Culture and Sensitivity Patterns of Various Antibiotics Used for the Treatment of Pediatric Infectious Diarrhea in Children under 5 Years of Age: A Tertiary Care Experience from Karachi

Background and objective: Infectious diarrhea is one of the most common infections and a significant cause of morbidity and mortality in children under 5 years of age. Frequent and irrational use of antibiotics has resulted in increased bacterial resistance. The aim of our study was to determine the culture and sensitivity patterns of antibiotics used for the treatment of diarrhea in children less than 5 years of age in a tertiary care hospital of Karachi, Pakistan. Methodology: This cross sectional study was conducted for a period of six months in the children of ages ranging between 6 months and 5 years. Stool samples were obtained from the patients presented with signs and symptoms of diarrhea in OPD or being referred to microbiology department for stool C/S (culture and sensitivity). Data were analyzed on SPSS version 19.0. Results: A total number of 325 stool samples were collected, out of which 152 samples were positive for pathogens. The most common pathogen isolated was E. coli 92 (60%), followed by Klebsiella 56 (37%) and Salmonella 4 (3%). All 152 isolates were highly resistant to majority of the standard antibiotics. Conclusion: Infectious diarrhea is highly prevalent among children under 5 years of age. E. coli was found to be the most frequent pathogen isolated in stool samples of the patients presenting with diarrhea and was highly resistant to many of the commonly used standard antibiotics in our clinical set up. Male children were predominantly affected by infectious diarrhea as compared to female children. Therefore, injudicious use of antibiotics should be stopped as well as should not be prescribed empirically for the treatment of all cases of pediatric diarrhea.

Background: Diarrhoea is the second leading cause of childhood mortality in children under five years old. Water is a major contributing risk factor for this disease that is a common illness and a global killer. Material and Methods: Water samples from different areas of Karachi were collected and were processed by MPN technique to evaluate the presence of microbiological substances. Results: Out of the processed samples, 64% were positive for the presence of mix enteric pathogen from different areas of Karachi;from Bhens colony, 74% were positive for the presence of fecal coli forms, among which 4% were positive for EHEC isolated from storage tank and water tank. Conclusion: The abovementioned results reflect the presence of organisms of public health importance in different sources of drinking water. According to WHO guidelines, there should be zero tolerance of these organisms in water.

Background and objectives: Irrational and repeated use of broad spectrum antibiotics for infectious diarrhea in children has resulted in their increased resistance along with several systemic toxic effects. Probiotics are also used in the management of infectious diarrhea as these are supposed to be favorable in promoting overall health benefits including stability of the intestinal flora. However, these agents are not used as an alternative to antibiotics as their exact bactericidal/bacteriostatic effects have not been evaluated on the basis of any clinical or in-vitro samples (Culture and Sensitivity test). Hence the aim of our study was to compare the culture and sensitivity patterns of standard antibiotics and two probiotics, Lactobacilli (Lactobacillus paracasei/Lactobacillus acidophilus) and Saccharomyces boulardii used for the treatment of infectious diarrhea in children less than 5 years of age in a tertiary care hospital of Karachi, Pakistan. Methodology: This prospective quasi experimental study was conducted for a period of six months. After getting informed consent from parents/guardians, the stool samples were obtained from children of ages, 6 months to 5 years, presented with signs and symptoms of diarrhea in outpatient department (OPD) or being referred to microbiology department for stool C/S (culture and sensitivity). The sensitivity patterns of the cultured isolates were assessed for standard antibiotics according to the CLSI guidelines (2018), while the two probiotics (Lactobacilli and Saccharomyces boulardii) were evaluated by means of Dried Modification method. The data was analyzed using statistical software SPSS version 19.0. Results: A total number of 325 stool samples were collected, out of which 152 samples were positive for pathogens i.e. E. coli, Klebsiella and Salmonella typhi. Hepatitis C virus is associated with Hepatitis C which can be acute or chronic. Acute hepatitis C is usually short term illness that can lead to chronic infection, while chronic hepatitis C is a chronic liver disease leading to cirrhosis, hepatocellular carcinoma and even death. The mode of transmission is mainly parenteral and vertical. Hepatitis C is the leading cause of morbidity and mortality and a serious public health problem, worldwide as well as in Pakistan. There is no vaccine available for hepatitis C so the only way to prevent hepatitis C is by changing behaviors which can spread the disease. Pakistan is also facing a huge burden of this disease. In Pakistan, the frequency of hepatitis C virus infection ranges from 8% - 15% in the general population with variations in different parts of the country. The general method of detecting infection with HCV is to observe the presence of antibodies to the virus. The aim of this study was to determine frequency of hepatitis C among general population and to see the increasing frequency in our part. Selection of Patients: All patients visiting outpatient clinics, Hepatitis clinic and indoor patients at Civil Hospital Karachi, Pakistan from January 1, 2010 to December 31, 2014 were screened for anti HCV. Methods: Blood samples were collected from the patients in Red Topped vaccutainers and allowed to clot, then centrifuged and serum was screened for anti HCV by Immunochromatography (ICT) method. Results: A total of 156,990 patients were screened for anti-HCV, 23,511 (14.98%) patients were reported positive. During the year 2010, total 11,058 patients were screened for anti HCV, 1791 (16%) were positive, while in the year 2011 total 26,921 were screened and 3908 (14.5%) were positive, during the year 2012, total 23,321 were screened, 3036 (14%) were positive, in the year 2013 we screened total 40,483 patients 6070 (15%) were positive, while in 2014, total 55,484 patients were screened and 8584 were positive (15.5%). The primary technique used for air traffic surveillance is ***,nowadays,its role in surveillance is gradually being replaced by the recently adopted Automatic Dependent Surveillance-Broadcast(ADS-B).ADS-B offers a higher accuracy,lower power consumption,and longer range than radar,thus providing more safety to *** coverage of terrestrial radar and ADS-B is confined to continental parts of the globe,leaving oceans and poles uncovered by real-time surveillance *** study presents an optimized Low-Earth Orbit(LEO)-based ADS-B constellation for global air traffic surveillance over intercontinental trans-oceanic flight *** optimization algorithm is based on performance evaluation parameters,i.e.,coverage time,satellite availability,and orbit stability(precession and perigee rotation),and communication *** results indicate that the constellation provides ample coverage in the simulated global oceanic *** constellation is a feasible and cost-effective solution for global air supervision,which can supplement terrestrial ADS-B and radar systems.

