Comprehensive data from in situ and laboratory tests in residual soil have been presented by Zhang et al.(2023).A number of issues addressed in the paper have been the interest of the discussers,namely the characterisation and behaviour of residual soils,limitation of piezocone testing due to the capacity of the entire system,measurement of shear wave velocity,rate effect of piezocone(CPTU)testing and piezocone testing with dual pore pressure *** and complementation of these issues are required with regard to both the execution and interpretation of the tests.

AIM: To report a case of severe idiopathic gastroparesis in complete absence of Kit-positive gastric interstitial cells of Cajal (ICC). METHODS: Gastric tissue from a patient with severe idiopathic gastroparesis unresponsive to medical treatment and requiring surgery was analyzed by conventional histology and immunohistochemistry. RESULTS: Gastric pacemaker cells expressing Kit receptor had completely disappeared while the local level of stem cell factor, the essential ligand for its development and maintenance, was increased. No signs of cell death were observed in the pacemaker region. CONCLUSION: These results are consistent with the hypothesis that a lack of Kit expression may lead to impaired functioning of ICC. Total gastrectomy proves to be curative.

AIM： The utility of serum alpha-fetoprotein （α-FP） in the detection of hepatocellular carcinoma （HCC） is questionable. Very high circulating levels of nociceptin/ orphanin FQ （N/OFQ）, a ligand for a novel opioid receptor, have recently been reported in HCC. The aim of this study was to assess the role of plasma N/OFQ in the diagnosis of HCC arising in patients with liver cirrhosis. METHODS： Plasma N/OFQ levels were measured by ELISA in 58 patients （28 HCC and 30 liver cirrhosis） and in 25 healthy controls. The values were correlated with clinical and laboratory features including α-FP. Spearman index, biserial correlation coefficient, non parametric combination （NPC） test and discriminant stepwise analysis were used for statistical evaluation of data.RESULTS： The upper normal limit of nociceptin was 122 pg/mL. Plasma levels above this cut-off were found in 21.4% of patients with HCC, in 23.3% of those with cirrhosis and in 8% of healthy subjects. α-FP serum levels〉200 ng/mL were found in 46.4% of the patients with HCC and in none of those with cirrhosis. No correlation was found between N/OFQ levels and any of the clinical and laboratory features, including α-FP. By NPC test, HCC and cirrhotic patients were different with regard to α-FP （P = 0.000） but not in terms of nociceptin （P = 0.595）. By point biserial correlation, HCC presence was positively correlated with α-FP （rpb = 0.52, P = 0.000） but not with N/OFQ （rpb = 0.16, P = 0.157）. In a discriminant analysis, α-FP was significant in the Wilks test （Y = -0.709 ＋ 0.03 α-FP） and properly classified 81% of all patients and 61% of HCC. N/OFQ had lower sensitivity, specificity and predictive values than α-FP. CONCLUSION： Nociceptin is increased in patients with chronic liver disease, independently of the presence of HCC, although the underlying mechanism has yet to be clarified. We conclude it is not a useful marker for HCC.

Objective:To evaluate in vitro effects of Tagetes minuta L. essential oil(TEO) on L3 Anisakis larvae type 1. Methods:In order to evaluate the potential use of Tagetes minuta essential oil against L3 Anisakis larvae three different media were tested:1) a saline solution(SS); 2) an industrial marinating solution; 3) sunflower seeds oil(SO). For each media and concentrations of TEO(0.1%,0.5%,1.0% and 5.0% v/v),20 parasites were introduced into plastic Petri dishes(diameter 90 mm) and maintained at room temperature. As controls,larvae were maintained without TEO under identical experimental conditions in SS,MS and SO. A total of 900 larvae were tested. The normalized mean viability,LT100,LT50 and the percentage of inactivation at 24 hours were calculated. Results:In vitro tests revealed a complete inactivation of parasites in saline solution after 2 hours with 5% and 1% of TEO. In marinating solution,a complete inactivation of parasites was observed after 4 hours at all concentrations used. A slower activity for all TEO concentration was reported in SO. Conclusions:The results obtained,showing a strong activity against Anisakis larvae,confirm TEO as a larvicidal agent in the treatment of human anisakidosis and in the industrial marinating process.

