Facial Cellulitis of Dental Origin: Experiments from the University of bouakéHealth Centre (Cote d’Ivoire)

Context: Facial cellulitis of dental origin is relatively frequent and severe. Objective: The aim of this document to describe the epidemiological, clinical, therapeutic and evolutionary aspects of facial cellulitis of dental origin in bouaké. Methodology: This was a retrospective and descriptive study carried out in the stomatology and maxillofacial surgery department of the University of bouaké Health centre over a period of 19 months (January 2018 to October 2019). All patients with facial cellulitis of dental origin were included. The parameters studied were epidemiological, clinical, therapeutic and evolutionary. Results: 179 patients were collected (hospital prevalence of 20.79%). The average age was 34 years (min 1-year-old and max 80-year-old). The sex ratio was 1.18. Among the patients, some were craftsmen (31.28%) and others were farmers (21.79%). The favourable factors included the nonsteroidal anti-inflammatory (77.65%) and traditional therapeutics (44.13%). The average number of days before consultation was 13 days (min 1 day and max 75 days). There were 7 cases of chronic cellulitis (3.91%) and 172 cases of acute cellulitis, including 107 (62.21%) circumscribed cellulitis, 51 (29.65%) diffused cellulitis, and 14 (8.14%) necrotizing fasciitis. The causal lesion was tooth decay (96.09%) and dental avulsion without antibiotherapy (3.91%). Incision and drainage was made in 145 patients (81%) necrosectomy surgery in 54 patients (30.17%). The mortality rate was 13.96%. Conclusion: Facial cellulitis of dental origin are clinically polymorphic with significant mortality prompting increased dental decay prevention actions.

Aim: Describe the epidemiological, clinical and therapeutic particularities of craniofacial trauma by encornment. Type of study: This was a retrospective and descriptive study. Material and methods: It took place in the surgical emergency departments of bouaké (Côte d’Ivoire) University Hospital from January 2013 to December 2017, for a period of 5 years. It focused on patients of 0 to 5 years old admitted for craniofacial trauma involving the horn of a bovine. Results: Of the 26 cases of traumatic encornment, 11 were craniofacial location (42%). There were 9 male and 2 female victims. The average age was 10.54 years with extremes of 6 months and 24 years. Patients came from rural areas of Côte d’Ivoire. bovids were responsible for all causal encornements. The lesions involved the scalp (3 cases) and the face (8 cases). The lesions were cutaneous-musculo skeletal in 7 cases. The surgical treatment included debridement associated to a suture in all cases and associated with maxillo mandibular blockage in 3 cases and duro-cranioplasty in one case. Conclusion: In the local context, facial cranio traumatism by encornement is rare and seems to be essentially infantile, masculine and rural problem.

Management of Congenital Malformations of the Face in the GbèkèRegion (Côte d’Ivoire)

background: Congenital malformations of the face are relatively frequent in the Gbèke region (Côte d’Ivoire). Objectives: Identify the epidemiological aspects, describe the management method, distinguish the anatomical-clinical aspects and identify the therapeutic methods and their results. Material and methods: A retrospective and descriptive study was carried out over a period of 8 years (2012-2019) in the Odontostomatology and Maxillofacial Surgery Department of the University Hospital Centre (bouaké) and in the “Jean baptiste Vatelot” religious hospital of bouaké. The study concerned patients whose congenital malformations of the face had undergone primary surgery. Results: 439 patients were collected, i.e. an annual frequency of 54.88 cases per year. The sex ratio was 1.07 and the mean age was 6.82 years (range 5 months to 48 years). There were 434 (98.86%) cleft lips and palates and 5 (1.14%) Tessier’s clefts. Among the cleft lip and palate, there were 214 (49.31%) cleft lip, 127 (29.26%) cleft palate and 93 (21.43%) combined cleft lip and cleft palate. Surgical treatments numbered 532 and included 293 (55.08%) cheiloplasties, 113 (21.24%) pushback uranostaphyloraphies and 90 (16.92%) double flap palatoplasties. Complications were noted in 43 cases. Conclusion: Congenital malformations of the face were managed relatively late in the Gbèkè region with satisfactory results. Ante-natal diagnosis, early psychotherapy and a better screening system should allow an improvement of the management.

