Upfront consolidation treatment with 131I-mIBG followed by myeloablative chemotherapy and hematopoietic stem cell transplantation in high-risk neuroblastoma

Importance:131I-metaiodobenzylguanidine(131I-mIBG)has a significant targeted antitumor effect for neuroblastoma.However,currently there is a paucity of data for the use of 131I-mIBG as a"front-line"therapeutic agent in those patients with newly diagnosed high-risk neuroblastoma as part of the conditioning regimen for myeloablative chemotherapy(MAC).Objective:To evaluate the feasibility of upfront consolidation treatment with 131I-mIBG plus MAC and hematopoietic stem cell transplantation(HSCT)in high-risk neuroblastoma patients.Methods:A retrospective,single-center study was conducted from 2003-2019 on newly diagnosed high-risk neuroblastoma patients without progressive disease(PD)after the completion of induction therapy.They received 131I-mIBG infusion and MAC followed by HSCT.Results:A total of 24 high-risk neuroblastoma patients were enrolled with a median age of 3.0 years at diagnosis.After receiving this sequential consolidation treatment,3 of 13 patients who were in partial response(PR)before 131I-mIBG treatment achieved either complete response(CR)(n=1)or very good partial response(VGPR)(n=2)after HSCT.With a median follow-up duration of 13.0 months after 131I-mIBG therapy,the 5-year event-free survival and overall survival rates estimated were 29%and 38%for the entire cohort,and 53%and 67%for the patients who were in CR/VGPR at the time of 131I-mIBG treatment.Interpretation:Upfront consolidation treatment with 131I-mIBG plus MAC and HSCT is feasible and tolerable in high-risk neuroblastoma patients,however the survival benefit of this 131I-mIBG regimen is only observed in the patients who were in CR/VGPR at the time of 131I-mIBG treatment.

Liver malignancy,including primary liver cancer and metastatic liver cancer has become one of the most common causes of cancer-related death worldwide due to the high malignant degree and limited systematic treatment strategy.Radioembolization with yttrium-90(^(90)Y)-loaded microspheres is a relatively novel technology that has made significant progress in the local treatment of liver malignancy.The different steps in the extensive work-up of radioembolization for patients with an indication for treatment with^(90)Y microspheres,from patient selection to follow up,both technically and clinically,are discussed in this paper.It describes the application and development of^(90)Y microspheres in the treatment of liver cancer.

AIM:To investigate the effect of Scutellariae Radix extract(SRE)on ulcerative colitis(UC)in rats induced by dextran-sulfate sodium(DSS).METHODS:Colitis was induced in male Sprague-Dawley(SD)rats(170-180 g)by 4% dextran sulfate sodium(DSS,wt/v;MW 54 000)in drinking water for 8 d.The treated rats received 4% DSS and SRE orally(100 mg/kg per day).Control rats received either tap water or SRE only.Macroscopic assessment which included body weight changes,fecal occult blood and stool consistency were determined daily.At the appointed time,the rats were sacrificed and the entire colons were removed.The colon length and the myeloperoxidase(MPO)activity were measured.The severity of colitis was graded by morphological and histological assessments.The ion transport activity of the colonic mucosa was assessed by electrophysiological technique.RESULTS:Rats treated with oral administration of 4% DSS regularly developed clinical and macroscopic signs of colitis.Treatment with SRE relieved the symptoms,including the reduction in body weight,shortening and ulceration of the colon.Administration of SRE also significantly reduced the histological damage induced by DSS.Moreover,the ISC responses of the colonic mucosato forskolin were suppressed after the induction of colitis.The stimulated ion transport activity of DSS-rats treated with SRE displayed significant improvement in the secretory responsiveness.CONCLUSION:SRE was effective in treating acute DSS-induced ulcerative colitis,as gauged by reduced clinical disease,improved macroscopic and histological damage scores,and enhanced recovery of normal colonic secretory function.

Diagnostic delay and associated factors among patients with pulmonary tuberculosis in Dar es Salaam,Tanzania

Background:Tanzania is among the 30 countries with the highest tuberculosis(TB)burdens.Because TB has a long infectious period,early diagnosis is not only important for reducing transmission,but also for improving treatment outcomes.We assessed diagnostic delay and associated factors among infectious TB patients.Methods:We interviewed new smear-positive adult pulmonary TB patients enrolled in an ongoing TB cohort study in Dar es Salaam,Tanzania,between November 2013 and June 2015.TB patients were interviewed to collect information on socio-demographics,socio-economic status,health-seeking behaviour,and residential geocodes.We categorized diagnostic delay into≤3 or>3 weeks.We used logistic regression models to identify risk factors for diagnostic delay,presented as crude(OR)and adjusted Odds Ratios(aOR).We also assessed association between geographical distance(incremental increase of 500 meters between household and the nearest pharmacy)with binary outcomes.Results:We analysed 513 patients with a median age of 34 years(interquartile range 27-41);353(69%)were men.Overall,444(87%)reported seeking care from health care providers prior to TB diagnosis,of whom 211(48%)sought care>2 times.Only six(1%)visited traditional healers before TB diagnosis.Diagnostic delay was positively associated with absence of chest pain(aOR=7.97,95%confidence intervals[CI]:3.15-20.19;P<0.001),and presence of hemoptysis(aOR=25.37,95%CI:11.15-57.74;P<0.001)and negatively associated with use of medication prior to TB diagnosis(aOR=0.31,95%CI:0.14-0.71;P=0.01).Age,sex,HIV status,education level,household income,and visiting health care facilities(HCFs)were not associated with diagnostic delay.Patients living far from pharmacies were less likely to visit a HCF(incremental increase of distance versus visit to any facility:OR=0.51,95%CI:0.28-0.96;P=0.037).Conclusions:TB diagnostic delay was common in Dar es Salaam,and was more likely among patients without prior use of medication and presenting with hemoptysis.Geographical distance to HCFs may ha

