Modified deep sclerectomy combined with Ex-PRESS filtration device versus trabeculectomy for primary open angle glaucoma

AIM：To compare the efficacy of modified deep sclerectomy combined with Ex-PRESS shunt versus trabeculectomy in primary open angle glaucoma. METHOdS：This is a prospective cohort comparative single-center study. Forty-nine eyes of 49 patients were enrolled in the study. Patients were randomly divided into two groups. Group A（22 patients） underwent classic trabeculectomy and group B（27 patients） underwent modified deep sclerectomy combined with insertion of Ex-PRESS model P50 drainage device. RESULTS：Mean age was 69±7y in group A and 64±8y in group B（P=0.03）. The mean reduction was 11.1±5.7 mm Hg in group A compared to 15.8±5.7 mm Hg in group B at 6mo（P=0.006）, and 9.8±4.9 mm Hg and 15.4±4.7 mm Hg respectively at 1y（P=0.0001）. Regarding the postoperative glaucoma medication, significant difference was observed between the two groups（in favour of group B） only at 6mo（P=0.017）. At the end of the follow-up period complete success rate in group A was 68.2% compared to 92.6% in group B（χ^2 test, P=0.07） and qualified success rate was 100% in both ***：Modified deep sclerectomy combined with Ex-PRESS shunt may provide comparable IOP reduction with fewer complications in management of primary open angle glaucoma.

Keratoconus is a progressive, non-inflammatory disease of the cornea, which is characterized by marked cornealsteepening and thinningrll. It induces myopia and irregular astigmatism leading frequently to severe visual impairmentu. Although several aetiological factors have been implicated in its pathophysiology, the exact mechanisms underlying keratoconus are not fully elucidated yet. Corneal crosslinking is the treatment of choice in order to inhibit the progression of keratoconus, whereas advanced cases require penetrating or lamellar keratolalasty for visual restoration.

Imaging of filtering blebs after implantation of the Ex-PRESS shunt with the use of the Visante optical coherence tomography

AIM: To analyze the features of the filtering blebs following implantation of the Ex-PRESS glaucoma device(model P-50) with the aid of the Visante anterior segment optical coherence tomography(AS-OCT)METHOdS: Five patients with open angle glaucomas were included in the study. They all underwent implantation of the Ex-PRESS device under a scleral flap. The surgical procedure was augmented with the use of mitomycin C subconjunctivally. The filtering blebs were analyzed with the Visante AS-OCT with the scans taken along the axis of the implantation of the glaucoma ***: All filtering blebs were graded as diffuse functioning. The morphological characteristics of the blebs were similar to those of the *** the use of the Ex-PRESS implant tend to form a characteristic episcleral lake at the site of the plate of the implant. CONCLUSION: The use of the Ex-PRESS implant produces filtering blebs similar to those of the trabeculectomy with the formation of a characteristic episcleral lake at the site of the plate of the implant.

BACKGROUNd Transplant recipients commonly harbor multidrug-resistant organisms(MdROs),as a result of frequent hospital admissions and increased exposure to antimi-crobials and invasive *** To investigate the impact of patient demographic and clinical characteristics on MdRO acquisition,as well as the impact of MdRO acquisition on intensive care unit(ICU)and hospital length of stay,and on ICU mortality and 1-year mortality post heart *** This retrospective cohort study analyzed 98 consecutive heart transplant patients over a ten-year period(2013-2022)in a single transplantation *** was collected regarding MdROs commonly encountered in critical *** Among the 98 transplanted patients(70%male),about a third(32%)acquired or already harbored MdROs upon transplantation(MdRO group),while two thirds did not(MdRO-free group).The prevalent MdROs were Acinetobacter baumannii(14%),Pseudomonas aeruginosa(12%)and Klebsiella pneumoniae(11%).Compared to MdRO-free patients,the MdRO group was characterized by higher body mass index(P=0.002),higher rates of renal failure(P=0.017),primary graft dysfunction(10%vs 4.5%,P=0.001),surgical re-exploration(34%vs 14%,P=0.017),mechanical circulatory support(47%vs 26%P=0.037)and renal replacement therapy(28%vs 9%,P=0.014),as well as longer extracorporeal circulation time(median 210 vs 161 min,P=0.003).The median length of stay was longer in the MdRO group,namely ICU stay was 16 vs 9 d in the MdRO-free group(P=0.001),and hospital stay was 38 vs 28 d(P=0.006),while 1-year mortality was higher(28%vs 7.6%,log-rank-χ2:7.34).CONCLUSION Following heart transplantation,a predominance of Gram-negative MdROs was *** acquisition was associated with higher complication rates,prolonged ICU and total hospital stay,and higher post-transplantation mortality.

