Hepatocellular carcinoma(HCC)is the most common primary liver malignancy and one of the most common causes of cancer-related death with the projection to increase during the next few decades(1).During the last 3 decades,several staging systems have been introduced for the stratification of the prognosis and management of HCC of all *** is no consensus regarding the universal implementation of one staging system since all the proposed classifications have limitations(2).The Barcelona Clinic Liver Cancer(BCLC)Staging System remains the most widely classification system used for HCC management guidelines that incorporates tumor size,presence of metastatic disease,portal hypertension,Child-Turcotte-Pugh score,total bilirubin and performance ***,it does not account for tumoral behavior,and it is very“conservative”(restrictive)regarding resectability,especially for BCLC-B patients who might benefit from surgical management(3,4).Recent advances in systemic and locoregional therapies have led to changes in many guidelines regarding systemic therapy,as well as the possibility for downstaging patients to undergo surgery or transplant with curative intent.

Esophageal cancer (EC) presents a high mortality rate, mainly due to its aggressive nature. Squamous cell carcinoma is the most common histological type world-wide, though, a continuous increase in esophageal adenocarcinomas has been noted in the past decades. Common risk factors associated with EC include smoking, alcohol consumption, gastroesophageal reflux disease, Barrett's esophagus and obesity. In an effort to overcome chemotherapy resistance in oncology, it was discovered that histone acetylation/deacetylation equilibrium is altered in carcinogenesis, leading to changes in chromatin structure and altering expression of genes important in the cell cycle, differentiation and apoptosis. Based on this knowledge, histone acetylation was addressed as a potential novel chemotherapy drug target to repress cancer cell proliferation. There are four classes of histone deacetylases (HDACs) inhibitors with a variety of different mechanisms of actions that render them possible anticancer drugs. They arrest the cell cycle, inhibit differentiation and angiogenesis and induce apoptosis. They do not necessarily act on histone proteins, since they can also exert indirect anti-cancer effects, by modifying various cellular *** addition,HDACs have also been associated with increased chemotherapy *** on the literature,HDACs have been associated with EC,with surveys revealing that increased expression of certain HDACs correlates with advanced TNM stages,tumor grade,metastatic potential and decreased 5-year overall and disease-free *** aim of this survey is to elucidate the molecular identity and mechanism of action of HDAC inhibitors as well as verify their potential utility as anti-cancer agents in esophageal cancer.

Post-partum reactivation of chronic hepatitis B virus infection among hepatitis B e-antigen-negative women

AIM: To investigate the frequency and timing of post-partum chronic hepatitis B virus(HBV) reactivation and identify its pre-partum predictors. METHODS: Forty-one hepatitis B e antigen(HBe Ag)-negative chronic HBV infected pregnant women were prospectively evaluated between the 28 th and the 32 nd week of gestation. Subjects were re-evaluated at 3-mo intervals during the first post-partum year and every 6 mo during the following years. HBV DNA was determined using real-time reverse transcription polymerase chain reaction(Cobas Taq Man HBV Test) with a lower detection limit of 8 IU/m L. Post-partum reactivation(PPR) was defined as abnormal alanine aminotransaminase(ALT) levels and HBV DNA above 2000 IU/m L. RESULTS: Fourteen out of 41 women(34.1%) had prepartum HBV DNA levels > 2000 IU/m L, 18(43.9%) had levels 10000 IU/m L exhibited PPR compared with 3 of the 21(14.3%) women with HBV DNA 2000 and the third with HBV DNA of 1850IU/m L), P = 0.004. An HBV DNA level ≥ 10000 IU/m L independently predicted post-partum HBV infection reactivation(OR = 57.02, P = 0.033). Mean pre-partum ALT levels presented a non-significant increase in PPR cases(47.3 IU/L vs 22.2 IU/L, respectively, P = 0.094).CONCLUSION: In the present study, PPR occurred in approximately 30% of HBe Ag-negative pregnant women; all events were observed during the first semester after delivery. Pre-partum HBV DNA level > 10000 IU/m L predicted PPR.

Objective:To determine the levels of burnout among Greek nurses during the first wave of Coronavirus disease 2019(COVID-19)pandemic and to investigate the relationship among levels of burnout,satisfaction and concern about information,personal protection equipment(PPE),and safety for ***:A cross-sectional study was *** were collected by using the Burnout subscale of the Greek version of the Professional Quality of Life *** concerning satisfaction and concern about information,PPE,and safety for COVID-19 were *** sampling method was *** statistics and multiple linear regression analysis were applied,and distributions were tested for their symmetry using Bloom’s ***:The sample comprised of 301 nurses working in public and private hospitals in Greece.A moderate level of burnout was found among the Greek nurses(mean=29.95,±4.75).Regarding satisfaction and concern about information,PPE,and safety for COVID-19,nurses expressed overall low satisfaction with a mean score of 1.96(±0.77),and high levels of concern with a mean score of 3.19(±0.85).Increased satisfaction regarding the information provided was significantly associated with decreased levels of burnout(β=-1.32,P<0.001),and increased concern for COVID-19 was significantly associated with increased levels of burnout(β=0.83,P=0.010).Conclusions:Greek nurses expressed low satisfaction concerning information,PPE,and safety for COVID-19 and high levels of concern of possible COVID-19 ***,a moderate level of burnout was found.

