Long-term survival of a patient with advanced pancreatic cancer under adjunct treatment with Viscum album extracts: A case report

BACKgROUND Advanced pancreatic cancer(aPC) has a poor prognosis with limited survival benefit from current standard treatment. Viscum album extracts(VAE) are used by many cancer patients, showing immune-stimulating effects, improved quality of life, and a survival benefit in patients with *** SUMMARY A 59-year-old architect developed epigastric pain. A cystic lesion of the pancreas of 45-mm diameter was detected. In a follow-up magnetic resonance imaging,about one year later, multiple lesions were seen in the corpus and the tail of the pancreas; CA-19-9 was elevated to 58.5 U/mL. A distal pancreatectomy with splenectomy was performed, and a tumor of 7 cm × 5 cm × 3.5 cm was *** investigation showed an intraductal papillary mucinous neoplasmassociated invasive adenocarcinoma with invasion of the lymph vessels,perineural invasion, and positive nodes(2/27); surgical margins showed tumor cells, and the tumor was classified as pT3 N1 M0 R1. The patient was treated with radiation of the tumor bed and capecitabine/oxaliplatin followed by gemcitabine and FOLFIRINOX. Seven months after surgery, a liver metastasis was detected and treatment with FOLFIRINOX was started. Four months after detection of the metastasis, the patient opted for additional treatment with VAE. Another month later, the metastasis was treated with radiofrequency ablation(RFA). Eight months later, the hepatic lesion recurred and was again treated with RFA. Thecontinuous VAE treatment was increased in dose, and the patient stayed recurrence-free for the next 39 mo in good health and working full-time(as of the time this case report was written).CONCLUSION We present the case of a patient with aPC with R1-resection with development of liver metastasis during the course of treatment who showed an overall survival of 63 mo and a relapse-free survival of 39 mo under increasing VAE therapy. Multimodality Treatment in Adult Patients with High-risk Soft-tissue Sarcomas

Objective： We enrolled retrospective data to determine the efficacy of combined chemotherapy and surgery for local tumour control and survival in patients with high-risk soft-tissue sarcomas. Methods： We collected data from 25 patients with high-risk soft-tissue sarcomas treated with four cycles of etoposide, ifosfamide and doxorubicin （EIA） followed by definitive surgery with or without postoperative radiotherapy and adjuvant chemotherapy. 21 patients received chemotherapy in a neoadjuvant/adjuvant clinical setting; eighteen of them completed adjuvant chemotherapy. Four patients received chemotherapy in an adjuvant setting only. Results： The objective response rate of neoadjuvant chemotherapy assessable in 21 patients was 43%. Including NED （n=7） and partial remissions （n=3）, the radiographic response rate was 47.6% with additional 42.9% stable diseases （n=9）. Surgery was performed in two patients before completing four neoadjuvant chemotherapy cycles because of disease progression. Median overall survival for all patients was 21.6＋ months. After completion of chemotherapy, in 62% of patients R0-resection could be performed. Conclusion： High proportion of R0-resections supports the idea of tumour down-staging after neoadjuvant treatment. Response to neoadjuvant chemotherapy is predictive for improved local tumour control resulting in long-term survival benefit.

本文报道用作者建立的流式细胞仪红细胞微核自动检测技术，将染色体断裂剂丝裂霉素Ｃ（ＭＭＣ）和非整倍体毒剂秋水仙碱（ＣＯＭ）诱导的大量微核分选在载玻片上，然后使用小鼠着丝粒γ－卫星ＤＮＡ探针（约为２３４ｂｐ），对分选微核进行荧光原位杂交（ＦＩＳＨ），以显示微核（ＭＮ）内着丝粒的情况，进而判定ＭＮ是由整条染色体还是由染色体断片组成。结果ＭＮ内着丝粒荧光阳性比例为ＣＯＭ５０．１％，ＭＭＣ２２．３％。两者相差显著，藉此方法可以准确有效地将两类毒剂区分开。

Real case of primitive embryonal duodenal carcinoma in a young man

We report here the case of a young man suffering from a rare germ cell tumour. The patient was a 25-year-old man who was referred to our centre for asthenia, stinging epigastric pain, and an iron deficiency anaemia. gastroscopy revealed a circumferential vegetating lesion on the second portion of the duodenum. The lesion was indurated at the third portion of the duodenum, responsible for a tight stenosis. A computerized tomography-scan of the chest, abdomen and pelvis, and a pancreatic MRI showed a circumferential lesion with a bi-ductal dilatation (i.e., of the common bile duct and Wirsung’s duct) without metastatic localisation. The patient underwent a pancreaticoduodenectomy with lymph node dissection including all cellular adipose tissues of the hepatic pedicle from the hepatic common artery and of the retroportal lamina. Histological findings were suggestive of a duodenal embryonal carcinoma with pancreatic infiltration. This is the second published case highlighting the duodenal primitive localisation of an embryonal carcinoma with pancreatic infiltration.

＜正＞Cold strip producers are permanently seeking for mill equipment and automation solutions,which ensure the most efficient rolling operation and fulfill the demands of the future market in terms of strip quality,dimensional performances and steel grade mix to be produced. Today the trend to use high-strength steel grades such as dual phase or TRIP steels in the automotive industries motivates the steel producers to invest in rolling mill equipment and automation solutions,which are able to produce these grades although the percentage of the product mix is rather small.A big portion of the product mix still presents the traditional steel grades. Siemens VAI sees its responsibility as cold rolling mill supplier to understand all requirements of the cold strip producer including strip quality,investment,production and maintenance *** in depth analysis of the product mix is necessary to determine the best equipment and automation solution to fulfil customer requirements for high strength *** paper explains how the maximum reduction,the required flatness adjustment range and mill stiffness influence the optimum solution especially when it comes to rolling of advanced high strength *** projects executed or currently under way by Siemens VAI for each of the mill types will be explained as well as the reasons for the individual solutions:
