Efficacy of transcatheter embolization/chemoembolization (TAE/TACE) for the treatment of single hepatocellular carcinoma

AIm:To investigate the efficacy of transcatheter embolization/chemoembolization (TAE/TACE) in cirrhotic patients with single hepatocellular carcinoma (HCC) not suitable for surgical resection and percutaneous ablation therapy. mETHODS:A cohort of 176 consecutive cirrhotic patients with single HCC undergoing TAE/TACE was reviewed; 162 patients had at least one image examination (helical CT scan or triphasic contrastenhanced mRI) after treatment and were included into the study. TAE was performed with Lipiodol followed by Gelfoam embolization; TACE was performed with Farmorubicin prepared in sterile drip at a dose of 50 mg/m2,infused over 30 min using a peristaltic pump,and followed by Lipiodol and Gelfoam embolization. RESULTS:patients characteristics were:mean age,62 years; male/female 117/45; Child-pugh score 6.2 ± 1.1; mELD 8.7 ± 2.3; mean HCC size,3.6 (range 1.0-12.0) cm. HCC size class was ≤ 2.0 cm,n = 51; 2.1-3.0 cm,n = 35; 3.1-4.0 cm,n = 29; 4.1-5.0 cm,n = 22; 5.1-6.0 cm,n = 11; and > 6.0 cm,n = 14. patients received a total of 368 TAE/TACE (mean 2.4 ± 1.7). Complete tumor necrosis was obtained in 94 patients (58%),massive (90%-99%) necrosis in 16 patients (10%),partial (50%-89%) necrosis in 18 patients (11%) and poor (maining 34 patients (21%). The rate of complete necrosis according to the HCC size class was:69%,69%,52%,68%,50% and,13% for lesions of ≤ 2.0,2.1-3.0,3.1-4.0,4.1-5.0,5.1-6.0,and > 6.0 cm,respectively. Kaplan-mayer survival at 24-mo was 88%,68%,59%,59%,45%,and 53% for lesions of ≤ 2.0,2.1-3.0,3.1-4.0,4.1-5.0,5.1-6.0,and > 6.0 cm,respectively. CONCLUSION:Our study showed that in cirrhotic patients with single HCC smaller than 6.0 cm,TAE/TACE produces complete local control of tumor in a significant proportion of patients. TAE/TACE is an effective therapeutic option in patients with single HCC not suitable for surgical resection or percutaneous ablation therapies. Early peak of hydrogen during lactose breath test predicts intestinal motility

Lactose breath test (LBT) is considered the gold standard for the diagnosis of lactose malabsorption. The test is considered positive for a peak of hydrogen (H2) ≥ 20 parts per million (ppm) above the baseline. Some patients (pts) showed a rapid peak between 30 and 90 minutes after lactose ingestion. The aim of this study was to evaluate the predictive value of an early peak during a LBT and an accelerated oro-cecal transit time (OCTT). We retrospectively analyzed all pts who referred to our Gastroenterology unit for Irritable Bowel Syndrome, from January to September 2012, who performed LBT, glucose and lactulose breath test. We consider a positive LBT for a peak of H2 > 20 ppm, a positive GHBT for a peak >12 ppm and we considered a normal OCCT a peak of H2 ≥ 10 ppm between 75 ± 105 min after lactulose load. The correlation between LBT and OCTT was evaluated by pearson score. 93 pts (65 F/28 m mean age 47 ± 6 years) with a positive LBT, without small intestinal bacterial overgrowth were analyzed: 46 pts (32 F/14 m;mean age 48 ± 6 years) with an early peak (2 (≥20 ppm) were enrolled as case, and 47 pts matched for sex and age with a peak of H2 after 90 min were enrolled as controls. 72% (33/46) of the group with an early peak showed an accelerated, 17% (8/46) a normal and 11% (5/46) a delayed OCTT. meanwhile, in control group 40.4% (19/47) showed a normal, 57.5% (27/47) a delayed and just 1 pts an accelerated OCTT. The specificity and sensibility of LBT for an accelerated OCTT were 97.9% and 71.7% respectively. The positive predictive value of LBT for an accelerated OCTT is 97.1%;the negative predictive value is 78%. There is a significant correlation between LBT and OCTT (p The presence of an early peak of H2 between 30 and 90 min after the ingestion of 25 gr of lactose could predict the presence of an accelerated OCTT in 97% of pts. If confirmed by further study, in this subset of pts, lactulose breath test for evaluating OCTT could be avoided.

