Transurethral prostate surgery in prostate cancer patients: A population-based comparative analysis of complication and mortality rates

Objective:Prostate cancer(PCa)patients might experience lower urinary tract symptoms as those diagnosed with benign prostatic hyperplasia(BPH).Some of them might be treated for their lower urinary tract symptoms instead of PCa.We aimed to test the effect of PCa versus BPH on surgical outcomes after transurethral prostate surgery,namely complication and mortality rates.Methods:Within the American College of Surgeons National Surgical Quality Improvement Program database(2011-2016),we identified patients who underwent transurethral resection of the prostate,photoselective vaporization,or laser enucleation.Patients were stratified according to postoperative diagnosis(PCa vs.BPH).Univariable and multivariable logistic regression models evaluated the predictors of perioperative morbidity and mortality.A formal test of interaction between diagnosis and surgical technique used was performed.Results:Overall,34542 patients were included.Of all,2008(5.8%)had a diagnosis of PCa.The multivariable logistic regression model failed to show statistically significant higher rates of postoperative complications in PCa patients(odds ratio:0.9,95%confidence interval:0.7-1.1;p=0.252).Moreover,similar rates of perioperative mortality(p=0.255),major acute cardiovascular events(p=0.581),transfusions(p=0.933),and length of stay of more than or equal to 30 days(p=0.174)were found.Additionally,all tests failed to show an interaction between post-operative diagnosis and surgical technique used.Conclusion:Patients diagnosed with PCa do not experience higher perioperative morbidity or mortality after transurethral prostate surgery when compared to their BPH counterparts.Moreover,the diagnosis seems to not influence surgical technique outcomes.

BACKGROUND Hepatocellular carcinoma(HCC)is the most common primary liver malignancy and the second leading cause of cancer deaths worldwide.It is often diagnosed at an advanced stage and therefore its prognosis remains poor with a low 5-year survival rate.HCC patients have increasingly complex and constantly changing characteristics,thus up-to-date and comprehensive data are fundamental.AIM To analyze the epidemiology and main clinical characteristics of HCC patients in a referral center hospital in the northwest of Italy between 2010 and 2019.METHODS In this retrospective study,we analyzed the clinical data of all consecutive patients with a new diagnosis of HCC recorded at"Santa Croce e Carle"Hospital in Cuneo(Italy)between 1 January 2010 and 31 December 2019.To highlight possible changes in HCC patterns over the 10-year period,we split the population into two 5-year groups,according to the diagnosis period(2010-2014 and 2015-2019).RESULTS Of the 328 HCC patients who were included(M/F 255/73;mean age 68.9±11.3 years),154 in the first period,and 174 in the second.Hepatitis C virus infection was the most common HCC risk factor(41%,135 patients).The alcoholic etiology rate was 18%,the hepatitis B virus infection etiology was 5%,and the non-viral/non-alcoholic etiology rate was 22%.The Child-Pugh score distribution of the patients was:class A 75%,class B 21%and class C 4%.The average Mayo end-stage liver disease score was 10.6±3.7.A total of 55 patients(17%)were affected by portal vein thrombosis and 158(48%)by portal hypertension.The average nodule size of the HCC was 4.6±3.1 cm.A total of 204 patients(63%)had more than one nodule<3,and 92%(305 patients)had a non-metastatic stage of the disease.The Barcelona Clinic Liver Cancer(BCLC)staging distribution of all patients was:4%very early,32%early,23%intermediate,34%advanced,and 7%terminal.Average survival rate was 1.6±0.3 years.Only 20%of the patients underwent treatment.Age,presence of ascites,BCLC stage and therapy were predictors of a better prognosis(P<0.01).A c

We report the charge-changing cross sections(σ_(cc))of 24 p-shell nuclides on both hydrogen and carbon at about 900A MeV,of which^(8,9)Li,^(10–12)Be,^(10,14,15)B,^(14,15,17–22)N and^(16)O on hydrogen and^(8,9)Li on carbon are for the first time.Benefiting from the data set,we found a new and robust relationship between the scaling factor of the Glauber model calculations and the separation energies of the nuclei of interest on both targets.This allows us to deduce proton radii(R_p)for the first time from the cross sections on hydrogen.Nearly identical R_p values are deduced from both target data for the neutron-rich carbon isotopes;however,the R_p from the hydrogen target is systematically smaller in the neutron-rich nitrogen isotopes.This calls for further experimental and theoretical investigations.

