,the proposed nomogram could be a useful tool for the preoperative counseling of PP implantation.

Lingual mucosal graft urethroplasty 12 years later: Systematic review and meta-analysis

Objective:To evaluate the functional results and complications of the lingual mucosal graft(LMG)urethroplasty and to sum up the current state of the art of this surgical technique.Methods:A systematic search of PubMed and Scopus electronic databases was performed,according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses(PRISMA)statement.Studies involving male patients treated with LMG urethroplasty for urethral stricture were included.Complete protocol is available at http://www.***.york.***.uk/PROSPERO/display_record.php?ID=CRD42017080121.A meta-analysis comparing functional and long-term oral complication outcomes of LMG and buccal mucosal graft(BMG)was performed,calculating the odds ratio(OR)and 95%confidence interval(CI).Results:Twenty original articles were included in the qualitative analysis.Strictures of 1.5e16.5 cm have been treated with LMG urethroplasty,due to the improvement of harvesting technique and very low rate of long-term oral complications.Very good functional results have been reported by different authors for LMG urethroplasty,with lower rate of oral complications than BMG.The meta-analysis included six comparative studies involving 187 and 178 patients treated with LMG and BMG urethroplasty,respectively.An OR of 1.65(95%CI[0.95e2.87],I^2=0%)and 0.18(95%CI[0.03e1.26],I^2=68%)were found for LMG vs.BMG urethroplasty,in terms of success and oral complication rate,respectively.Conclusion:LMG urethroplasty can be reasonably considered a first choice technique for urethral stricture with very good results.Oral complications are temporary and minimally disabling,basically less than those for BMG,and depend mainly on the graft extent.

Early Permian arc-related volcanism and sedimentation at the western margin of Gondwana： Insight from the Choiyoi Group lower section

Permian sedimentary and basic to intermediate volcanic rocks assigned to the Conglomerado del Río Blanco and Portezuelo del Cenizo Formation, lower part of the Choiyoi Group, crop out between the Cordon del Plata, Cordillera Frontal and Precordillera of Mendoza Province, Argentina. The sedimentary rocks are represented by six lithofacies grouped in three facies associations. They were deposited by mantled and gravitational flows modified by high-energy fluvial currents that evolved to low-energy fluvial and lacustrine environments. They constitute the Conglomerado del Río Blanco, which cover unconformably marine Carboniferous sequences. Five volcanic and volcaniclastic facies make up the beginning of volcanic activity. The first volcanic event in the Portezuelo del Cenizo is basaltic to andesitic lava-flows emplaced in the flanks of volcanoes. Lava collapse produced thick block and ash flows.Interbedding in the intermediate volcanic rocks, there are dacites of different geochemical signature,which indicate that the development of acidic volcanism was coetaneous with the first volcanic activity.The geochemistry of these rocks induces to consider that the Choiyoi Group Lower section belongs to a magmatic arc on continental crust. The age of this section is assigned to the lower Permian（277 ？ 3.0Ma, Kungurian age）.

Seismic hazard analysis for central-western Argentina

In this study, we present a PSHA（Probabilistic Seismic Hazard Analysis） for the city of San Juan, which is located in the central-western region of Argentina（30°S-35.5°S; 66.5°W-71°W）. In addition to crustal earthquakes provided by catalogues, recent paleoseismological and neotectonic investigations have permitted to consider events which occurred during the last 400 years.Four seismogenic sources that could cause damages to the studied site corresponding to Precordillera,Western Sierras Pampeanas, Basement of the Cuyana Basin and Cordillera Principal were identified.Based on the evaluation of the contribution of these sources,maximum moment magnitudes above 7.5（Mw） are expected.High values of SA（spectral acceleration）（0.2 and 1 s periods） and PGA（peak ground acceleration） were found in the city of San Juan, which suggests that it is located in a zone of high seismic hazard.Finally, the obtained SA spectra were compared with the seismic-resistant construction standards of Argentina INPRES-CIRSOC 103 [1]. Results suggest that for the city of San Juan and for a return period of475 years, it covers the seismic requirements of the structures.

The interaction between a molecular cloud and an external agent(e.g.,a supernova remnant,plasma jet,radiation,or another cloud)is a common phenomenon throughout the Universe and can significantly change the star formation rate within a galaxy.This process leads to fragmentation of the cloud and to its subsequent compression and can,eventually,initiate the gravitational collapse of a stable molecular cloud.It is,however,difficult to study such systems in detail using conventional techniques(numerical simulations and astronomical observations),since complex interactions of flows occur.In this paper,we experimentally investigate the compression of a foam ball by Taylor–Sedov blast waves,as an analog of supernova remnants interacting with a molecular cloud.The formation of a compression wave is observed in the foam ball,indicating the importance of such experiments for understanding how star formation is triggered by external agents.

Timber-concrete composite bridges:Three case studies

During the last years, timber-concrete composite(TCC) structures have been extensively used in Europe both in new and existing buildings. Generally speaking, a composite structure combines the advantages of both materials employed: the strength and stiffness of the concrete in compression and the tensile strength, lightweight, low embodied energy, and aesthetical appearance of the timber. The concrete slab provides protection of the timber beams from direct contact with water, which is crucial to ensure the durability of the timber beams, particularly when used for bridges. Different types of connectors can be used to provide force exchange between the concrete slab and the timber beam. The choice of a structurally effective yet cheap shear connection between the concrete topping and the timber joist is crucial to make the TCC structures a viable solution that can compete with reinforced concrete and steel structures. In this paper, the possibilities offered by TCC structures for short-span bridge decks are discussed. The technology of TCC structures and the general design rules are illustrated. Three case studies are reported, including a short-span bridge tested in Colorado, USA, with the timber layer being constructed from recycled utility poles and notch connection; a TCC bridge with glulam beams and triangular notches with epoxyglued rebar connectors built in Portugal; and a TCC bridge with glulam beams and rectangular notches built in Germany. All the solutions were found to be structurally effective and aesthetically pleasing. They can all provide a sustainable option for short-span bridges.

The Gondwana Orogeny in northern North Patagonian Massif:Evidences from the Caita Có granite, La Se?a and Pangaré mylonites,Argentina

Structural analyses in the northern part of the North Patagonia Massif, in the foliated Caita Co granite and in La Sefia and Pangare mylonites, indicate that the pluton was intruded as a sheet-like body into an opening pull-apart structure during the Gondwana Orogeny. Geochronological studies in the massif indicate a first, lower to middle Permian stage of regional deformation, related to movements during indentation tectonics, with emplacement of foliated granites in the western and central areas of the North Patagonian Massif. Between the upper Permian and lower Triassic, evidence indicates emplacement of undeformed granitic bodies in the central part of the North Patagonian Massif. A second pulse of deformation between the middle and upper Triassic is related to the emplacement of the Caita CO granite, the development of mylonitic belts, and the opening of the Los Menucos Basin. During this pulse of deformation, compression direction was from the eastern quadrant.
