Acute diverticulitis in younger patients:Any rationale for a different approach?

AIM:To compare the natural history and course of acute diverticulitis in a younger age group with an older population and to evaluate whether younger patients should be managed differently. METHODS:This study was a retrospective review of 157 patients treated with acute diverticulitis between January 1,2004 and December 31,*** was stratified according to the Hinchey classification. Patients were divided into 2 populations:group A≤ 50 years(n=31) ;group B>50 years(n=126) .Mean patient follow-up was 15 mo. rESULTS:The median age was 60 years.A significantly higher proportion of patients in group B presented with complicated diverticulitis(36.5%vs 12.9%,P=0.01) .recurrence was more frequent in group A(25.8%vs 11.1%,P=0.03) and the mean time-torecurrence was shorter(12 mo vs 28 mo,P=0.26) . The most severe recurrent episodes of acute diverticulitis were classified as Hinchey stageⅠand none of the patients required emergency *** multivariate analysis,only age(P=0.024) was identified as an independent prognostic factor for recurrence. CONCLUSION:Based on the results of this study,we recommend that diverticulitis management should be based on the severity of the disease and not on the age of the patient.

Background: In patients undergoing pneumonectomy, intraoperative pulmonary and cardiac complications are the major cause of morbidity and mortality. Protective lung ventilation strategies may decrease the overall lung injury. right, ventricular dysfunction may occur during the surgery and after the pneumonectomy, in the early postoperative period, with reduced rV ejection fraction and increased rV end-diastolic volume index, caused by increased rV afterload. Case report: We describe the case of a 28-year-old non-smoker female who underwent to a right pneumonectomy. The patient presented intraoperative hemodynamic instability and signs of rV dysfunction, requiring vasoactive amines and nitric oxide. Discussion: This article is intended to provide an overview of the anesthetic management for pneumonectomy including the hemodynamic management and considerations of the causes and management of right ventricular dysfunction.

Allelopathy of the Leaf Extract of Eucalyptus Genetic Material on the Physiological Performance of Millet Seeds

This study aims to evaluate the possible allelopathic effects of young and mature leaf extract from of two eucalyptus genetic materials on the physiological performance of millet seeds. The experiment was carried out in a completely randomized design, in a 5 × 2 × 2 factorial arrangement [five concentrations of the extract (0, 25%, 50%, 75%, and 100%) × two genetic materials (1 - Eucalyptus grandis × Eucalyptus urophylla and 2 - Eucalyptus grandis × Eucalyptus camaldulensis) × two maturation stages of leaves (young and mature leaves)] with four replicates, each one with 25 seeds. Seeding was performed on the germitest paper substrate distributed in Petri dishes, moistened with the extract according to the treatments, being allocated in biochemical oxygen demand (B.O.D) adjusted at 25°C and 12 h photoperiod. Percentage, speed index, mean germination time, aerial part and primary root length, and dry matter weight of aerial part and root system of seedlings were evaluated. The leaf extracts of the two eucalyptus clones had all the analyzed variables changed with effect more evident on young leaf extract of the genetic material 1. It can be concluded that the tested genetic materials have a detrimental effect on the physiological performance of millet seeds.
