Although the prognosis of early gastric cancer (EGC) is considered to be satisfactory, some patients experience tumor relapse after curative surgery. Both pathogenesis and risk factors of recurrence remain unclear. We describe a case report of a 49-year-old male who underwent subtotal gastric resection D2A for angular gastric cancer. Histological examination revealed gastric adenocarcinoma with low grade of differentiation and colloid areas, intramucosal, and absence of neoplastic proliferation in the surgical margins, in omental stroma and in the six examined lymph nodes (pT1, pN0). 11 years later, the same patient underwent D2 total gastrectomy for gastric cancer in the remnant stomach. New histological examination revealed again gastric adenocarcinoma, intramucosal, medium degree of differentiation, no documentable neoplastic proliferation within the limits of surgical resection, in the thirty-three examined lymph nodes and in the omentum (pT1, pN0).

Background: Gradients in local environmental characteristics may favour the abundance of species with particular traits, while other species decline, or favour species with different traits at the same time, without an increase in average species abundances. Therefore, we asked: do variations in species and traits differ along gradients of deadwood variables? Do species abundance and trait occurrence change with species richness within or between functional groups? Thus, we analysed the beetle assemblages of five forest sites located in Italy, along the Apennines ***: From 2012 to 2018 we sampled beetles and five deadwood types in 193 plots to characterise the deadwood gradient: standing dead trees, snags, dead downed trees, coarse woody debris, and stumps. We modelled beetle species relative abundances and trophic traits occurrences against the deadwood variables using joint species distribution ***: Out of 462 species, only 77 showed significant responses to at least one deadwood type, with a weak mean response across species. Trophic groups showed mostly negative responses to deadwood variables. Species abundance increased with species richness among sites only for phytophagous and saproxylophagous. Trait occurrence did not increase with species richness among sites, except for phytophagous and ***, trait occurrence changed significantly with species richness of several trophic groups within some *** found that increases in species richness do not result in decreases in species abundance of a given trophic group, but rather null or positive relationships were found suggesting low interspecific ***: Our findings suggest that in Mediterranean mountain forests there is still room for increasing the level of naturalness, at least for what concerns deadwood management. Parathyroid hormone pulsatility: physiological and clinical aspects

Parathyroid hormone （PTH） secretion is characterized by an ultradian rhythm with tonic and pulsatile components. In healthy subjects, the majority of PTH is secreted in tonic fashion, whereas approximately 30% is secreted in low-amplitude and high-frequency bursts occurring every 10-20 min, superimposed on tonic secretion. Changes in the ultradian PTH secretion were shown to occur in patients with primary and secondary osteoporosis, with skeletal effects depending on the reciprocal modifications of pulsatile and tonic components. Indeed, pathophysiology of spontaneous PTH secretion remains an area potentially suitable to be explored, particularly in those conditions such as secondary forms of osteoporosis, in which conventional biochemical and densitometric parameters may not always give reliable diagnostic and therapeutic indications. This review will highlight the literature data supporting the hypothesis that changes of ultradian PTH secretion may be correlated with skeletal fragility in primary and secondary osteoporosis.

Fecal calprotectin(FC) has emerged as one of the most useful tools for clinical management of inflammatory bowel diseases(IBD). Many different methods of assessment have been developed and different cutoffs have been suggested for different clinical settings. We carried out a comprehensive literature review of the most relevant FC-related topics: the role of FC in discriminating between IBD and irritable bowel syndrome(IBS) and its use in managing IBD patients In patients with intestinal symptoms, due to the high negative predictive value a normal FC level reliably rules out active IBD. In IBD patients a correlation with both mucosal healing and histology was found, and there is increasing evidence that FC assessment can be helpful in monitoring disease activity and response to therapy as well as in predicting relapse, post-operative recurrence or pouchitis. Recently, its use in the context of a treat-to-target approach led to a better outcome than clinically-based therapy adjustment in patients with early Crohn's disease. In conclusion, FC measurement represents a cheap, safe and reliable test, easy to perform and with a good reproducibility. The main concerns are still related to the choice of the optimal cut-off, both for differentiating IBD from IBS, and for the management of IBD patients.
