Anemia in the Elderly: Frequency and Etiologies at University Hospital Center of Point G

Introduction: Older people are exposed to particular health problems that must be taken into account, including anemia defined by the World Health Organization (WHO) in people aged 65 and over hemoglobin < 13 g/dl in men and < 12 g/dl in women. Objectives: To determine the frequency of anemia, to describe its clinical and paraclinical aspects, and to determine the etiologies. Material and Methods: We conducted a cross-sectional study with prospective data collection that took place in the Department of Internal Medicine of the Point G CHU in Mali from January 1st, 2018 to August 31st, 2018, covering all elderly patients. At least 65 years old hospitalized and/or having consulted and presenting with anemia. Anemia was defined as Hb < 13 g/dl in men and < 12 g/dl in women (WHO definition). Biological markers were collected at admission. Results: Anemia was observed in 42 patients in 160 elderly patients, a prevalence of 26%, the age of our patients ranged from 65 to 90 years. The sex ratio was 1.33. Fifty percent (50%) of the patients had high blood pressure as antecedents. Weight loss was found in 69% of cases;47.6% of patients had dyspnea on admission. Tachycardia was the physical sign associated with anemia present in 85.7% of them. The mean hemoglobin level was 9.2 g/dl ± 2.03: thus 50% of the subjects had microcytic and or hypochromic anemia. The etiologies found were infectious in 31.0% of cases, inflammatory in 31.0% of cases, renal in 14.2% of cases. Chronic hemorrhage, acute haemorrhage, hemolysis accounted for 2.4% each. We observed 9.5% of unexplained causes. Conclusion: The frequency of anemia is high. Nearly half of the geriatric population has anemia. The lack of means and the nonperformance of the technical platform to perform all examinations in the etiological research of anemia have a significant impact on the rate of unexplained anemias.

The unique topographic,climatic,and latitudinal heterogeneity of Mesoamerica is crucial to understanding the evolutionary emergence of one of the richest cultural and biological regions in the planet and its impact on food production.Broadly divided between two regions of large biodiversity-lowland tropical or coastal zones and temperate or subtropical highlands sculpted by parallel sierras and their central plateau-the Mexican territory harbors more than 2000 endemic monocotyledonous species,including several hundred grasses(Poaceae),particularly those belonging to the genus Zea,composed of maize(Zea mays ssp.mays)and its wild relatives,collectively referred as teosintes.

Evaluation of Rhizobium tropici-Derived Extracellular Polymeric Substances on Selected Soil Properties, Seed Germination, and Growth of Black-Eyed Peas (Vigna unguiculata)

Rhizobium tropici-derived extracellular polymeric substances (EPS) have been used in soils to enhance soil structures and mitigate soil erosions. However, information on their use to improve soil health and fertility indicators, and plant growth is limited. In a greenhouse study, we investigated their effects on some soil health, soil fertility indices, and the growth of black-eyed peas (Vigna unguiculate). Results showed that soils incubated with EPS significantly increased basal soil respiration, soil microbial biomass, permanganate oxidizable carbon (POC), and potentially mineralizable nitrogen (PMN). The EPS shifted microbial populations from bacteria to fungi and Gram (−ve) to Gram ( ve) bacteria. However, it had little or no effects on soil pH, soil organic matter (SOM), and cation exchange capacity (CEC). The EPS decreased soil moisture loss, increased soil aggregate stability, but delayed blacked-eyed peas germinations in the soils. At 0.1% (w/w) concentrations in soils, there was increase in plant root nodulations and vegetative growth. This study was carried out within 40 days of incubating soils with EPS or growing the black-eyed peas in a greenhouse study. The plant growth parameters were taken before flowering and fruiting. Further studies of the effects of incubating soils with the extracellular polymeric substances on plant growth. Soil microbial biomass, microbial diversities, and other soil fertility indices are deemed necessary.

