BACKGROUND Mediastinal leakage(ML) is one of the most feared complications of esophagectomy. A standard strategy for its diagnosis and treatment has beendifficult to establish because of the great variability in their incidence and mortality rates reported in the existing *** To assess the incidence, predictive factors, treatment, and associated mortality rate of mediastinal leakage using the standardized definition of mediastinal leaks recently proposed by the Esophagectomy Complications Consensus Group(ECCG).METHODS Seven Italian surgical centers(five high-volume, two low-volume) affiliated with the Italian Society for the Study of Esophageal Diseases designed and implemented a retrospective study including all esophagectomies(n = 501) with intrathoracic esophagogastric anastomosis performed from 2014 to *** MLs were defined according to the classification recently proposed by the *** Fifty-nine cases of ML were recorded, yielding an overall incidence of 11.8%(95%CI: 9.1%-14.9%). The surgical approach significantly influenced the occurrence of ML: the proportion of leakage was 10.5% and 9% after open and hybrid esophagectomy(HE), respectively, and doubled(20%) after totally minimally invasive esophagectomy(TMIE)(P = 0.016). No other predictive factors were found. The 30-and 90-d overall mortality rates were 1.4% and 3.2%,respectively; the 30-and 90-d leak-related mortality rates were 5.1% and 10.2%,respectively; the 90-d mortality rates for TMIE and HE were 5.9% and 1.8%,respectively. Endoscopy was the first-line treatment in 49% of ML cases, with the need for retreatment in 17.2% of cases. Surgery was needed in 44.1% of ML *** treatment had the lowest mortality rate(6.9%). We present a dense crustal velocity field and corresponding strain-rate pattern computed using Global Positioning System（GPS）- Global Navigation Satellite System（GNSS） data from several hundred permanent stations in the Italian Peninsula. GPS data analysis is based on the GAMIT/GLOBK 10.6 software, which was developed and maintained mainly by Massachusetts Institute of Technology（MIT）, using tools based on the distributed-sessions approach implemented in this package. The GPS data span the period from January 2008 to december 2012 and come from several different permanent GPS networks in Italy. The GLOBK package implemented in the last version of the GAMIT package is used to compute the position time-series and velocities registered in the International Terrestrial Reference Frame（ITRF） 2008. The resulting high-density intra-plate velocity field provides indications of the tectonics of the Mediterranean region. A computation of the strain-rate pattern from GPS data is performed and compared with the map of the epicentral locations of historical earthquakes that occurred in the last 1000 years in the Italian territory, showing that, in general, higher crustal deformation rates are active in regions affected by seismicity of greater magnitude.

Endives(Cichorium endivia L.)are popular vegetables,diversified into curly/frisée-and smooth/broad-leafed(escaroles)cultivar types(cultigroups),and consumed as fresh and bagged *** are rich in sesquiterpene lactones(STL)that exert proven function on bitter taste and human *** assembly of a reference transcriptome of 77,022 unigenes and RNA-sequencing experiments were carried out to characterize the differences between endives and escaroles at the gene structural and expression levels.A set of 3177 SNPs distinguished smooth from curly cultivars,and an SNP-supported phylogenetic tree separated the cultigroups into two distinct clades,consistently with the botanical varieties of origin(crispum and latifolium,respectively).A pool of 699 genes maintained differential expression pattern(core-deGs)in pairwise comparisons between curly vs smooth cultivars grown in the same *** annotation allowed the identification of 26 genes in the sesquiterpenoid biosynthesis pathway,which included several germacrene A synthase,germacrene A oxidase and costunolide synthase members(GAS/GAO/COS module),required for the synthesis of costunolide,a key precursor of lactucopicrin-and lactucin-like sesquiterpene *** core-deGs contained a GAS gene(contig83192)that was positively correlated with STL levels and recurrently more expressed in curly than smooth endives,suggesting a cultigroup-specific *** significant positive correlation of GAS/GAO/COS transcription and STL abundance(2.4-fold higher in frisée endives)suggested that sesquiterpenoid pathway control occurs at the transcriptional *** on correlation analyses,five transcription factors(MYB,MYB-related and WRKY)were inferred to act on contig83192/GAS and specific STL,suggesting the occurrence of two distinct routes in STL biosynthesis.

AIM: To evaluate the predictors of 10-year survival of patients with hepatitis C recurrence. METHODS: Data from 358 patients transplanted between 1989 and 2010 in two Italian transplant centers and with evidence of hepatitis C recurrence were analyzed. A χ2, Fisher's exact test and Kruskal Wallis' test were used for categorical and continuous variables, respectively. Survival analysis was performed at 10 years after transplant using the Kaplan-Meier method, and a log-rank test was used to compare groups. A P level less than 0.05 was considered significant for all tests. Multivariate analysis of the predictive role of different variables on 10-year survival was performed by a stepwise Cox logistic ***: The ten-year survival of the entire population was 61.2%. Five groups of patients were identified according to the virological response or lack of a response to antiviral treatment and, among those who were not treated, according to the clinical status(mild hepatitis C recurrence, "too sick to be treated" and patients with comorbidities contraindicating the treatment). While the 10-year survival of treated and untreated patients was not different(59.1% vs 64.7%, P = 0.192), patients with a sustained virological response had a higher 10-year survival rate than both the "non-responders"(84.7% vs 39.8%, P ders had a survival rate comparable to patients untreated with mild recurrence(84.7% vs 89.3%). A sustained virological response and young donor age were independent predictors of 10-year survival. CONCLUSION: Sustained virological response significantly increased long-term survival. Awaiting the interferon-free regimen global availability, antiviral treatment might be questionable in selected subjects with mild hepatitis C recurrence.

AIM: To validate the feasibility of digital tomosynthesis of the abdomen(DTA) combined with contrast enhanced ultrasound(CEUS) in assessing complications after endovascular aortic aneurysm repair(EVAR) by using computed tomography angiography(CTA) as the gold ***: For this prospective study we enrolled 163 patients(123 men; mean age, 65.7 years) referred for CTA for EVAR follow-up. CTA, DTA and CEUS were performed at 1 and 12 mo in all patients, with a maximum time interval of 2 ***: Among 163 patients 33 presented complications at CTA. DTA and CTA correlated for the presence of complications in 32/33(96.96%) patients and for the absence of complications in 127/130(97.69%) patients; the sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and accuracy of DTA were 97%, 98%, 91%, 99%, and 98%, respectively. CEUS and CTA correlated for the presence of complications in 19/33(57.57%) patients and for the absence of complications in 129/130(99.23%) patients; the sensitivity, specificity, PPV, NPV and accuracy of CEUS were 58%, 99%, 95%, 90%, and 91%, respectively. Sensitivity, specificity and accuracy of combining DTA and CEUS together in detecting EVAR complications were 77%, 98% and 95%, ***: Combining DTA and CEUS in EVAR follow-up has the potential to limit the use of CTA only in doubtful cases.