Cognitive correlates of neuroimaging abnormalities in the onset of schizophrenia: A case report

Increasing evidence shows that cognitive impairment and brain abnormalities can appear early in the first episodes of schizophrenia, but it is currently debated how brain changes can correlate with clinical presentation of schizophrenic patients. Of note, this report describes the case of a young schizophrenic male presenting parietal magnetic resonance/positron emission tomography abnormalities and cognitive impairment, documented by specific neuropsychological tests. In our knowledge only few studies have investigated if neuropsychological abnormalities could be concomitant with both structural and functional neuroimaging. This case shows that impairment in specific cognitive domains is associated with structural/functional brain abnormalities in the corresponding brain areas(frontal and parietal lobes), supporting the hypothesis of disconnectivity, involving a failure to integrate anatomical and functional pathways. Future research would define the role of cognitive impairment and neurodegeneration in psychiatric nosography and, in particular, their role in the early phases of illness and long-term outcome of schizophrenic patients.

Andrographolide Loaded in Micro-and Nano-Formulations: Improved Bioavailability, Target-Tissue Distribution, and Efficacy of the “King of Bitters”

Andrographolide (AG) is the characteristic constituent of Andrographis paniculata, of the Acanthaceae family. This plant is a well-known Asian medicinal plant that is widely used in India, China, and Thailand. A monograph of Herba Andrographidis (Chuanxinlian) is included in the Chinese Pharmacopoeia, which reports that this decoction can “remove heat, counteract toxicity, and reduce swellings.” The numerous potential activities of AG range from anti-inflammatory to anti-diabetic action, from neuroprotection to antitumor activity, and from hepatoprotective to anti-obesity properties. However, AG has low bioavailability and poor water solubility, which can limit its distribution and accumulation in the body after administration. In addition, AG is not stable in gastrointestinal alkaline and acidic environments, and has been reported to have a very short half-life. Among the diverse strategies that have been adopted to increase AG water solubility and permeability, the technological approach is the most useful way to develop appropriate delivery systems. This review reports on published studies related to microparticles (MPs) and nanoparticles (NPs) loaded with AG. MPs based on polylactic-glycolic acid (PLGA), alginic acid, and glucan derivatives have been developed for parenteral oral and pulmonary administration, respectively. NPs include vesicles (both liposomes and niosomes);polymeric NPs (based on polyvinyl alcohol, polymerized phenylboronic acid, PLGA, human serum albumin, poly ethylcyanoacrylate, and polymeric micelles);solid lipid NPs;microemulsions and nanoemulsions;gold NPs;nanocrystals;and nanosuspensions. Improved bioavailability, target-tissue distribution, and efficacy of AG loaded in the described drug delivery systems have been reported.

Dear Editor,Anaplastic lymphoma kinase-positive(ALK+)anaplastic large cell lymphoma(ALCL)is an aggressive peripheral Tcell lymphoma representing 10%-15%of pediatric lymphoid neoplasms.The constitutively activated nucleophosmin-ALK(NPM-ALK)fusion protein is the main driver of ALK+ALCL oncogenesis[1].Current clinical protocols can achieve high cure rates,with 75%event-free survival(EFS)at 5 years[2],but the patients’outcome with relapsed/refractory disease still remains poor[3].Liquid biopsy is widely applied for the prognostic and therapeutic stratification of onco-hematological patients.As a non-invasive procedure,it can be used to identify several biomarkers,such as small extracellular vesicles(S-EVs)[4].S-EVs are stable carriers of molecules that mediate genetic exchange between distant cells,alter the microenvironment,facilitate cancer cell progression and promote drug resistance[5].

