Aim:This paper reported the experience of one center on repeat laparoscopic liver surgery for metastasis and hepatocellular carcinoma(HCC)with a review of the ***:This retrospective study included 24 patients who underwent laparoscopic re-intervention(hepatic resection and radiofrequency ablation)for recurrent HCC in cirrhosis(n=17)and for recurrent malignant metastases(n=7)after a previous open or laparoscopic *** were divided into two groups according to the fi rst surgical *** 1 underwent open resection and laparoscopic procedure(7 patients),and Group 2 underwent laparoscopic resection and laparoscopic procedure(17 patients).Results:Mean operative time for re-intervention was signifi cantly longer for Group 1(220.14±80.06 min)than for Group 2(150±56.18 min;P=0.001),whereas the mean blood loss and mean hospital stay were comparable in both *** to Dindo-Clavien classifi cation,overall morbidity ranged between Grade I and IIIa and was similar in both ***:This study suggests that repeat laparoscopic surgery for recurrent hepatic malignant diseases in selected patients is a feasible and safe procedure with good short-term outcomes,but further prospective studies are needed to support these results.

The prognosis of patients with metastatic colorectal cancer (mCRC) remain poor despite the impressive improvement of treatments observed over the last 20 years that led to an increase in median overall survival from 6 mo, with the only best supportive care, to approximately 30 mo with the introduction of active chemotherapy drugs and targeted agents. The monoclonal antibodies (moAbs) cetuximab and panitumumab, directed against the epidermal growth factor receptor (EGFR), undoubtedly represent a major step forward in the treatment of mCRC, given the relevant efficacy in terms of progression-free survival, overall survival, response rate, and quality of life observed in several phase III clinical trials among different lines of treatment. However, the anti-EGFR moAbs were shown only to be effective in a subset of patients. For instance, KRAS and NRAS mutations have been identified as biomarkers of resistance to these drugs, improving the selection of patients who might derive a benefit from these treatments. Nevertheless, several other alterations might affect the response to these drugs, and unfortunately, even the responders eventually become resistant by developing secondary (or acquired) resistance in approximately 13-18 mo. Several studies highlighted that the landscape of responsible alterations of both primary and acquired resistance to anti-EGFR drugs biochemically converge into MEK-ERK and PIK3CA-AKT pathways. In this review, we describe the currently known mechanisms of primary and acquired resistance to anti-EGFR moAbs together with the various strategies evaluated to prevent, overcame or revert them.

A Computational Modeling on Flow Bifurcation and Energy Distribution through a Loosely Bent Rectangular Duct with Vortex Structure

The present study investigates the non-isothermal flow and energy distribution through a loosely bent rectangular duct using a spectral-based numerical approach over a wide range of the Dean number 0 < D n ≤ 3000 . Unlike previous research, this work offers novel insights by conducting a grid-point-specific velocity analysis and identifying new bifurcation structures. The study reveals how centrifugal and buoyancy forces interact to produce steady, periodic, and chaotic flow regimes significantly influencing heat transfer performance. The Newton-Raphson method is employed to explore four asymmetric steady branches, with vortex solutions ranging from 2- to 12 vortices. Unsteady flow characteristics are analyzed exquisitely by performing time-advancement of the solutions and the flow regimes are shown as a percentage of total flow with longitudinal vortex generation. Axial flow, secondary flow, and temperature profiles have been depicted in accordance with Dn to wander the flow pattern, and it is predicted that the time-dependent flow (TDF) consists of asymmetric 2- to 10-vortex solutions. The significant findings of this study include the axial displacement of the circulations due to the influence of the time-varying temperature dispersal applied along the wall. Chaotic flows, which dominate the higher Dean number range, are shown to enhance heat convection due to increased fluid mixing. A detailed comparison with prior research demonstrates the advantages of this approach, particularly in capturing complex non-linear behaviors. The findings of this study provide practical guidelines for optimizing duct designs to maximize heat transfer and suggest future research directions, such as using nanofluids or studying Magneto-hydrodynamics in the same configuration.

We consider the topological entropy h(θ) of real unimodal maps as a function of the kneading parameter θ(equivalently, as a function of the external angle in the Mandelbrot set). We prove that this function is locally Hlder continuous where h(θ) > 0, and more precisely for any θ which does not lie in a plateau the local Hlder exponent equals exactly, up to a factor log 2, the value of the function at that *** confirms a conjecture of Isola and Politi(1990), and extends a similar result for the dimension of invariant subsets of the circle.

Although the no-cloning theorem forbids perfect replication of quantum information, it is sometimes possible to produce large numbers of replicas with vanishingly small error. This phenomenon, known as quantum superreplication, can occur for both quantum states and quantum gates. The aim of this paper is to review the central features of quantum superreplication and provide a unified view of existing results. The paper also includes new results. In particular, we show that when quantum superreplication can be achieved, it can be achieved through estimation up to an error of size O（M/N2）, where N and M are the number of input and output copies, respectively. Quantum strategies still offer an advantage for superreplication in that they allow for exponentially faster reduction of the error. Using the relation with estimation, we provide i） an alternative proof of the optimality of Heisenberg scaling in quantum metrology, ii） a strategy for estimating arbitrary unitary gates with a mean square error scaling as log N/N2, and iii） a protocol that generates O（N2） nearly perfect copies of a generic pure state U｜0） while using the corresponding gate U only N times. Finally, we point out that superreplication can be achieved using interactions among k systems, provided that k is large compared to M2/N2.
