One step minilaparotomy-assisted transmesenteric portal vein recanalization combined with transjugular intrahepatic portosystemic shunt placement: A novel surgical proposal in pediatrics

Transjugular intrahepatic portosystemic shunt(TIPS) placement is a standard procedure for the treatment of portal hypertension complications. When this conventional approach is not feasible, alternative procedures for systemic diversion of portal blood have been proposed. A one-step interventional approach, combining minilaparotomy-assisted transmesenteric(MAT) antegrade portal recanalization and TIPS, is described in an adolescent with recurrent esophageal varice bleeding and portal cavernoma(PC). A 16-yearold girl was admitted to our Unit because of repeated bleeding episodes over a short period of time due to esophageal varices in the context of a PC. A portal vein recanalization through an ileocolic vein isolation with the MAT approach followed by TIPS during the same session was performed. In the case of failed portal recanalization, this approach, would also be useful for varice endovascular embolization. Postoperative recovery was uneventful. Treatment consisting of propanolol, enoxaparin and a proton pump inhibitor was prescribed after the procedure. One month post-op, contrast enhanced computed tomography confirmed the patency of the portal and intrahepatic stent grafts. No residual peritoneal fluid was detected nor opacification of the large varices. Endoscopy showed good improvement of the varices. Doppler ultrasound confirmed the accelerated flow in the portal stent and hepatopetal flow inside the intrahepatic portal branches. Three months post-op, TIPS maintained its hourglass shape despite a slight expansion. Portal hypertension and life threatening conditions related to PC would benefit from one-step portal recanalization. MAT-TIPS is feasible and safe for the treatment of PC even in children. This minimally invasive procedure avoids or delays surgical treatment or re-transplantation when necessary in pediatric patients.

To assess dietary myo-inositol in reducing stem cell activation in colitis, and validate pβ-cateninS552 as a biomarker of recurrent *** examined the effects of dietary myo-inositol treatment on inflammation, pβ-cateninS552 and pAkt levels by histology and western blot in IL-10-/- and dextran sodium sulfate-treated colitic mice. Additionally, we assessed nuclear pβ-cateninS552 in patients treated with myo-inositol in a clinical trial, and in patients with and without a history of colitis-induced *** mice, pβ-cateninS552 staining faithfully reported the effects of myo-inositol in reducing inflammation and intestinal stem cell activation. In a pilot clinical trial of myo-inositol administration in patients with a history of low grade dysplasia (LGD), two patients had reduced numbers of intestinal stem cell activation compared to the placebo control patient. In humans, pβ-cateninS552 staining discriminated ulcerative colitis patients with a history of LGD from those with benign *** crypts with increased numbers of pβ-cateninS552 - positive cells can be utilized as a biomarker in colitis-associated cancer chemoprevention trials.

Participatory Early Warning Systems: Youth, Citizen Science,and Intergenerational Dialogues on Disaster Risk Reduction in Brazil

Building national people-centered early warning systems(EWS) is strongly recommended by the United Nations International Strategy for Disaster Reduction(UNISDR). Most of the scientific literature is critical of the conventional view of EWS as a linear model with a topdown approach, in which technological features are given more attention than human factors. It is argued that EWS should be people-centered, and used for risk prevention,with an emphasis on resilience, rather than only being triggered when a hazard occurs. However, both the UNISDR and the literature fail to say how a people-centered EWS should be built, and what steps are needed to put EWS into effect. This article examines the obstacles and measures required to promote people-centered EWS,with a focus on the situation in Brazil. After assessing the institutional vulnerability of EWS, we analyze some measures that can be taken to reduce institutional vulnerability,based on experiences with a participatory citizen science educational project that involved high school *** guidelines are developed for adopting a bottom-up approach towards achieving the four elements of EWS—risk knowledge, monitoring, communication of warnings,and response capability—with the help of school curricula.

