Multivariate Analysis of Extreme Physical, Biological and Chemical Patterns in the Dynamics of Aquatic Ecosystem

This study is a part of the research in monitoring systems of environmental impacts in coastal re-gions in order to develop trophic dynamic models to be used in the aquatic systems management. Meteorological influences in the variability of the nutrients, larvae concentration, dissolved oxygen (DO) and chlorophyll a were investigated in a region where upwelling occurs. Extreme seasonal variations of reanalysis, QuikSCAT, and surface stations from the southeast coast of Brazil, as well as, surface seawater data collected in Anjos Bay, Arraial do Cabo city northeast of Rio de Janeiro state, are analyzed. Seasonality and correlations are applied to verify the relationship between them, considering minimum values of sea surface temperature (SST) and sea level variation and maximum values of the other variables. Principal Component Analysis (PCA) and Hierarquical Cluster Analysis (HCA) are applied to verify spatial and temporal variances and to describe more clearly the structure of the local ecosystem. The seasonality of northeasterly extreme wind stress follows the seasonal pattern expected for the study area with peaks during spring. The SST has a well-defined seasonal pattern with maximum peaks from February to July and minimum peaks from September to January. Chlorophyll a presents higher seasonal peak in February, being in accordance with DO;both are related to the maximum primary productivity. Correlations of the physical variables (local and remote) with nutrients and larvae present a relatively similar pattern around 0.5, showing these variables have a reasonable interaction with the meteorological forcing. PCA shows a strong variability in pressure data around 0.9, which may be related to the seasonal variations in South Atlantic subtropical anticyclone (SASA) and consequently to the occurrence of upwelling in the region. Appropriateness of the study of iron deficiency anemia prior to referral for small bowel evaluation at a tertiary center

To evaluate the adequacy of the study of iron deficiency anemia (IDA) in real life practice prior to referral to a gastroenterology department for small bowel evaluation. METHODSAll consecutive patients referred to a gastroenterology department for small bowel investigation due to iron deficiency anemia, between January 2013 and December 2015 were included. Both patients referred from general practitioners or directly from different hospital departments were selected. Relevant clinical information regarding prior anemia workup was retrospectively collected from medical records. An appropriate pre-referral study was considered the execution of esophagogastroduodenoscopy (EGD) with Helicobacter pylori (H. pylori) investigation, colonoscopy with quality standards (recent, total and with adequate preparation) and celiac disease (CD) screening (through serologic testing and/or histopathological investigation). RESULTSA total of 77 patients (58.4% female, mean age 67.1 ± 16.7 years) were included. Most (53.2%) patients were referred from general practitioners, 41.6% from other hospital specialties and 5.2% directly from the emergency department. The mean pre-referral hemoglobin concentration was 8.8 ± 2.0 g/dL and the majority of anemias had microcytic (71.4%) and hypochromic (72.7%) characteristics. 77.9% of patients presented with an incomplete pre-referral study: EGD in 97.4%, with H. pylori investigation in 58.3%, colonoscopy with quality criteria in 63.6%, and CD screening in 24.7%. Patients with an appropriate study at the time of referral were younger (48.7 ± 17.7 vs 72.3 ± 12.3 years, P < 0.001). Small bowel evaluation was ultimately undertaken in 72.7% of patients, with a more frequent evaluation in patients with a quality colonoscopy at referral (78.6% vs 23.8%); P < 0.001 (OR = 11.7, 95%CI: 3.6-38.6). Schistosomiasis mansoni is a neglected disease and key public health problem,mainly due to its high prevalence,the scarcity of public policies,and the severity of some clinical *** fibrosis(PPF)is the commonest complication of chronic schistosomiasis mansoni and its diagnosis requires different *** though wedge biopsy of the liver is considered the gold standard,it is not justified in non-surgical patients,and percutaneous liver biopsy may be informative but does not have sufficient *** PPF tests mostly include biological(serum biomarkers or combined scores)or physical assessments(imaging assessment of fibrosis pattern or tissue stiffness).Moreover,imaging techniques,such as ultrasound,computed tomography,magnetic resonance imaging,and elastography are applied not only to support the diagnosis of schistosomiasis,but also to assess and detect signs of portal hypertension and organ damage due to chronic schistosomiasis.A combination between a comprehensive history and physical examination with biomarkers for liver fibrosis and imaging methods seems to offer the best approach for evaluating these *** addition,understanding their strengths and limitations will allow a more accurate interpretation in the clinical context and can lead to greater accuracy in estimating the degree of fibrosis in patients with Schistosomiasis mansoni(***)*** review will discuss the different noninvasive methods that are currently available for the evaluation of PPF in *** infection,and their application,advantages,and limitations in clinical practice.
