Factors affecting technical efficiency of small-scale apple farms in Balochistan Plateau, Pakistan

In Balochistan, Agriculture is dominated by small-scale farmers and the apple production sector is confronted with the chronic constraint of low output per unit of input. Although some agricultural policies have been implemented in this province, studies on effectiveness and impacts of such policies over apple production and farmers' performance are still limited. This study is an effort to bridge the research gap on this potential crop in the plateau by investigating technical and scale efficiency of 181 officially designated small-scale apple farmers in mountainous district Mastung in Balochistan; then explored factors that underlie differences in production inefficiency. A two-stage input-oriented Data Envelopment Analysis(DEA) methodology was employed to evaluate technical and scale efficiency followed by truncated bootstrapped regression framework to analyze the correlative determinants to efficiency. Average technical and scale efficiencies of larger landholders were higher than medium and small landholders. Analysis indicated that there is still ample scope for inefficient sample farmers to reduce the input use by 33% without compromising the given yield level. Maximum contributions to the total input savings were from the use of urea, farm yard manure(FYM) and labor-use. Farmer's experience, farm ownership, irrigation constraints, and farmer's access to trainings, credit and extension services significantly affected apple farmers' technical efficiency. Based on the findings of this study, strategies are suggested to enhancing efficiencies by farmers' capacity development through effective extension services and trainings and provision of adequate credit.

A Study to Observe Pulse Pressure Variation after Induction with Propofol for General Anesthesia

Background and Aims: Pulse pressure variation (PPV) is a reliable and predictive dynamic parameter presently being utilized for fluid responsiveness. In the operating room, fluid administration based on PPV monitoring helps the physician in deciding whether to volume resuscitate or use interventions in patients undergoing surgery. Propofol is an intravenous induction agent which lowers blood pressure. There are multiple causes such as depression in cardiac output, and peripheral vasodilatation for hypotension. We undertook this study to observe the utility of PPV as a guide to fluid therapy after propofol induction. Primary outcome of our study was to monitor PPV as a marker of fluid responsiveness for the hypotension caused by propofol induction. Secondary outcome included the correlation of PPV with other hemodynamic parameters like heart rate (HR), systolic blood pressure (SBP), and diastolic blood pressure (DBP);after induction with propofol at regular interval of time. Methods: A total number of 90 patients were recruited. Either of the radial artery was then cannulated under local anaesthesia with 20G VygonLeadercath arterial cannula and invasive monitoring transduced. A baseline recording of heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP) and PPV was then recorded. Patients were then induced with predetermined doses of propofol (2 mg/kg) and recordings of HR, SBP, DBP, and PPV were taken at 5, 10 and 15 minutes. Results: Intraoperatively, PPV was significantly higher at 5 minutes and significantly lower at 15 minutes after induction. It was observed that there were no statistically significant correlations between PPV and SBP or DBP. PPV was strongly and directly associated with HR. Conclusion: We were able to establish that PPV predicts fluid responsiveness in hypotension caused by propofol induction;and can be used to administer fluid therapy in managing such hypotension. A Comparative Study between Landmark Based and Real Time Ultrasound Guided Sub Arachnoid Block

Background: Sub arachnoid block (SAB) performed by traditional landmark palpation technique can be inaccurate. This problem is exacerbated by altered patient anatomy due to obesity and age-related changes. A pre-procedural ultrasound scan of the lumbar spine has been shown to be of benefit in guiding lumbar epidural insertion in obstetric patients. Information on the use of real-time ultrasound (RUS) guided SAB, to date, been limited. This study compared RUS guided SAB to traditional landmark guided technique in patients undergoing spinal anesthesia for different surgical procedures. Methods: This was a prospective, single center, comparative observational study conducted in the department of anesthesiology at our center. 560 patients who underwent spinal anesthesia either by landmark based technique or real-time ultrasound-guided methods. The primary outcome was the first attempt success rate of dural puncture when employing the two methods. Results: Baseline characteristics were similar in the two study groups. The first attempt success rate of dural puncture in landmark guided group was 64.3% compared to 72.6% in the ultrasound guided group. This difference was not statistically significant. The procedure performance time was significantly shorter with landmark palpation compared to use of real-time ultrasound guided method. Conclusion: Use of RUS-guided technique does not significantly improve the first attempt success rate of SAB dural puncture during spinal anesthesia compared to the traditional landmark-guided technique.

Comparison of efficacy of lung ultrasound and chest X-ray in diagnosing pulmonary edema and pleural effusion in ICU patients: A single centre, prospective, observational study

Background and Aims While chest X-ray (CXR) has been a conventional tool in intensive care units (ICUs) to identify lung pathologies, computed tomography (CT) scan remains the gold standard. Use of lung ultrasound (LUS) in resource-rich ICUs is still under investigation. The present study compares the utility of LUS to that of CXR in identifying pulmonary edema and pleural effusion in ICU patients. In addition, consolidation and pneumothorax were analyzed as secondary outcome measures. Material and Methods This is a prospective, single centric, observational study. Patients admitted in ICU were examined for lung pathologies, using LUS by a trained intensivist;and CXR done within 4 hours of each other. The final diagnosis was ascertained by an independent senior radiologist, based on the complete medical chart including clinical findings and the results of thoracic CT, if available. The results were compared and analyzed. Results Sensitivity, specificity and diagnostic accuracy of LUS was 95%, 94.4%, 94.67% for pleural effusion;and 98.33%, 97.78%, 98.00% for pulmonary edema respectively. Corresponding values with CXR were 48.33%, 76.67%, 65.33% for pleural effusion;and 36.67%, 82.22% and 64.00% for pulmonary edema respectively. Sensitivity, specificity and diagnostic accuracy of LUS was 91.30%, 96.85%, 96.00% for consolidation;and 100.00%, 79.02%, 80.00% for pneumothorax respectively. Corresponding values with CXR were 60.87%, 81.10%, 78.00% for consolidation;and 71.3%, 97.20%, 96.00% for pneumothorax respectively. Conclusion LUS has better diagnostic accuracy in diagnosis of pleural effusion and pulmonary edema when compared with CXR and is thus recommended as an effective alternative for diagnosis of these conditions in acute care settings. Our study recommends that a thoracic CT scan can be avoided in most of such cases.

