Single vs dual(en bloc) kidney transplants from donors ≤ 5 years of age: A single center experience

AIM: To compare outcomes between single and dual en bloc(EB) kidney transplants(KT) from small pediatric donors. METHODS: Monocentric nonprospective review of KTs from pediatric donors ≤ 5 years of age. Dual EB KT was defined as keeping both donor kidneys attached tothe inferior vena cava and aorta, which were then used as venous and arterial conduits for the subsequent transplant into a single recipient. Donor age was less useful than either donor weight or kidney size in decision-making for kidney utilization as kidneys from donors cm in length were not transplanted. Post-transplant management strategies were standardized in all ***: From 2002-2015, 59 KTs were performed including 34 dual EB and 25 single KTs. Mean age of donors(17 mo vs 38 mo, P compared to the single KT group, respectively. Mean cold ischemia time(21 h), kidney donor profile index(KDPI; 73% vs 62%) and levels of serum creatinine(Scr, 0.37 mg/d L vs 0.49 mg/d L, all P = NS) were comparable in the dual EB and single KT groups, respectively. Actuarial graft and patient survival rates at 5-years follow-up were comparable. There was one case of thrombosis resulting in graft loss in each group. Delayed graft function incidence(12% dual EB vs 20% single KT, P = NS) was slightly lower in dual EB KT recipients. Initial duration of hospital stay(mean 5.4 d vs 5.6 d) and the one-year incidences of acute rejection(6% vs 16%), operative complications(3% vs 4%), and major infection were comparable in the dual EB and single KT groups, respectively(all P = NS). Mean 12 mo Scr and abbreviated MDRD levels were 1.17 mg/d L vs 1.35 mg/d L and 72.5 m L/min per 1.73 m^2 vs 60.5 m L/min per 1.73 m^2(both P = NS) in the dual EB and single KT groups, respectively. cONcLUSION: By transplanting kidneys from young pediatric donors into adult recipients, one can effectively

AIM: To check the safety and efficacy of boceprevir/telaprevir with peginterferon/ribavirin for hepatitis c virus(HcV) genotype 1 in the real-world settings. METHODS: This study was a non-randomized, observational, prospective, multicenter. This study involved 47 centers in Italy. A database was prepared for the homogenous collection of the data, was used by all of the centers for data collection, and was updated continuously. All of the patients enrolled in this study were older than 18 years of age and were diagnosed with chronic infection due to HcV genotype 1. The HcV RNA testing was performed using cOBAS-Taq Man2.0(Roche, LLQ 25 IU/m L). RESULTS: All consecutively treated patients were included. Forty-seven centers enrolled 834 patients as follows: Male 64%; median age 57(range 18-78), of whom 18.3% were over 65; mean body mass index 25.6(range 16-39); genotype 1b(79.4%); diagnosis of cirrhosis(38.2%); and fibrosis F3/4(71.2%). The following drugs were used: Telaprevir(66.2%) and PEG-IFN-alpha2a(67.6%). Patients were na?ve(24.4%), relapsers(30.5%), partial responders(14.8%) and null responders(30.3%). Overall, adverse events(AEs) occurred in 617 patients(73.9%) during the treatment. Anemia was the most frequent AE(52.9% of cases), especially in cirrhotic. The therapy was stopped for 14.6% of the patients because of adverse events or virological failure(15%). Sustained virological response was achieved in 62.7% of the cases, but was 43.8% in cirrhotic patients over 65 years of age. cONcLUSION: In everyday practice, triple therapy is safe but has moderate efficacy, especially for patients over 65 years of age, with advanced fibrosis, nonresponders to peginterferon + ribavirin.

This is a continuity of a series of taxonomic papers where materials are examined,described and novel combinations are proposed where necessary to improve our traditional species concepts and provide updates on their *** addition to extensive morphological descriptions and appropriate asexual and sexual connections,DNA sequence data are also analysed from concatenated datasets(rDNA,TEF-a,RBP2 and b-Tubulin)to infer phylogenetic relationships and substantiate systematic position of taxa within appropriate *** new species or combinations are being proposed,we apply an integrative approach(morphological and molecular data as well as ecological features wherever applicable).Notes on 125 fungal taxa are compiled in this paper,including eight new genera,101 new species,two new combinations,one neotype,four reference specimens,new host or distribution records for eight species and one alternative *** new genera introduced in this paper are Alloarthopyrenia,Arundellina,camarosporioides,Neomassaria,Neomassarina,Neotruncatella,Paracapsulospora and *** new species are Alfaria spartii,Alloarthopyrenia italica,Anthostomella ravenna,***,Arthrinium paraphaeospermum,Arundellina typhae,Aspergillus koreanus,Asterina cynometrae,Bertiella ellipsoidea,Blastophorum aquaticum,cainia globosa,camarosporioides phragmitis,ceramothyrium menglunense,chaetosphaeronema achilleae,chlamydotubeufia helicospora,ciliochorella phanericola,clavulinopsis aurantiaca,colletotrichum insertae,comoclathris italica,coronophora myricoides,cortinarius fulvescentoideus,***,***,***,cunninghamella gigacellularis,cyathus pyristriatus,cytospora cotini,Dematiopleospora alliariae,***,Diaporthe aseana,***,Distoseptispora multiseptata,***,***,Dothiora buxi,Emericellopsis persica,Gloniopsis calami,Helicoma guttulatum,Helvella floriforma,***,Hermatomyces subiculosa,Ju

