Short benign ileocolonic anastomotic strictures-management with bi-flanged metal stents:Six case reports and review of literature

BACKGROUND The endoscopic management of benign short post-anastomotic ileocolonic stricture(pAICS) that is refractory to primary and secondary treatment modalities remains *** lumen-apposing metal stent(LAMS) is a novel device recently developed for therapeutic gastrointestinal *** have demonstrated significantly better results with regard to stent migration than fully covered self-expandable metal stents(FCSEMSs).CASE SUMMARY This article presents six cases of symptomatic pAICS successfully treated with a LAMS and a review of the relevant *** report a life-saving technique not previously documented and the use of technology to improve patient *** six patients(median age,75 years) suffered from vomiting,constipation and recurrent abdominal pain,with symptoms starting 23-25 wk *** median stricture length was 1.83 *** six patients underwent successful and uneventful bi-flanged metal stent(BFMS)-LAMS placement for benign *** patients remained asymptomatic during the three months of stent indwelling and up to a median of 7 mo after stent *** to the literature,the application of LAMS for pAICS is associated with a py and the availability of LAMSs provide patients with minimally invasive treatment options,allowing them to avoid more invasive surgical *** BFMS(NAGI stent) is longer and larger than the prototype AXIOS-LAMS,which should be considered in the management of short ileocolonic post-anastomotic strictures longer than 10 mm and shorter than 30 mm.

Contribution of ghrelin to functional gastrointestinal disorders' pathogenesis

Functional gastrointestinal disorders(FGID) are heterogeneous disorders with a variety of clinical manifestations, primarily defined by signs and symptoms rather than a definite underlying cause. Their pathophysiology remains obscure and, although it is expected to differ according to the specific FGID, disruptions in the brain-gut axis are now thought to be a common denominator in their pathogenesis. The hormone ghrelin is an important component of this axis,exerting a wide repertoire of physiological actions, including regulation of gastrointestinal motility and protection of mucosal tissue. Ghrelin's gene shows genetic polymorphism, while its protein product undergoes complex regulation and metabolism in the human body. Numerous studies have studied ghrelin's relation to the emergence of FGIDs, its potential value as an index of disease severity and as a predictive marker for symptom relief during attempted treatment. Despite the mixed results currently available in scientific literature, the plethora of statistically significant findings shows that disruptions in ghrelin genetics and expression are plausibly related to FGID pathogenesis. The aim of this paper is to review current literature studying these associations, in an effort to uncover certain patterns of alterations in both genetics and expression, which could delineate its true contribution to FGID emergence, either as a causative agent or as a pathogenetic intermediate.

On the connection between the solutions to the Dirac and Weyl equations and the corresponding electromagnetic four-potentials

In this work we study in detail the connection between the solutions to the Dirac and Weyl equations and the associated electromagnetic ***,it is proven that all solutions to the Weyl equation are degenerate,in the sense that they correspond to an infinite number of electromagnetic *** far as the solutions to the Dirac equation are concerned,it is shown that they can be classified into two *** elements of the first class correspond to one and only one four-potential,and are called non-degenerate Dirac *** the other hand,the elements of the second class correspond to an infinite number of four-potentials,and are called degenerate Dirac ***,it is proven that at least two of these fourpotentials are gauge-inequivalent,corresponding to different electromagnetic *** order to illustrate this particularly important result we have studied the degenerate solutions to the forcefree Dirac equation and shown that they correspond to massless *** have also provided explicit examples regarding solutions to the force-free Weyl equation and the Weyl equation for a constant magnetic *** all cases we have calculated the infinite number of different electromagnetic fields corresponding to these ***,we have discussed potential applications of our results in cosmology,materials science and nanoelectronics.

