Enhanced Adherence Counselling, Support Groups and Viral Load Suppression amongst HIV-Positive Adolescents in a Tertiary Health Care Facility in Cameroon

Background: Globally, HIV viral load suppression rate, which is an indirect measure of the efficacy of antiretroviral (ART) medication, is 47% and 52% in Africa. In Cameroon, the viral load (VL) suppression rate is 44.7% and poor adherence is widely documented as being responsible for the large gap in VL Suppression. Enhanced adherence counselling (EAC) sessions, and enrolment and participation in support groups are specific interventions to improve ART adherence and improve viral load suppression. Purpose: This study assesses the uptake and contribution of support groups and EAC sessions in the management of adolescents with unsuppressed VL results at Centre Hospitalier d’Essos, Yaounde. Methods: A retrospective correlational quantitative patient files review was conducted for 138 files of HIV positive adolescents aged between 10 - 19 years with HIV VL above 1000 copies/ml enrolled in care between January 2009 and December 2019. Data from the questionnaire was entered into CSPRO version 7.4. and analyzed by using SPSS version 25.0. Results: A total of 138 participants (75 females and 63 males) with a mean age of 15 ± 3 years were included in our study. Sixty-nine (50%) participants were in World Health Organization (WHO) stage I;32.6% were in Stage II;13.0% and 4.3% were in stages III and IV, respectively. Thirty (21.7%) had a history of tuberculosis and 76% of the adolescents were being cared for primarily by their parents. The charts of the adolescents revealed that there was an association between completion of EAC sessions in adolescents with unsuppressed VL and eventual VL suppression (R.R = 2.5;CI 0.848 - 6.162;p = 0.033). However, there was no significant association between support group enrolment and active participation, and eventual VL Suppression. Furthermore, combining EAC and support group interventions was strongly associated with eventual VL Suppression in this group of initially unsuppressed adolescents (R.R = 7.5;C.I 2.544 - 22.360;p Conclusion: S

Eco-ableism and access circularity in natural building

The climate crisis disproportionately impacts disabled people. Yet climate-related advocacy, planning, and policymaking often neglect to thoughtfully include disabled people. Responding to this gap, disabled and neurodivergent environmental activists coined the term eco-ableism to describe discrimination and silencing toward disabled and neurodivergent people (i.e., ableism) arising in environmental spaces (i.e., eco-ableism). Relatedly, building operations and construction practices contribute a significant percentage of global, energy-related CO_(2) emissions annually, which calls into question the relationships between the impending climate crisis, disability justice, and architecture. Climate-specific, natural building materials and methods present a potential pathway toward a more sustainable built future: low-carbon, locally sourced, minimally processed, and nontoxic materials. Despite a critical overlap, there is little published research on material access in the production phase and human access in the occupation phase of natural buildings. Applying eco-ableism and material circularity in an architectural framework, this research aims to investigate the gaps and possibilities of access, natural material applications, and resulting US natural buildings informed by scholarship in critical disability studies and semi-structured interviews with natural building professionals.

Acute Colonic Pseudo-Obstruction (ACPO): An Expanding Colon with Unusual Risk Factors

The distention of the colon without mechanical or anatomical obstruction, Acute Colonic Pseudo-obstruction (ACPO), is a common condition occurring in the critically ill. ACPO in the setting of an acute pulmonary embolism and embolic stroke is a rarity. A 76-year-old female with shortness of breath, left hemiparesis and right-sided paresthesias presented with acute pulmonary embolism and acute infarcts of the left caudate nucleus, thalamus and occipital lobe. Her hospitalization was complicated with persistent distention of the large bowel without dilation of the small bowel. Empiric antibiotics were initiated without improvement and laboratory studies including Clostridium difficile were negative. She underwent nasogastric decompression and two decompressive colonoscopies with a resolution of her symptoms. This case illustrates an example of acute abdominal distension, without underlying etiology, in the setting of acute embolism of the pulmonary and cerebral vasculature. Early identification and action with decompressive colonoscopy were key to preventing further bowel damage or rupture.

Contraceptive behaviors and awareness in an urban slum community of south India

In this study, we explore the application of ACP (asymptotic curve based and proportionality oriented) Alpha Beta (αβ) Nonlinear Math to analyze arithmetic and radiation transmission data. Specifically, we investigate the relationship between two variables. The novel approach involves collecting elementary “y” data and subsequently analyzing the asymptotic cumulative or demulative (opposite of cumulative) Y data. In part I, we examine the connection between the common linear numbers and ideal nonlinear numbers. In part II, we delve into the relationship between X-ray energy and the radiation transmission for various thin film materials. The fundamental physical law asserts that the nonlinear change in continuous variable Y is negatively proportional to the nonlinear change in continuous variable X, expressed mathematically as dα = −Kdβ. Here: dα {Y, Yu, Yb} represents the change in Y, with Yu and Yb denoting the upper and baseline asymptote of Y. dβ {X, Xu, Xb} represents the change in X, with Xu and Xb denoting the upper and baseline asymptote of X. K represents the proportionality constant or rate constant, which varies based on equation arrangement. K is the key inferential factor for describing physical phenomena.