A Hybrid Model for Improving Software Cost Estimation in Global Software Development

Accurate software cost estimation in Global Software Development(GSD)remains challenging due to reliance on historical data and expert *** models,such as the Constructive Cost Model(COCOMO II),rely heavily on historical and accurate *** addition,expert judgment is required to set many input parameters,which can introduce subjectivity and variability in the estimation ***,there is a need to improve the current GSD models to mitigate reliance on historical data,subjectivity in expert judgment,inadequate consideration of GSD-based cost drivers and limited integration of modern technologies with cost *** study introduces a novel hybrid model that synergizes the COCOMO II with Artificial Neural Networks(ANN)to address these *** proposed hybrid model integrates additional GSD-based cost drivers identified through a systematic literature review and further vetted by industry *** article compares the effectiveness of the proposedmodelwith state-of-the-artmachine learning-basedmodels for software cost *** the NASA 93 dataset by adopting twenty-six GSD-based cost drivers reveals that our hybrid model achieves superior accuracy,outperforming existing *** findings indicate the potential of combining COCOMO II,ANN,and additional GSD-based cost drivers to transform cost estimation in GSD.

Exploring factors of disaster preparedness in UNESCO-designated heritage sites

Context-aware reinforcement learning for cooling operation of data centers with an Aquifer Thermal Energy Storage

Data centers are often equipped with multiple cooling units. Here, an aquifer thermal energy storage (ATES) system has shown to be efficient. However, the usage of hot and cold-water wells in the ATES must be balanced for legal and environmental reasons. Reinforcement Learning has been proven to be a useful tool for optimizing the cooling operation at data centers. Nonetheless, since cooling demand changes continuously, balancing the ATES usage on a yearly basis imposes an additional challenge in the form of a delayed reward. To overcome this, we formulate a return decomposition, Cool-RUDDER, which relies on simple domain knowledge and needs no training. We trained a proximal policy optimization agent to keep server temperatures steady while minimizing operational costs. Comparing the Cool-RUDDER reward signal to other ATES-associated rewards, all models kept the server temperatures steady at around 30 °C. An optimal ATES balance was defined to be 0% and a yearly imbalance of −4.9% with a confidence interval of [−6.2, −3.8]% was achieved for the Cool 2.0 reward. This outperformed a baseline ATES-associated reward of 0 at −16.3% with a confidence interval of [−17.1, −15.4]% and all other ATES-associated rewards. However, the improved ATES balance comes with a higher energy consumption cost of 12.5% when comparing the relative cost of the Cool 2.0 reward to the zero reward, resulting in a trade-off. Moreover, the method comes with limited requirements and is applicable to any long-term problem satisfying a linear state-transition system.

Adverse Events following AstraZeneca COVID-19 Vaccination: A Case Study in Abidjan, Côte d’Ivoire

Introduction: Pharmaceutical companies have boosted vaccine production following the global COVID-19 pandemic. In Côte d’Ivoire, the first vaccination campaign with the AstraZeneca vaccine began on March 1, 2021, as part of the Covax program. Despite the positive benefit/risk balance, the adverse effects of vaccination should not be minimized. Objective: To identify adverse events of AstraZeneca’s COVID-19 vaccination for better management. Materials and Methods: This is a case of a 57-year-old obese (bMI = 39 kg/m2) female health care worker who experienced adverse events in March 2021 after the second dose of AstraZeneca vaccine administered 4 weeks apart. These were subject to mandatory case reporting. Results: Major post-vaccination events occurred in a noisy systemic picture with parameters showing significant disturbances. biological surveillance remains costly and makes the accountability of the vaccine complex. Conclusion: Vaccination remains the ultimate weapon in the fight against endemic diseases but should not overshadow the reporting of adverse events.
