Clinical impact of selective transarterial chemoembolization on hepatocellular carcinoma:A cohort study

AIM： To prospectively long term clinical impact evaluate the short and of selective transarterial chemoembolization （TACE） on fiver function in patients with hepatocellular carcinoma （HCC）. To assess side effects in relation to treatments. To analyze the overall survival and HCC progression free survival probability. METHODS： One hundred and seventeen cirrhotic patients with HCC were enrolled. Baseline liver function included Child-Pugh score and serum levels of alanine- aminotransferase （ALT）, prothrombin time （PT） and bilirubin. According to Cancer Liver Italian Program （CLIP） and Barcelona Clinic Liver Cancer （BCLC） staging systems, 71 patients were eligible for TACE; 32 had previously received treatment for HCC. No significant differences in liver function were observed between previously treated and not treated patients. TACE was performed by selective catheterization of the arteries nourishing the lesions. While hospitalized, patients underwent clinical, hematologic and ultrasonographic assessments. One month after TACE a CT scan was performed to assess tumor response. A second TACE was performed ＂on demand＂ Liver function tests were checked in all patients every four months. RESULTS： After first TACE, the mean Child-Pugh score increased from a mean baseline 5.62 ±1.12 to 6.11 ±1.57 at discharge time （P 〈 0.0001）, decreasing after four months to 5.81 ± 0.73 （not significant）. ALT, PT and bilirubin significantly （P 〈 0.0001） increased 24 h after TACE and progressively decreased until discharge. After the second TACE, variations in Child-Pugh score, ALT, PT and bilirubin were comparable to that described after the first TACE. No major complications were observed. The mean follow-up was 14.7 ＋ 6.3 mo （median： 16 mo）. Only one patient died. No other patient experienced important long term worsening of clinical status. The overall survival probability at twenty-four months was 98.18% with a correspondent HCC progression free survival probability of 69%. CONCLUSION：

Assessment of clinical and radiological response to sorafenib in hepatocellular carcinoma patients

Sorafenib is an effective anti-angiogenic treatment forhepatocellular carcinoma(HCC). The assessment of tumor progression in patients treated with sorafenib is crucial to help identify potentially-resistant patients,avoiding unnecessary toxicities. Traditional methods to assess tumor progression are based on variations in tumor size and provide unreliable results in patients treated with sorafenib. New methods to assess tumor progression such as the modified Response Evaluation Criteria in Solid Tumors or European Association for the Study of Liver criteria are based on imaging to measure the vascularization and tumor volume(viable or necrotic). These however fail especially when the tumor response results in irregular development of necrotic tissue. Newer assessment techniques focus on the evaluation of tumor volume,density or perfusion. Perfusion computed tomography and Dynamic ContrastEnhanced-UltraS ound can measure the vascularization of HCC lesions and help predict tumor response to antiangiogenic therapies. Mean Transit Time is a possible predictive biomarker to measure tumor response. Volumetric techniques are reliable,reproducible and time-efficient and can help measure minimal changes in viable tumor or necrotic tissue,allowing the prompt identification of non-responders. Volume ratio may be a reproducible biomarker for tumor response. Larger trials are needed to confirm the use of these techniques in the prediction of response to sorafenib.

In the last decade trans-arterial radioembolization has given promising results in the treatment of patients with intermediate or advanced stage hepatocellular carcinoma(HCC),both in terms of disease control and tolerability *** technique consists of the selective intra-arterial administration of microspheres loaded with a radioactive compound(usually Yttrium90),and exerts its therapeutic effect through the radiation carried by these microspheres.A careful and meticulous selection of patients is crucial before performing the radioembolization to correctly perform the procedure and reduce the incidence of *** is a technically complex and expensive technique,which has only recently entered clinical practice and is supported by scant results from phase Ⅲ clinical ***,it may represent a valid alternative to transarterial chemoembolization(TACE) in the treatment of intermediate-stage HCC patients,as shown by a comparative retrospective assessment that reported a longer time to progression,but not of overall survival,and a more favorable safety profile for *** addition,this treatment has reported a higher percentage of tumor shrinkage,if compared to TACE,for pre-transplant downsizing and it represents a promising therapeutic option in patients with large extent of disease and insufficient residual liver volume who are not immediately eligible for *** might also be a suitable companion to sorafenib in advanced HCC or it can be used as a potential alternative to this treatment in patients who are not responding or do not tolerate sorafenib.

Pin-on-disc study of dry sliding behavior of Co-free HVOFcoated disc tested against different friction materials

The dry sliding behavior of three commercial friction materials(codenamed FM1,FM2,and FM3)tested against a Co‐free cermet coating produced by high‐velocity oxy‐fuel(HVOF)on gray cast‐iron discs is ***1 is a conventional low‐metallic friction material,FM2 is developed for using against HVOF‐coated discs,and FM3 is a Cu‐free friction material with a low content of abrasives and a relatively high concentration of steel *** the tribological evaluation,they are tested on a pin‐on‐disc(PoD)test rig against Co‐free HVOF‐coated discs,with particular attention to the running‐in stage,which is fundamental for the establishment of a friction layer between the two mating surfaces,i.e.,the pin and *** PoD tests are performed at room temperature(RT)and a high temperature(HT)of 300℃.At RT,all materials exhibit a long running‐in *** HT,no running‐in is observed in FM1 and FM2,whereas a shorter running‐in period,with respect to the RT case,is observed in FM3 followed by the attainment of a comparatively high coefficient of *** RT,the pin wear is mild in all cases but severe at ***3 shows the lowest wear rate at both ***,the coated disc shows no wear when sliding against the FM3 friction *** the results are interpreted considering the microstructural characteristics of the friction layers formed on the sliding *** findings of the present study provide insights into reducing wear in braking system components and hence reducing environmental particulate matter emissions from their wear,through the use of disc coatings.

