Gangrenous Cholecystitis with Atypical Presentation in an Elderly Diabetic Woman

In elderly patients, numerous factors subsidize the diagnostic challenge and high incidence of complications in this specific population, taking gangrenous cholecystitis as a critical example. The aim of this work is to report an unusual case of gangrenous choleystitis in an elderly diabetic women and its atypical clinical presentation. A 79-year-old female patient came to our observation;her medical history showed nausea and vomiting of about 2 hours which rapidly ended with symptomatic therapy, without recurrence, and a 3-week history of intermittent fever associated with productive cough. No abdominal discomfort was declared. Physical examination of the abdomen was negative. Laboratory analysis revealed leukocytosis with the remaining criteria within the normal range. After 2 days, she started with a mildabdominal pain in the epigastric region that rapidly progressed to the right upper quadrant, right flank and right iliac fossa, without nausea, vomiting or fever. Abdominal computer tomography findings revealed thickness of the gall-bladder and important densification of the vascular bed. Acute cholecystitis was diagnosed. The patient was then submitted to a laparoscopic cholecystectomy under general anaesthesia with findings suggestive of gangrenous acute cholecystitis confirmed by histologic examination of the specimen. Delays in diagnosing acute cholecystitis in specific populations, such as elderly diabetics, result in a higher prevalence of morbidity and mortality due to potentially serious complications as gangrenous cholecystitis. Consequently, the diagnosis should be measured and investigated promptly in order to prevent poor outcomes.

Dynamics of Primary Productivity and Oceanographic Parameters under Influence of the Amazon River Plume

The watershed of the Amazon River discharges about 120,000 m3·s-1?of freshwater into the adjacent platform and oceanic region. The aim of this work was to analyze the distribution of oceanographic parameters, chlorophyll a and primary productivity under the influence of the Amazon River plume, during the period of greatest extension of the Amazon plume. Collections were carried out in September 14 in 16 stations including continental platform and oceanic region. It was possible to observe superficial currents along the coast in the northwest direction, but with less intensity and currents with greater speeds towards the east due to the North Brazil Current retroflexion at this time of the year, transporting the plume to the central Atlantic Ocean. The spatial influence of the plume was observed in the salinity, which ranged from 28 to 36.75, although the high precipitation in the region may also have contributed as a source of freshwater. However, the sampled region showed strong negative linear correlation of silicate, chlorophyll a and primary productivity with salinity. The primary productivity values ranged from 0.04 to 18.81 mg C m-3?day-1, whereas chlorophyll a concentrations ranged from 0.15 mg·m-3?to 1.83 mg·m-3, decreasing their values as they move away from the coast. The Amazon River plume can reach and influence the oceanographic and biological parameters in a large area of this oligotrophic region. However, the results also suggest that the export of material from the adjacent coastal region is another determinant of the region’s productivity.

Neonatal Umbilical Cord Infections: Incidence, Associated Factors and Cord Care Practices by Nursing Mothers at a Tertiary Hospital in Western Uganda

Background: Umbilical cord infections complicate to neonatal sepsis that significantly contributes to neonatal mortality worldwide. There is paucity of data on the incidence, factors associated with neonatal umbilical cord infections in western Uganda, yet nursing mothers continue to practice potentially dangerous cord care practices. We described the incidence, factors associated with umbilical cord infections and cord care practices by nursing mothers at a tertiary hospital in western Uganda. Methods: This was a hospital based cross sectional study at a tertiary hospital in western Uganda between the months of March and June 2019. Two hundred and forty (240) neonates aged between 2 to 14 days attending the neonatal unit were recruited in the study. Umbilical cord infection was assessed based on the World Health Organisation (WHO) clinical definition: finding of any of discharge, redness and swelling on the umbilical stump. Data on sociodemographic characteristics, maternal and neonatal perinatal factors and cord care practices of the nursing mothers were analysed by bivariate and multivariate logistic regression using STATA 13.0 to determine factors associated with umbilical cord infection. Patterns of Genetic Diversity of the White-Nosed Coati Reveals Phylogeographically Structured Subpopulations in Mexico