Statistical analysis on the influence of mechanical parameters in the vibration of pylons

We present a statistical investigation of the degree of influence that assumptions made in relation to the mechanical parameters of a pylon have on its ground-induced *** study is set up by using as a key kinematic variable the displacement at the top of a reference,a stand-alone pylon with a uniform cross-section and fixity at its ***,statistics are produced using a dimensionless displacement ratio defined between the‘parental’and the‘subsidiary’cases,the latter defined for the pylon(a)resting on compliant soil,(b)having an attached top mass,and(c)being non-uniform with ***,two materials are examined,namely,steel and reinforced concrete(R/C).More specifically,this displacement ratio is independent of the excitation and plays the role of a transfer function between the base and the top of the *** horizontal and vertical motions are considered,and the equations of motion are solved in the frequency *** ensuing statistical analysis is conducted for the following parameter combinations:(a)pylon founded on soft,intermediate,and stiff soil;(b)low,intermediate,and high-mass ratios of the attached mass to the pylon′s mass;(c)a constant and quadratic degree of pylon tapering with *** correlation coefficients are calculated for all the above combinations to arrive at statistical results that establish validity bounds and quantify the degree of influence of each assumption on the pylon′s response.

Risk of colorectal neoplasm in patients with acromegaly: A meta-analysis

AIM: To examine the risk of colorectal neoplasm in acromegalic patients by meta-analyzing all relevant controlled studies. METHOdS: Extensive English language medical literature searches for human studies, up to december 2007, were performed using suitable keywords. Pooled estimates [odds ratio (OR) with 95% confidence intervals (CI)] were obtained using either the fixed or random-effects model as appropriate. Heterogeneity between studies was evaluated with the Cochran Q test whereas the likelihood of publication bias was assessed by constructing funnel plots. Their symmetry was estimated by the adjusted rank correlation test. RESULTS: For hyperplastic polyps the pooled ORs with 95% CI were 3.557 (2.587-4.891) by fixed effects model and 3.703 (2.565-5.347) by random effects model. The Z test values for overall effect were 7.81 and 6.984, respectively (P denomas the pooled ORs with 95% CI were 2.486 (1.908-3.238) (fixed effects model) and 2.537 (1.914-3.364) (random effects model). The Z test values were 6.747 and 6.472, respectively (P d OR with 95% CI was identical for both fixed and random effects model (OR, 4.351; 95% CI, 1.533-12.354; Z = 2.762, P = 0.006]. There was no significant heterogeneity and no publication bias in all the above meta-analyses. CONCLUSION: Acromegaly is associated with an increased risk of colorectal neoplasm.

Hepatocellular carcinoma(HCC)constitutes the fifth most frequent malignancy worldwide and the third most frequent cause of cancer-related ***,treatment selection is based on the stage of the *** fields such as three-dimensional(3d)printing,3d bioprinting,artificial intelligence(AI),and machine learning(ML)could lead to evidence-based,individualized management of *** this review,we comprehensively report the current applications of 3d printing,3d bioprinting,and AI/ML-based models in HCC management;we outline the significant challenges to the broad use of these novel technologies in the clinical setting with the goal of identifying means to overcome them,and finally,we discuss the opportunities that arise from these ***,regarding 3d printing and bioprinting-related challenges,we elaborate on cost and cost-effectiveness,cell sourcing,cell viability,safety,accessibility,regulation,and legal and ethical ***,regarding AI/ML-related challenges,we elaborate on intellectual property,liability,intrinsic biases,data protection,cybersecurity,ethical challenges,and *** findings show that AI and 3d printing applications in HCC management and healthcare,in general,are steadily expanding;thus,these technologies will be integrated into the clinical setting sooner or ***,we believe that physicians need to become familiar with these technologies and prepare to engage with them constructively.
