Percutaneous nephrostomy versus ureteral stent in hydronephrosis secondary to obstructive urolithiasis:A systematic review and meta-analysis

objective: To assess if there is a preferable intervention between retrograde ureteral stent (RUS) and percutaneous nephrostomy (PCN) tube, in cases of upper urinary tract stone obstruction with complications requiring urgent drainage, by evaluating outcomes regarding urinary symptoms, quality of life (QoL), spontaneous stone passage, and length of hospital stays, since there is no literature stating the superiority of one modality over the ***: We searched MEDLINE and other sources for relevant articles in June 2019 without any date restrictions or filters applied. The selection was done first by the title and abstract screening and then by full-text assessment for eligibility. only randomized controlled trials or cohort studies in patients with hydronephrosis secondary to obstructive urolithiasis that presented comparative data between PCN and RUS placement concerning at least one of the defined outcome measures were included. Lastly, MEDLINE database and PubMed platform were screened again using the same terms, from June 2019 until November ***: of 556 initial articles, seven were included in this review. Most works were considered of moderate-to-high quality. Three studies regarding QoL showed a tendency against stenting, even though only one demonstrated statistically significant negative impact on overall health state. Two works reported significantly more post-intervention urinary symptoms in stenting patients. one article found that PCN is a significant predictor of spontaneous stone passage, when adjusted for stone size and location. Findings on length of hospital stays were not consistent a.ong ***: PCN appears to be the intervention better tolerated, with less impact on the patient’s perceived QoL and less post-operative urinary symptoms, in comparison with RUS. Nevertheless, further studies with larger samples and a randomized controlled design are suggested.

Evaluation of the Efficiency of the Magicramp^® Device for Reducing Cramps Resulting from oncological Treatments: “Double-Blind, Randomized Clinical Trial”

We identified that oncological treatments in general (chemotherapies, immunotherapies and radiotherapies) frequently cause peripheral neuropathy, including cramps, characterized by excess protons due to meta.olic and neuronal factors, such as sudden changes in pH, uremia and aspects that affect neuromotor functions. Such situations and others like them are often neglected in treatment, which naturally concerns itself with the main problem: Cancer. Sometimes toxic solutions are implemented that have comorbid side effects, such as duloxetine (standard treatment). Based on monitoring of cancer patients who used the non-toxic product, called “Magicramp® Electrostatic Charge Reduction Cushion” (MECRC), approved in Europe more than 10 years a.o, we carried out a controlled test in Brazil. In this clinical trial, we hypothesized that reducing excessive ionic charges (electrostatic charge), which is one of the side effects often described in the literature as “Chemotherapy-Induced Peripheral Neuropathy” (CIPN), would decrease or eliminate cramping, under the hypothesis that such elimination would prevent or attenuate muscular vulnerability to action impulses, and increase the power of relaxation through the same mechanism. In this double-blind and randomized clinical trial, 40 (forty) adult patients with muscle cramps caused by oncological treatments were tested, evaluating the degree of efficiency of the product that aims to reduce muscle cramps, by eliminating and/or reducing excess loads electrostatic ionic. Dear editor,Vulvar sarcomas are rare and represent 1%–3%of cancers in this region.1 Leiomyosarcoma is the most common type of vulvar *** contrast,epithelioid sarcoma(ES)is extremely rare,accounting for a.ound 1%of soft-tissue vulvar *** the ESs of the vulva,proximal variants tend to have a more aggressive clinical course,in comparison to distal variants.2 The treatment of choice in proximal-type ES of the vulva is surgical resection,usually with surgical margins greater than 2 *** to the extreme rarity of these malignancies,clear algorithms for adjuvant treatment of these tumors have still not been *** and/or chemotherapy may be indicated in some cases.

Implantosuported Rehabilitation after Marginal Recession of Mandibular Calcifying Epithelial odontogenic Tumor: Case Report

The Calcifying Epithelial odontogenic Tumor (CEoT) also called the Pindborg’s Tumor represents a rare benign epithelial odontogenic neoplasm with locally aggressive behavior. Surgical excision with a safety margin is one of the treatment options, requiring bone and soft tissue reconstruction for implant-supported rehabilitation, providing the individual with a return to adequate oral functions. The objective of the present study is to report on the case of the treatment stages and the factors involved in the rehabilitation of a 23-year-old man with a history of mandibular lesion diagnosed as CEoT. A marginal resection of the mandible lesion was performed, with posterior reconstruction with iliac crest bone graft and soft tissue graft to allow the implant-supported rehabilitation. Currently, in the postoperative control, the individual shows no signs of tumor recurrence and is rehabilitated without functional and/or aesthetic complaints. Surgical excision should be framed as a form of treatment, aiming for better resolution in the more complex cases of these invasive tumors.

