Background:Sepsis is the leading cause of death in hospitalized patients and significant effort has been made to facilitate early diagnosis and ***,aggressive treatment can have negative effects,especially in patients with unstable volume status,such as those with congestive heart ***:We used electronic medical records to perform a retrospective study looking at hospital outcomes in patients from Southwest Missouri who were admitted with sepsis and had a comorbid diagnosis of hypertensive heart *** primary outcome was in-hospital ***:We studied a total of 184 patients with the diagnosis of hypertensive heart failure with sepsis,and 348 patients with the diagnosis of sepsis that served as the baseline *** was a total of 37(20%)deaths in the hypertensive heart failure group and a total of 94(27%)deaths in the baseline ***:Our study showed no significant difference between the baseline group and those with hypertensive heart disease with heart failure.

Cloud distribution characteristics over the Tibetan Plateau in the summer monsoon period simulated by the Australian Community Climate and Earth System Simulator(ACCESS) model are evaluated using COSP [the CFMIP(Cloud Feedback Model Intercomparison Project) Observation Simulator Package]. The results show that the ACCESS model simulates less cumulus cloud at atmospheric middle levels when compared with observations from CALIPSO and CloudSat, but more ice cloud at high levels and drizzle drops at low levels. The model also has seasonal biases after the onset of the summer monsoon in May. While observations show that the prevalent high cloud at 9–10 km in spring shifts downward to 7–9 km,the modeled maximum cloud fractions move upward to 12–15 km. The reason for this model deficiency is investigated by comparing model dynamical and thermodynamical fields with those of ERA-Interim. It is found that the lifting effect of the Tibetan Plateau in the ACCESS model is stronger than in ERA-Interim, which means that the vertical velocity in the ACCESS model is stronger and more water vapor is transported to the upper levels of the atmosphere, resulting in more high-level ice clouds and less middle-level cumulus cloud over the Tibetan Plateau. The modeled radiation fields and precipitation are also evaluated against the relevant satellite observations.

Habitat suitability models have been used to predict the present and future potential distribution of a variety of species. Eurasian tree sparrowsPasser montanus, native to Eurasia, have established populations in other parts of the world. In North America, their current distribution is limited to a relatively small region around its original introduction to St. Louis, Missouri. We combined data from the Global Biodiversity Information Facility with current and future climate data to create habitat suitability models using Maxent for this species. Under projected climate change scenarios, our models show that the distribution and range of the Eurasian tree sparrow could increase as far as the Pacific Northwest and Newfoundland. This is potentially important information for prioritizing the management and control of this non-native species.