Ultrafast laser-scanning time-stretch imaging at visible wavelengths

Optical time-stretch imaging enables the continuous capture of non-repetitive events in real time at a line-scan rate of tens of MHz—a distinct advantage for the ultrafast dynamics monitoring and high-throughput screening that are widely needed in biological microscopy.However,its potential is limited by the technical challenge of achieving significant pulse stretching(that is,high temporal dispersion)and low optical loss,which are the critical factors influencing imaging quality,in the visible spectrum demanded in many of these applications.We present a new pulse-stretching technique,termed free-space angular-chirpenhanced delay(FACED),with three distinguishing features absent in the prevailing dispersive-fiber-based implementations:(1)it generates substantial,reconfigurable temporal dispersion in free space(41 ns nm^(−1))with low intrinsic loss(o6 dB)at visible wavelengths;(2)its wavelength-invariant pulse-stretching operation introduces a new paradigm in time-stretch imaging,which can now be implemented both with and without spectral encoding;and(3)pulse stretching in FACED inherently provides an ultrafast all-optical laser-beam scanning mechanism at a line-scan rate of tens of MHz.Using FACED,we demonstrate not only ultrafast laser-scanning time-stretch imaging with superior bright-field image quality compared with previous work but also,for the first time,MHz fluorescence and colorized time-stretch microscopy.Our results show that this technique could enable a wider scope of applications in high-speed and high-throughput biological microscopy that were once out of reach.

Profiles and removal efficiency of polycyclic aromatic hydrocarbons by two different types of sewage treatment plants in Hong Kong

Sewage discharge could be a major source of polycyclic aromatic hydrocarbons （PAHs） in the coastal waters. Stonecutters Island and Shatin Sewage Treatment Works （SCISTW and STSTW） in Hong Kong, adopted chemically enhanced primary treatment and biological treatment, respectively. This study aimed at （1） determining the removal efficiencies of PAHs, （2） comparing the capabilities in removing PAHs, and （3） characterizing the profile of each individual PAHs, in the two sewage treatment plants （STPs）. Quantification of 16 PAHs was conducted by a Gas Chromatography. The concentrations of total PAHs decreased gradually along the treatment processes （from 301 ± 255 and 307 ± 217 ns/L to 14.9 ± 12.1 and 63.3 ± 54.1 ng/L in STSTW and SCISTW, respectively）. It was noted that STSTW was more capable in removing total PAHs than SCISTW with average total removal effidency 94.4% ± 4.12% us. 79.2% ± 7.48% （p 〈 0.05）. The removal of PAHs was probably due to sorption in particular matter, confirmed by the higher distribution coefficient of individual and total PAHs in solid samples （dewatered sludge contained 92.5% and 74.7% of total PAHs in SCISTW and STSTW, respectively） than liquid samples （final efffluent-total contained 7.53% and 25.3% of total PAHs in STSTW and SCISTW, respectively）. Despite the impressive capability of STSTW and SCISTW in removing PAHs, there was still a considerable amount of total PAHs （1.85 and 39.3 ks/year, respectively for the two STPs） being discharged into Hone Kon coastal waters, which would be an environmental concern.

Lymphoproliferative disorders in inflammatory bowel disease patients on immunosuppression: Lessons from other inflammatory disorders

Immunosuppressive agents, such as thiopurines, methotrexate, and biologics, have revolutionized the treatment of inflammatory bowel disease(IBD). However, a number of case reports, case control studies and retrospective studies over the last decade have identified a concerning link between immunosuppression and lymphoproliferative disorders(LPDs), the oncological phenomenon whereby lymphocytes divide uncontrollably. These LPDs have been associated with Epstein-Barr virus(EBV) infection in which the virus provides the impetus for malignant transformation while immunosuppression hampers the immune system's ability to detect and clear these malignant cells. As such, the use of immunosuppressive agents may come at the cost of increased risk of developing LPD. While little is known about the LPD risk in IBD, more is known about immunosuppression in the post-transplantation setting and the development of EBV associated posttransplantation lymphoproliferative disorders(PTLD). In review of the PTLD literature, evidence is available to demonstrate that certain immune suppressants such as cyclosporine and T-lymphocyte modulators in particular are associated with an increased risk of PTLD development. As well, high doses of immunosuppressive agents and multiple immunosuppressive agent use are also linked to increased PTLD development. Here,we discuss these findings in context of IBD and what future studies can be taken to understand and reduce the risk of EBV-associated LPD development from immunosuppression use in IBD.

FGK 22 μm excess stars in lamOST DR2 stellar catalog

Since the release of the Large Sky Area Multi-Object Fiber Spectroscopic Telescope（lamOST）catalog, we have had the opportunity to use the lamOST DR2 stellar catalog and the WISE All-Sky Data Release catalog to search for 22 μm excess candidates. In this paper, we present 10 FGK candidates which show an excess in the infrared at 22 μm. All the 10 sources are newly identified 22 μm excess candidates.Of these 10 stars, five stars are F type and five stars are G type. The criterion for selecting candidates is ks[22].387. In addition, we present the spectral energy distributions covering wavelengths from the optic-≥0al to mid-infrared band. Most of them show an obvious excess from the 12 μm band and three candidates even show excess from 3.4 μm. To characterize the amount of dust, we also estimate the fractional luminosity of10 22 μm excess candidates.