The Jiangmen Underground Neutrino Observatory(JUNO)features a 20 kt multi-purpose underground liquid scintillator sphere as its main *** of JUNO's features make it an excellent location for^8B solar neutrino measurements,such as its low-energy threshold,high energy resolution compared with water cherenkov detectors,and much larger target mass compared with previous liquid scintillator *** this paper,we present a comprehensive assessment of JUNO's potential for detecting^8B solar neutrinos via the neutrino-electron elastic scattering process.A reduced 2 MeV threshold for the recoil electron energy is found to be achievable,assuming that the intrinsic radioactive background^(238)U and^(232)Th in the liquid scintillator can be controlled to 10^(-17)g/*** ten years of data acquisition,approximately 60,000 signal and 30,000 background events are *** large sample will enable an examination of the distortion of the recoil electron spectrum that is dominated by the neutrino flavor transformation in the dense solar matter,which will shed new light on the inconsistency between the measured electron spectra and the predictions of the standard three-flavor neutrino oscillation *** m^(2)_(21)=4.8times10^(-5);(7.5times10^(-5))eV^(2),JUNO can provide evidence of neutrino oscillation in the Earth at approximately the 3sigma(2sigma)level by measuring the non-zero signal rate variation with respect to the solar zenith ***,JUNO can simultaneously measureDelta m^2_(21)using^8B solar neutrinos to a precision of 20% or better,depending on the central value,and to sub-percent precision using reactor antineutrinos.A comparison of these two measurements from the same detector will help understand the current mild inconsistency between the value of Delta m^2_(21)reported by solar neutrino experiments and the KamLAND experiment.

Scanning electrochemical microscopy(SEcM)and scanning vibrating electrode technique(SVET)were used to investigate the electrochemical behaviour of the top surface of the 2098-T351 alloy welded by friction stir welding(FSW).The SVET technique was efficient in identifying the cathodic and anodic weld *** welding joint(WJ),which comprises the thermomechanically affected zone(TMAZ)and the stir zone(SZ),was cathodic relative to the heated affected zone(HAZ)and the base metal(BM).The reactivities of the welding joint at the advancing side(AS)and the retreating side(RS)were analyzed and compared using SEcM technique in the competition mode by monitoring the dissolved oxygen as a redox mediator in 0.005 mol L^-1 Nacl *** RS was more electrochemically active than the AS,and these results were correlated with the microstructural features of the welded alloy.

JUNO is a multi-purpose neutrino observatory under construction in the south of *** publication presents new sensitivity estimates for the measurement of the △m_(31)^(2),△m_(21)^(2),sin^(2)θ_(12),and sin^(2)θ_(13) oscillation parameters using reactor antineutrinos,which is one of the primary physics goals of the *** sensitivities are obtained using the best knowledge available to date on the location and overburden of the experimental site,the nuclear reactors in the surrounding area and beyond,the detector response uncertainties,and the reactor antineutrino spectral shape constraints expected from the TAO satellite *** is found that the △m_(21)^(2) and sin^(2)θ_(12) oscillation parameters will be determined to 0.5%precision or better in six years of data *** the same period,the △m_(31)^(2) parameter will be determined to about 0.2%precision for each mass ordering *** new precision represents approximately an order of magnitude improvement over existing constraints for these three parameters.

The Jiangmen Underground Neutrino Observatory(JUNO)is a large liquid scintillator detector designed to explore many topics in fundamental *** this study,the potential of searching for proton decay in the p→νK^(+)mode with JUNO is *** kaon and its decay particles feature a clear three-fold coincidence signature that results in a high efficiency for ***,the excellent energy resolution of JUNO permits suppression of the sizable background caused by other delayed *** on these advantages,the detection efficiency for the proton decay via p→νK^(+)is 36.9%±4.9%with a background level of 0.2±0.05(syst)±0.2(stat)events after 10 years of data *** estimated sensitivity based on 200 kton-years of exposure is 9.6×1033 years,which is competitive with the current best limits on the proton lifetime in this channel and complements the use of different detection technologies.
