We investigated the performance characteristics of prostate-specific antigen （PSA） and PSA density （PSAD） in Chinese men. All Chinese men who underwent transrectal ultrasound-guided prostate biopsy （TRUS-PB） from year 2000 to 2013 were included. The receiver operating characteristic （ROC） curves for both PSA and PSAD were analyzed. The sensitivity, specificity, positive predictive value （PPV） and negative predictive value （NPV） at different cut-off levels were calculated. A total of 2606 Chinese men were included. For the ROC, the area under curve was 0.770 for PSA （P〈 0.001） and 0.823 for PSAD （P〈 0.001）. PSA of 4.5 ng ml^-1 had sensitivity of 94.4%, specificity of 14.1%, PPV of 29.5%, and NPV of 86.9%; PSAD of 0.12 ng ml^-1cc^-1 had sensitivity of 94.5%, specificity of 26.6%, PPV of 32.8%, and NPV of 92.7%. On multivariate logistic regression analyses, PSA cut-off at 4.5 ng ml^-1 （OR 1.61, 95% CI 1.05-2.45, P = 0.029） and PSAD cut-off at 0.12 ng ml^-1 cc^-1 （OR 6.22, 95% CI 4.20-9.22, P 〈 0.001） were significant predictors for prostate cancer detection on TRUS-PB. In conclusion, the performances of PSA and PSAD at different cut-off levels in Chinese men were very different from those in Caucasians. PSA of 4.5 ng ml^-1 and PSAD of O. 12 ng ml^-1 cc^-1 had near 95% sensitivity and were significant predictors of prostate cancer detection in Chinese men.

The silk sericin is the main residue in silk production and it is found to be a low cost and efficient bio-sorbent. In this study, sericin was characterized with various techniques including SEM （scanning electron micro- scope）, XRD, N2 physisorption, FTIR （Fourier transformed infrared spectroscopy） and XPS （X-ray photoelectron spectroscopy）. The nitrogen content of sericin was ca. 8.5 mmol.g-1 according to elemental analysis. Dye adsorption by sericin biosorbent was investigated with the acid yellow （AY）, methylene blue （MB） and copper （II） phthalocyanine-3,4＇4＂4＇＂-tetrasulfonic acid （CuPc） dyes from water. Sericin displayed large capacity for AY andCuPc adsorption with adsorption capacities of respectively 3.1 and 0.35 mmol.g-1, but it did not adsorbed methyl- ene blue dye. This selectivity is due to the basicity of amide groups in seriein biosorbents.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

Objectives: To 1) explore the planar scintigraphic findings in asymmetry patients caused by unilateral condylar hyperplasia (UCH) or asymmetric mandibular hyperplasia (AMH);2) develop a local agecondylar activity reference norm;and 3) check the accuracy of the new norm and compare with other analytical methods. Material and Method: Chinese patients with mandibular asymmetry and scintigraphic findings available were recruited. Clinical, radiographic records and scintigraphic condylar activity ratio (CAR) were studied. Regression analysis was performed to quantify the relationship between age and CAR to develop a new norm. The sensitivity of condylar activity assessment using a) traditional norm;b) new norm;and c) percentile difference was compared. In patients with serial data available, longitudinal analysis of the scintigraphic changes were checked. Result: 109 patients were eligible for the study. Significant difference in CAR was noted between UCH and AMH patients. Linear relationship was observed between age and CAR. A new norm of the age-CAR was established, which showed improved sensitivity in condylar activity prediction in UCH and AMH when compared with traditional norm. Relative to percentile difference, the sensitivity of new norm was lower in AMH but not in UCH patients. Serial analysis revealed gradual decline in CAR with minimal change in percentile difference. Conclusion and Clinical Relevance: There is difference in scintigraphic condylar activity between UCH and AMH patients. A norm of age-CAR relationship was established, which showed improved sensitivity in condylar activity prediction in UCH patients. Further study is required to confirm the role of scintigraphy in AMH.