Acute coronary syndrome after infliximab therapy in a patient with Crohn’s disease

Infliximab is a potent anti-TNF antibody, which is used with great success in Crohn’s disease patients. Since its release in clinical practice, several adverse reactions have been observed. The interest in possible consequences of its administration is still high because of the recent introduction of the drug for the long-term maintenance therapy of refractory luminal and fistulizing Crohn’s disease. We present a case of acute coronary syndrome (non-STEMI) in a patient with corticoid resistant Crohn’s disease after his first dose of infliximab. By reviewing the scant articles that exist in the literature on this topic we made an effort to delineate the possible mechanisms of this phenomenon.

The use of an artificial neural network in the evaluation of the extracorporeal shockwave lithotripsy as a treatment of choice for urinary lithiasis

Objective:Artificial neural networks(ANNs)are widely applied in medicine,since they substantially increase the sensitivity and specificity of the diagnosis,classification,and the prognosis of a medical *** this study,we constructed an ANN to evaluate several parameters of extracorporeal shockwave lithotripsy(ESWL),such as the outcome and safety of the ***:patients with urinary lithiasis suitable for ESWL treatment were *** ANN was designed using *** data were collected from all patients and 12 nodes were used as *** statistical analysis was also ***:Finally,716 patients were included in our *** analysis revealed that diabetes and hydronephrosis were positively correlated with ESWL *** efficacy,univariate analysis revealed that stone location,stone size,the number and density of shockwaves delivered,and the presence of a stent in the ureter were independent factors of the ESWL *** was further confirmed when adjusted for sex and age in a multivariate *** performance of the ANN at the end of the training state reached 98.72%.The four basic ratios(sensitivity,specificity,positive predictive value,and negative predictive value)were calculated for both training and evaluation data *** performance of the ANN at the end of the evaluation state was 81.43%.Conclusion:Our ANN achieved high score in predicting the outcome and the side effects of the ESWL treatment for urinary stones.

Lingual Nerve Entrapment in Muscular and Osseous Structures

Running through the infratemporal fossa is the lingual nerve （i.e. the third branch of the posterior trunk of the mandibular nerve）. Due to its location, there are various anatomic structures that might entrap and potentially compress the lingual nerve. These anatomical sites of entrapment are：（a） the partially or completely ossified pterygospinous or pterygoalar ligaments; （b） the large lamina of the lateral plate of the pterygoid process; and （c） the medial fibers of the anterior region of the lateral pterygoid muscle. Due to the connection between these nerve and anatomic structures, a contraction of the lateral pterygoid muscle, for example, might cause a compression of the lingual nerve. Any variations in the course of the lingual nerve can be of clinical significance to surgeons and neurologists because of the significant complications that might occur. To name a few of such complications, lingual nerve entrapment can lead to：（a） numbness, hypoesthesia or even anesthesia of the tongue’s mucous glands; （b） anesthesia and loss of taste in the anterior two-thirds of the tongue; （c） anesthesia of the lingual gums; and （d） pain related to speech articulation disorder. Dentists should, therefore, be alert to possible signs of neurovascular compression in regions where the lingual nerve is distributed.

Management of hepatitis B virus infection in patients with inflammatory bowel disease under immunosuppressive treatment

Thermal Characterization of Lauric Acid and Stearic Acid Binary Eutectic Mixture in Latent Heat Thermal Storage Systems with Tube and Fins

In order to obtain the suitable phase change material（pCM） with low phase change temperature and improve its heat transfer rate, experimental investigation was conducted. Firstly, different mass ratios of lauric acid（LA） and stearic acid（SA） eutectic mixtures were prepared and characterized by differential scanning calorimetry（DSC）. Then, the performance of eutectic mixture during charging process under different fin widths in vertical condition, and performance during charging and discharging processes under different inlet temperature heat transfer fluid（HTF） in horizontal condition were investigated, respectively. The results revealed that the LA-SA eutectic mixture had the suitable phase change temperature and desired latent heat for low-temperature water floor heating system. Wide fins and high inlet temperature HTF significantly enhanced the transfer rate and decreased the melting time.
