Hepato-splenic lymphoma:a rare entity mimicking acute hepatitis:A case report

We reported a case of non-Hodgkin's lymphoma where liver involvement was the predominant clinical manifestation.A 27-year old man presented wiht markedly elevated serum aspartate aminotrasferase,alanine aminotransferase and lactate dehydrogenase,reduced prothrombin activity,thrombocytopenic purpura and hepato-splenomegaly without ***,toxic, autoimmune and metabolic liver diseases were excluded. Bone marrow biopsy showed an intracapillary infiltration of T-lymphocytes with no evidence of lipid storage disease. Because of a progressive spleen enlargement,splenectomy was *** examination showed lymphomatous intrasinuses invasion of the spleen. Indications and outcomes of endoscopic resection for nonpedunculated colorectal lesions:A narrative review

Colonoscopic evaluation of hematochezia in low and average risk patients for colorectal cancer:A prospective study

AIM: To relate the endoscopic findings in patients with hematochezia with regard to age in “low and average risk” for colorectal cancer (CRC) and to localize signifi cant lesions in order to identify patients who need sigmoidoscopy or total colonoscopy. METHODS: This prospective study was performed in an open access GI endoscopy unit. Out of 4322 consecutive patients undergoing colonoscopy, 918 reported hema- tochezia. The fi nal study group comprized 180 patients aged below 45 and 237 over 45. Main exclusion criteria were a 1st-degree family history of colorectal carcinoma, patients reporting blood mixed with stools and/or pro- gressive colonic symptoms, or patients who had under- gone colon surgery for neoplastic lesions. RESULTS: Total colonoscopy could be performed in 96% of patients. Abnormal findings were observed in 34.3% of the younger and in 65.7% of the older ones. Findings were the presence of polyps in the distal colon (n = 2) and IBD in the proximal colon (n = 29) in the group of the younger patients, and polyps (n = 15), IBD (n = 13), and carcinoma (n = 6, 4 of the lesions were located proximal to the splenic flexure) in the elderly. Our f indings suggest that the diagnostic potential of total colonoscopy in patients younger than 45 referring scant hematochezia, is not mandatory. By exploring only the distal tract of the colon we have misdiagnosed two cases of IBD located in the ascending colon. In this group of patients additional risk factors must be identifi ed before performing a total colonoscopy. Regarding the patients older than 45 yr, the exploration of the distal colon would have led to our overlooking a carcinoma, two neoplastic polyps and one IBD located in the proximal colon. CONCLUSION: Young patients with scant hematochezia but without risk factors for neoplasia do not need a totalcolonoscopy, whereas is mandatory performing a total colonoscopy in older patients even in the presence of anal pathology.

Background:Hypertrophic scars and keloids,common complications following median sternotomy for cardiac surgery,signiﬁcantly impact patient quality of life due to their aesthetic and symptomatic *** advancements in laser therapy have made it a prominent option for managing these complex scars,yet a com-prehensive understanding of its efﬁcacy is *** aim of this scoping review is to explore the effects of laser therapy in managing hypertrophic scars and keloids after median ***:This scoping review ana-lyzed studies up to February 2024 from databases including PubMed,EMBASE,CINAHL,Scopus,Web of Science,and the Cochrane *** included any study that assessed laser therapy’s effects on hypertrophic scars and keloids following median *** were selected based on predeﬁned inclusion criteria with-out publication year,design,or origin ***:Six studies met the inclusion criteria,involving a total ofﬁve RCTs and one *** studies primarily tested 585 and 595-nm pulsed dye laser(PDL)treatments,focusing on scar appearance,patient symptoms,and treatment *** studies reported signiﬁcant improvements in scar height reduction and patient symptom relief after treatment,with mixed results for scar erythema and *** events were generally mild and ***:Laser therapy offers a beneﬁcial approach for improving the appearance and symptoms of hypertrophic scars and keloids post-median ***,further research is necessary to optimize treatment parameters and explore the long-term psychosocial impacts of this *** review highlights the need for more comprehensive studies to establish standardized treatment protocols and evaluate their effectiveness.

Taguchi-Optimized Catalytic Growth of Carbon Nanotubes for Applications in Electro-Catalysis

Taguchi method, largely used to optimize processes controlled by manifold parameters, has been utilized to improve the synthesis of carbon nanotubes by chemical vapor deposition of isobutane. Analyzing results obtained in nine suitably designed reactions, the influence of synthesis （773-973 K）, calcinations （723-1,023 K） and reduction （773-973 K） temperatures and catalyst-support （alumina, magnesia or Na＋-exchanged K10 clay） on specific yield and crystallization degree （i.e., C sp2 content） of the nanotubes has been ranked. After critical examination and adjusting of conditions predicted to give optimal results, -50 g of nanotubes per gram of metal are obtained at 973 K over Fe/alumina catalysts calcined at 723 K and reduced at 773 K. Under the same conditions, highly crystallized nanotubes （with 73% of C sp2 bonds, as qualitatively estimated from Raman spectroscopy）, suitable for electro-catalysis applications, are synthesized over Fe/clay catalysts.

Family history of irritable bowel syndrome is the major determinant of persistent abdominal complaints in young adults with a history of pediatric recurrent abdominal pain

AIM： To assess the late outcome of teen-agers with a previous history of recurrent abdominal pain （RAP） or irritable bowel syndrome （IBS）. METHODS： A group of 67 children with RAP referred to the department from January 1986 to December 1995 was followed up between 5 and 13 years after the initial diagnosis by means of a structured telephone interview. We hypothesized that those patients with persistent adult IBS-like symptoms would be significantly more likely to report a family history oflBS in comparison with adults with no persistent abdominal complaint. RESULTS： Out of the 52 trackable subjects, 15 were found to present IBS-like symptoms at follow-up （29%） whereas the majority （37 subjects） did not. Subjects with IBS-like symptoms were almost three times more likely to present at least one sibling with similar symptoms compared to subjects not complaining （40.0% vs 16.0%）, respectively （P 〈 0.05 at Student t test）. Subjects with IBS-like symptoms also reported a higher prevalence of extra-intestinal symptoms, such as back pain, fibromyalgia, headache, fatigue and sleep disturbances. CONCLUSION： The study confirms previous observations indicating that pediatric RAP can predict later development of IBS. The latter appears to be greatly influenced by intrafamilial aggregation of symptoms, possibly through the learning of a specific illness behavior.
