Considering the variation in metabolic evaluation and medical management of kidney stone disease,this consensus review was created to discuss the metabolic activity of nephrolithiasis,define the difference between single and recurrent stone formers,and develop a schema for metabolic and radiologic follow-up.A systematic review of the literature was performed to identify studies of metabolic evaluation and follow-up of patients with *** single and recurrent stone formers share many similarities in metabolic *** study group determined that based on an assessment of risk for stone recurrence and metabolic activity,single and recurrent stone formers should be evaluated comprehensively,including two 24 h urine studies on a random *** medication and dietary recommendations are effective for many patients in reducing the risk of stone ***-up of those with stone disease should be obtained depending on the level of metabolic activity of the patient,the risk of chronic kidney disease and the risk of osteoporosis/osteopenia.A standard scheme includes a baseline metabolic profile,a repeat study 3e6 months after initiation of treatment,and then yearly when stable,with abdominal imaging obtained every 1-2 years.

目的为了比较FREDDY激光机、钬激光机和气压弹道碎石机在结石上移和碎石方面的临床效果。方法上移的水下试验装置包括一个水平方向放置的硅树脂导管(直径为1.3cm),和一个固定支架,使激光机的石英光纤和气压弹道碎石机的撞针可以直接接触石头。将预先称过重的圆柱状Bego石头(BegoUSA公司提供)放于该装置中,分别由FREDDY激光机、钬激光机和气压弹道碎石机进行碎石。FREDDY激光机和钬激光机都采用相近的脉冲能量和频率。为了具有可比性,气压弹道碎石机采用了半硬性撞针和单脉冲,在100、200、300kPa各档压力设置下分别对石头进行30次撞击。“平均后退距离”定义为直接测得的石头最终静止点到原点的距离(每档设置都记录测试结果)。另外,两台激光机分别对称过重的石头(熟石膏成分)进行碎石,以比较碎石效果。石头固定于一个水下装置(由一个倒置的硅注射器和一个固定装置组成)中。激光光纤(钬激光和FREDDY激光的光纤内径分别是365μm和280μm)穿过注射器的针头出来抵住石头。总共24颗石头被分成4组,每组6颗,其中两组用FREDDY激光分别以300J和400J的总能量碎石,另两组用钬激光分别以300J和480J的总能量碎石。“碎石率”定义为石头重量缺失的百分比。结果FREDDY激光机在能量设置为160mJ时,分别以5、10和15Hz的频率碎石,石头的平均后退距离为7.6、8.1和6.8cm;钬激光机在能量设置为0.8J时,分别以5和10Hz的频率碎石,石头的平均后退距离为3.3和4.9cm;气压弹道碎石机在压力设置分别为100、200、300kPa时,石头的平均后退距离为8.5、9.9和13.8cm。一般来说,FREDDY激光机造成的石头后退距离都要少于气压弹道碎石机,尽管这只有在后者处于最高压力设置时才有统计意义(p<0.05)。而FREDDY激光机造成的石头后退距离要多于钬激光机,在临床上常见的设置中有统计意义(p<0.05)。FREDDY激光机以300J和400J的总能量碎石时,石头重量的缺失率分别为44.9%和86.8%,都有统计意义(p<0.05);钬激光机以300J和480J的总能量碎石时,石头重量的缺失率分别为3.3%和7.1%,都有统计意义(p<0.05)。结论FREDDY激光机造成的石头后退距离要明显大于钬激光机(在低频设置时),但又明显少于气压弹道碎石机(在所有设置时)。因此我们建议,在进行腔内输尿管镜碎石术,或在输尿管肾盂连接处进行腔内激光碎石术时,于接近结石处使用阻断装置,如StoneCone-石椎(BostonScientific公司提供)。FREDDY激光机在体外碎石的效率要明显好于钬激光机,对于结石病人来说,前者是个更优、更经济的选择。

Objective:To evaluate the potential toxic effects of mine tailings exposure among the fisher folks residing near and far from the Calancan Bay,marinduque,using the micronucleus assay as an ***:The fisher folks residing near and far from the Calancan Bay were interviewed and the presence and frequency of cells with micronucleus in exfoliated buccal epithelial cells were ***:Results showed that the prevalence of cells with micronucleus was higher among the fisher folks who were directly exposed to the mine tailings as compared with those fisher folks who reside in a community without exposure of mine tailings and history of mining(Pmicronuclei observed near the Calancan Bay could possibly indicate a prolonged chemical stress caused by the toxic heavy metals in the mine tailings and the environment.

Louisiana’s chronic wetland deterioration has resulted in massive soil organic matter loss and subsequent carbon release through *** combat these losses,and reestablish ecosystem function,goods,and services,many restoration projects have been constructed or planned throughout coastal *** are significant data gaps and conflicting results regarding wetland contributions to global warming,especially related to carbon sequestration in restored *** exceptionally large data set was used to derive carbon accumulation rates from key soil characteristics and *** and comparisons of bulk density,organic matter,total carbon,vertical accretion(short-and longer-term),and carbon accumulation rates were made across time(chronosequence) and space(i.e.,coastwide,watershed basins,and vegetation zones).Carbon accumulation rates in the Louisiana coastal zone were generally correlated to hydrogeomorphology,with higher rates occurring in zones of high river connectivity or in swamp or higher salinity tolerant *** average,naturally occurring wetlands had higher carbon accumulation rates than restoration *** some restoration measures were higher,and most showed increasing carbon accumulation rates over *** demonstrate that although wetland restoration provides many ecosystem benefits,the associated carbon sequestration may also provide useful measures for climate change management.

B lymphocytes regulate airway granulocytic inflammation and cytokine production in a murine model of fungal allergic asthma

Sensitization to fungi often leads to a severe form of asthma that is particularly difficult to manage clinically, resulting in increased morbidity and hospitalizations in these patients. Although B lymphocytes might exacerbate asthma symptoms through the production of IgE, these cells might also be important in the protective response against inhaled fungi. Through cytokine release and T-cell interactions, these lymphocytes might also influence the development and maintenance of airway wall fibrosis. JH-/- mice lack the JHgene for the heavy chain component of antibodies, which is critical for B-cell function and survival. These animals have facilitated the elucidation of the role of B iymphocytes in a number of immune responses; however, JH-/- mice have not been used to study fungal allergy. In this study, we examined the role of B lymphocytes using an Aspergillus fumigatus murine fungal aeroallergen model that mimics human airway disease that is triggered by environmental fungal exposure. We compared disease progression in sensitized wild-type BALB/c and JH-/- mice that were exposed to repeated fungal exposure and found no differences in airway hyperresponsiveness, overall pulmonary inflammation or collagen deposition around the large airways. However, the levels of the Th2-type cytokines IL-4 and IL-13 were significantly attenuated in the airways of JH-/- mice relative to the BALB/c controls. By contrast, levels of the inflammatory cytokines IL-17A and IL-6 were significantly elevated in the JH-/- animals, and there was significantly more robust airway eosinophilia and neutrophilia than in control animals. Taken together, these findings demonstrate that B lymphocytes help to regulate granulocytic responses to fungal exDosure in the pulmonary compartment.

In large Solar Energetic Particle （SEP） events, ions can be accelerated at coronal mass ejection （CmE）-driven shocks to very high energies. The spectra of heavy ions in many large SEP events show features such as roll-overs or spectral breaks. In some events when the spectra are plotted in terms of energy/nucleon, they can be shifted relative to each other to make the spectral breaks align. The amount of shift is charge to mass ratio （Q/A） dependent and varies from event to event. This can be understood if the spectra of heavy ions are organized by the diffusion coefficients （Cohen et al. 2005）. In the work of Li et al. （2009）, the Q/A dependence of the scaling is related to shock geometry when the CmE-driven shock is close to the Sun. For events where multiple in-situ spacecraft observations exist, one may expect that different spacecraft are connected to different portions of the CmE-driven shock that have different shock geometries, therefore yielding different Q/A dependence. In this work, we examine one SEP event which occurred on 2013 November 4. We study the Q/A dependence of the energy scaling for heavy ion spectra using helium, oxygen and iron ions. Observations from STEREO-A, STEREO-B and ACE are examined. We find that the scalings are different for different spacecraft. We suggest that this is because ACE, STEREO- A and STEREO-B are connected to different parts of the shock that have different shock geometries. Our analysis indicates that studying the Q/A scaling of in-situ particle spectra can serve as a powerful tool to remotely examine the shock geometry for large SEP events.

Kharsali village, located in the Northwest Himalaya near the confluence of the Yamuna River and Unta Gad, is situated on a thick(>150 m) paleolandslide deposit. The village is continuously being eroded at its base by the two rivers. Cracks are noted in most houses while the ancient Shani Temple lying to the south of the village has tilted ~5° towards the northeast. Three slope sections(S-1, S-2, S-3) were modelled and analysed to determine the displacement and shear strain patterns of the slopes. Based on surface failure conditions, potential slope instability of the Kharsali village was evaluated from 2D Finite Element method(FEm) using Shear Strain Reduction(SSR) analysis in the Phase2 software. Results indicate a critical Stress Reduction Factor(SRF) of 1.5 for the southern edge of the village(S-1) housing the Shani Temple. The development of failure surfaces at its lower portion signifies the propagating, progressive nature of the slope. The S-2 slope section is most vulnerable to slope failure, with a critical SRF of 1.08. This has been inferred by the formation of failure surfaces with displacements of 0.05-0.08 m. The S-3 section in the northern part of the Kharsali shows highest critical SRF of 2.76. The un-metalled road section in the north of the village near S-3 hasdeveloped a failure surface with displacement of 0.003-0.004 m, and a zone of subsidence. The S-3 section is relatively stable, whereas the S-2 section is the most vulnerable portion of the village.
