Clinicopathological features and surgical outcomes of resected functional pancreatic neuroendocrine tumors: a single institution experience

Background:Functional pancreatic neuroendocrine tumors(f-PanNETs)are rare tumors of the pancreas that are associated with excess hormone production.A majority of literature available on these tumors is limited to case reports and small series and discusses a particular subtype.Here,we report the clinicopathological features and outcomes of all f-PanNETs from a high-volume pancreatectomy center.Methods:A prospectively maintained database was used to identify patients who underwent resection for f-PanNETs between January 1995 and December 2015.Patients were stratified by the type of f-PanNET,and their clinicopathological features and outcomes were analyzed and reported.Results:We identified 69 patients with a mean age of 52±16 years,and the majority were female(62%).The most common type of f-PanNET were insulinomas(70%)followed by gastrinomas(15%),and vasoactive intestinal peptidomas(VIPomas)(10%).There was considerable heterogeneity in the presenting symptoms.Patients with insulinomas presented with smaller tumors compared to the other subtypes.Nodal disease was most frequent in patients with VIPomas(71%),and gastrinomas(50%).The median overall survival(OS)was 7.4 years.Elevated Ki-67 and age>65 years were associated with poorer OS.Recurrence was observed in 8 patients(12%)and was most frequent in patients with VIPomas.Recurrence was associated with a poorer OS(P=.012).Conclusion:f-PanNETs are exceedingly rare and present with a wide variety of symptoms,histopathological findings,and outcomes.Surgical resection can help achieve good long-term survival.

Investments in the exploitation of the continent’s raw material reserves have resulted in little (positive) impact on the energy security situation within Africa. This paper attempts to explore factors influencing the level of energy security of Small Medium Enterprises (SMEs) in the Greater Accra Region of Ghana, and from the dimension of availability, affordability, economic efficiency and environmental stewardship or sustainability of energy. The cross-sectional survey design was employed as the design for the collection and analysis of data. The study applied a quantitative research method in examining the indicators of energy security for SMEs in Accra Metropolis in the Greater Accra Region of Ghana. The population covered about 500 SMEs who have about 100 employees and total revenue of $1 million or the equivalence in Ghana Cedis. A sample of 246 was selected through a cluster and simple random sampling. The analysis adopted statistical techniques to conduct descriptive, exploratory and inferential analysis of the data collected. Factors that influence the level of energy security of SMEs are broadly categorized into the environment and economic change, political factors, physical disruptions and market conditions. Ge- nerally, the level of energy security under the availability dimension is medium among the SMEs. Issues of affordability, economic efficiency and environmental stewardship provide high medium interventions on energy security for SMEs. It is recommended that managers of Ghana’s energy sector should ensure prudent management of the country’s energy resources since the disruption of supply lines or sources form the key conditions that influence the energy security level of SMEs. SMEs should adopt energy efficiency measures to ensure that energy is conserved for future use.