Aim:Gliomatosis cerebri(GC)is defined as a rare pattern of growth of diffuse gliomas involving three or more cerebral lobes.Given its rarity,it is difficult to define prognostic factors and standard of treatment.We retrospectively analyzed patients(PT)with GC from a single institution with the aim of identifying the main prognostic factors and to assess optimal management.Methods:Medical records were reviewed of patients≥18 years with a histological and/or radiological diagnosis of GC(with no contrast enhancement)occurring between 2006 and 2017.Median progression free survival(PFS)and overall survival(OS)were calculated by the Kaplan-Meier method.Results:We analyzed 33 PT,22 males and 11 females;Eastern Cooperative Oncology Group(ECOG)performance status(PS)was 0-1 in 21 of the patients.Twenty-two PT underwent biopsy:16 were astrocytomas and 6 oligodendrogliomas.O6-methylguanin-DNA-methyltransferase(MGMT)was detected in 14 cases,and it was methylated in eight cases.Isocitrate dehydrogenase 1(IDH1)was analyzed in 16 PT,and it had mutated in 10 of them.Nine PT(27%)were treated with radiation therapy(RT)plus concurrent temozolomide(TMZ),22 PT(67%)received TMZ alone,and 2 PT(6%)underwent RT alone.We reported“complete response”in 1 patient(3%),partial response in 9 PT(27%),and stable disease in 15 PT(45%),while 8 PT(25%)had a progressive disease.For all PT,PFS and OS were 19.1 and 30.7 months,respectively.For ECOG PS 0-1 and≥2,PFS was 34.6 months vs.3.4 months(P<0.0001)and OS was 42 months vs.8.9 months(P<0.0001),respectively.Methylated MGMT was associated with longer PFS(41.6 months vs.8.9 months,P=0.05)and OS(52.7 months vs.14.6 months,P=0.009);PFS for IDH1 mutation and IDH wild-type was 52.7 months vs.8.9 months(P=0.006)and OS was 52.7 months vs.41.7 months(P=0.02),respectively.No significant difference was detected as regards treatments.With regard to histological subtype,OS was 42.0 months vs.52.7 months(P=0.8)and PFS was 41.6 months vs.28.6 months(P=0.7)for astrocytoma vs.oligodendroglioma,respectively.PT with tr

BACKGROUND An altered (dysbiosis) and unhealthy status of the gut microbiota is usually responsible for a reduction of short chain fatty acids (SCFAs) concentration. SCFAs obtained from the carbohydrate fermentation processes are crucial in maintaining gut homeostasis and their determination in stool samples could provide a faster, reliable and cheaper method to highlight the presence of an intestinal dysbiosis and a biomarker for various gut diseases. We hypothesize that different intestinal diseases, such as celiac disease (CD), adenomatous polyposis (AP) and colorectal cancer (CRC) could display a particular fecal SCFAs’ signature. AIM To compare the fecal SCFAs’ profiles of CD, AP, CRC patients and healthy controls, using the same analytical method. METHODS In this cross-sectional study, we defined and compared the SCFAs’ concentration in fecal samples of 9 AP, 16 CD, 19 CRC patients and 16 healthy controls (HC). The SCFAs’ analysis were performed using a gas-chromatography coupled with mass spectrometry method. Data analysis was carried out using Wilcoxon ranksum test to assess pairwise differences of SCFAs’ profiles, partial least squaresdiscriminate analysis (PLS-DA) to determine the status membership based on distinct SCFAs’ profiles, and Dirichlet regression to determine factors influencing concentration levels of SCFAs. RESULTS We have not observed any difference in the SCFAs’ amount and composition between CD and healthy control. On the contrary, the total amount of SCFAs was significantly lower in CRC patients compared to HC (P = 0.044) and CD (P = 0.005). Moreover, the SCFAs’ percentage composition was different in CRC and AP compared to HC. In detail, HC displayed higher percentage of acetic acid (P value = 1.3 × 10-6) and a lower amount of butyric (P value = 0.02192), isobutyric (P value = 7.4 × 10-5), isovaleric (P value = 0.00012) and valeric (P value = 0.00014) acids compared to CRC patients.