Bacteriophages(hence termed phages)are viruses that target bacteria and have long been considered as potential future treatments against antibiotic-resistant bacterial ***,the molecular nature of phage interactions with bacteria and the human host has remained elusive for decades,limiting their therapeutic *** many phages and their functional repertoires remain unknown,the advent of next-generation sequencing has increasingly enabled researchers to decode new lytic and lysogenic mechanisms by which they attack and destroy ***,the last decade has witnessed a renewed interest in the utilization of phages as therapeutic vectors and as a means of targeting pathogenic or commensal bacteria or inducing ***,the narrow host range,immense antibacterial repertoire,and ease of manipulating phages may potentially allow for their use as targeted modulators of pathogenic,commensal and pathobiont members of the microbiome,thereby impacting mammalian physiology and immunity along mucosal surfaces in health and in microbiomeassociated *** this review,we aim to highlight recent advances in phage biology and how a mechanistic understanding of phage-bacteria-host interactions may facilitate the development of novel phage-based *** provide an overview of the challenges of the therapeutic use of phages and how these could be addressed for future use of phages as specific modulators of the human microbiome in a variety of infectious and noncommunicable human diseases.

Characterization of airborne wear debris produced by brake pads pressed against HVOF-coated discs

A significant fraction of the non-exhaust particulate matter emissions from vehicular traffic comprises fine particles from the wear debris of brake pads and *** studies have shown that these emissions can be consistently reduced by using wear resistant disc *** study thoroughly analyses the debris produced by a low-met brake pad,which is dyno-bench tested against both cast iron and WC-CoCr-coated brake *** achieve this,particles in the size range of 2.5|im to 30 nm were collected and *** results showed a consistent reduction in the particle emission as well as in the concentration of iron oxides,which are mainly released from the disc tribo-oxidation in the coated ***,a few tungsten carbides,released from the coating,were also observed in the wear *** results of this study can be useful for improving the protective coating and consequently help in reducing particulate matter emission further.

Machine learning hydrogen adsorption on nanoclusters through structural descriptors

背景:组织工程是一个多学科研究的交叉学科,其目标是使人体损伤的组织和器官再生,通过这种假设,几乎所有的动物组织都可以在实验室进行培养。一般的方法是从需要移植的患者身上提取干细胞,在一定的支持条件下允许其生长、增殖、生产为可替换的组织。另一方面,寻找细胞能够互相联结并形成分层结构的合适的支持条件,如基质或支架等非常必要。目前用于烧烫伤治疗的材料有很多种,如胶原,透明质酸、纤维蛋白和聚乳酸及其共聚物。目的:讨论以新型的生物材料自体血浆为支架对自体成纤维细胞和角质化细胞生长、扩张、增殖的影响。设计:建立一种真皮再生的方法,将自体成纤维细胞浸于人血浆基质中,排除各种影响样本安全性和排斥反应的问题,应用同样的方法在新的真皮上获取角质化细胞。时间及地点:实验于 2008 年在意大利曼多瓦 C. Poma 医院动物工厂完成。材料:人角质化细胞和成纤维细胞取自 1 例58 岁乳房切除患者的皮肤碎片。实验得到C. Poma 医院独立伦理委员会批准,患者知情同意。方法:从自体活组织皮肤样本中分离人角质化细胞和成纤维细胞,置于培养瓶中增殖,随后将自体血浆作为皮肤移植形成构造的支架,免疫和免疫组织化学特征显示与正常皮肤相似。主要观察指标:将血浆样本用甲醛固定,埋入石蜡,苏木精-伊红染色,用显微镜进行细胞计数。所有样本均经免疫组织化学评估。结果:在血浆基质上获得了多层规则形状的角质化细胞和成纤维细胞,基底膜的形成说明自体血浆是角质化细胞和成纤维细胞的生长、分化、扩展的良好支架,真皮和表皮细胞联结重建皮肤移植与正常皮肤无异。结论:血浆作为角质化细胞和成纤维细胞分化、扩展的支架表现出良好的性能,同时实验还特别发现成纤维细胞浓缩血浆可以暂时替代皮肤,也是角质化细胞生长的强有力的支架。此外血浆还具有廉价,易于制备等优点。自体角质化细胞和成纤维细胞及自体血浆的应用,可以避免血种类型的免疫排斥反应。这种治疗方法给有慢性缺损并且需要连续移植的患者带来了希望。

Anderson-Hynes dismembered ureteropyeloplasty has been the gold standard surgical treatment for ureteropelvic junction obstruction (UPJO) caused either by crossing renal vessel or by a stenotic junction in children. Nowadays it is still discussed which could be the best surgical approach. All the techniques actually used have the goal to improve functional outcome and to reach better results in terms of reducing traumatic damage, postoperative pain and therefore reduction of hospitalization. We are presenting our experience in the treatment of UPJO by open dismembered pyeloplasty with a minimal invasive approach using the Alexis®(Applied Medical, Rancho Santa Margherita, CA) autostatic wound retractor.
