Autoantibodies related to ataxia and other central nervous system manifestations of gluten enteropathy

Gluten ataxia and other central nervous system disorders could be linked to gluten enteropathy and related autoantibodies.In this narrative review,we focus on the various neuro-logical manifestations in patients with gluten sensitivity/celiac disease,immunological and autoimmune mechanisms of ataxia in connection to gluten sensitivity and the autoantibodies that could be used as a biomarker for diagnosing and following.We focused on the anti-gliadin antibodies,antibodies to different isoforms of tissue transglutaminase(TG)(anti-TG2,3,and 6 antibodies),anti-glycine receptor antibodies,anti-glutamine acid decarboxylase antibodies,anti-deamidated gliadin peptides antibodies,etc.Most studies found a higher prevalence of these antibodies in patients with gluten sensitivity and neurological dysfunction,presented as different neurological disorders.We also discuss the role of a gluten-free diet on the clinical improvement of patients and also on imaging of these disorders.

Effectiveness and safety of COVID-19 vaccines in patients with diabetes as a factor for vaccine hesitancy

Diabetes mellitus is one of the most common comorbid conditions encountered in patients with severe acute respiratory syndrome coronavirus 2 infection accompanied by significantly increased mortality,prolonged hospital stay,and requirement of invasive mechanical ventilation.This review aims to present the effectiveness and safety profile of available coronavirus disease 2019(COVID-19)vaccines in people with diabetes as a potential cause of hesitancy for vaccination.Data from published research proves a robust immune response following immunization for COVID-19 in diabetic patients with substantial production of virus-neutralizing antibodies;however,the observed immune response was unequivocally weaker than that in individuals without diabetes.This observation was further enhanced by the findings that worse glycemic control was associated with more suppressed antibody production.In contrast,individuals with optimal glycemic control performed similarly to healthy controls.In addition to the need for strict glucose monitoring and adequate diabetes treatment,those findings reinforce the concept of diabetes-induced secondary immune deficiency and necessitate the application of booster doses to diabetic patients with priority.Nevertheless,after vaccination,reported adverse events were not different from those in the general population.No increase in severe adverse events was documented.While single case reports detected transient increases in blood glucose post-vaccination,more extensive trials could not replicate such a relationship.

The interaction of the structure-directing agent with the zeolite framework determines germanium distribution in SCM-15 germanosilicate

We report results from computational modeling of the relative stability of germanosilicate SCM-15 structure due to different distribution of germanium heteroatoms in the double four-member rings(D4Rs)of the framework and the orientation of the structure directing agent(SDA)molecules in the as-synthesized zeolite.The calculated relative energies of the bare zeolite framework suggest that structures with germanium ions clustered in the same D4Rs,e.g.,with large number of Ge–O-Ge contacts,are the most stable.The simulations of various orientations of the SDA in the pores of the germanosilicate zeolite show different stability order-the most stable models are the structures with germanium spread among all D4Rs.Thus,for SCM-15 the stabilization due to the presence of the SDA and their orientation,is thermodynamic factor directing both the formation of specific framework type and Ge distribution in the framework during the synthesis.The relative stability of bare structures with different germanium distribution is of minor importance.This differs from SCM-14 germanosilicate,reported earlier,for which the stability order is preserved in presence of SDA.Thus,even for zeolites with the same chemical composition and SDA,the characteristics of their framework lead to different energetic preference for germanium distribution.

Pancreatic islet transplantation is a minimally invasive procedure aiming to reverse the effects of insulin deficiency in patients with type 1 diabetes(T1D)by transplanting pancreatic beta cells.Overall,pancreatic islet transplantation has improved to a great extent,and cellular replacement will likely become the mainstay treatment.We review pancreatic islet transplantation as a treatment for T1D and the immunological challenges faced.Published data demonstrated that the time for islet cell transfusion varied between 2 and 10 h.Approximately 54%of the patients gained insulin independence at the end of the first year,while only 20%remained insulin-free at the end of the second year.Eventually,most transplanted patients return to using some form of exogenous insulin within a few years after the transplantation,which imposed the need to improve immunological factors before transplantation.We also discuss the immunosuppressive regimens,apoptotic donor lymphocytes,anti-TIM-1 antibodies,mixed chimerism-based tolerance induction,induction of antigen-specific tolerance utilizing ethylene carbodiimide-fixed splenocytes,pretransplant infusions of donor apoptotic cells,B cell depletion,preconditioning of isolated islets,inducing local immunotolerance,cell encapsulation and immunoisolation,using of biomaterials,immunomodulatory cells,etc.

Dietary imbalance and overeating can lead to an increasingly widespread disease-obesity.Aesthetic considerations aside,obesity is defined as an excess of adipose tissue that can lead to serious health problems and can predispose to a number of pathological changes and clinical diseases,including diabetes;hypertension;atherosclerosis;coronary artery disease and stroke;obstructive sleep apnea;depression;weight-related arthropathies and endometrial and breast cancer.A body weight 20%above ideal for age,gender and height is a severe health risk.Bariatric surgery is a set of surgical methods to treat morbid obesity when other treatments such as diet,increased physical activity,behavioral changes and drugs have failed.The two most common procedures currently used are sleeve gastrectomy and gastric bypass.This procedure has gained popularity recently and is generally considered safe and effective.Although current data show that perioperative mortality is low and better control of comorbidities and short-term complications is achieved,more randomized trials are needed to evaluate the long-term outcomes of bariatric procedures.This review aims to synthesize and summarize the growing evidence on the long-term effectiveness,outcomes and complications of bariatric surgery.