Long Lasting Effects of Breastfeeding on metabolism in Women with prior Gestational Diabetes

Background & Aims: Breastfeeding improves glucose tolerance in the early postpartum period of women with prior gestational diabetes GDm, but it is unclear whether future risk of metabolic alterations, like type 2 diabetes, is reduced. The aim of this study was to investigate the effect of lactation, three years after pregnancy, on glucose and lipid metabolism in women with prior gestational diabetes. materials & methods: A population of women with prior gestational diabetes (Carpenter and Coustan Criteria) was evaluated with comparison of results for “lactating” [BF] versus “nonlactating women” [non BF]. Breast feeding was defined [BF] if lasting? 4 weeks. In each woman a 75-g oral glucose tolerance test (OGTT) was performed to analyze the glucose tolerance, insulin sensitivity/resistance and b-cell function. Fasting serum was used to study their lipid profile (total cholesterol, high-density lipoprotein [HDL] cholesterol, low-density lipoprotein [LDL] cholesterol, and triglycerides), apolipoprotein B, apolipoprotein A1, homocysteine, fibrinogen, hsCRp, uric acid, microalbuminuria. Statistics: paired and Un-paired t-test, mann-Whitney and?χ2?tests were used, as appropriate. Results: A total of 81 women were evaluated (62 [BF] and 19 [non BF]). maternal age (37.1?±?4.6 vs 37.4?±?4.9 years), body mass index (26.3?±?5.6 vs 26.4?±?5.3 kg/m2), parity (1.9?±?0.8 vs 1.7?±?0.8) and length of follow-up (32.2?±?20.2 vs 32.1±?20,0) were not different between the two groups. No effect was visible on glucose tolerance, HOmA-IR and other b-cell function indexes as well as hs-CRp (not significantly lower in non BF), uric acid, total cholesterol, HDL and LDL cholesterol. Levels of significance were only reached for “HOmA-IS” [BF] 1.0?±?0.7 vs [non BF] 0.6?±?0.4, p = 0.04) and triglycerides [BF] 83.8?±46.7 vs [non BF] 123.2?±?94.0 mg/dl, p = 0.02). Long-term epilepsy associated tumors(LEAT) represent a well known cause of focal epilepsies. Glioneuronaltumors are the most frequent histological type consisting of a mixture of glial and neuronal elements and most commonly ariseing in the temporal lobe. Cortical dysplasia or other neuronal migration abnormalities often coexist. Epilepsy associated with LEAT is generally poorly controlled by antiepileptic drugs while, on the other hand, it is high responsive to surgical treatment. However the best management strategy of tumor-related focal epilepsies remains controversial representing a contemporary issues in epilepsy surgery. Temporo-mesial LEAT have a widespread epileptic networkwith complex epileptogenic mechanisms. By using an epilepsy surgery oriented strategy LEAT may have an excellent seizure outcome therefore surgical treatment should be offered early, irrespective of pharmacoresistance, avoiding both the consequences of uncontrolled seizures as well as the side effects of prolonged pharmacological therapy and the rare risk of malignant transformation.

In this paper we present the science potential of the enhanced X-ray Timing and polarimetry(eXTp) mission for studies of strongly magnetized objects. We will focus on the physics and astrophysics of strongly magnetized objects, namely magnetars, accreting X-ray pulsars, and rotation powered pulsars. We also discuss the science potential of eXTp for QED studies. Developed by an international Consortium led by the Institute of High Energy physics of the Chinese Academy of Sciences, the eXTp mission is expected to be launched in the mid 2020s.

背景Dupilumab是一种抗IL-4受体α亚基的抗体,可以阻断IL-4/IL-13的信号转导。IL-4/IL-13是2型/Th2型免疫疾病(如特应性/过敏性疾病)的关键驱动因子。一项重要的2b期临床研究(NCT01854047)显示,在使用了中高剂量吸入激素联合长效β2受体激动剂(ICS/LABA)后仍控制不佳的持续性哮喘患者中,dupilumab可减少哮喘重度急性发作,改善患者的肺功能和生活质量,且耐受性良好。目的探讨在哮喘并发常年性变应性鼻炎(pAR)患者中,dupilumab对SNOT-22的总分及其中变应性鼻炎(AR)相关症状评分的影响。方法对2b临床研究中每2周(q2w) 200 mg和300 mg dupilumab治疗后的数据进行析因分析。这两种剂量的dupilumab目前正在3期临床研究(NCT02414854)中进行验证。pAR定义为:在研究开始时,患者出现对典型的常年性抗原产生特异性应答(Ig E≥0. 35 Ku/L)。结果总共241例(61%)患者患有pAR。与安慰剂相比,每两周300 mg dupilumab显著改善了在哮喘并发pAR的患者中SNOT-22的总分(最小二乘平均差,-5. 98; 95%CI,-10. 45～-1. 5; p=0. 009)和4项AR相关症状的评分(鼻塞,-0. 60; 95%CI,-0. 96～-0. 25;流鼻涕,-0. 67; 95%CI,-1. 04～-0. 31;打喷嚏,-0. 55; 95%CI,-0. 89～-0. 21;鼻后分泌物,-0. 49;95%CI,-0. 83～-0. 16; 4项评分都p mg dupilumab治疗减少了并发pAR的患者的SNOT-22总分(-1. 82; 95%CI,-6. 46～2. 83; p=0. 443)和AR相关的症状评分,但与安慰剂相比无统计学差异。在无pAR的哮喘患者中,dupilumab与安慰剂治疗这些评分无差异。结论每两周300 mg dupilumab治疗显著改善了在控制不佳的持续性哮喘并发pAR患者中的AR相关的鼻部症状。
