obstructive uropathy is defined as the structural or functional interruption of urinary outflow at any level in the urinary *** is regarded as one of the most prevalent causes of acute kidney injury(AKi),accounting for 5%–10%of *** severe obstruction of the urinary tract is a potentially threatening situation for the kidneys and therefore requires prompt identification and management to relieve *** aim of the present article is to review and synthesize available evidence on obstructive uropathy,providing a clinical guideline for clinicians.A literature review on obstructive uropathy in the context of AKi was performed,focusing on the least clarified aspects regarding diagnosis and *** literature searching was conducted in English and top-level evidence articles including systematic reviews,metanalyses and large series were *** obstruction of the urinary tract is a diagnostic and therapeutical challenge that may lead to important clinical complications together with direct structural and hemodynamic damage to the *** recognition of the leading cause and its exact location is essential to ensure prompt urinary drainage together with the most suitable drainage technique selection.A multidisciplinary approach,including urologists,nephrologists,and other medical specialties,is best suited to correctly manage concomitant hemodynamic changes,fluid and electrolyte imbalances,and other related *** uropathy is one of the leading causes of *** of patients suitable for early diversion and feasibility or adequate selection of the indicated technique is sometimes challeng-ing.A thorough understanding of the physiopathology behind the development of urinary obstruction is vital for correct diagnosis and management.

AiM： To determine the association of Helicobacter pylori （H pylorl） CagA^＋ infection and pro-inflammatory polymorphisms of the genes interleukin （iL）-irN and iL-1B with the risk of gastric atrophy and peptic ulcers in a dyspeptic population in Costa rica, a country with high incidence and mortality of gastric cancer. METHoDS： Seven biopsy specimens, a fasting blood sample and a questionnaire concerning nutritional and sociodemographic factors were obtained from 501 consecutive patients who had undergone endoscopy for dyspeptic symptoms. A histopathological diagnosis was made. Pepsinogen concentrations were analyzed by enzyme linked immunosorbent assay （ELiSA）. infection with H pylori CagA^＋ was determined by serology and polymerase chain reaction （PCr）. iL-1B and iL-1rN polymorphisms genotyping was performed by PCrrestriction fragment length polymorphism （PCr-rFiP）and PCr respectively. rESULTS： in this dyspeptic population, 86% were Hpylori positive and of these, 67.8% were positive for CagA. Atrophic antral gastritis （AAG） was associated with CagA^＋ status [odd ratio （or） = 4.1; P 〈 0.000] and fruit consumption （or = 0.3; P 〈 0.00）. Atrophic body gastritis （ABG） was associated with pepsinogen PGi/PGii 〈 3.4 （or = 4.9; P 〈 0.04） and alcohol consumption （or = 7.3; P 〈 0.02）. Duodenal ulcer was associated with CagA^＋（or = 2.9; P 〈 0.04） and smoking （or = 2.4; P 〈 0.04）. PGi 〈 60 μg/L as well as PGi/PGii 〈 3.4 were associated with CagA^＋. CoNCLUSioN： in a dyspeptic population in Costa rica, H pylori CagA^＋ is not associated with ABG, but it is a risk factor for AAG. The pro-inflammatory cytokine poly- morphisms iL-1B ＋ 3945 and iL-1rN are not associated with the atrophic lesions of this dyspeptic population.

An improvement in pancreatic cancer treatment represents an urgent medical goal. Late diagnosis and high intrinsic resistance to conventional chemotherapy has led to a dismal overall prognosis that has remained unchanged during the past decades. increasing knowledge about the molecular pathogenesis of the disease has shown that genetic alterations, such as mutations of K-ras, and especially epigenetic dysregulation of tumor-associated genes, such as silencing of the tumor suppressor p16ink4a, are hallmarks of pancreatic cancer. Here, we describe genes that are commonly affected by epigenetic dysregulation in pancreatic cancer via DNA methylation, histone acetylation or mirNA (microrNA) expression, and review the implications on pancreatic cancer biology such as epithelial-mesenchymal transition, morphological pattern formation, or cancer stem cell regulation during carcinogenesis from PaniN (pancreatic intraepithelial lesions) to invasive cancer and resistance development. Epigenetic drugs, such as DNA methyltransferases or histone deactylase inhibitors, have shown promising preclinical results in pancreatic cancer and are currently in early phases of clinical development. Combinations of epigenetic drugs with established cytotoxic drugs or targeted therapies are promising approaches to improve the poor response and survival rate of pancreatic cancer patients.

Flexural Performance of i-Joist Fabricated with Glue-Laminated Bamboo and <i>Gmelina arborea</i>Plywood

The search for efficient and versatile structural elements, leads to the fabrication of i-joists (6.5 cm × 18.5 cm × 600 cm (width × depth × length) with glue-laminated bamboo (Guada angustifolia) in the flanges and Gmelina arborea 12-mm structural plywood in the web. The results showed a modulus of rupture (Mor) of 39.45 MPa and an effective modulus of elasticity (MoE) of 17.05 GPa. Shearing in the glue line was 5.95 MPa and the lamination strength was 6.45 MPa. Structural design values averaged 9.43 MPa for bending and 4.72 MPa in shear according to Costa rican structural standards. Both resistance value (flexure and shear) were considered satisfactory for structural proposes and i-joists fabricated with bamboo and G. arborea plywood are comparable with the Andean classification group “C” structural grade. The use of this i-joist was also shown in roofing and flooring systems. This beam can be used in allowable spans from 2 to 4 m in span for flooring systems and from 5 to 7 m for roofing applications.

Acute Urinary retention among Adult Men at Bobo-Dioulasso University Teaching Hospital: Epidemiology, Aetiologies and initial Management

We conducted a cross-sectional study between February 1st, 2012 and September 30, 2012 at Bobo-Dioulasso University Teaching hospital. The target population was all patients seen at the emergency services for acute urinary retention. Among the 155 patients admitted for urological emergencies, 104 (67.1%) had acute urinary retention. The average age of patients was 65 years, ranging from 23 to 89 years and the majority was more than 60 years old (77.8%) and lived in rural areas (64.4%). Prostate tumor pathology and urethral stricture were the most frequent diagnosis, and the renal function was impaired in 33.7% of cases. Urethrovesical drainage, cystocatheterism, and suprapubic cystostomy were the treatment approach in 56.0%, 28.0% and 15.2% of the cases. Acute urinary retention is the most common urological emergency and many complications are associated with urethrovesical sounding. These complications should therefore be prevented by improving acute urinary care.

The identification of jets originating from quarks and gluons,often referred to as quark/gluon tagging,plays an important role in various analyses performed at the Large Hadron Collider,as Standard Model measurements and searches for new particles decaying to quarks often rely on suppressing a large gluon-induced *** paper describes the measurement of the efficiencies of quark/gluon taggers developed within the ATLAS Collaboration,using √s=13 TeV proton–proton collision data with an integrated luminosity of 140 fb^(-1) collected by the ATLAS *** taggers with high performances in rejecting jets from gluon over jets from quarks are studied:one tagger is based on requirements on the number of inner-detector tracks associated with the jet,and the other combines several jet substructure observables using a boosted decision tree.A method is established to determine the quark/gluon fraction in data,by using quark/gluon-enriched subsamples defined by the jet *** in tagging efficiency between data and simulation are provided for jets with transverse momentum between 500 GeV and 2 TeV and for multiple tagger working points.