Background: In 2018, the Centers for Medicaid and Medicare Services (CMS) issued a protocol for the treatment of sepsis. This bundle protocol, titled SEP-1 is a multicomponent 3 h and 6 h resuscitation treatment for patients with the diagnosis of either severe sepsis or septic shock. The SEP-1 bundle includes antibiotic administration, fluid bolus, blood cultures, lactate measurement, vasopressors for fluid-refractory hypotension, and a reevaluation of volume status. We performed a retrospective analysis of patients diagnosed with either severe sepsis or septic shock comparing mortality outcomes based on compliance with the updated SEP-1 bundle at a rural community hospital. Methods: Mortality outcome and readmission data were extracted from an electronic medical records database from January 1, 2019, to June 30, 2020. International Classification of Diseases (ICD)-10 codes were used to identify patients with either severe sepsis or septic shock. Once identified, patients were separated into four populations: patients with severe sepsis who met SEP-1, patients with severe sepsis who failed SEP-1, patients with septic shock who met SEP-1, and patients with septic shock who failed SEP-1. A patient who met bundle criteria (SEP-1 criteria) received each component of the bundle in the time allotted. Using chi-squared test of homogeneity, mortality outcomes for population proportions were investigated. Two sample proportion summary hypothesis test and 95% confidence intervals (CI) determined significance in mortality outcomes. Results: Out of our 1122 patient population, 437 patients qualified to be measured by CMS criteria. Of the 437 patients, 195 met the treatment bundle and 242 failed the treatment bundle. Upon comparing the two groups, we found the probable difference in mortality rate between the met(14.87%) and failed bundle(27.69%) groups to be significant(95% CI: 5.28-20.34, P = 0.0013). BACKGROUND Italy retains a distinctive organization of mental health services according to a community-based model of care with a multidisciplinary team serving a welldefined catchment area under the coordination of the local department of mental *** coronavirus disease 2019(COVID-19)pandemic is forcing Italian mental health services to develop new organizational strategies at all levels of care in order to face the associated *** To explore factors associated with changes in psychiatric admissions to an inpatient psychiatric unit located in Lombardia Region,*** All hospital admissions(n=44)were recorded to an inpatient psychiatric unit during a three month national lockdown in Italy in 2020 and compared with those occurring over the same time period in 2019(n=71).For each admission,a 20-item checklist was completed to identify factors leading to *** analyses were performed using Statistical Package for Social Sciences for Windows,release ***-square test(or Fisher’s exact test)and Mann-Whitney U-test were applied,where *** Hospital admissions dropped by 38%during the COVID-19 *** significant differences were found in demographics,clinical variables associated with hospital admissions and length of stay between 2019 and *** with 2019,a significantly greater proportion of hospital admissions in 2020 were related to difficulties in organizing care programs outside the hospital(chi-square=4.91,df 1,one-way P=0.035)and in patients’family contexts(chi-square=3.71,df 1,one-way P=0.049).On the other hand,logistic and communication difficulties pertaining to residential facilities and programs were significantly more common in 2019 than in 2020(chi-square=4.38,df 1,one-way P=0.032).CONCLUSION Admissions to the inpatient psychiatric unit dropped significantly during the COVID-19 pandemic in 2020,with difficulties in organizing care programs outside the hospital and in patients’family contexts

High pressure, radon bearing water has been identified as one of the most critical challenges in mining the high-grade uranium deposit at the Mc Arthur River Operation, Cameco Corporation. The ore deposits are located between 490 and 640 m below the surface and surrounded by water bearing Athabasca sandstone, a graphitic P2 fault zone, and highly altered ground. This paper introduces the inflow risk management program at Mc Arthur River Operation, which includes various hydrogeological challenges and the corresponding strategies applied, such as risk-based probe and grout programs(geological, hydrogeological, and geotechnical), ground freezing programs, and comprehensive ground control programs. These programs have been developed, tested, and proven successful over years of mining practices. Working with this world class deposit of high risk and low tolerance, it is believed that these experiences might be beneficial to other mining operations with similar hydrogeological characteristics.

An acoustic based instrument, the ABS Acoustic Bubble Spectrometer@@（ABS）, was investigated for the detection and quantification of bubbles in biological media. These include viscoelastic media （blood）, materials of varying density （bone in tissue）, non-homogenous distribution of bubbles （intravenous bubbly flow）, and bubbles migrating in tissue （decompression sickness, DCS）. The performance of the ABS was demonstrated in a series of laboratory experiments. Validation of the code was performed using a viscoelastic polymer solution, Polyox, in which the bubble size distribution and void fraction were determined by ABS measurements and with image analysis of high speed videos. These tests showed that the accuracy of the ABS was not significantly affected by viscoelasticity for bubbles smaller than 200 microns. The ABS detection and measurement of non-homogenous bubble distributions was demonstrated using a bubbly flow through a simulated vein surrounded by tissue. The scatter of acoustic signals due to bones in the acoustic pathway was also investigated. These in-vitro experiments were done using meat （beef） as a tissue simulant. Decompression experiments were done using beef meat which was held underwater at high pressure （9.9 atm） then rapidly decompressed. Bubble size distributions and void fraction calculations in these experiments were then validated using image analysis of high speed video. In addition, preliminary experiments were performed with the US Navy Medical Research Center, demonstrating the utility of the modified ABS system in detecting the evolution of bubbles in swine undergoing decompression sickness （DCS）. These results indicate that the ABS may be used to detect and quantify the evolution of bubbles in-vivo and aid in the monitoring of DCS.