Coatis (Procyonidae;Nasua) are considered the only truly social meso-carnivore mammals in Neotropical forests. In Mexico, white-nosed coatis (Nasua narica) are suspected to have undergone population reduction due to habitat loss and fragmentation and led to a lack of genetic adaptability and genetic isolation throughout its range. We examined patterns of genetic diversity and connectivity of five populations of Nasua narica distributed throughout Mexico (n = 60) by sequencing an ≈ 800 bp fragment of the mitochondrial cytochrome-b gene and also by screening 12 microsatellite loci. We found moderate to high levels of genetic variability for both genetic markers. We recorded twenty-two different cytochrome-b haplotypes throughout the 5 sampled areas and found that each of the sampled population of white-nosed coatis in Mexico harbors unique haplotypes and only three haplotypes were shared among two different populations that were closer geographically. All populations had high haplotype diversity (h) (0.968 ± 0.008 (SD)) but lower levels of nucleotide diversity (π) of 0.007 ± 0.001 (SD). All microsatellite loci were polymorphic in all of the populations and the mean number of alleles per locus was 5.033 ± 1.545 (SD) with expected (HE) and observed (HO) heterozygosity values of 0.774 and 0.664, respectively. However, low Wright F statistic values suggest the existence of a reduced heterozygosity (FST = 0.203, FIS = 0.134 and FIT = 0.310). Significant differences between the five populations confirmed isolation by distance, which suggests genetic structure among five subpopulations.

Enterprise architecture is a subject that has increased its importance in the small and medium enterprises in the manufacturing sector of the industry in Mexico. The global competitiveness of the markets has influenced the adoption of methodologies that support the strategic alignment of the processes with the goals and strategic objectives of the firms. The components of the business architecture like mission, vision, strategic objectives, products, organizational structure, business processes, clients and geographic region, were collected from the firm of the case study for the design of the architecture. As a result of the practical application, an implementation model has been created and four strategic objectives were established for to improve productivity and competitiveness. This paper is a result of the research project of analysis, design and implementing business architecture in a medium size manufacturing company like partial architecture of an enterprise using ontologies for representing the core elements of the business architecture; the study presents clearly the importance of the strategic planning for the analysis and the detection of the main faults for the success of the achievement of goals and objectives.

The aim of this study was to gather information on the use of stable isotopes to measure total body water and body composition scan in children. We selected studies in the last 13 years, in addition to classical studies on the subject, indexed in the database PubMed, LILACS, BVS and SciELO. The body composition was characterized by the amount of bone tissue, muscle and adipose tissue, also including the organs as well as levels of body water. Your knowledge becomes increasingly important in light of the changes that occur in the nutritional status of various types of diseases in frameworks, such as diabetes mellitus, protein energy malnutrition, in cases of obesity and metabolic syndrome. The ability to accurately assess body fat mass especially in children is associated with the importance of effective strategies for prevention and treatment of childhood obesity. Historically, in addition to clinical applications, measurements of total body water were used to determine body composition in nutritional studies. To the knowledge of the body composition, the body water can be measured and used by the ingestion of a dose of labeled water. The measured isotope enrichment is a function of the amount of body water. The method of deuterium is particularly interesting for the assessment of body composition in children, due to its characteristics of collection and analysis.

Population growth has consequences for intense use of aquatic ecosystems. Samples were taken, between 2013 and 2014, in the reservoirs ＂Algodoeiro＂ and ＂gloria＂ in Sergipe, Brazil. TSI （Trophic State Index） and WQI （Water Quality Index） were used. It was determined the parameters such as conductivity, dissolved oxygen, total nitrogen, total phosphorus and chlorophyll. TSI has been applied, in ＂Algodoeiro＂, which was classified as mesotrophic. With the increase of rainfall in 2014, this reservoir changed its condition to eutrophic. ＂gloria＂ reservoir was already classified in super-eutrophication. Regarding the WQI, the reservoirs were classified in Class IV, indicating a critically degraded condition.