The ongoing SARS-CoV-2 outbreak has rapidly increased the desire to manage bioaerosol exposures in indoor settings. Studies using chlorine dioxide gas (ClO2) at low concentrations have shown this intervention to be an effective mitigation strategy against viral, bacterial, and fungal elements in ambient air. There is an array of available products for generating ClO2 gas however most involve the use of expensive or sophisticated technology that makes their applicability limited to specialized consumers. The purpose of this study was to determine the virucidal efficacy of three pragmatic and affordable, ClO2 generating products using an aerosolized MS2 surrogate in a sealed chamber room under five different scenarios. The products tested included: Ultrashock—a ClO2 releasing pod (30 ppmv), Filter Media—a ClO2 impregnated zeolite media made to fit into an air blower housing (2 impregnated media filled air reactor tube (2 air samples were collected from sampling ports on the outer chamber wall at 0, 90 and 180 minutes. The Ultrashock and Filter Media with air flow in the rapid sweep scenario showed the greatest reduction in air MS2 (T180 = 99.992% and T180 = 99.996% respectively) compared to the control (T180 = 99.6%). When compared to the control results, the filter media with air flow engaged prior to the introduction of MS2 yielded reductions of 99.87% and 99.93% in air and on surfaces respectively at T0, demonstrating the protective effect residual ClO2 has against air and surface contamination. Previously, we demonstrated the virucidal efficacy of low concentration chlorine dioxide (ClO2) gas in room settings. The purpose of these studies was to evaluate novel ClO2 formats as potential biocidal interventions for real world congregate settings and air systems. Three types of studies were conducted to determine the efficacy of ClO2 in reducing bacteria and mold in various workspaces: hard and soft surfaces (gymnasium & equipment), aerosol (in-room), and within a laboratory environment. The study demonstrated that ClO2 was highly effective against both bacteria and mold with reduction ranging from 85.0% - > 99.4% for bacteria and >99.4% for yeast and mold. Treatments on hard and soft surfaces (gymnasiums and sports equipment), reduced bacteria by an average of 90% - 95%. The following treatments were applied overnight: 1) hard surface spraying with dilute ClO2 solutions, 2) carpet and tumbling treatments with powdered ClO2 releasing impregnates, and 3) HVAC treatment and overall room deodorization with low dose ClO2 gas from controlled releasing sachets. The in-room study treating air with a ClO2 filtration media also indicated significant air and surface room efficacy, with an average of 94% reduction in bacteria after 24-hour, and 99.4% reduction in mold after 24-hours. In a related air study, a biological combination of Raoultella terrigena and Staphylococcus aureus was injected as a bio-aerosol into a 4-inch diameter pipe with air flowing at approximately 1200 ft/min. Dry ClO2 gas was introduced into the air flow to achieve an effective concentration of 5 or 10 ppmv. Air samples were collected at sampling ports downstream from the fan at 10, 22, 55 and 100 ft along the pipe and used to evaluate changes in airborne bacteria and mold. Multiflux, a new thermal, hydrologic, and airflow model and software was being employed to solve the flow of heat, moisture, and air in and around an underground opening. The airway domain was solved with an integrated-parameter Computational Fluid Dynamic (CFD) module, which is an embedded part of the Multiflux code. The CFD model includes convection, conduction, and radiation for heat, as well as convection and diffusion for moisture transport in an air-filled opening. The surrounding rockmass model may be from any analytical solution, or from a complex thermal-hydrologic numerical model such as NUFT or TOUGH2. The rockmass model is interfaced to Multiflux using a novel technique called Numerical Transport Code Functionalization (NTCF). The purpose was to briefly describe the Multiflux model and show four example applications. The first example reports the results of Multiflux simulations for a mine drift, comparing calculations with CLIMSIM, a well known mine climate software, and with measured data. The second and third examples involve development ends in two coal mines. Another development-end ventilation model in Multiflux is also shown as the 4th example compared with field measurements from the Lucky Friday Mine in Idaho, USA for comparison. The results of the study show very good agreement between the Multiflux model and the available measured field results.

The in situ physicochemical analysis of nanostructured functional materials is crucial for advances in their design and production. X-ray coherent diffraction imaging （CDI） methods have recently demonstrated impressive potential for characterizing such materials with a high spatial resolution and elemental sensitivity; however, moving from the current ex situ static regime to the in situ dynamic one remains a challenge. By combining soft X-ray ptychography and single-shot keyhole CDI, we performed the first in situ spatiotemporal study on an electrodeposition process in a sealed wet environment, employed for the fabrication of oxygen-reduction catalysts, which are key components for alkaline fuel cells and metal-air batteries. The results provide the first experimental demonstration of theoretically predicted Turing-Hopf electrochemical pattern formation resulting from morphochemical coupling, adding a new dimension for the in-depth in situ characterization of electrodeposition processes in space and time.

A boronic acid-based anthracene fluorescent probe was functionalised with an acrylamide unit to incorporate into a hydrogel system for monosaccharide detection.In solution,the fluorescent probe displayed a strong fluorescence turn-on response upon exposure to fructose,and an expected trend in apparent binding constants,as judged by a fluorescence response where D-fructose>D-galactose>D-mannose>D-glucose.The hydrogel incorporating the boronic acid monomer demonstrated the ability to detect monosaccharides by fluorescence with the same overall trend as the monomer in solution with the addition of D-fructose resulting in a 10-fold enhancement(≤0.25 mol/L).
