Prevalence and Factors Associated with Dry Eyes Syndrome in Glaucomatous Patients on Anti-Glaucoma Eye Drops Followed up at the Garoua Regional Hospital-Northern Cameroon

Background: Dry eye and glaucoma are two common pathologies in the elderly, and are very often associated. This association suggests a link between them and between their treatments. Our purpose for this study is to determine the prevalence of dry eye in our glaucoma patients treated with eye drops and deduce the factors associated with it. Patients and Methods: We conducted a cross-sectional, analytical study from October 2022 to September 2023 in the ophthalmology department of Garoua Regional Hospital in glaucoma patients aged over 15 years able to answer the Ocular Surface Disease Index (OSDI) questionnaires and treated with antiglaucoma eye drops for more than 3 months. These patients underwent a complete ophthalmological examination and a tear film break-up time test. Results: A total of 73 patients (146 eyes) were examined. The average age of the patients was 47.2 ± 16.5 years, with a male predominance of 54.8%. The prevalence of dry eye syndrome in our patients according to the OSDI score was 56.2% (95% CI) (44.8;67.6). The prevalence of dry eye syndrome on clinical examination of the BUT in the right eye was 83.7% (95% CI) (75.1;92.1) and 79.4% (70.2;88.7) in the left eye. The duration of antiglaucoma treatment was the factor associated with dry eye syndrome (p < 0.05) in glaucoma patients in our setting. Conclusion: Dry Eye Syndrome is common in glaucoma patients on drops in our setting. Treatment duration of more than 1 year seems to be associated with dry eyes.

Facial and Periorbital Emphysemas Following a Violent Sneezing: An Atypical Clinical Situation

Introduction: Facial emphysema is the presence of air in the subcutaneous tissues of the facial region. They can be clinically recognized by the crackling sensation felt when the affected area is palpated. Observation: The authors describe left orbito-facial emphysema that occurred after a violent sneezing episode in a 36-year-old patient. He had significant edema of the left facial and ipsilateral periorbital region associated with major emphysema and complete closure of the left eye. Nasal cavities endoscopy revealed inflammation of the distal orifice of the nasolacrimal duct. The clinical ophthalmologic examination performed in emergency showed left chemosis, slight ocular hypertonia of mechanical origin, and a slight decrease in visual acuity. Pupillary reflexes and retinography were normal. A craniofacial computed tomography (CT) revealed a significant left orbital emphysema, a fracture of the left medial orbital wall (ethmoidal lamina papyracea) with intraconal fat incarceration without entrapment of the medial rectus and significant air infiltration of all the left hemifacial soft tissues. A broad-spectrum antibiotic and anti-inflammatory treatment were instituted, as well as practical advice to prevent a recurrence. We observed progressive resorption of the edema with a return to the normal of the soft tissues and the palpebral cleft in 15 days. Conclusion: These atypical cases can be serious. It is essential to exclude signs of visual deficit and ocular compression. Multidisciplinary management is important.

Awareness of Diabetic Retinopathy in Diabetic Individuals Based on Knowledge, Attitude, and Practice in Cameroon: A Multicentre Study

Introduction: Diabetic retinopathy accounts for 5% of all causes of blindness. We set out to assess knowledge, attitude, and practice patterns in patients with diabetes regarding diabetic retinopathy (DR) and identify barriers that may exist in this context. Material and Methods: We conducted a cross-sectional study by consecutively enrolling patients with diabetes consulting at four hospitals in Cameroon between November 2021 and March 2023. We surveyed participants about their understanding of diabetic retinopathy (DR), their approach to it, and their visits to eye specialists by means of a single-investigator-interviewer-administered questionnaire. Data was anonymously analysed using STATA/BE 17 and presented in frequencies and Spearman’s correlation coefficient. The error margin was 5% and all results with p-value Results: We enrolled 152 patients with type 2 diabetes mellitus, with a mean age of 60.30 years and a male-to-female ratio of 0.9. Out of the 152 patients enrolled, 138 (90.59%) agreed that the eyes could be damaged by diabetes. Meanwhile, only 21 (15.79%) associated diabetes with DR. Of the 41.18% who were occasionally sent for an eye exam by their consulting physicians, 91.72% made it to the consultations. Spearman’s correlation showed no significant relationship between the knowledge of eye involvement in diabetes and visits to eye specialists, regardless of blood sugar levels (p = 0.30). Conclusion: We were able to show that there is a lack of sensitization of patients with diabetes on diabetic retinopathy and referral to ophthalmologists.

Cyber Deception Using NLP

Cyber security addresses the protection of information systems in cyberspace. These systems face multiple attacks on a daily basis, with the level of complication getting increasingly challenging. Despite the existence of multiple solutions, attackers are still quite successful at identifying vulnerabilities to exploit. This is why cyber deception is increasingly being used to divert attackers’ attention and, therefore, enhance the security of information systems. To be effective, deception environments need fake data. This is where Natural Language (NLP) Processing comes in. Many cyber security models have used NLP for vulnerability detection in information systems, email classification, fake citation detection, and many others. Although it is used for text generation, existing models seem to be unsuitable for data generation in a deception environment. Our goal is to use text generation in NLP to generate data in the deception context that will be used to build multi-level deception in information systems. Our model consists of three (3) components, including the connection component, the deception component, composed of several states in which an attacker may be, depending on whether he is malicious or not, and the text generation component. The text generation component considers as input the real data of the information system and allows the production of several texts as output, which are usable at different deception levels.