Imaging techniques play a crucial role in the modern era of medicine,particularly in gastroenterology.Nowadays,various non-invasive and invasive imaging modalities are being routinely employed to evaluate different gastrointestinal(GI)diseases.However,many instrumental as well as clinical issues are arising in the area of modern GI imaging.This minireview article aims to briefly overview the clinical issues and challenges encountered in imaging GI diseases while highlighting our experience in the field.We also summarize the advances in clinically available diagnostic methods for evaluating different diseases of the GI tract and demonstrate our experience in the area.In conclusion,almost all imaging techniques used in imaging GI diseases can also raise many challenges that necessitate careful consideration and profound expertise in this field.

We studied the effect of titanium dioxide(TiO_(2))nanoparticles(NPs)on dielectric behavior of Naþion-conducting salt-complexed polymer nanocomposite system formed from a binary polymer blend of poly(ethylene oxide)(PEO)and polyvinyl pyrrolidone(PVP),with the addition of both sodium metaperiodate(NaIO_(4))at concentration 10 wt.%and TiO_(2) NPs of size10 nm,at concentrations 1,2,3,4 and 5 wt.%.Free standing nanocomposite PEO/PVP/NaIO_(4)/TiO_(2) films(150m)were characterized at room-temperature by analyzing their complex electrical impedance and dielectric spectra in the range 1 Hz–1 MHz.At the concentration of 3 wt.%of TiO_(2) NPs,both ion conductivity and dielectric permittivity of the PEO/PVP/NaIO_(4)/TiO_(2) ion-conducting dielectrics reach an enhancement by more than one order of magnitude as compared to nanoadditive-free case.

Lateral epicondylitis(LE)is a chronic aseptic inflammatory condition caused by repetitive microtrauma and excessive overload of the extensor carpi radialis brevis muscle.This is the most common cause of musculoskeletal pain syndrome in the elbow,inducing significant pain and limitation of the function of the upper limb.It affects approximately 1-3%of the population and is frequently seen in racquet sports and sports associated with functional overload of the elbow,such as tennis,squash,gymnastics,acrobatics,fitness,and weight lifting.Typewriters,artists,musicians,electricians,mechanics,and other professions requiring frequent repetitive movements in the elbow and wrists are also affected.LE is a leading causation for absence from work and lower sport results in athletes.The treatment includes a variety of conservative measures,but if those fail,surgery is indicated.This review summarizes the knowledge about this disease,focusing on risk factors,expected course,prognosis,and conservative and surgical treatment approaches.

Landscape alteration affects the demography of an endangered avian predator by reducing the habitat quality

Habitat transformation is identified as major threat to biodiversity loss globally,affecting threatened raptors.Changes in land use can alter the abundance and distribution of birds of prey by affecting habitat availability and quality.In this study,we used multivariate analyses to investigate the effect of habitat alteration on the demography of a declining Eastern Imperial Eagle(Aqufla heliaca),making assumptions for future population trajectories.We used the Generalized Linear Mixed Models with Poisson distribution and Log link function,searching a relationship between the demographic parameters and the landscape structure and possible effects of fragmentation.In our study area,habitat change affected dramatically permanent grasslands,shrinking their availability.As we expected,the share of grasslands in eagles’territories significantly affected occupancy rate,but not productivity and breeding frequency.We found that occupancy rate decreased significantly,while productivity and breeding frequency showed no trend.Modeling the effect of habitat alteration on Eastern Imperial Eagle demography,we found out that territory quality was a more powerful factor driving the response of a top predator to the alteration of favorite foraging habitats.Only the habitat quality in source territories had a significant positive effect on eagle productivity.We found that simple rules to predict adverse agricultural impact on territory abandonment or breeding productivity of a top predator were not reliable.Our results could be used for planning conservation of other large territorial raptors,facing the same type of threat.

Molecular Mechanism of Electroacupuncture Regulating Cerebral Arterial Contractile Protein in Rats with Cerebral Infarction Based on MLCK Pathway

Objective To explore the effect of electroacupuncture(EA)intervention on the vasoconstriction of cerebral artery smooth muscle cells after cerebral infarction.Methods Male Wistar rats were randomly divided into 3 groups by a random number table:the model group(n=24),the EA group(n=24),and the normal group(n=6).The model and the EA groups were divided into different time subgroups at 0.5,1,3,and 6 h after middle cerebral artery occlusion(MCAO),with 6 rats in each subgroup.MCAO model was established using intraluminal suture occlusion method.The EA group was given EA treatment at acupoint Shuigou(GV 26)instantly after MCAO for 20 min.The contents of cerebrovascular smooth muscle MLCK,the 3 subunits of myosin light chain phosphatase(MLCP)MYPT1,PP1c-δand M20,as well as myosin-ATPase activity were detected using immunohistochemistry and Western blotting.Results The overall expression level of the MYPT1 and PP1c-δin the model group was significantly higher(P0.05),and the other subgroups were still significantly higher than the normal group(P<0.01).After EA intervention,the expression level of each subgroup was significantly lower than the corresponding model group.There was a significant difference between the 0.5 and 1 h subgroups(P<0.01),while a difference was also observed between the 3 and 6 h subgroups(P<0.05).The dynamic change rule gradually increased with the prolongation of infarction time within 6 h after infarction.Conclusion EA intervention can inhibit contraction of cerebral vascular smooth muscle cells and regulate smooth muscle relaxation by regulating MLCK pathway.