Background and objective: Classically, diabetic subjects are at high risk of anaesthesia compared with general population. However, some recent publications have shown contrasting and sometimes contrary results. The aim of our study was to evaluate morbidity and mortality during and after anaesthesia in patients with versus without diabetes operated on at Monkole Hospital over the last ten years. Methods: Retrospective cohort study including all patients who underwent all-comers surgery excluding cardiac surgery between 2011 and 2021. Each diabetic patient was matched to 2 non-diabetic controls on age and sex. The evaluation criterion was the frequency of occurrence of at least one perioperative complication and/or death up to day 30. A multivariate analysis using a Cox model was used to determine the factors associated with the occurrence of this morbidity and mortality. The model was adjusted for comorbidities, preoperative hyperglycaemia, ASA score, type of anaesthesia and severity of surgery. Results: A total of 351 diabetic patients (mean age 53.3 ± 14.18 years) and 701 non-diabetic patients (mean age 53.52 ± 14.7 years) were included and analysed. Preoperatively, hyperglycaemia (blood glucose > 180 mg/dl) was observed in 24.3% of diabetic patients compared with 1.6% of non-diabetic patients. The incidence of overall perioperative complications was 25.6% in diabetic patients compared with 28.6% in non-diabetic patients (p = 0.27). The risk factors associated with this morbidity were general anaesthesia with oro-tracheal intubation vs loco-regional anaesthesia (OR = 3.06 [95%CI: 1.91 - 4.94];p Conclusion: This study shows that there is not significant increase in perioperative morbidity and mortality in diabetic patients compared with non-diabetic ones of similar severity. These results suggest that diabetes itself (excluding associated comorbidities) has only a minor impact on perioperative morbidity and mortality.

Can preoperative planning using IRIS™three-dimensional anatomical virtual models predict operative findings during robot-assisted partial nephrectomy?

Objective To evaluate the predictive validity of IRIS™(Intuitive Surgical®,Sunnyvale,CA,USA)as a planning tool for robot-assisted partial nephrectomy(RAPN)by assessing the degree of overlap with intraoperative execution.Methods Thirty-one patients scheduled for RAPN by four experienced urologists were enrolled in a prospective study.Prior to surgery,urologists reviewed the IRIS™three-dimensional model on an iphone Operating System(iOS)app and completed a questionnaire outlining their surgical plan including surgical approach,and ischemia technique as well as confidence in executing this plan.Postoperatively,questionnaires assessing the procedural approach,clinical utility,efficiency,and effectiveness of IRIS™were completed.The degree of overlap between the preoperative and intraoperative questionnaires and between the planned approach and actual execution of the procedure was analyzed.Questionnaires were answered on a 5-point Likert scale and scores of 4 or greater were considered positive.Results Mean age was 65.1 years with a mean tumor size of 27.7 mm(interquartile range 17.5-44.0 mm).Hilar tumors consisted of 32.3%;48.4%of patients had R.E.N.A.L.nephrometry scores of 7-9.On preoperative questionnaires,the surgeons reported that in 67.7%cases they were confident that they can perform the procedure successfully,and on intraoperative questionnaires,the surgeons reported that in 96.8%cases IRIS™helped achieve good spatial sensation of the anatomy.There was a high degree of overlap between preoperative and intraoperative questionnaires for the surgical approach,interpreting anatomical details and clinical utility.When comparing plans for selective or off-clamp,the preoperative plan was executed in 90.0%of cases intraoperatively.Conclusion A high degree of overlap between the preoperative surgical approach and intraoperative RAPN execution was found using IRIS™.This is the first study to evaluate the predictive accuracy of IRIS™during RAPN by comparing preoperative plan and intraoperative execution.

This paper is concerned first with the behaviour of differences T（t） - T（s） near the origin, where （T（t）） is a semigroup of operators on a Banach space, defined either on the positive real line or a sector in the right half-plane （in which case it is assumed analytic）. For the non-quasinilpotent case extensions of results in the published literature are provided, with best possible constants; in the case of quasinilpotent semigroups on the half-plane, it is shown that, in general, differences such as T（t） -T（2t） have norm approaching 2 near the origin. The techniques given enable one to derive estimates of other functions of the generator of the semigroup; in particular, conditions are given on the derivatives near the origin to guarantee that the semigroup generates a unital algebra and has bounded generator.

Background:Daily moderate-to-vigorous physical activity(MVPA)is vital to the physical,mental,and social well-being of children.Early restrictions during the coronavirus disease 2019(COVID-19)pandemic included the closure of schools and physical activity(PA)amenities across the US.This study aimed to examine the impact of the pandemic on the PA and play behavior of U.S.children and to provide evidencebased recommendations to improve their PA.Methods:A cross-sectional,online,parent-reported survey was conducted of children aged 318 years between April and June 2020 to assess light PA and MVPA using a modified Godin Leisure-Time Exercise Questionnaire.Additional items included family/child socioeconomic demographics,child adaptability to the pandemic,and community access.The survey was shared through social media and snowball sampling distribution.Results:Analysis of 1310 surveys indicated child PA scores declined significantly during the pandemic(from 56.6 to 44.6,max 119,p<0.001).Specifically,MVPA score decreased(from 46.7 to 34.7,max 98,p<0.001)while light PA remained the same.Age-based changes were seen in the quantity,variety,and intensity of PA,with the lowest pandemic-related impact seen in preschoolers and the highest in high schoolers(4.7 vs.17.2,p<0.001).Community-based peer PA decreased across all age groups.Conclusion:This study shows decreased PA levels in U.S.children,according to parent reporting,during the COVID-19 pandemic.Recommendations for community leaders,educators,and parents to improve PA in children are provided.With continued spread of COVID-19,these results and recommendations may be imperative to the physical well-being of U.S.children.