Agricultural Extension in Balochistan, Pakistan:Date Palm Farmers' Access and Satisfaction

This study analyzed information gathered from 200 date palm households in the mountainous Panjgur District of Balochistan in Pakistan with the aim of determining date farmers＇ access to and satisfaction with agricultural extension services. Information was collected through a structured questionnaire survey, group discussions and semi- structured personal interviews. The findings of the analysis revealed very poor access to extension services. The results of logistic regression analysis identified five variables out of six which significantly influence date farmers＇ access to extension services： household bead＇s age, household bead＇s literacy, number of date palm trees owned, mostly inherited date palm trees, and percentage of dead date palm trees. The overwhelming majority of farmers were dissatisfied with all three main types of extension services being provided by public extension officials. Explanations for this as well as for associations between access to extension services and the six independent variables are provided. Important policy conclusions are drawn based on the findings.

An Efficient Frontier for Sum Deviation JIT Sequencing Problem in Mixed-model Systems via Apportionment

In this paper, the sum deviation just-in-time （JIT） sequencing problem in mixed-model production systems is studied relating with the discrete apportionment problem together with their respective mathematical formulations. The assignment formulation for the first problem is briefly discussed followed by the existence of JIT cyclic sequences. Presenting the concise discussion on divisor methods for the discrete apportionment problem, we have proposed two mean-based divisor functions for this problem claiming that they are better than the existing divisors; hence, we found a better bound for the JIT sequencing problem. The linkage of both the problems is characterized in terms of similar type of objective functions. The problems are shown equivalent via suitable transformations and similar properties. The joint approaches for the two problems are discussed in terms of global and local deviations proposing equitably efficient solution.

Effects of conservation measures on crop diversity and their implications for climate-resilient livelihoods: the case of Rupa Lake Watershed in Nepal

Agrobiodiversity conservation is vital for achieving sustainability, but empirical studies on the effects of different practices or measures on crop diversity are rare. This study aims to estimate the effects of raising conservation awareness(RCA),building diversity blocks(BDB), and their combination on crop diversity among 240 randomly selected households surrounding the Rupa Lake Watershed in Nepal. Based on descriptive analysis and multiple regression models, the results indicate that the two single measures had no significant effect on the numbers of crop species and varieties grown by households in 2018. However, the combination of RCA and BDB had a significantly positive effect on the number of crop varieties, especially for grain and vegetable crops. Considering that these crops are essential in the daily lives of local people, the results indicate that a strategy that combines both awareness raising and on-farm conservation measures can generate higher crop diversity and better serve the climate-resilient livelihoods of people in mountainous areas.

Yarsagumba(Ophiocordyceps sinensis), an endemic species to the Himalayas and Tibetan Plateau, is one of the most valuable medicinal mushrooms in the world. In Nepal, it is distributed largely in isolated patches of alpine grasslands of 3,000-5,000 m elevation. Although it is reported from 27 northernmost districts of Nepal, the local distribution pattern of this species is largely ***, the collection system and local management regime of this species are not well documented. We conducted a field survey at Dhorpatan Hunting Reserve(DHR) among the different stakeholders in January-June 2012 to assess collection sites, patterns and trends and to understand the management regime. We estimated that about 75 kg of Yarsagumba is collected every year from DHR and the amount has been declining since 2008. To manage the resource, locals have initiated regulating the collection by issuing permits, taxing to the collectors, and monitoring the activities of harvesters with the help of park authorities. The revenue generated at local level from the permits has been used for community developmental activities.

Analysis of Fatigue Strain, Fatigue Modulus and Fatigue Damage for the Model Formulation of Concrete Based on Strain Life Approach

Analysis of fatigue strain, fatigue modulus and fatigue damage for the modeling of concrete plays a vital role in the evolution material behaviour which is heterogeneous and anisotropic in nature. The Level-S nonlinear fatigue strain curve, fatigue modulus curve, residual strain curve of concrete in compression, tension, flexure and torsional fatigue loading were proposed using strain life approach. The parameters such as physical meaning, the ranges, and the impact on the shape of the curve were discussed. Then, the evolution model of fatigue modulus was established based on the fatigue strain, fatigue modulus, residual strain and secondary strain evolution model. The hypothesis of fatigue modulus is inversely related with the fatigue strain amplitude. The fatigue evolution of concrete damages the bond between material grains, changed the orientation of structure of molecules and affects the elastic properties resulting in the reduction of material stiffness and modulus by formation of microcracking, macro cracking, cracking and finally damage. This paper presents the fatigue strain life model and analysis of fatigue strain, fatigue modulus and damage parameters of concrete which is capable of predicting stiffness degradation, inelastic deformation, and strength reduction under fatigue loading and experimental results were employed for the validation of the theoretical model.
