Capecitabine for locally advanced and metastatic colorectal cancer:A review

Capecitabine (Xeloda) is an oral fluoropyrimidine which is produced as a pro-drug of fluorouracil, and shows improved tolerability and intratumor drug concentrations following its tumor-specif ic conversion to the active drug. We have searched the Pubmed and Cochrane databases from 1980 to 2009 with the purpose of reviewing all available information on Capecitabine, focusing on its clinical effectiveness against colorectal cancer. Special attention has been paid to trials that compared Capecitabine with standard folinic acid (leucovorin, LV)-modulated intravenous 5-fluorouracil (5-FU) bolus regimens in patients with metastatic colorectal cancer. Moreover the efficacy of Capecitabine on metastatic colorectal cancer, either alone or in various combinations with other active drugs such as Irinotecan and Oxaliplatin was also assessed. Finally, neoadjuvant therapy con- sisting of Capecitabine plus radiation therapy, for locally advanced rectal cancer was analysed. This combination of chemotherapy and radiotherapy has a special role in tumor down staging and in sphincter preservation for lower rectal tumors. Comparative trials have shown that Capecitabine is at least equivalent to the standard LV-5-FU combination in relation to progression-free and overall survival whilst showing a better tolerability prof ile with a much lower incidence of stomatitis. It is now known that Capecitabine can be combined with other active drugs such as Irinotecan and Oxaliplatin. The combination of Oxaliplatin with Capecitabine represents a new standard of care for metastatic colorectal cancer. Combinating the Capecitabine-Oxaliplatin regimen with promising new biological drugs such as Bevacizumab seems to give a realistic prospect of further improvement in time to progression of metastatic disease. Energy storage and harvesting in BaTiO_(3)/epoxy nanodielectrics

Metastatic disease from cutaneous melanoma can af-fect all organs of the body, and varies in its biological behavior and clinical presentation. We present the case of a 58-year-old man who arrived at our clinic with acute abdominal pain, which, after investigation, was diagnosed as acute cholecystitis. The patient under-went laparotomy and cholecystectomy. Two years ago, he underwent surgical removal of a primary cutaneous melanoma on his right upper back. Pathological exami-nation revealed the presence of malignant melanoma with a metastatic lesion of the gallbladder.

Frailty and Liver resection: where do we stand?

As the world population is continuously aging, the number of older patients requiring liver surgery is also on the rise. Data have shown that age should not be a limiting factor for liver resection, as it cannot accurately predict postoperative outcomes. Instead, frailty can serve as a more reliable measure of the patient's overall health and functional reserves. Several frailty assessment tools have been implemented for preoperative risk stratification before liver surgery, and higher scores have commonly been associated with postoperative morbidity, mortality, and length of hospital stay. However, no consensus has been reached on the most useful screening tool. Future studies should focus on comparing the currently available assessment tools, constructing a liver resection-specific tool, and assessing the role of frailty assessment tools in preoperative patient optimization.

NORMA:The Network Makeup Artist—A Web Tool for Network Annotation Visualization

The Network Makeup Artist(NORMA) is a web tool for interactive network annotation visualization and topological analysis, able to handle multiple networks and annotations simultaneously. Precalculated annotations(e.g., Gene Ontology, Pathway enrichment, community detection,or clustering results) can be uploaded and visualized in a network, either as colored pie-chart nodes or as color-filled areas in a 2D/3D Venn-diagram-like style. In the case where no annotation exists,algorithms for automated community detection are offered. Users can adjust the network views using standard layout algorithms or allow NORMA to slightly modify them for visually better group separation. Once a network view is set, users can interactively select and highlight any group of interest in order to generate publication-ready figures. Briefy, with NORMA, users can encode three types of information simultaneously. These are 1) the network, 2) the communities or annotations of interest, and 3) node categories or expression values. Finally, NORMA offers basic topological analysis and direct topological comparison across any of the selected networks. De novo lipogenesis(DNL)plays an important role in the pathogenesis of hepatic steatosis and also appears to be implicated in hepatic inflammation and ***,the inhibition of acetyl-CoA carboxylase,which catalyzes the ratelimiting step of DNL,might represent a useful approach in the management of patients with nonalcoholic fatty liver disease(NAFLD).Animal studies and preliminary data in patients with NAFLD consistently showed an improvement in steatosis with the use of these ***,effects on fibrosis were variable and an increase in plasma triglyceride levels was ***,more longterm studies are needed to clarify the role of these agents in NAFLD and to determine their risk/benefit profile.