Giant Sia.olith with 20 Years of Evolution in the Submandibular Duct

Sia.olithiasis is a common disease that affects the ma.or salivary glands, which is characterized by obstruction of the gland itself or its excretory duct due to the formation of a sia.olith, resulting in decreased salivary flow. It mainly affects the submandibular glands (80% - 90%), probably because it has a long duct that surrounds the mylohyoid muscle and emerges against gravity through the Wharton’s duct. The size of the salivary calculus can vary from below 1 millimeter (mm) to a few centimeters in diameter, where most stay below 10 mm. Only 7% are greater than 15 mm and these are classified as giant calculi. This report presents a case of asymptomatic giant sialolith (approximately 60 mm in diameter) located in the duct of the right submandibular gland, with 20 years of evolution. The treatment of choice was the surgical removal of the sialolith (calculus) through the salivary duct with maintenance of the gland. Although the individual did not present complaints related to the presence of the giant calculi, its removal had a great impact on their quality of life, since a significant respiratory improvement was reported by the individual. The major difference reported was better sleep quality.

Comparative Dimensional and Microscopic Analysis of the Strength and Hardness of Internal Fixation Plates

The fixation stability achieved with the use of plates and screws in oral and maxillofacial surgery is a decisive factor in treatment success. The mechanical and structural properties of the internal fixation materials have direct influence on the dimensional stability and resistance of a fixation system, thus influencing treatment outcomes. This study proposed to analyze the dimensional and resistance patterns of titanium plates used for obtaining stable fixation in orthognathic surgery and craniofacial trauma. For this study, 30 conventional 2.0 mm straight four hole plates with bridge from three brands commercialized in Brazil, were subjected to macroscopic, microscopic, strength and hardness analysis. The dimensional measurements were performed using a digital caliper. Energy-dispersive X-ray spectroscopy analysis was performed by scanning electron microscopy to analyze the chemical composition of the samples. The mechanical resistance tests were performed with a universal testing machine. The samples were then submitted to Vickers hardness analysis, complying with the standards of ASTM E92. The data collected from the dimensional study was submitted to statistical analysis of the coefficient of variation, while the values obtained during the mechanical tests were analyzed by variance (ANOVA) and Tukey’s test (p 0.05). The sample groups presented different performances in resistance, hardness, size and surface, even though they were reported to be chemically similar compounds that allowed us to conclude the plates from Group 2 were more resistant than groups 1 and 3.

Facial Asymmetry Caused by Mandibular osteoma: Case Report

osteoma is a benign tumor that often affects the maxillofacial region. It exhibits slow growth and remains asymptomatic for prolonged time and may cause facial asymmetry or functional disorders when it reaches vast proportions. When osteomalacia is diagnosed it is important to check a.out Gardner’s Syndrome because up to 90% of patients with this syndrome may present skeletal abnormalities and osteomas. The objective of this paper is to report a case of a 54-year-old woman that related an 8-year evolution of a volume increase in the face. Clinical examination showed facial asymmetry and presence of nodular lesion located in the left region of the mandibule, painless and hard on palpation. Pa.oramic radiography showed radiopaque image limited to the left mandibular body and angle, suggestive of osteoma. Gastrointestinal review was requested and discarded intestinal polyps and Gardner’s Syndrome. The patient was submitted to excisional biopsy and microscopic examination of the tumor that confirmed the diagnosis of osteoma. osteoma needs special attention due to the possible association with malignant lesions intestine and also by psychological and functional problems, when it reaches great proportions.

Caffeine is an alka.oid present in a wide variety of plants. Currently the most consumed psychostimulant worldwide, its consumption is associated with several health benefits, including modulation of the innate and adaptive immune response, reduction of oxidative cellular stress, and decreased incidence of some cancers, including breast cancer. Breast cancer is the most common cause of cancer a.ong women and the second leading cause of cancer death in women worldwide. The interaction between biomaterials and drugs has enabled a great advance in science for developing controlled drug delivery systems and has been used to treat numerous pathologies. This work aimed to evaluate the immunomodula.ory effects of caffeine associated or not with polyethylene glycol adsorbed in microemulsion (MLP) on MCF-7 cells, pha.ocytic cells (MN), and coculture. For biological assays, ATCC (American Type Culture Collection, USA) cell lines of breast adenocarcinoma (MCF-7) and pha.ocytes (MN) obtained from voluntary donors were used. The cells (MN and MCF-7) and coculture were treated with caffeine and MLP and incubated for rheological characterization analyses: flow curve and viscosity, oxidative stress: superoxide anion assay (), and activity of the enzyme superoxide dismutase (Cu-Zn-SoD). Caffeine and MLP increased viscosity and blood and MCF-7 cells and affected the immunomodulation of oxidative stress meta.olism of MN and MCF-7 cells treated with caffeine and associated caffeine to the MLP. These data suggest that caffeine is associated or not with MLP-induced immunomodula.ory effects on MN pha.ocytes and MCF-7 cells, demonstrating the antitumor activity via.oxidative stress and can be a complementary alternative for treating breast cancer.
