Nano albumin bound-paclitaxel in pancreatic cancer: Current evidences and future directions

Pancreatic cancer(PDAC) is an aggressive and chemoresistant disease, representing the fourth cause of cancer related deaths in western countries. Majority of patients have unresectable, locally advanced or metastatic disease at time of diagnosis and the 5-year survival rate in these conditions is extremely low. For more than a decade gemcitabine has been the cornerstone of metastatic PDAC treatment, although survival benefit was very poor. PDAC cells are surrounded by an intense desmoplastic reaction that may create a barrier to the drugs penetration within the tumor. Recently PDAC stroma has been addressed as a potential therapeutic target. Nano albumin bound(Nab)-paclitaxel is an innovative molecule depleting tumor stroma, through interaction between albumin and secreted protein acidic and rich in cysteine. Addition of nab-paclitaxel to gemcitabine has showed activity and efficacy in metastatic PDAC first-line treatment improving survival and overall response rate vs gemcitabine alone in the MPACT phase Ⅲ study. This combination represents one of the standards of care in advanced PDAC therapy and is suitable to a broader spectrum of patients compared to other schedules. Nab-paclitaxel is under investigation as a backbone of chemotherapy in novel combinations with target agents or immunotherapy in locally advanced or metastatic PDAC. In this article, we provide an updated and critical overview about the role of nab-paclitaxel in PDAC treatment based on the latest advances in preclinical and clinical research. Furthermore, we focus on the use of nab-paclitaxel within the context of metastatic PDAC treatment landscape and we discuss about future implications in the light of current clinical ongoing trials.

Insulin （Ins） has recently been demonstrated to have the ability to induce the capacitation process in pig spermatozoa. In various mammalian species, capacitation has been linked to the nitric oxide （NO） signalling; therefore, this study investigated NO production in Ins-treated pig spermatozoa by fluorescence-activated cell sorting. For the same samples, sperm capacitation was evaluated chlortetracycline staining, protein tyrosine phosphorylation pattern and acrosomal status. A significant increase of the intrasperm NO level and the activation of three capacitation indices were detected in response to Ins treatment. Conversely, sperm preincubation with an NO synthase inhibitor （N-nitro-L-arginine methyl ester） or with the anti-Ins receptor β （IRβ） antibody reversed all of the Ins-related effects. These results suggest that Ins has the capacity to enhance intracellular NO concentrations in pig spermatozoa and indicate a oossible NO implication upon Ins oromotion of caoacitation.

In real scenarios,the spacecraft deviates from the intended paths owing to uncertainties in dynamics,navigation,and command *** quantifying these uncertainties is crucial for assessing the observability,collision risks,and mission *** issue is further magnified for CubeSats because they have limited control authority and thus require accurate dispersion estimates to avoid rejecting viable trajectories or selecting unviable *** uncertainties arise from random variables(e.g.,measurement errors and drag coefficients)and processes(e.g.,solar radiation pressure and low-thrust acceleration).Although random variables generally present minimal computational complexity,handling stochastic processes can be challenging because of their noisy ***,accurately modeling these processes is essential,as they significantly influence the uncertain propagation of space trajectories,and an inadequate representation can result in either underestimation or overestimation of the stochastic characteristics associated with a given *** study addresses the gap in characterizing process uncertainties,represented as Gauss–Markov processes in mission analysis,by presenting models,evaluating derived quantities,and providing results on the impact of spacecraft *** study emphasizes the importance of accurately modeling random processes to properly characterize stochastic spacecraft paths.

The stability analysis for nonlinear differentialalgebraic systems is addressed using tools from classical control theory. Sufficient stability conditions relying on matrix inequalities are established via Lyapunov Direct Method. In addition, a novel interpretation of differential-algebraic systems as feedback interconnection of a purely differential system and an algebraic system allows reducing the stability analysis to a smallgain-like condition. The study of stability properties for constrained mechanical systems, for a class of Lipschitz differential-algebraic systems and for an academic example is used to illustrate the theory.

In this work, we focus on designing efficient numerical schemes to approximate a ther- modynamically consistent Navier-Stokes/Cahn-Hilliard problem given in [3] modeling the mixture of two incompressible fluids with different densities. The model is based on a diffuse-interface phase-field approach that is able to describe topological transitions like droplet coalescense or droplet break-up in a natural way. We present a splitting scheme, decoupling computations of the Navier-Stokes part from the Cahn-Hilliard one, which is unconditionally energy-stable up to the choice of the potential approximation. Some nu- merical experiments are carried out to validate the correctness and the accuracy of the scheme, and to study the sensitivity of the scheme with respect to different physical pa- rameters.

Ideal surgical treatment for anal fistula should aim to eradicate sepsis and promote healing of the tract,whilst preserving the sphincters and the mechanism of *** the simple and most distal fistulae,conventional surgical options such as laying open of the fistula tract seem to be relatively safe and therefore,well accepted in clinical ***,for the more complex fistulae where a significant proportion of the anal sphincter is involved,great concern remains about damaging the sphincter and subsequent poor functional outcome,which is quite inevitable following conventional surgical *** this reason,over the last twodecades,many sphincter-preserving procedures for the treatment of anal fistula have been introduced with the common goal of minimising the injury to the anal sphincters and preserving optimal *** them,the ligation of intersphincteric fistula tract procedure appears to be safe and effective and may be routinely considered for complex anal *** technique,the anal fistula plug,derived from porcine small intestinal submucosa,is safe but modestly effective in long-term follow-up,with success rates varying from 24%-88%.The failure rate may be due to its extrusion from the fistula *** obviate that,a new designed plug(GORE BioA ) was introduced,but long term data regarding its efficacy are *** glue showed poor and variable healing rate(14%-74%).Fi La C and video-assisted anal fistula treatment procedures,respectively using laser and electrode energy,are expensive and yet to be thoroughly assessed in clinical ***,a therapy using autologous adiposederived stem cells has been *** properties of regenerating tissues and suppressing inflammatory response must be better investigated on anal fistulae,and studies remain in *** aim of this present article is to review the pertinent literature,describing the advantages and limitations of new sphincterpreserving techniques.

The catalytic conversion of methanol to dimethyl ether(DME) over a series of home-made FER-type zeolites having different acidities and commercial γ-Al_2O_3 has been studied with the aim to understand the impact of adding water in the reactant stream on the catalytic behavior on investigated materials. Cofeeding water with methanol, the alcohol conversion was slightly reduced over the investigated zeolites while the catalytic activity of γ-Al_2O_3, the traditional catalyst of MeOH-to-DME conversion, was strongly inhibited. It was also found that, for the investigated zeolites, both the amount and the initial deposition rate of the coke formed during the reaction were reduced when water was co-fed with methanol while no significant effects on both methanol conversion and DME selectivity were observed under the investigated conditions.