Background Cerebral cavernous malformations(CCMs)frequently manifest with *** radiosurgery(SRS)has been employed for CCM not suitable for *** effect on reducing haemorrhage risk is still *** aim of this study was to expand on the safety and efficacy of SRS for haemorrhagic *** This retrospective multicentric study included CCM with at least one haemorrhage treated with single-session *** annual haemorrhagic rate(AHR)was calculated before and after *** event analysis and Cox regression were used to evaluate factors associated with *** radiation effects(AREs)and occurrence of new neurological deficits were *** The study included 381 patients (median age:37.5 years(Q1–Q3:25.8–51.9))with 414 *** AHR from diagnosis to SRS excluding the first haemorrhage was 11.08 per 100 CCM-years and was reduced to 2.7 per 100 CCM-years after *** recurrent event analysis,SRS,HR 0.27(95%CI 0.17 to 0.44),p13 Gy,HR 2.27(95%CI 1.20 to 4.32),p=0.012 and the presence of DVA,HR 2.08(95%CI 1.00 to 4.31),p=0.049 were factors associated with higher probability of post-SRS ***-SRS haemorrhage was symptomatic in 22 out of 381(5.8%)patients,presenting with transient(15/381)or permanent(7/381)neurological *** occurred in 11.1%(46/414)CCM and was responsible for transient neurological deficit in 3.9%(15/381)of the patients and permanent deficit in 1.1%(4/381)of the *** doses>13 Gy and CCM volume>0.7 cc were associated with increased risk of ARE.

Neuroendocrine tumors of the gastrointestinal tract are rare neoplasms. Rectal neuroendocrine tumors consist approximately the 5%-14% of all neuroendocrine neoplasms in Europe. These tumors are diagnosed in relatively young patients,with a mean age at diagnosis of 56 years. Distant metastases from rectal neuroendocrine tumors are not very common. Herein we describe a case of a rectal neuroendocrine tumor which metastasized to the lung,mediastinum and orbit. This case underscores the importance of early identification and optimal management to improve patient's prognosis. Therefore,the clinical significance of this case is the necessity of physicians' awareness and education regarding neuroendocrine tumors' diagnosis and management.

Primary and secondary organic aerosol in an urban/industrial site: Sources, health implications and the role of plastic enriched waste burning

PM_(1)0 samples were collected from an urban/industrial site nearby Athens,where uncontrolled burning activities ***,monocarboxylic,dicarboxylic,hydroxycarboxylic and aromatic acids,tracers from BVOC oxidation,biomass burning tracers and bisphenol A were ***,monocarboxylic acids,biomass burning tracers and bisphenol A were increased during autumn/winter,while BSOA tracers,dicarboxylic-and hydroxycarboxylic acids during *** aromatic acids,different sources and formation mechanisms were indicated as benzoic,phthalic and trimellitic acids were peaked during summer whereas p-toluic,isophthalic and terephthalic were more abundant during autumn/*** Benzo[a]pyrene-equivalent carcinogenic power,carcinogenic and mutagenic activities were calculated showing significant(p<0.05)increases during the colder ***,succinic and malic acids were the most abundant monocarboxylic,dicarboxylic and hydrocarboxylic acids during the entire sampling *** oxidation was the most significant contributor to BSOA as the isoprene-SOA compounds were two times more abundant than the pinene-SOA(13.4±12.3 and 6.1±2.9 ng/m^3,respectively).Ozone has significant impact on the formation of many studied compounds showing significant correlations with:isoprene-SOA(r=0.77),hydrocarboxylic acids(r=0.69),pinene-SOA(r=0.63),dicarboxylic acids(r=0.58),and the sum of phthalic,benzoic and trimellitic acids(r=0.44).PCA demonstrated five factors that could explain sources including plastic enriched waste burning(30.8%),oxidation of unsaturated fatty acids(23.0%),vehicle missions and cooking(9.2%),biomass burning(7.7%)and oxidation of VOCs(5.8%).The results highlight the significant contribution of plastic waste uncontrolled burning to the overall air quality degradation.