＜正＞Recent advances in technology permit the disabled to use computers as one of their personal assistive aids in daily functional activities. Many rehabilitation organisations or professionals have adopted specialised computer - related treatment for their clients. However, to date, little evidence is available on the effectiveness of various approaches to the computer training for the disabled. The present study was designed to perform theory driven empirical work that contributes a better under- standing of effects using different memory retraining strategies for persons with brain injury. The present research project attempted to develop and evaluate computer applications for memory retraining. The study also investigated usability and effectiveness of applying computer technology in cognitive rehabilitation. Eighteen persons with brain injury participated in 3 study groups. Six of them participated in each of the three computer -assisted memory retraining that adopted different strategy. The subjects’ progress and performance in the training modules were closely monitored. The results of the different study groups were compared and contrasted through pre - test and post - test data analysis. It was found that the three study groups that adopted different computer - assisted memory retraining strategies （self-paced, feedback enhanced, and visual presentation） showed significant training effects. However, the visual presentation group showed insignificant improvement in their memory self-efficacy. There is also significant correlation between the pre - training self-efficacy and the memory retraining outcomes. Further data analysis will be performed to compare the computer -assisted memory retraining method to the control conventional therapist-administered method.

BACKGROUND: Simulation-based medical education has been growing rapidly and becomes one of the most popular teaching methods for improving patient safety and patient care. The Simulation Subcommittee of the Hong Kong College of Emergency Medicine organized an educational program emphasizing the team training, clinical decision-making and communication skills. This study aimed to evaluate the attitude of the participants toward a new training program and the change in the knowledge on clinical performance in emergency physicians and nurses after attending the educational ***: A course evaluation form was filled in by the participants at the end of the workshop. An assessment of 20 multiple-choice questions with 5 options was administered to the participants before and after the 2-day simulation-based training ***: A total of 72 doctors and nurses working in the Accident and Emergency Department were enrolled. The average pretest and posttest scores were 12 and 14.3 respectively. The percentage improvement in the mean score of the pretest and posttest was 11.5%. The Chi-square test showed signifi cant improvement in the pretest and posttest score grading(P=0.00). Paired t-test revealed signifi cant difference between the mean scores of the pretest and posttest(P=0.00).CONCLUSIONS: Participants had positive attitude toward this new training program. Significant improvement of the knowledge on clinical performance in healthcare professionals in the Accident and Emergency Department was observed after the participation in this simulation-based educational program.

应用微阵列-比较基因组杂交（microarray comparative genomic hybridization，aCGH）技术检测基因拷贝数变异（copy number variation，CNV）可对染色体亚微结构异常进行定位，在研究领域和临床实验室中迅速成为一个了解基因和患者遗传病因的有力的工具。由于它在产前诊断上的应用时间较短，目前对aCGH是否可以作为产前诊断工具及其潜在的不确定性存在比较大的争议。此外，对于大多数CNV的表型效应还知之甚少，因此，临床医生向患者进行解释的难度很大，导致许多临床医生拒绝以临床诊断为目的使用aCGH。根据已知的与临床疾病密切相关的一些区域定制的寡核苷酸芯片应运而生，可以降低这种不确定性，在过量信息和信息量不足之间寻求到了一个平衡。本综述的目的是从临床医生的角度探讨aCGH技术用于产前诊断所面临的问题和应用前景。

Objective: To develop an auto-segmentation method for delineating the biological tumor volume of nasopharyngeal carcinoma from positron emission tomography images. Methods: A phantom consisting of a water tank with fixed background fluorodeoxyglucose [18F-FDG] activity and spheres with diameters ranging from 2.1 to 5 cm with varying activities of FDG were used to simulate tumors of different sizes and FDG uptake. The phantom was scanned with a PET/CT scan at different sphere to background intensity ratios. An optimum fixed percentage threshold (FT) approach and a signal-to-background ratio (SBR) approach were developed to estimate the true size of the spheres from the PET images. Both approaches were further evaluated in patient images for validation. Twenty-two patients with NPC from stage T1 to T4 were included. The PET based biological tumor volumes (BTV) were delineated with both FT (BTVFT) and SBR (BTVSBR) approaches and compared with the gross tumor volume localized from MRI (GTVMR). The mean volumes of BTVFT and BTVSBR were compared and the degree of overlap between GTVMR and both BTVs was evaluated. Paired t-tests were used for statistical analysis. Results: The optimal FT value was 36.5% of maximal intensity, and SBR approach was represented by an inverse linear regression model. The estimated volume of spheres segmented by both approaches shows no significant difference from the true volume of spheres (p > 0.05), but the average absolute errors were smaller from SBR approach than FT approach (p = 0.008). GTVMR was larger than both BTVFT (p = 0.003) and BTVSBR (p SBR with GTVMR is significantly larger than with BTVFT (0.52 and 0.42 respectively, p MR for radiotherapy planning purpose.