Developing a simple scalable method to fabricate electrodes with high capacity and wide voltage range is desired for the real use of electrochemical ***,we synthesized amorphous NiCo-LDH nanosheets vertically aligned on activated carbon cloth substrate,which was in situ transformed from Co-metal-organic framework materials nano-columns by a simple ion exchange process at room *** to the amorphous and vertically aligned ultrathin structure of NiCo-LDH,the NiCo-LDH/activated carbon cloth composites present high areal capacities of 3770 and 1480 mF cm^(-2)as cathode and anode at 2 mA cm^(-2),and 79.5%and 80%capacity have been preserved at 50 mA cm^(-2).In the meantime,they all showed excellent cycling performance with negligible change after>10000 *** fabricating them into an asymmetric supercapacitor,the device achieves high energy densities(5.61 mWh cm^(-2)and 0.352 mW cm^(-3)).This work provides an innovative strategy for simplifying the design of supercapacitors as well as providing a new understanding of improving the rate capabilities/cycling stability of NiCo-LDH materials.

The high-gap plant protection machine is taken in this paper as the research object to ensure the good driving power and safety of the high-gap plant protection machine,and the control strategy of inter-shaft torque distribution is established under different working conditions to improve vehicle power and lateral *** anticipated demand torque is initially determined based on the structural characteristics and operational principles of the plant protection ***,a hierarchical control framework is devised by incorporating a formulated switching control ***,a simulation model for torque distribution control strategy between shafts is developed on the Matlab/Simulink platform,followed by simulation and experimental *** results are presented as follows:the inter-shaft torque distribution strategy established in this paper increases the average longitudinal acceleration by 0.13 m/s^(2) and 0.14 m/s^(2) under the control of low and high to low adhesion road surfaces,*** the control of the single-line shifting condition,the yaw velocity can successfully follow the expected value with a maximum value of 0.61 rad/*** side deflection angle of the center of mass does not exceed 2.8°,which can follow the ideal trajectory and improve power and safety.

A Robust Method of Bipolar Mental Illness Detection from Facial Micro Expressions Using Machine Learning Methods

Bipolar disorder is a serious mental condition that may be caused by any kind of stress or emotional upset experienced by the *** affects a large percentage of people globally,who fluctuate between depression and mania,or vice versa.A pleasant or unpleasant mood is more than a reflection of a state of ***,it is a difficult task to analyze through physical examination due to a large patient-psychiatrist ratio,so automated procedures are the best options to diagnose and verify the severity of *** this research work,facial microexpressions have been used for bipolar detection using the proposed Convolutional Neural Network(CNN)-based *** Action Coding System(FACS)is used to extract micro-expressions called Action Units(AUs)connected with sad,happy,and angry *** have been conducted on a dataset collected from Bahawal Victoria Hospital,Bahawalpur,Pakistan,Using the Patient Health Questionnaire-15(PHQ-15)to infer a patient’s mental *** experimental results showed a validation accuracy of 98.99%for the proposed CNN modelwhile classification through extracted featuresUsing SupportVectorMachines(SVM),K-NearestNeighbour(KNN),and Decision Tree(DT)obtained 99.9%,98.7%,and 98.9%accuracy,***,the outcomes demonstrated the stated method’s superiority over the current best practices.

采用冷冻/碳化法制备了铁钴双金属粒子电极(Fe,Co/NPs@GO),并通过改善铁钴的前驱体比例优化了Fe,Co/NPs@GO双金属三维粒子电极体系对CBZ的降解性能,探讨电流密度和PMS投加量对CBZ降解的影响结果表明:0.15g/L的Fe,Co/NPs@GO(Fe:Co=1:1),3mmol/L PMS,在20mA/cm^(2)的电流密度下,CBZ(1mg/L)在30min内的去除率达99.5%.与单纯的催化剂活化PMS体系相比,加入电催化条件后,CBZ的去除率提升了约37%.通过自由基淬灭实验及电子顺磁共振(EPR)证实,羟基自由基和单线态氧是降解CBZ的主导活性物种,并推测出CBZ在电催化耦合三维粒子电极活化过一硫酸盐(PMS)体系的降解机理.同时,粒子电极的循环利用实验表明,Fe,Co/NPs@GO粒子电极具有较好的催化稳定性和重复利用性.