The Ability to Visually Recognize Facial Emotions during the Initial Stages of Alzheimer’s

Background: Alzheimer’s sufferers (AS) are unable to visually recognize facial emotions (VRFE). However, we do not know the kind of emotions involved, the timeline for the onset of this loss of ability to recognize facial emotional expressions during the natural course of this disease and the existence of any correlation with other comorbid cognitive disorders. For that reason, the authors aimed to determine whether a deficit in facial emotion recognition is present at the onset of Alzheimer disease, distinctly and concurrently with the onset of cognitive impairment or is it a prodromal syndrome of Alzheimer’s Disease before the onset of cognitive decline and what emotions are involved. A secondary aim was to investigate relationships between facial emotion recognition and cognitive performance on various parameters. Method: Single Blind Case-control study. Setting in Memory clinic. Participants: 12 patients, (AS) and 12 control subjects (CS) were enrolled. Measurements: Quantitative information about the ability for facial emotion recognition was obtained from Method of Analysis and Research on the Integration of Emotions (MARIE). The Mini Mental Status Examination (MMSE), the Picture Naming, the Mattis Dementia Rating Scale (DRS), and the Grober & Buschke Free and Cued Selective Reminding Test (FCSRT) tests were used to measure cognitive impairment. Results: We note that the AS have a problem with the visual recognition of facial emotions with existence of a higher threshold for visual recognition. The AS is less sensitive to the visual recognition cues of facial emotions. AS is unable to distinguish anger from fear. A Study of Visual Recognition of Facial Emotional Expressions in a Normal Aging Population in the Absence of Cognitive Disorders

Objective: To examine and measure the decision-making processes involved in Visual Recognition of Facial Emotional Expressions (VRFEE) and to study the effects of demographic factors on this process. Method: We evaluated a newly designed software application (M.A.R.I.E.) that permits computerized metric measurement of VRFEE. We administered it to 204 cognitively normal participants ranging in age from 20 to 70 years. Results: We established normative values for the recognition of anger, disgust, joy, fear, surprise and sadness expressed on the faces of three individuals. There was a significant difference in the: 1) measurement (F (8.189) = 3896, p = 0.0001);2) education level (x2(12) = 28.4, p = 0.005);3) face (F(2.195) = 10, p = 0.0001);4)series (F (8.189)=28, p = 0.0001);5) interaction between the identity and recognition of emotions (F (16, 181 =11, p = 0.0001). However, performance did not differ according to: 1) age (F (6.19669) = 1.35, p = 0.2) or 2) level of education (F (1, 1587) = 0.6, p = 0.4). Conclusions: In healthy participants, the VRFEE remains stable throughout the lifespan when cognitive functions remain optimal. Disgust, sadness, fear, and joy seem to be the four most easily recognized facial emotions, while anger and surprise are not easily recognized. Visual recognition of disgust and fear is independent of aging. The characteristics of a face have a significant influence on the ease with which people recognize expressed emotions (idiosyncrasy). Perception and recognition of emotions is categorical, even when the facial images are integrated in a spectrum of morphs reflecting two different emotions on either side.

Hip Disarticulation for Severely Injured Lowerlimbs about 11 Cases and Literature Review

Introduction: Hip disarticulation (HD) consists of a removal of the entire lower limb at the height of the hip-femoral joint. It is an extremely mutilating intervention with a major functional and psychological impact. We report 11 cases of hip disarticulation following severe trauma of the lower limb. The aim of the study is to analyze the frequency, the clinical aspect and present their management results and the patients’ becoming. Patients and methods: A 13-year retrospective study was conducted in our traumatology department. Eleven cases of hip disarticulation were identified. We analyzed the following data: the epidemiological parameters (age, sex, accidentology), Clinico-radiological (gravity injury severity score (ISS), mangled extremity severity score (MESS), associated lesion), admission time, evolutionary aspects and finally the patients’ becoming). Results: The series consisted of 3 males and 8 females, with an average age of 24 years. All experienced a HD following the lower limb severe traumatism due to a road accident. The category of victims was dominated by pedestrians. The average admission time was 13 hours with extremes of 9 and 28 hours. Our series average ISS was 28 with extremes of 15 - 69. The associated lesions were dominated by Gustilo Type III A III B open fractures. The mean MESS at admission was 10 with extremes from 8 to 14. Complications were dominated by infection (at least 72%) with a mortality rate of 36.36%. Algohallucinosis affects all these patients post-operatively at varied degrees. Aggressive components and denial of bereavement were most noticeable. Only one patient was able to honor her Canadian prosthetic equipment. All the victims changed their personal and professional activities. Conclusion: HD is an extremely mutilating surgery with a major functional and psychological impact.