Hyperthermic intraperitoneal chemotherapy:Rationale and technique

A decrease of docosahexaenoic acid in testes of mice fed a high-fat diet is associated with impaired sperm acrosome reaction and fertility

Obesity is a major worldwide health problem that is related to most chronic diseases,including male *** to its wide impact on health,mechanisms underlying obesity-related infertility remain *** this study,we report that mice fed a high-fat diet(HFD)for over 2 months showed reduced fertility rates and increased germ cell apoptosis,seminiferous tubule degeneration,and decreased intratesticular estradiol(E2)and E2-to-testosterone ***,we also detected a decrease in testicular fatty acid levels,behenic acid(C22:0),and docosahexaenoic acid(DHA,22:6n-3),which may be related to the production of dysfunctional ***,we did not detect any changes in the frequency of seminiferous tubule stages,sperm count,or rate of in vitro ***,there was an increase in spontaneous and progesterone-induced acrosomal exocytosis(acrosome reaction)in spermatozoa from HFD-fed *** data suggest that a decrease in E2 and fatty acid levels influences spermatogenesis and some steps of acrosome biogenesis that will have consequences for ***,our results add new evidence about the adverse effect of obesity in male reproduction and suggest that the acrosomal reaction can also be affected under this condition.

New Reports of Spiders in Three Mexican Ant-Acacias

The richness of spider species on the foliage of Mexican ant-acacias in Los Tuxtlas (Veracruz), and Santiago Pinotepa Nacional (Oaxaca) was documented. A total of 11 species, grouped into five families were found;in Los Tuxtlas five species representing two families were identified, whereas in Santiago Pinotepa Nacional, the number of taxa was eight, belonging to five families. The richness of spiders in Santiago Pinotepa Nacional was higher than that found in Los Tuxtlas. In both locations Nephila clavipes Linnaeus and Eustala illicita O. Pickard-Cambridge, both from Araneidae, were found;these species had been previously reported as common inhabitants of Central American ant-acacias. The presence of E. illicita in Mexican myrmecophytic acacias was confirmed. A total of 10 species grouped into five families are reported for the first time inhabiting Mexican ant-acacias, increasing the richness of spider interactions documented in Mexico. This study showed that the occurrence of spiders in the Vachellia-Pseudomyrmex mutualism system has been overlooked and is likely more common than what has been reported until now. This opens an opportunity for the study of the evolution of spider-ant-plant interactions.

Estimation of Partition Coefficients of Fe-S Protein Models： Se Substitutions in Fe4S4Cysn

The SCAP method allows a semiquantitative estimation of the partitioning of many solutes between numerous pairs of solvents. The organic solvent-water partition coefficients P are calculated. For FeaS4Cysn, the organic solvent-water partition coefficients for l-octanol Po, cyclohexane Pch and chloroform Pcf decay 4.46, 6.25 and 4.60 log units per Cys, respectively. The Po is of the same order of magnitude as CDHI （conformationally dependent hydrophobic index） calculations and Pch-cf as computations performed with a method by Leo and Hansch. The logPo-ch-cf mean relative errors are 17%, 25% and-17%, which represent mean and unsigned relative errors of -3% and 20%. On varying the number of Cys, the structures show hydrophobic moments indicative of amphipathic compounds. For Se substitutions in Fe4SeaCys,, Po-ch-cf drop 4.52, 6.30 and 4.66 exponents per Cys. With the reference calculations, Po-ch-cf decay 4.03, 4.80 and 5.76 exponents per Cys. For FeaS4 mSemCys4, the calculated similar partition coefficients and hydrophobic moments, and lower distance in dendrogram suggest a role of Fe-Se clusters in physiological processes. The analysis is extended to lysozyme and its secondary structures.

Binding of Mono, Bi and Tridomain Proteins to Zwitterionic and Anionic Vesicles： Asymmetric Location of Anionic Phospholipids in Mixed Zwitterionic/Anionic Vesicles and Cooperative Binding

Lysozyme, myoglobin and BSA were used as models of globular proteins covering a wide range of pl. The purpose is to extend the studies to anionic lipid bilayers. Electrostatics is studied in cationic protein adsorption to zwitterionic PC and anionic mixed PC/PG SUVs. Protein adsorption is investigated in SUVs along with changes of fluorescence emission spectra. Partition coefficients and cooperativity parameters are calculated. At pl binding obtains maximum while at lower or higher pHs binding decreases. In Gouy-Chapman formalism activity coefficient goes with square charge, which deviations indicate asymmetric location of anionic phospholipid in the inner leaflet, in mixed SUVs for lysozyme- and myoglobin-PC/PG systems, in agreement with experiments and molecular dynamics simulations. Vesicles bind myoglobin anti-cooperatively while lysozyme-BSA cooperativitivey. A model is proposed for both, which composes two protein sub-layers with different structures and properties. Hill coefficient reflects subunit cooperativity of bi and tridomain proteins.
