Surgical repair of intractable chylous ascites following laparoscopic anterior resection

Chylous ascites is the accumulation of a milk-like peritoneal fluid rich in triglycerides and it is an unusual complication following surgical treatment of colorectal cancer. Conservative management is usually sufficientin patients with chylous ascites after surgery. However,we describe a patient with intractable chylous ascites after laparoscopic anterior resection for sigmoid colon cancer who failed initial conservative treatment. This patient was successfully managed by surgery.

Cholecystocolic fistula secondary to gallbladder carcinoma is extremely rare and has been reported in very few studies. Most cholecystocolic fistulae are late complications of gallstone disease, but can also develop following carcinoma of the gallbladder when the necrotic tumor penetrates into the adjacent colon. Although no currently available imaging technique has shown great accuracy in recognizing cholecystocolic fistula, abdominopelvic computed tomography may show fistulous communication and anatomical *** we report an unusual case of cholecystocolic fistula caused by gallbladder carcinoma, which was preoperatively misdiagnosed as hepatic flexure colon carcinoma.

In this work,variation in the dynamic recrystallization(DRX)and dynamic precipitation behavior of AZ80 alloy during extrusion due to changes in extrusion temperature was investigated,and the resultant microstructure and yield asymmetry were *** the extrusion temperature increases from 250℃to 350℃,the primary DRX mechanism changes from twin-induced DRX to discontinuous DRX,resulting in an increase in the DRX area fraction and un DRXed grain *** addition,as the extrusion temperature rises,Mg17Al12 precipitation during extrusion decreases sharply throughout the extruded *** reduction in the compressive yield strength(CYS)with increasing extrusion temperature is more pronounced than it is for the tensile yield strength(TYS),which ultimately increases the yield asymmetry of the extruded *** higher extrusion temperature has less of an influence on the TYS due to the promotion of certain hardening *** the other hand,the greater reduction in the CYS is attributed to the increased fraction and size of regions in which{1012}twins predominantly form and the lower amount of precipitates,which effectively facilitates{1012}twinning.

A strategy for fabricating microcrystalline cellulose–Ti_(3)C_(2)T_(x)(MCC–MXene)nanocomposite films with high relative permittivity,high thermal conductivity,and excellent mechanical properties was *** MCC–MXene nanocomposite film was fabricated by casting a solution containing N,N-dimethylacetamide/lithium chloride(DMAc/LiCl)-soluble MCC and DMAcdispersible MXene nanosheets,followed by humidity control *** MXene nanosheets greatly enhanced the permittivity of the nanocomposite films owing to interfacial ***,the nanocomposite film with 20 wt.%MXene content achieved a desirable permittivity of 71.4 at 102 Hz(a 770%improvement against that of neat cellulose),while the dielectric loss only increased by 1.8 times(from 0.39 to 0.70).The obtained nanocomposite films with 20 wt.%and 30 wt.%MXene exhibited remarkable in-plane thermal conductivities of 8.523 and 9.668 W∙m^(−1)∙K^(−1),respectively,owing to the uniform dispersion and selfalignment of the MXene layered ***,the uniformly dispersed MXene nanosheets in the MCC network with interfacial interaction(hydrogen bonding)and mechanical entanglement endowed the nanocomposite films with excellent mechanical properties and ***,the thermal stability,water resistance,and antibacterial properties of the nanocomposite films were effectively improved with the introduction of ***,using DMAc/LiCl as the solvent system not only improves the compatibility between MCC and MXene but also avoids the problem of easy oxidation of MXene in aqueous *** the high stability of the MCC–MXene solution and enhanced properties of the MCC–MXene films,the proposed strategy manifests great potential for fabricating natural biomass-based dielectric materials.

BACKGROUND: Although perioperative portal vein (PV) stent implantation is an effective treatment for steno-occlusive disease in adult living donor liver transplantation (LDLT) re-cipients, we experienced high incidence of biliary anastomotic strictures (BAS) after PV stenting. In this study, we sought to clarify the relation between BAS and PV stenting and to suggest the possible mechanism of BAS and measures to reduce its in-cidence. METHODS: We retrospectively analyzed 44 LDLT recipients who underwent PV stent implantation across the line of PV anastomosis regardless of the location of steno-occlusion (stent group) and their matched controls (non-stented LDLT recipients, n=131). RESULTS: The incidence of BAS was higher in patients in the stent group than that in the control group (43.2% vs 17.6%, P=0.001). Cumulative 6-month and 1-, 2- and 5-year BAS rates were 31.8%, 34.1%, 41.4% and 43.2%, respectively, in the stent group and 13.0%, 13.8%, 16.1% and 17.8%, respectively, in the control group (P=0.001). Multivariate analysis revealed that PV stenting was an independent risk factor for BAS. CONCLUSIONS: Although PV stent implantation is a reliable treatment modality for steno-occlusive PV in adult LDLT recipients, innovative methods to prevent the PV stent from crossing the line of PV anastomosis may be necessary to reduce the incidence of postoperative BAS.

Cowden syndrome is a rare autosomal dominant disorder that is characterized by multiple hamartomas in a variety of tissues and this is associated with germline mutations in the phosphatase and tensin homologue (PTEN) gene, which is the tumor suppressor gene located on chromosome 10q23.3. It is characterized by multiple hamartomatous neoplasms of the skin, oral mucosa, gastrointestinal (gi) tract, bones, central nervous system, eyes, and genitourinary tract. Cowden syndrome does not have increased risk of gi malignancy; however, it has an increased risk of breast, thyroid and endometrial cancer development. Herethe authors report a rare case of Cowden syndrome incidentally diagnosed from multiple gastric polyposis. A 29-year-old woman presented with multiple gastric polyps. The laboratory results were normal except for mild anemia, with a hemoglobin level of 11.9 g/dL. Esophagogastroduodenoscopy revealed multiple gastric, duodenal polyps and esophageal acanthosis. Colonoscopy revealed possible hamartomatous polyps in the rectum. Under the suspicion of Cowden syndrome, sonography of the thyroid and breasts was carried out, which revealed multiple thyroid masses. Subsequent fine-needle aspiration biopsy revealed the presence of clusters of follicular epithelial cells, and due to the possibility of malignancy, the patient underwent total thyroidectomy. The pathology was reported as invasive follicular carcinoma. A gene study by direct sequencing showed the presence of a PTEN mutation (c.633C > A /***211*).