The Benefits of Patients’ Information regarding Dietary Habits before Percutaneous Coronary Intervention: A 3- and 6-Month Follow-Up

Aim: To evaluate the benefits of counseling intervention related to dietary habits changes on patients following percutaneous coronary intervention (PCI). Materials and Methods: A randomized counseling intervention study, with a 3- and 6-month follow-up was performed on 230 patients who underwent PCI. They were randomly allocated to the intervention (n = 93) or the control group (n = 137). A 3- and 6-month telephone follow-up was performed for dietary habits evaluation. Data analysis was performed by using the statistical package SPSS, ver. 20.?Results: Compared with control group, intervention group had higher prevalence of hypertension, history of diabetes and dyslipidemias and history of acute myocardial infraction, with no statistical difference. Moreover, patients in control group were more likely to be ex-smokers (p = 0.01). Post hoc tests using the Bonferroni correction revealed that mean TCHOL concentration while patients admitted to hospital differed statistically significantly between the time points of 3 and 6 months (209 ± 67 mg/dl vs 174 ± 34 mg/dl vs 176 ± 36 mg/dl), p= 0.005 and p = 0.042 respectively. However, there was no statistical significant difference between 3 months and 6 months measurements. Mean glucose concentration while patients admitted to hospital differed statistically significantly between the time points of 3 and 6 months (108 ± 40 mg/dl vs 95 ± 21 mg/dl vs 95 ± 23 mg/dl), p =0.009 and p = 0.012 respectively. However, there was no significant statistical difference between 3 months and 6 months measurements, (p = 1.000). Conclusion: A nurse-led program regarding dietary habits modifications on patients undergoing PCI should be performed along with a long-term follow up after hospital discharge.

Effortful control (EC) is a temperamental self-regulatory capacity, defined as the efficiency of executive attention [1], which is related to individual differences in self-regulation. Although effortful control covers some dispositional self-regulatory abilities important to cope with social demands of successful adaptation to school, such as attention regulation, individual differences in EC have recently been associated with school functioning through academic achievement including the efficient use of learning-related behaviors, which have been found to be a necessary precursor of learning and they refer to a set of children’s behaviors that involve organizational skills and appropriate habits of study. Therefore, the aim of this study is to review the literature on EC’s relationship to academic achievement via learning-related behaviors, which reflect the use of metacognitive control processes in kindergarten and elementary school students. The findings indicate that EC affects academic achievement through the facilitation of the efficient use of metacognitive control processes.

Aim: The authors conducted a case-control study to estimate predictive factors for timely identification of patients at higher risk for developing drug resistant epilepsy. Methods:The retrospective case-control study was conducted among people diagnosed as having drug resistant epilepsy (cases) and their controls, identified as having drug-responsive seizures. All participants were admitted to the tertiary Epilepsy Center at the Institute of Neurology and Neuropsychology (Tbilisi, georgia) during 2011. The data on demographic features and disease characteristics were analyzed. Multiple logistic regression analysis was used to identify independent risk factors for development of intractable epilepsy. Results:A total 334 patients were identified;84 (34%) met the criteria for drug resistant epilepsy. One hundred and sixty-four age- and gender-matched controls with drug-responsive epilepsy were identified. Relative to the control group, the drug resistant seizure group had increased frequency of perinatal pathology (24% vs. 12%), febrile seizures (22% vs. 12%), seizure frequency at disease manifestation (62% vs. 19%), occurrence of convulsive seizures (84%vs. 70%), electroencephalo-graph (EEG) epileptiform discharges (94% vs. 77%), polytherapy (90% vs. 12%), multilobar lesions (30% vs. 16%), hippocampal sclerosis (18% vs. 5%), and malformations of cortical development (8% vs. 2%). Multivariate analysis indicated four factors with independent predictive value for development of intractable epilepsy: frequency of seizure, polymorphism of seizure, polytherapy, and epileptiform EEG *** presence of all four factors in combination resulted in a 98% of probability of developing drug resistant epilepsy. Conclusion:Several factors appear to have prognostic value in identifying the risk for drug resistant epilepsy.