A Comparative Structural Study of Southern Region Shallow Basement of the North-Kivu Province (DR Congo) by Gravity and Magnetic Data Analysis

The aim of this study is to characterize the subsoil in the southern region of the North-Kivu province (DR Congo). Gravity and geomagnetic data were used in this study. Five different filters—the horizontal gradient magnitude, the analytic signal, the tilt derivative, the horizontal derivative of tilt derivative and the tilt angle of horizontal gradient—enabled us to delineate the gravity and magnetic anomaly sources present in the shallow subsurface of the study area. The plains of the Rutshuru territory are dominated by sources of weak gravity anomalies and sources of very weak magnetic anomalies located almost in the same places. The southern part of Rutshuru territory and a large part of Masisi territory are underlain by shallow sources of high gravity and magnetic anomalies. Gravity and magnetic anomaly sources are almost identical in the study area. The shallow sources of gravity and magnetic anomalies encountered in our study area are more or less linear and connected. The numerous gravity and magnetic lineaments present in our study region have three major directions: oriented East-West, North-South and North-East-South-West.

Structural Study of the Kakobola Area and Its Surroundings by Detecting the Edges of Gravity Anomaly Sources

The area covered by this study is the county of Kakobola and its surroundings. Previous studies show that those related to the study of depths by the gravity method, using other techniques, are not always carried out until now. The main goal of this article is the gravimetric characterization of our area by other approach. The interest is not only to map the lineaments and to know their dip, but also to estimate the depths of these different anomalies. The methods used for this study are the first total horizontal derivative (FTHDT), tilt angle (TA), analytical signal (AS) and horizontal gradient magnitude (HGM). The processing of the complete Bouguer anomalies (CBA) data was done mainly through software. Data analysis using the semi-finished body depth method shows depths ranging from 7.49 m to 224.6 m. Data analysis using the AS method shows values ranging from 41.7 mGal/m to 510 mGal/m. The fractures and/or geological contacts in our study area show dips ranging from -73.73° to 68.16° and North-South orientation according to the tilt angle method. The FTHDT shows several lineaments, a NE oriented fracture of Kakobola and low dip values which suggest a tabular structure of the subsurface in our study area. According to the HGM, the study area shows several preferential directions of fractures and/or geological contacts whose the most frequent directions are the NNE-SSW and WNW.

This study focuses on the Kakobola region and its surroundings where cavities discovered in its basement may represent a major risk for the hydroelectric dam erected on the Lufuku River near the Kakobola city and the civil engineering works in the study area. In order to deepen the studies related to this understudied region and provide decision-makers with information that will enable them to make the necessary and appropriate decisions regarding the development of this area, a study based on the analysis of geomagnetic data was carried out using certain methods revealing more shallow than deep structures, and others highlighting the limits of both shallower and deeper structures. Total magnetic anomalies and reduced to equator (RTE) magnetic anomalies were used to map the subsurface of the Kakobola region and its surroundings. In order to detect the edges of magnetized structures, the horizontal gradient magnitude (HGM), the analytic signal (AS), the horizontal gradient of tilt angle (HGTA), the tilt angle (TA), the theta map (TM), the enhanced total horizontal derivative of the tilt angle (ETHDR), the tilt angle of the horizontal gradient (TAHG), and the tilt angle of analytic signal (TAAS) were used. The study area is characterized by two areas of low values of magnetic anomalies and two other sources of high magnetic anomalies located in the bed and the neighborhood of the two major rivers in the region. The shallow sources of magnetic anomalies are lying in the bed and the vicinity of the same rivers in the study area. The magnetic sources in the study area are connected and almost linear. Several magnetic lineaments identified in this region by different methods present several preferential directions, but the most predominant directions are NE-SW, NW-SE, W-E and NE-SW.