Serum chromogranin-A in hepatocellular carcinoma: Diagnostic utility and limits

AIM: The utility of serum alpha-fetoprotein (α-FP) for the detection of hepatocellular carcinoma (HCC) is questionable. High serum levels of chromogranin-A (CgA) have recently been reported in HCC. Impaired hepatic, renal, and heart functions influence circulating CgA. The aim of this study was to assess sensitivity and specificity of serum CgA as a marker of HCC in patients with liver cirrhosis (LC). METHODS: Serum CgA levels were measured by RIA in 339 patients of which 54 HCC, 132 LC, 45 chronic hepatitis (CH), 27 chronic heart failure (CHF), 36 chronic renal failure (CRF), 45 chronic inflammatory bowel disease (IBD) as disease controls and in 75 healthy controls. Patients with liver disease or IBD and concomitant renal and/or heart failure were excluded. Pearson correlation, non-parametric combination test and confidence interval analysis were used for statistical analysis. RESULTS: Serum CgA above normal values (100 ng/mL) were found in 83% of HCC patients, in 48% of LC patients, in 20% of CH patients, in 33% of IBD patients, in 92% of CRF patients, in 100% of CHF patients, and in none of the healthy controls. The mean CgA values in HCC (769±1046), in LC (249±369), in CH (87±94), in CRF (1390±1401), in CHF (577±539), in IBD (146±287) were significantly higher than those in healthy controls (48±18). HCC patients had higher CgA values (P200 ng/mL in 21% of the HCC patients and in none of the LC patients. No significant correlation was found between a-FP and CgA in patients with HCC and in patients with cirrhosis. CONCLUSION: When HCC is suspected and a-FP

Unsuspected imported malaria in a case of sudden infant death

Here we describe the case of a 4-mo-old female who died suddenly without any apparent cause that was initially mistaken as a case of sudden infant death syndrome. Histologic observation of brain sections revealed blue-black bodies in erythrocytes of the blood vessels, suggestive of specific stages of the hematic schizogonic cycle. Further examinations revealed hemozoin and hemosiderin deposits in the parenchyma of all organs, leading to the diagnosis of malaria by Plasmodium falciparum(P. falciparum). The death occurred in Italy, the native country of the infant, two weeks after a Christmas holiday spent in Pakistan, the parents' birthplace, which has a high malarial endemicity. As this case demonstrates, the diagnosis of malaria should always be considered as a differential diagnosis in subjects, including infants, that die unexpectedly after returning from P. falciparum endemic areas.

Effects of cigarette smoking on the evolution of hearing loss caused by industrial noise

The few studies evaluating the changes caused by cigarette smoking on hearing loss induced by occupational exposure to noise have reached discordant conclusions. The aim of this study is to investigate the interactions between cigarette smoking and occupational exposure to noise as risk factors in the onset and development of hearing loss. The study was performed on a sample of 557 shipyard workers exposed to noise at an Equivalent Level (Leq) of 93 dBA. On the basis of their smoking habits, they were divided into three groups: group (A), non-smokers;group (B), smokers (15-30 cigarettes per day);and group (C), heavy smokers (over 30 cigarettes per day). The study focussed on the audiometric responses of the subjects at the frequencies of 500, 1000, 2000, 3000 and 4000 Hz. The results were then compared using statistical techniquees (Internal correlation coefficient, exponential model, ANCOVA, NPC test). Comparison of the audiometric responses showed statistically significant differences between the three groups. Non-parametric analysis, performed using the NPC test, highlighted that the interaction between smoking and exposure to noise has an influence on hearing loss at all frequencies, and particularly at high frequencies (3000-4000 Hz). The data obtained from the examined sample show that smoking and exposure to noise cause an increase in occupational hearing loss and that this is directly related to the number of cigarettes smoked.
