Is positivity for hepatitis C virus antibody predictive of lower risk of death in COVID-19 patients with cirrhosis?

Liver injury has been reported in coronavirus disease 2019(COVID-19)cases but the impact of pre-existing liver damage and related etiology have not been completely *** research interests include the potential reciprocal influence of COVID-19 and pre-existing liver damage related to hepatitis C virus(HCV)infection,in *** this end,we have evaluated three cohorts of patients admitted at three Italian hospitals during the coronavirus pandemic;these included 332 patients with COVID-19 and 1527 patients with HCV who were from established real-world antiviral treatment study cohorts(sofosbuvir/velpatasvir),with either liver disease(various severities;n=1319)or cirrhosis(n=208).Among the COVID-19 patients,10 had cirrhosis(3%),including 7 of metabolic origin and 3 of viral *** among the COVID-19 patients was 27.1%,with 70%of those with cirrhosis of metabolic etiology having ***,older age,low white blood cell count and lymphocyte count being identified as risk predictors of death[odds ratio(OR)=13.7,95%confidence interval(CI):2.59-83.01,P=0.006;OR=1.05,95%CI:1.03-1.08,P=0.0001;OR=1.09,95%CI:1.36-1.16,P=0.001;OR=0.61,95%CI:0.39-0.93,P=0.023,respectively].In the two cohorts of HCV patients,COVID-19 diagnosis was made in 0.07%of those with liver disease and 1%of those with ***,the prevalence of HCV antibodies among COVID-19-infected patients was comparable to that currently reported for the general population in *** the COVID-19 patients,pre-existing metabolic cirrhosis appears to be associated with higher mortality,while HCV antibodies may be suggestive of“protection”against COVID-19.

Effects of chronic therapy with non-steroideal antinflammatory drugs on gastric permeability of sucrose: A study on 71 patients with rheumatoid arthritis

AIM： To evaluate the gastric permeability after both acute and chronic use of non-steroidal anti-inflammatory drugs （NSAIDs） and to assess the clinical usefulness of sucrose test in detecting and following NSAIDs-induced gastric damage mainly in asymptomatic patients and the efficacy of a single pantoprazole dose in chronic users. METHODS： Seventy-one consecutive patients on chronic therapy with NSAIDs were enrolled in the study and divided into groups A and 13 （group A receiving 40 mg pantoprazole daily, group b only receiving NSAIDs）. Sucrose test was performed at baseline and after 2, 4 and 12 wk, respectively. The symptoms in the upper gastrointestinal tract were recorded. RESULTS： The patients treated with pantoprazole had sucrose excretion under the limit during the entire follow-up period. The patients without gastroprotection had sucrose excretion above the limit after 2 wk, with an increasing trend in the following weeks （P = 0.000）. A number of patients in this group revealed a significantly altered gastric permeability although they were asymptomatic during the follow-up period. CONCLUSION： Sucrose test can be proposed as a valid tool for the clinical evaluation of NSAIDs-induced gastric damage in both acute and chronic therapy. This tecnique helps to identify patients with clinically silent gastric damages. Pantoprazole （40 mg daily） is effective and well tolerated in chronic NSAID users .

An Operational Hydro-Meteorological Chain to Evaluate the Uncertainty in Runoff Forecasting over the Verbano basin

The development and implementation of a real-time flood forecasting system with a hydro-meteorological operational alert procedure during the MAP-D-PHASE Project is described in this paper. This chain includes both probabilistic and deterministic forecasts. The hydrological model used to generate the runoff simulations is the rainfall-runoff distributed FEST-Wb model, developed at Politecnico di Milano. The observed data to run the control simulations were supplied by ARPA-Piemonte. The analysis is focused on Maggiore Lake basin, an Alpine basin between North-West of Italy and Southern Switzerland. Two hindcasts during the D-PHASE period are discussed in order to evaluate certain effects regarding discharge forecasts due to hydro-meteorological sources of uncertainties. In particular, in the June convective event it is analysed how the effect of meteorological model spatial resolution can influence the discharge forecasts over mountain basins, while in the November stratiform event how the effect of the initial conditions of soil moisture can modify meteorological warnings. The study shows how the introduction of alert codes appears to be useful for decision makers to give them a spread of forecasted QDFs with the probability of event occurrence, but also how alert warnings issued on the basis of forecasted precipitation only are not always reliable.

Rituximab therapy for primary glomerulonephritis: Report on two cases

The evidence in the medical literature on the efficacy and safety of rituximab therapy for primary glomerulonephritis is limited and controversial. We describe two male Caucasian patients with rapidly progressive kidney failure due to primary proliferative glomerulonephritis. both of them received high-dose intravenous corticosteroids and oral cyclophosphamide with limited benefit. The first patient(hepatitis C virus-negative mixed cryoglobulinemia) underwent plasma-exchange with intravenous immunoglobulins; he showed significant benefit on kidney function(he became dialysis independent with serum creatinine going back to 1.6 mg/d L) after one rituximab pulse even if urinary abnormalities were still present. No improvement in renal function or urinary changes occurred in the second patient. both these individuals developed sepsis over the follow-up, the first patient died two months after rituximab therapy. This report is in keeping with the occurrence of severe infections after rituximab therapy in patients with renal impairment at baseline and concomitant high-dose steroids.

Outcomes assessment of hepatitis C virus-positive psoriatic patients treated using pegylated interferon in combination with ribavirin compared to new Direct-Acting Antiviral agents

AIM To evaluate the outcomes in biological treatment and quality of life of psoriatic patients with chronic hepatitis C(CHC) treated with new Direct-Acting Antiviral agents(DAAs) compared to pegylated interferon-2α plus ribavirin(P/R) *** This is a retrospective study involving psoriatic patients in biological therapy who underwent anti-hepatitis C virus(HCV) treatment at the Department of Dermatology Galeazzi Orthopaedic Institute Milan, Italy from January 2010 to November 2017. The patients were divided into two groups: patients that underwent therapy with DAAs and patients that underwent HCV treatment with P/R. Patients were assessed by a dermatologist for psoriasis symptoms, collecting Psoriasis Area Severity Index(PASI) scores and the Dermatology Quality of Life Index(DLQI). PASI and DLQI scores were evaluated 24 wk after the end of HCV treatment and were assumed as an outcome of the progression of psoriasis. Switching to a different b DMARD was considered as an inadequate response to biological therapy. The dropout of HCV therapy and sustained virological response(SVR) were considered as outcomes of HCV *** Fifty-nine psoriatic patients in biological therapy underwent antiviral therapy for CHC. Of this, 27 patients were treated with DAAs and 32 with P/R. After 24 wk post treatment, the DLQI and the PASI scores were significantly lower(P biological *** DAAs seem to be more effective and safe than P/R in HCV-positive psoriatic patients on biological treatment. Fewer dermatological adverse events may be due to interferon-free therapy.

A Pilot Study on the Effects of Temperature on the blooms of the benthic Mucilage-Forming Microalga Chrysophaeum taylorii

C. taylorii （Chrysophaeum taylorii） Lewis and bryan （Pelagophyceae） are benthic mucilage-producing microalgae recently recorded in the Mediterranean Sea, where its summer blooms are having detrimental effects on benthic communities, fisheries and tourism, especially along the Sardinian coasts （western Mediterranean Sea）. Although few informations on the bloom dynamics of this microalga are available in literature jet, recent studies have hypothesized that sea water temperature plays a major role in determining the distribution and abundance of C. taylorii. The goal of this pilot study was to verify the above-mentioned statement providing experimental evidence for it. To this purpose, C. taylorii presence and abundance were investigated along a temperature gradient near the powerhouse of Torre Valdaliga （Latium coasts）, where a stream of hot water is continuously pumped out from the powerhouse into the sea. The obtained data showed that C. taylorii abundance was significantly higher in the study sites nearer to the powerhouse and a positive correlation between water temperature and cell densities was found, thus confirming that water temperature is one of the environmental variables directly influencing the bloom dynamics of this microalga in the Mediterranean Sea.

Effectiveness of chest radiography,lung ultrasound and thoracic computed tomography in the diagnosis of congestive heart failure

Hydrostatic pulmonary edema is as an abnormal in-crease in extravascular water secondary to elevatedpressure in the pulmonary circulation, due to conges-tive heart failure or intravascular volume *** of hydrostatic pulmonary edema is usuallybased on clinical signs associated to conventional ra-diography findings. Interpretation of radiologic signsof cardiogenic pulmonary edema are often question-able and subject. For a bedside prompt evaluation,lung ultrasound(LUS) may assess pulmonary conges-tion through the evaluation of vertical reverberationartifacts, known as b-lines. These artifacts are relatedto multiple minimal acoustic interfaces between smallwater-rich structures and alveolar air, as it happens incase of thickened interlobular septa due to increase of extravascular lung water. The number, diffusion and in-tensity of b lines correlates with both the radiologic andinvasive estimate of extravascular lung water. The inte-gration of conventional chest radiograph with LUS canbe very helpful to obtain the correct diagnosis. Com-puted tomography(CT) is of limited use in the work upof cardiogenic pulmonary edema, due to its high cost,little use in the emergencies and radiation ***, a deep knowledge of CT signs of pulmonaryedema is crucial when other similar pulmonary condi-tions may occasionally be in the differential diagnosis.
